한국, 미국, 일본의 간호인력 양성 및 활동 현황 비교 (Comparisons of the Nursing Workforce with Japan, and the U.S.)

  • 본 연구의 목적은 미국과 일본의 간호인력 양성 및 활동현황과의 비교를 통해 현재 우리나라 간호인력 정책 및 관리에 주는 시사점을 도출하는 것이다. 우리나라는 간호사, 간호조무사 및 간호보조인력의 높은 이직율과 이로 인한 간호서비스의 질 저하, 수급관리, 인력관리 비용의 증가 등 간호인력 정책제도 운영에 있어 여러 가지 어려움을 겪고 있으며, 이를 해결하기 위한 다양한 정책방안의 모색이 시급하다. 미국, 일본 등 간호인력 부족에 대한 어려움은 공통적이나, 이들 국가는 간호사 취업률이 우리나라의 40% 수준 대비 70~80% 수준으로 높고, 간호인력 확충을 위한 간호사 배치기준 법제화 및 수가정책, 근로조건 개선 등의 정책 마련이 중요함을 시사하고 있다. 우수한 간호인력의 안정적인 수급을 통해 환자안전 및 간호서비스의 질 향상을 달성하기 위해서는 보건당국의 보다 적극적인 간호인력 확충, 근로조건 개선 의지가 필요하며, 이를 위한 법 제도 개선이 요구된다.

의료서비스경험조사의 신뢰도 및 타당도 검토: 의료기관 특성별 차이를 중심으로 (Review of Reliability and Validity of Medical Service Experience Survey: Focused on the Differences by Type of Medical Institutions)

  • Background: The efforts to build more "people centered," "patient centered" health system has been emerging all over the world. Aligning with it, the Korean government is conducing the survey called "Medical Service Experience Survey (MSES)." There are critics, however, that MSES is not scrutinizing the medical experiences of patients in various healthcare settings. For this reason, this study aims to perform an empirical analysis of the differences in answers of patients responding to various healthcare settings. Methods: There are two steps in this study. First, explanatory analysis is conducted to compare the tendency of statistical concentration on questionnaires by divided healthcare settings. Second, confirmative analysis is carried out to evaluate the construct validity, reliability, and discriminant validity of the questionnaire in each healthcare setting. The raw data of MSES, which was conducted in 2020 by the Ministry of Health and Welfare in Korea and the Korean Institute for Health and Social Affairs is used. Results: As a result of exploratory factor analysis for all outpatients, the items were classified into four factors statistically: "doctor experience," "nurse experience," "outpatient service experience," and "patient satisfaction." It was confirmed that the reliability of all factors extracted was secured. However, for patients who visited hospitals, questionnaires related to personal privacy, such as "experiences on medical staffs considering physical exposure" or "experiences related to personal information exposure," were answered in conjunction with items of "nurse experience." Besides, patients responded that administrative elements of medical services, such as "experiences of comfort in medical institutions" and "experiences of satisfactory administrative services," were related to the items of "nurse experience." The answers of patients who visited traditional medical hospitals and clinics about "doctor experience" and "nurse experience" were not discerned statistically, and the answers to "doctor experience," "nurse experience," and "medical institution experience" were entangled with the responses of patients who visited dental hospitals and clinics. On the other hand, as a result of the confirmatory factor analysis, it was found that the inquiries of MSES generally had intensive validity. Conclusion: The collection of objective and scientific data is the prominent component to enlighten the patient-centered healthcare system alongside with change of the worldwide paradigm of measuring the healthcare system performance as follows the transition of perspective of health care from provider-centered to patient-centered. This study empirically shows that the patient experience can vary as the healthcare settings. Furthermore, to make an advance in measuring the experience of patients with medical services, this article proposes the deliberate consideration of the different kinds of healthcare settings and articulate design of the survey.

병원 내 EMR 시스템 연동을 통한 맞춤형 헬스케어 서비스 시스템 개발 (Customized healthcare service platform connecting EMR system)

  • 최근 대학병원급의 종합병원에서는 환자의 데이터가 많아짐에 따라 문제를 겪고 있다. 의료진들은 담당 환자들이 처방에 따라 생활을 하고 있는지 확인하기 어려울 때가 많다. 그리고 환자들은 처방에 대한 정보를 기억 못 할 때가 많으며, 처방에 대해 실생활에서 어떤 식으로 적용해야 할지 모를 때가 많다. 이런 문제들을 해결하기 위해 MQTT Push 알림을 활용한 맞춤형 헬스케어 서비스를 접근성이 좋은 웹 애플리케이션(이하 웹앱) 형식으로 제안한다. 의사의 처방에 따라 환자는 복약 시기, 진료 예약, 그 밖의 처방 관련 정보들을 알림 받고, 이에 따른 행동의 결과를 웹앱에 입력할 수 있다. 그리고 의료진 측에서 이를 확인하여 환자가 처방을 잘 따르고 있는지 확인하고, 특별한 사항을 환자에게 알릴 수 있는 기능을 웹앱에서 제공한다. 본 논문에서는 위의 기능들을 웹앱과 RESTful API 서버 플랫폼을 통해 제공하고, MQTT 브로커를 통해 웹앱에 알림 기능을 효율적으로 수행할 수 있도록 하여, 효과적인 맞춤형 헬스케어 서비스가 제공될 수 있음을 확인하였다.

간호·간병통합서비스병동 간호스테이션의 소음 특성 분석 (Characteristics of Noise in the Nurse Station of Comprehensive Care Wards)

  • Purpose: It is recently reported that the increasing noise in the hospitals has caused psychological and physiological stress problems with patients, and medical staffs. This study intends to investigate and analyze the noise levels in the sub stations in comparison with those in the wards in general. This study tries also to find some alterative solutions to the immediate problems. Methods: Noise measurements are conducted in advance prior to analyzing their results at two general hospitals with more than 900 beds, in the comprehensive nursing service wards and in the main/ sub stations located in the general wards Results: Although the noise level in the comprehensive nursing service wards has been slightly lower than that in the general wards, this result is over the recommended noise levels. Therefore it is recommended that efforts should be made to low down the noise level as an alternative and to replace the aged carts as well. Implications: The comprehensive nursing service wards are required to take measures against the various noise sources.

복지관의 경로식당 공간 디자인의 개선 방안에 관한 연구 (A Research on the Improvement of Dining Space Design at Elderly Welfare Facility)

  • Purpose : Balanced meal service is very important for the elderly to maintain good physical health. Good food and comfortable dining environment is also important for the elderly to prevent emotional depression. The purpose of this study is to investigate current problems of dining space in Korean elderly welfare facility. De-institutionalizing dining environment of elderly welfare facility will bring more meaningful social interactions among elderly users. Methods : Through literature research and case studies of welfare centers in the US and Japan, Elderly Dining Space Checklist was developed. Dining spaces of 11 welfare facilities in Seoul was analyzed by this checklist. It was administered by interior designers as well as facility staffs to achieve more credible results. Questionnaire for the elderly users of those dining space also prosecuted to broaden knowledge for creating improved environment. Results : Dining spaces of elderly welfare centers are lack of privacy and furnitures are not comfortable enough for the general users as well as wheelchair users. Elderly dining space users are seeking more private and comfortable spaces for the social communications. Implications : Dining environment for the elderly in Korea should develop more de-institutionalized design concepts.

EasyCare : 심계질환자 관리를 위한 에이전트 기반의 u-헬스케어 시스템 (EasyCare : An Agent-based u-Healthcare System for Managing Patients with Heart Diseases)

  • Due to the growth of economy and the advancement of IT, the life expectancy has been prolonged and the interests in health have greatly increased. Recently the request for systems that enable measuring the bio-signals of patients in the non medical organizations, such as home, and transmitting them to medical staffs at remote sites for monitoring them. In this paper, we present an agent-based u-health system for patients or suspects with heart diseases. Our system consists of portable devices for measuring bio-signals and agents that perform data collection, data storage, automatic detection of abnormal status in patients, and HL7-based data exchange in a cooperative way. The main features of the system are : the agent-based architecture facilitates the addition of new service modules as well as the modification of existing ones; an intelligent agent is provided which automatically detects situations in which the bio-signals of patients are abnormal; the medical data standard is supported so that the communication with other systems is very easy. To our survey, there have been few previous systems which support all those features in a seamless way.

국내 종합병원의 에너지 사용량에 영향을 미치는 요인 도출 (Study on the elements affecting energy consumption of general hospitals in Korea)

  • Purpose : The purpose of this study is to analyze the elements that affect the energy consumption of general hospitals in Korea. Methods : Factors affecting energy consumption in general hospitals include facility elements and operating elements. The effects of these elements on energy consumption of general hospitals were analyzed by statistical methods such as t-test, anova, correlation analysis, and regression analysis. Results : Total floor area, number of operating rooms, number of MRI, number of staffs, and number of outpatients can be said as useful variables that can explain energy consumption in general hospitals. Implication : It is possible to save energy consumption in general hospitals by adjusting major variables that affect the energy consumption of general hospitals.

응급부 진입구역과 환자분류구역의 감염관리를 위한 환자동선과 공간구성 계획에 대한 연구 (A Study on the Guideline of Spatial Composition and Circulation in Triages and Entrances Area in Emergency Departments for Efficient Infection Control)

  • Purpose: After Mers breakout in 2015, major hospital in Korea have been renovated the emergency department to make a separate infection control zone for high-risk patient with potential infection and to improve a triage area and an entrance area for efficient patient evaluation. However, there are no specific design standards to reinforce infection control for patients and staffs safety. Therefore, it is important to establish of initial design factors in the triage and entrance area as a guideline. Methods: 5 cases which had been recently renovated are selected to analyze patient circulation and spatial composition in a triage area and an entrance area. The partial floor plans of each case are represented as bubble diagrams to help understanding of different patient circulation flows. Based on this analysis, significant design factors which should be considered in planning stage for infection control have been extracted. Results: 13 design factors are established. Using these design factors, patient circulation diagram is generated to provide an optimized suggestion for efficient infection control. Implications: This suggestion provides basic databases to start to establish design guideline in the triage area and the entrance area to minimize infection spreading in the emergency department.

감염병전문병원의 시설 가이드라인에 관한 연구 - 운영방식과 건축계획을 중심으로 (A Study on the Establishment of Facility Guidelines for Infectious Diseases Hospitals - Focusing on Operational Methods and Architectural Planning)

  • Purpose: In terms of efficiency and safety, this study attempted to organize data on the operation methods and architectural planning of infectious diseases hospitals. Methods: The results obtained through on-site and interview surveys with hospital officials and medical staffs at four infectious diseases hospitals under construction were summarized based on those original business plans and facility guidelines. Results: First, the operational methods to secure safety and operational efficiency were summarized for each department which are major hospital functions of infectious disease hospitals. Second, as the architectural planning, the characteristics of space and circulation of each department are summarized. For safety of medical staff, negative pressure and non-negative pressure zone have to completely separated. In addition medical staff wears PPE and enters the negative pressure zone and returns in the order of admiral, shower, and gowning in the PPE undressing room after patient treatment. In case of operational efficiency, flexible operation is required in normal and crisis situations. For example, it is important for The Ward to gradually switch to negative pressure beds in times of crisis from normal situation and the outpatient department considers the composition of negative pressure and non-negative pressure outpatient spaces that can operate in parallel even in crisis situations. Implications: Infectious disease hospitals require flexible operation and appropriate facilities for normal and crisis situations.

병원 헬스케어 서비스 혁신: ICT 조직기능 연관성 (ICT Relatedness)에 기반한 상호작용과 지식공유 (Hospital Healthcare Service Innovation: Interaction and Knowledge Sharing based on ICT Relatedness)

  • 본 연구의 목적은 병원 헬스케어 서비스 분야에서 ICT 조직기능 연관성(Information and Communication Technology Relatedness)과 조직환경이 의료진간 또는 의료진과 환자간 상호작용과 지식공유에 어떻게 영향을 미치며, 지식공유는 의료 서비스 혁신, 그리고 서비스 혁신은 서비스성과에 어떻게 영향을 미치는지를 구조화된 모델을 통해 연구하는 것이다. 자원준거관점(Resource based view), 적합구조화이론(Adaptive Structuration Theory), 그리고 서비스 혁신이론을 기반으로 헬스케어 서비스 시스템에서 성과에 영향을 미치는 주요요인으로 ICT 조직기능 연관성, 구성원간의 상호작용과 지식공유를 설정했고, 성과요인으로는 서비스 혁신과 서비스 성과를 설정였다. 모델의 검증을 위해 병원에서 종사하는 의료직, 기능직, 사무직 종사자들을 대상으로 설문을 실시하여 분석하였다. 모델 검증 결과, 모든 가설들은 유의미하게 성과창출에 긍정적인 영향을 주는 것으로 밝혀졌다. 구조화된 조직환경과 ICT 조직기능 연관성은 서비스 조직구성원 및 환자들의 행위에 영향을 주어 상호작용 및 지식공유를 촉진하고, 병원 헬스케어 서비스의 혁신과 성과에 영향을 미치는 중요한 요소임을 시사하고 있다.
