중년전환기 여성의 건강관련 삶의 질 변화유형 분석: 여성가족패널 자료를 이용한 2차자료분석 (Identifying Trajectories of Health-related Quality of Life in Mid-life Transition Women: Secondary Data Analysis of Korean Longitudinal Survey of Women & Families)

  • Purpose: The purpose of this study was to identify latent classes of health-related quality of life trajectories in middle-aged women and investigate predictors for latent classes. Methods: This study utilized data from the 2nd, the 4th to the 7th Korean Longitudinal Survey of Women & Families. The subjects included 1,351 women aged 40~45 years. The data was analyzed using latent class growth analysis and logistic regression. Results: Two trajectories were identified for health-related quality of life in middle-aged women; 'persistently good' and 'increasing' groups. Predictors for the 'increasing' group were lower economic status, higher depression, and lower perceived health status. Conclusion: This study showed that characteristics of the individual, symptom status, and health perceptions were associated with health-related quality of life in middle-aged women. It is necessary to provide effective intervention for latent classes of health-related quality of life trajectories based on physical, mental, and social factors.

노인의 다중적 건강 변화궤적 유형화 및 관련요인 탐색 (Multidimensional Health Trajectories and Their Correlates Among Older Adults)

  • 본 연구는 고령화연구패널조사(KLoSA)의 1-6차 연도 자료를 분석하여 노년기의 우울, 만성질환, 인지기능의 다중적 측면에서의 건강 변화 유형을 파악하고 이와 관련이 있는 요인들을 분석하고자 하였다. 연구의 대상은 1차 연도 조사 시점에서 65세 이상이고 결측률이 없는 총 2,059명이다. 잠재성장모형을 통해 세 가지 건강 측면이 시간에 따라 어떠한 변화궤적을 보이는지 살펴보았으며, 성장혼합모형을 활용해 세 건강 요인에서 서로 다른 변화 궤적을 보이는 집단들을 유형화하였다. 또한 일원분산분석 및 Scheffé 사후검증, 교차분석을 통해 건강 변화유형 집단에 따라 인구사회학적 특성, 건강행동, 삶의 만족도에서 차이가 나타나는지를 살펴보았다. 본 연구의 주요 결과는 다음과 같다. 첫째, 잠재성장모형 분석 결과 우울과 만성질환의 수는 연령에 따라 점차 증가하고 인지기능은 점차 감소했다. 둘째, 성장혼합모형을 분석한 결과 노인의 건강 변화는 일반적 노화, 질환 증가, 만성적 문제의 세 집단으로 유형화되었다. 셋째, 노인의 인구사회학적 특성, 건강행동, 삶의 만족도는 세 집단에 따라 유의한 차이가 나타났다. 특히 만성적 문제 집단의 경우 교육 및 소득수준이 낮고, 전반적으로 낮은 삶의 만족도를 나타냈다. 본 연구는 노년기 건강증진 교육 프로그램에 있어 노인들이 경험하는 건강의 다중적 측면을 고려하는 것이 중요함을 보여주며, 노년기 건강증진 교육 프로그램을 위한 제언을 제공한다. 또한 전생애적 건강관리를 위한 건강정보문해력 증진에 있어서 가정과 교과과정이 갖는 중요성에 대해 논의하고자 한다.

Influence of Asthma on the Longitudinal Trajectories of Cigarette Use Behaviors From Adolescence to Adulthood Using Latent Growth Curve Models

  • Objectives: While epidemiologic research indicates that the prevalence of risk-taking behaviors including cigarette smoking among young people with asthma is substantial, the longitudinal patterns of cigarette smoking in this vulnerable population have received little attention. The aim of this study was to evaluate differences in the longitudinal trajectories of cigarette use behaviors from adolescence to adulthood between young people with and without asthma. Methods: Data from the National Longitudinal Study of Adolescent to Adult Health (Add Health) during the years 1994 to 1995 (Wave I, adolescence), 2001 to 2002 (Wave III, young adulthood), and 2007 to 2008 (Wave IV, adulthood) were analyzed (n=12 244). Latent growth curve models were used to examine the longitudinal trajectories of cigarette use behaviors during the transition to adulthood according to asthma status. Results: Regardless of asthma status, the trajectory means of cigarette use behaviors were found to increase, and then slightly decrease from adolescence to adulthood. In total participants, there were no statistically significant differences in initial levels and changes in cigarette use behaviors according to asthma status. However, in select sex and race subgroups (i.e., females and non-whites), former asthmatics showed greater escalation in cigarette use behaviors than did non-asthmatics or current asthmatics. Conclusions: This study indicated that the changing patterns of cigarette use behaviors during the transition to adulthood among young people with asthma are comparable to or even more drastic than those among young people without asthma.

1인가구의 주관적 건강상태 변화: 잠재계층성장모형을 활용하여 (Trajectories of Self-rated Health among One-person Households: A Latent Class Growth Analysis)

  • Purpose: The aim of this study is to explore different types of self-rated health trajectories among one-person households in Korea. Methods: We used five time-point data derived from Korea Health Panel (2011~2015). A latent growth curve modeling was used to assess the overall feature of self-rated health trajectory in one-person households, and a latent class growth modeling was used to determine the number and shape of trajectories. We then applied multinomial logistic regression on each class to explore the predicting variables. Results: We found that the overall slope of self-rated health in one-person households decreases. In addition, latent class analysis demonstrated three classes: 1) High-Decreasing class (i.e., high intercept, significantly decreasing slope), 2) Moderate-Decreasing class (i.e., average intercept, significantly decreasing slope), and 3) Low-Stable class (i.e., low intercept, flat and nonsignificant slope). The multinomial logistic regression analysis showed that the predictors of each class were different. Especially, one-person households with poor health condition early were at greater risk of being Low-Stable class compared with High-Decreasing class group. Conclusion: The findings of this study demonstrate that more attentions to one-person households are needed to promote their health status. Policymakers may develop different health and welfare programs depending on different characteristics of one-person household trajectory groups in Korea.

Neural Spike Train Decoding에 기반한 인공와우 어음처리방식 성능평가 (Performance Evaluation of Cochlear Implants Speech Processing Strategy Using Neural Spike Train Decoding)

  • We suggest a novel method for the evaluation of cochlear implant (CI) speech processing strategy based on neural spike train decoding. From formant trajectories of input speech and auditory nerve responses responding to the electrical pulse trains generated from a specific CI speech processing strategy, optimal linear decoding filter was obtained, and used to estimate formant trajectory of incoming speech. Performance of a specific strategy is evaluated by comparing true and estimated formant trajectories. We compared a newly-developed strategy rooted from a closer mimicking of auditory periphery using nonlinear time-varying filter, with a conventional linear-filter-based strategy. It was shown that the formant trajectories could be estimated more exactly in the case of the nonlinear time-varying strategy. The superiority was more prominent when background noise level is high, and the spectral characteristic of the background noise was close to that of speech signals. This confirms the superiority observed from other evaluation methods, such as acoustic simulation and spectral analysis.

청소년 우울 증상의 변화 궤적에 따른 잠재계층유형 및 영향요인 (Latent Classes of Depressive Symptom Trajectories of Adolescents and Determinants of Classes)

  • Purpose: Untreated depression in adolescents affects their entire life. It is important to detect and intervene early depression in adolescence considering the characteristics of adolescent's depressive symptoms accompanied by internalization and externalization. The aim of this study was to identify latent classes of depressive symptom trajectories of adolescents and determinants of classes in Korea. Methods: The three time-point (2018~2020) data derived from the Korean Children and Youth Panel Survey 2018 were used (N=2,325). Latent Growth Curve Modeling (LGCM) was conducted to explore the depressive symptom trajectories in all adolescents, and Latent Class Growth Modeling (LCGM) was conducted to identify each latent class. Multinomial logistic regression analysis was performed to confirm the determinants of each latent class. Results: The LGCM results showed that there was no statistically significant change in all adolescents' depressive symptoms for 3 years. However, the LCGM results showed that four latent classes showing different trajectories were distinguished: 1) Low-stable (intercept=14.39, non-significant slope), 2) moderate-increasing (intercept=19.62, significantly increasing slope), 3) high-stable (intercept=26.30, non-significant slope), and 4) high-rapidly decreasing (intercept=26.34, significantly rapidly decreasing slope). The multinomial logistic regression analysis showed that the significant determinants (i.e., gender, self-esteem, aggression, somatization, peer relationship) of each latent class were different. Conclusion: When screening adolescent's depression, it is necessary to monitor not only direct depression symptoms but also self-esteem, aggression, somatization symptoms, and peer relationships. The findings of this study may be valuable for nurses and policy makers to develop mental health programs for adolescents.

지하터널에서 주행하는 전동차의 하부에서 발생한 입자의 이동경로 예측 (Prediction of Trajectories of Particles Generated Underneath a Subway Train Running in An Underground Tunnel)

  • In this study, the flow around a subway train running in an underground tunnel was numerically estimated. For the validation of the numerical results, the airflow velocity at a point underneath a subway train was measured using an ultrasonic anemometer. Then, the trajectories of particles generated at the contact points between the wheels and rails were numerically predicted. By considering the airflow velocity and particle trajectories, the space underneath the T-Car (trailer car) was expected to be appropriate for the room for the installation of a dust-removal system.

PSCF 모델을 활용한 부산지역 PM10의 발생원 추정 (Estimation of PM10 source locations in Busan using PSCF model)

  • The purpose of this study is to find out the air flow patterns affecting the PM10 concentration in Busan and the potential sources within each trajectory pattern. The synoptic air flow trajectories are classified into four clusters by HYSPLIT model and the potential sources of PM10 are estimated by PSCF model for each cluster from 2008 to 2012. The potential source locations of PM10 are compared with the distribution of PM10 anthropogenic emissions in east Asia developed in 2006 for the NASA INTEX-B mission. The annual mean concentrations of PM10 in Busan decreased from $51ug/m^3$ in 2008 to $43ug/m^3$ in 2012. The monthly mean concentrations of PM10 were high during a spring season, March to May and low during a summer season, August and September. The cluster2 composed of the air trajectories from the eastern China to Busan through the west sea showed the highest frequency, 44 %. The cluster1 composed of the air trajectories from the inner Mongolia region to Busan through the northeast area of China showed the second high frequency, 26 %. The cluster3 and 4 were composed of the trajectories originated in the southeast sea and the east sea of Busan respectively and showed low frequencies. The concentrations of in each cluster were $47ug/m^3$ in cluster1, $56ug/m^3$ in cluster2, $42ug/m^3$ in cluster3 and $37ug/m^3$ in cluster4. From these results, it was proved that the cluster1 and 2 composed of the trajectories originated in the east and northeast area of China were the causes of high PM10 concentrations in Busan. The results of PSCF and CWT model showed that the potential sources of the high PM10 concentrations were the areas of the around Mongolia and the eastern China having high emissions of PM10 from Beijing, Hebei to Shanghai through Shandong, Jiangsu.

노년기 우울의 다중변화궤적에 관한 연구 (Multiple Trajectories of Depressive Symptoms Among Older Adults)

  • 본 연구는 우리나라 노년기 우울변화의 다중궤적을 확인하고, 각 궤적집단의 특성을 파악하는 것을 목적으로 한다. 이를 위해 한국복지패널 1차년도부터 5차년도(2006-2010) 자료를 활용하였으며, 만 60세 이상 4,181명의 자료를 분석하여 노인 우울의 다중궤적과 각 궤적집단의 결정요인을 분석하였다. 주요 분석방법으로는 잠재성장혼합모형과 다항 로지스틱 회귀분석 등이 사용되었다. 노년기 우울과 관련있는 여섯 가지 요인을 통제한 상태에서 노인 우울의 다중궤적을 분석한 결과, 감소형(10.6%), 증가후감소형(9.7%), 증가형(7.9%), 그리고 안정형(71.8%)의 네 가지 유형이 존재하는 것을 확인하였다. 우리나라 노인우울의 궤적집단을 결정하는 요인으로는 주관적 건강상태, 자아존중감, 가족관계만족, 경제활동기간, 그리고 빈곤기간은 안정형과 나머지 세 개의 우울궤적을 구분하는 공통요인으로 나타났다. 이밖에 만성질환, 배우자상실, 그리고 가구소득은 안정형과 증가후감소형을 구분하는 요인이었으며, 연령과 공적연금수급은 안정형과 증가형을 구분하는 것으로 나타났다. 연구결과를 토대로 노년기 우울을 예방하고 완화하기 위한 개입방안 제시하였다.

중·장년기 주관적 건강상태의 변화궤적 유형과 예측요인 탐색: 잠재계층성장분석(LCGA)을 이용한 종단연구 (A Study on Change Trajectories of Self-Rated Health in Middle Aged: Longitudinal Study Using Latent Class Growth Analysis)

  • 본 연구는 중·장년기 주관적 건강상태의 변화궤적 유형을 탐색적 차원에서 확인하고, 도출된 주관적 건강상태 변화궤적 유형과 사회경제적 요인, 개인 내적 요인, 가족관계 요인의 관계를 검증하여 중·장년기의 건강한 노후를 맞이하는 방안을 모색하는데 목적을 두고 있다. 이를 위해 한국복지패널 2~12차(2007년~2017년)의 종단자료를 활용하여 중·장년기 2,418명을 최종 분석대상으로 삼았다. 연구방법으로는 주관적 건강상태의 변화궤적 유형을 확인하기 위하여 잠재계층성장분석을 적용하였으며, 건강상태에 있어 안정집단과 위험집단을 예측하는 요인을 검증하기 위하여 다항 로지스틱 회귀분석을 실시하였다. 연구결과, '고수준-유지형(46.3%)', '저수준-유지형(19.6%)', '감소형(17.5%)', '증가형(16.6%)'의 4가지 유형이 확인되었다. 건강상태와 밀접한 요인인 만성질환 변화를 통제하였음에도, 지속해서 경제활동을 할수록 '저수준-유지형'보다'고수준-유지형'에 속하는 것으로 나타났다. 또한, 자아존중감이 높아질수록 '감소형' 보다 '고수준-유지형'에 속하고, 가족관계에 대한 만족이 증가할수록 '저수준-유지형' 보다 '고수준-유지형'으로, 가족 스트레스를 덜 받을수록 '저수준-유지형'보다 '증가형'에 속하게 되는 것으로 나타났다. 본 연구를 바탕으로 중·장년의 건강을 지속해서 관리할 수 있도록 돕는 국가 체계의 정책적 제언과 현장 중심의 실천적 제언을 제시하였다.
