The Effect of Screening of Stomach Cancer on Stage Shift (위암의 조기검진에 의한 병기이전(stage shift) 효과)

  • Objectives : This study was performed to evaluate the effect of stage shift according to screening of stomach cancer. Methods : Total 840 cases of stomach cancer patients, undergone a surgical operation at Department of Surgery, Kangnam St. Mary's Hospital, The Catholic University of Korea from Jan. 1989 to Dec. 1995, were reviewed by stomach cancer working sheet, and classified as asymptomatic and symptomatic group based on the presence of subjective symptoms on their hospital visit. Their histopathologic stages were analysed. We compared the histopathologic stages of asymptomatic stomach cancer patients with those of symptomatic patients. Results : From the total of 840 patients, asymptomatic patients group comprised 28 cases (3.3%). Proportion of asymptomatic patients tended to increase from 1.9% in 1990, 0.9% in 1991 to 8.6% in 1995. Proportions of asymptomatic patients by stages were 78.6% (stage I), 3.6% (stage III), 17.9% (stage IV) and that of symptomatic patients by stages were 38.2% (stage I), 16.5% (stage II), 24.8% (stage III), 19.1% (stage IV). In less than 40 years old, 50.5% of symptomatic patients were diagnosed as stage I. With increment of ages, proportions of stage I were makedly decreased. It was significantly different between proportion of early gastric cancer in asymptomatic patients (60.1%) and that in symptomatic patients (25.0%). Conclusions : We confirmed stage shift according to screening of stomach cancer. And proportion of early gastric cancer in asymptomatic patients was higher than that in symptomatic patients. This results suggest that screening of gastric cancer be important to reduce mortality and if be indirectly started from 40 years old.

Correlation of Body Mass Index and Neck Circumference in Patients with Obstructive Sleep Apnea-Hypopnea Syndrome (폐쇄성 수면무호흡 증후군과 목둘레 및 체질량 지수와의 상관성 연구)

  • Obstructive Sleep Apnea-Hypopnea Syndrome (OSAHS) is a disorder characterized by the repetitive collapse of the pharyngeal airway during sleep, which leads to oxygen desaturation, sleep fragmentation, daytime sleepiness, and increased risk for hypertension and stroke. We investigated the clinical factors related to the severity of OSAHS. Polysomnography was performed in three hundred and ninety five consecutive adult patients with clinical symptoms of obstructive sleep apnea syndrome. All patients completed the sleep questionnaire and the Epworth Sleepiness Scale before polysomnography. Patients were classified into four groups based on the severity of their polysomnographic data: Non-OSA group, characterized by Apnea-Hypopnea Index (AHI) < 5; mild OSA group, by AHI 5-15; moderate OSA group, by AHI 16-30; and severe OSA group, by AHI > 30. Neck circumference was also measured at the cricothyroid level. A total of 395 patients (336 men and 59 women) were studied. In the non-OSA group, there were 55 patients; their mean neck circumference was $39.63{\pm}4.24cm$ and mean BMI was $24.48{\pm}3.53$. In the mild group, there were 101 patients; their mean neck circumference was $41.93{\pm}3.75cm$ and mean BMI was $25.33{\pm}2.94$. In the moderated group, there were 93 patients; their mean neck circumference was $43.27{\pm}3.50cm$ and BMI was $25.90{\pm}2.88$. In the severe group, there were 146 patients; their mean neck circumference was $44.94{\pm}3.93cm$ and mean BMI was $26.81{\pm}3.76$. Men had significantly larger neck circumference than women ($Mean{\pm}SD$, $43.72{\pm}3.83$ vs $39.17{\pm}4.30$, p < 0.001), and higher AHI than women ($29.12{\pm}22.65$ vs $14.63{\pm}14.11$, p < 0.001). Multiple regression analysis revealed that neck circumference was the most significant predictor of AHI. Neck circumference and BMI were positively correlated with the severity of OSAHS. The severity of OSAHS was greater in men than in women.

The Epidemiologial Study on the Welders' Pneumoconiosis among Shipyard Welders in Pusan Area (부산지역 조선업 용접공들의 진폐증에 관한 역학적 조사연구)

  • Authors studied on the prevalence of 94 cases of pneumoconiosis who were found out through the screening test on 1,062 workers engaged in welding process at 36 shipyard in Pusan area from March 1st, 1986 to November 30th, 1986. The result were as follows; 1. Dust concentration was measured $4.49{\pm}0.54mg/m^3$ in the small scale shipyard while it was $6.25{\pm}1.08mg/m^3$ in the large one. 2. The prevalence of welder's lung was 8.9% (male:8.5%, female:12.0%) and this is consist of 4.9% suspected pneumoconiosis and 4.0% pneumoconiosis more than category 1/0. 3. The prevalence was significantly increased according to the duration of dust exposure(p<0.05), and it showed the increasing tendency by the age group. 4. The prevalence was significantly higher in the large scale shipyard than is the small scale one(p<0.01). 5. The type and shape of opacities were 71.4% of p type and 28.6% of q type, however no pneumoconiosis with r type observed in this study. 6. The main subjective symptoms were the sputum(29.8%), coughing(25.5%), shortness of breath(20.2%), fatique(6.4%), and chest pain(5.3%). In other hand, 57.4% of pneumoconiosis were asymptomatic. 7. The prevalence of pulmonary tuberculosis was radiologically 1.7% in all subjects including 5.3% in pneumoconiosis and 1.3% in no pneumoconiosis(p<0.01).

A Survey on the Prevalence of Musculoskeletal Symptom according to Work Task (작업유형별 근골격계 증상 호소율에 관한 조사연구)

  • Though people occupationally exposed to machineries and automation in the industrialized society desire work involving decreased strength, due to the continuous and repetitive activities, a new industrial stress is present. Studies on prevalence of musculoskeletal disease and their related risk factors have evolved. In this study in relation to work tasks, we investigated the differences in musculoskeletal symptoms occurring In each body region. The results of the survey were as follows. 1. When comparing age, level of education, work duration, job satisfaction and leisure time activities according to work task, age in control group was $38.83{\pm}5.5$, in comparison to the other 2 groups was smaller(p<0.05), and level of education in control group was higher (p<0.05). Work duration in the cutting department was $8.04{\pm}4.99$ years longer than the other 2 groups (p<0.05), but there was no difference in the job satisfaction and leisure time activities. 2. The mean of symptom scores of each work task was 1.54 in the cutting department, 1.57 in the press department and 1.59 in the control group, and there was no significant differnce in the 3 groups. The mean of symptom scores for upper extremities in the control group was low but no statistically significant diffrence was shown. 3. When comparing the mean of symptom score according to work task in the each body region, in the shoulder region, the symptom score in the press department which desired strength was higher than the other 2 groups but no significant difference was shown. In the wrist region the cutting department scored 1.01 and in comparison to the other 2 groups was significantly increased (p<0.05). 4. The results of the univariate regression analysis on the major individual risk factors associated with musculoskeletal symptom relating work showed that previous symptom complaints in the same body region was significant risk factor(p<0.001) in the whole body Besides wrist, hip, and knee, psychological problem was shown to be a significant factor(p<0.05). And the body regions which work task was significant risk factor were wrist and neck region (p<0.05). 5. The results of the multiple regression analysis involving significant factors of each body region from the univariate regression analysis showed that previous symptom complaint in whole body region(p<0.001) and psychological problem in the shoulder, elbow and lumbar region (p<0.05) were significant factors, and work task was significant factor in the wrist (p<0.05).

  • Aim : The purpose of this study was to investigate the hypothesis that treatment with self-checking behavior management program was valuable in the management of temporomandibular disorders compared with a existing method treatment group. Methods : All participants who are diagnosed with temporomandibular disorders were treated with medication and physical therapy. They came into the clinic at intervals of two weeks, three times. The signs and symptoms were assessed investigating Pain NRS(Numerical Rating Scale), MCO(Maximum comfortable opening), Noise NRS, LOM(Limitation of Mouth opening) NRS at every visit. Experimental group received instructions repeatedly by using self evaluation questionnaire. Control group was educated once at the beginning of treatment. The difference Pain NRS, MCO, Noise NRS, LOM NRS between first and second visits, first and third visits were estimated.. Result :Experimental group showed greater improvement on MCO difference after stopping medication(p=0.001). This improvements were prominent in the male(p=0.001) and the first or second decade of the patients(p=0.004). Conclusion : The present study showed that there was better result when educating repeatedly with using self evaluation questionnaire than educating once at the beginning of the treatment.

  • Purpose : To investigate the medical information related to HRT online and the medical treatment of perimenopausal and postmenopausal women in the Dept. of Ob&Gy Korean Medicine Hospital, after the discontinuance of the WHI trial in U.S, July 2002. Methods : With the key-words "HRT", "Hormone Replacement therapy", "호르몬 대체요법(HRT)", 갱년기 증후군(perimenopausal syndrome)", “폐경기후증후군(postmenopausal syndrome)", I searched for the information from July 2002 to 2005 on DAUM, the representative portal site in Korea, and I've got a grasp of the tendency of the informational propagation on HRT. Moreover, I investigated chief complaints and tendency of give up HRT of the perimenopausal and postmenopausal women(aged between 47 and 60) who visited Dept. of Ob&Gy Korean Medicine Hospital for 2 years and 6 months since July 2002. Results : 1) Searching for the news on DAUM, I found; 2 articles on the methods of HRT: 4 on the positive effects of HRT: 4 on the general items including the positive effects of HRT: 19 on the side effects of HRT: 1 on the insignificant effect of HRT :4 on the apprehensions about HRT: 3 on the strengthening of the criteria on medical fees review: 3 on the discontinuance of HRT: 8 on the alternative materials and medicines to HRT: 4 on the guidance for the phyto-estrogen. 2) I analyzed chief complaints of 120 women. The majority of chief complaints were vasomotor symptoms like hot flush and sweating. There were only 4 patients who wanted to give up HRT. Conclusion : The side effects of HRT were objectively dealt with online but there was not enough effective and continuous guidance. In the case that a woman not on HRT wishes to overcome perimenopausal period through KM therapy, this information may have affected her decision. However, not many women who were already on HRT terminated the therapy for fear of side effects and switched to KM therapy. Promotion of KM therapy in improving health during perimenopausal and postmenopausal period is desperately needed.

State of Visual Acuity Test and Factors Related to Vision Deterioration of Preschoolers (미취학 아동의 시력검진 실태와 시력저하 관련요인)

  • The study aimed to examine the state of visual acuity tests in preschoolers and determine the factors affecting the deterioration of their vision. A total of 172 pair, child aged 5-7 years and either one of their parents, were recruited from childcare centers or kindergarten between August 2015 and January 2016. Questionnaires were administered to the parents, and the children's visual acuities were measured. Using forward selection in logistics regression analysis, factors affecting vision deterioration were elucidated. Of 172 children, 128 (74.4%) had undergone a visual acuity test previously and 77 (44.8%) showed a deterioration in their vision. Children who had undergone their first visual acuity test after the age of 7 years (OR=7.425, CI=2.844- 19.385) and showed more abnormal eye symptoms, such as squinting or tilting the head to see or falling down frequently (OR=3.084, CI=1.202-7.914) and whose age was younger (OR=10.335, CI=3.252-32.848), were more likely to develop deterioration of vision. Children who had a posture such that they looked up at the computer monitor from below (OR=.075, CI=.022-.255), were less likely to show deterioration of vision. It can be inferred that early visual acuity tests is essential to detect deteriorated vision of preschoolers. Therefore, it is necessary to educate parents, personnels in day care center, and health clinics about the importance of early visual acuity tests and close observation of specific behaviors related to vision deterioration. Development of precautionary intervention program of vision deterioration in preschoolers and examination of its effects are needed.

The Effect of Short-term Forest Therapy Camp on Youths with Internet Addiction Risk Group: Focused on the Biological, Neurocognitive and Psychosocial Aspects (인터넷중독 위험 청소년에 대한 단기 산림치유 효과: 생물학적, 신경인지적 및 심리-사회적 측면을 중심으로)

  • This study examined the effectiveness of 'Forest Therapy (FT)' from the biological, neurocognitive and psychosocial perspectives. Adolescents who were classified as potential internet addicts took part in a FT program for two (once or twice) or three days. Before and after participating the program, Serum Brain-derived neurotrophic factor (BDNF), Comprehensive Attention Test (CAT), Children's Depression Inventory (CDI), and State Anxiety Inventory (STAI) were measured. It was found that FT led to positive consequences, indicated by increased serum BDNF, improved CAT performance, reduced internet use desire, greater resilience, and better social relationship. In addition, FT was partially effective in alleviating state anxiety level, when the participants were classified in accordance with FT exposure lengths. Overall, this study provides evidence that FT reduces symptoms related to internet addiction and promotes various qualities required for well-being.

  • This study was done to determine the relationship between the anxiety levels of hospitalized psychiatric patients and various influencing variables. The purpose of this study was to determine factors that may help hospitalized psychiatric patients to experience lower levels of anxiety in relation to changing situations and provide the basic data for a dynamic approach which is important in the field of modern psychiatric nursing that understands and analyses the meaning of patients behavior. The anxiety may produce stress, which is a common experience among all human beings. Patients may merely feel uncomfortable in the state of mild anxiety, however, the severe state could be an obstacle to treatment and recovery from disease. The anxiety of the psychiatric patient is a factor which greatly influencing the patient's behavior, so his disorderd behavior is an expression of defence or pathologically fixed behavior. According the psychiatric patient's anxiety at the time of admission is the concern of the health team. The nurse's special concern has to do with understanding and supporting the patient and meeting his individual needs by frequent close contact during the entire hospitalization period, compared to other teamembers the nurse's responsibility in this regard is greated. So this study emphasizes the necessity of creating conditions these, but above all the psychiatric nurse should create a therapeutic environment by not only regarding the patient's behavior or symptoms but understanding the meaning of them. The subjects of this study were 57 psychiatric patients selected from the K neuropsychiatric hospital located in Kunsan city. Data were collected twice from the same patients within a 24 hour period after admission and 10 days after admission. (September 18th to November 8th, 1980). The data collected method was through direct interview, and the interview time was 20 minutes for each patient. Data analysis included Item Analysis & Internal Consistency Reliability Tests, Percentages, t-test, analysis of variance and stepwise multiple regression analysis. The findings of this study were as follows. A. Test of Hypothesis a. Hypothesis 1 :“The anxiety level of psychiatric patients within 24 hours after admission will be higher than those of the same patients 10 days after admission,”was accepted.   • This study was conducted to analyze the patient's exposed dose targeting the patients who had acute ischemic stroke symptoms and CT brain perfusion scan, by comparing fixed time technique and bolus tracking technique which was provided by the manufacturer and to identify the Time graph to implement the usability of contrast medium's tracking technique the best contrast enhancement intervals. $CTDI_{VOL}$ of PCT in patient appeared to be 431.72mGy in fixed scan delay protocol, whereas 323.61mGy in Bolus tracking technique. The value of DLP appeared to be $1243.47mGy{\cdot}cm$ in fixed scan delay protocol, whereas $932mGy{\cdot}cm$ in Bolus tracking technique. Time graph appeared to be various in fixed scan delay protocol, whereas the optimal time graph could be obtained in Bolus tracking. The exposure dose could be reduced by 25% applying Bolus tracking technique when taking brain perfusion CT scan.
