Initial Report for the Radiation Effects Research Foundation F1 Mail Survey

  • To study the full health effects of parental radiation exposure on the children of the atomic bomb survivors, the Radiation Effects Research Foundation developed a cohort of 76,814 children born to atomic bomb survivors (F1 generation) to assess cancer incidence and mortality from common adult diseases. In analyzing radiation-associated health information, it is important to be able to adjust for sociodemographic and lifestyle variations that may affect health. In order to gain this and other background information on the F1 cohort and to determine willingness to participate in a related clinical study, the F1 Mail Survey Questionnaire was designed with questions corresponding to relevant health, sociodemographic, and lifestyle indicators. Between the years 2000 and 2006, the survey was sent to a subset of the F1 Mortality Cohort. A total of 16,183 surveys were completed and returned: 10,980 surveys from Hiroshima residents and 5,203 from Nagasaki residents. The response rate was 65.6%, varying somewhat across parental exposure category, city, gender, and year of birth. Differences in health and lifestyle were noted in several variables on comparison across city and gender. No major differences in health, lifestyle, sociodemographics, or disease were seen across parental exposure categories, though statistically significant tests for heterogeneity and linear trend revealed some possible changes with dose. The data described herein provide a foundation for studies in the future.

연도별 가습기 살균제 피해자 분포 고찰 - 폐 손상 중심 - (Distribution of Health Problems Associated with Humidifier Disinfectant by Year)

  • Objectives: Studies are needed to examine the characteristics of health effects reported by people who used humidifier disinfectant (HD), including the distribution of victims. Methods: We analyzed the distribution of health effects including lung injury that were asserted by a total of 699 individuals who registered with the first through third national programs to determine health effects associated with the use of HD. Results: We found that HD-associated lung injury (HDLI) occurred every year from 2002 through 2011, and in 2011 for 37.2% (n=96) of the total of 258 clinically evaluated HDLI victims. More than half of the victims responded that they were victimized between 2010 and 2011. This was consistent among all classifications by sex, age, HD brand and HD chemical ingredient. Conclusion: This study assumed that the major reason for the outbreak of the health effect between 2010 and 2011 could be the variations in concentrations and physical properties related to polyhexamethylene guanidine (PHMG). Further studies are necessary to examine if some factors related to the chemical disinfectants contained in HD brands may have caused the massive outbreak of health effects including HDLI.

  • Hypotheses In health services research are becoming increasingly more complex and specific. As a result, health services research studies often include multiple independent, intervening, and dependent variables in a single hypothesis. Nevertheless, the statistical models adopted by health services researchers have failed to keep pace with the increasing complexity and specificity of hypotheses and research designs. This article introduces a statistical model well suited for complex and specific hypotheses tests in health services research studies. The covariance structure modeling(CSM) methodology is especially applied to regional relevance indices(RIs) to assess the impact of health resources and healthcare utilization. Data on secondary statistics and health insurance claims were collected by each catchment area. The model for RI was justified by direct and indirect effects of three latent variables measured by seven observed variables, using ten structural equations. The resulting structural model revealed significant direct effects of the structure of health resources but indirect effects of the quantity on RIs, and explained 82% of correlation matrix of measurement variables. Two variables, the number of beds and the portion of specialists among medical doctors, became to have significant effects on RIs by being analyzed using the CSM methodology, while they were insignificant in the regression model. Recommendations for the CSM methodology on health service research data are provided.

  • Objectives: The aim of the present study is to elucidate the relationship of community capacity to health in a metropolitan area in Korea. To do so, a multi-level model to verify the contextual effects of community capacity is presented. Methods: The study materials are the "The 4th Seoul Citizens Health Indicators Surveys" on 404 dong in Seoul. The community capacity indicators were developed in two strata: individual-level indicators with community identity domain; and community-level indicators with participation in community organizations, number of non-profit organizations, degree of organizing of community-based organizations, and volunteer activities. Results: Higher unhealthy probability occurs among those with lower community capacity at the community level, lower individual income, and lower community satisfaction at the individual level. It contributed to explaining self-rated health status and showed that there were contextual effects of the community going beyond the compositional effects of the individual. Conclusions: In the process of building community capacity, a community autonomously finds pending issues and solves related problems, and in so doing, raises the social quality and establishes the conditions for health promotion. Thus, the significance of neighborhood needs to be discovered and created in a new way through the development of community capacity.

Endocrine Disrupting Effects of Several Pharmaceuticals to Oryzias Latipes

  • Endocrine disrupting effects of four pharmaceutical products were evaluated with fish. The test pharmaceuticals, i.e., sulfamethoxazole, sulfamethazine, oxytetracycline and tetracycline have been often detected in aquatic environment, but their ecological hazard on receptors of various trophic levels has seldom been evaluated. In the present study, we conducted acute toxicity assays with a fish, Japanese medake (Oryziα lαtipes). The vitellogenin induced in female fish normally, but a endocrine disrupting chemical could give effects even male fish. We have tried 4 pharmaceutic chemicals to find out the endocrine disrupting effects. Sulfamethoxazole 1, 0.5 ppm induced vitellogenin even at male Japanese medaka. Sulfamethazine 10, 5, 1 ppm could induced vitellogenin at male fish. Oxytetracycline 10, 5, 1ppm could induced vitellogenin With the fish. Tetracycline 10, 5 ppm could induced vitellogenin at male fish. Some pharmaceuticals such as sulfamethoxazole, sulfamethazine, oxytetracycline and tetracycline could give effects to male Oryzias latipes. They could induced vitellogenin under exposure range 0.5 ${\sim}$ 10 ppm of chemicals at male Oryzias latipes.

약선음식의 선택속성이 만족도와 관계지속의도에 미치는 영향 (Effects of Selection Attributes on Satisfaction and Relationship Retention in Yaksun Cuisine)

  • With increasing interest in the health and safety of food served in restaurants, more emphasis is being placed on the importance of health and food. As a result, Yaksun cuisine with new added effects and image is becoming more popular as a way to improve people's health. This study analyzed the effects of selection attributes relating to Yaksun cuisine on customer satisfaction and intention to maintain relationship in order to understand various needs of consumers as they relate to health food. The results showed that, among selection attributes of Yaksun cuisine, price, safety, and health were found to have significant effects on customer satisfaction; however, the effect of quality on satisfaction was not verified. Moreover, while the effect of safety and health of Yaksun cuisine on the intention to maintain relationship was found to be significant, the effects of quality and price on the intention to maintain relationship were not verified. These findings suggest that consumers believe Yaksun cuisine is more beneficial for health than general food and trust the origin label, hygiene, and safety of ingredients in Yaksun cuisine. Therefore, the results of this study suggests providing opportunities for consumers to experience diverse tastes in Yaksun cuisine and that development and promotion of different recipes using Yaksun ingredients will help reinforce competitiveness of Yaksun cuisine in the market and increase sales.

Estrogenic Activity of Persistent Organic Pollutants and Parabens Based on the Stably Transfected Human Estrogen Receptor-α Transcriptional Activation Assay (OECD TG 455)

  • Screening of estrogenic activity on dichloro diphenyl trichloroethane (DDT), dichloro diphenyl dichloro ethylene (DDE), dieldrin, heptachlor, aldrin, chlordane, lindane, polybrominated diphenyl ethers (PBDE) and parabens was compared using Organization for Economic Cooperation and Development (OECD) test guideline 455 (TG455). The estrogenic activity of DDT was 58,000-fold ($PC_{50}$, $1.67{\times}10^{-6}$ M) less than $17{\beta}$-estradiol($E_2$) ($PC_{50}$, $2.88{\times}10^{-11}$ M) but DDE, dieldrin, heptachlor, aldrin, chlordane, lindane and PBDE did not show any estrogenic activity in this assay system. In the case of paraben compounds, the rank of relative transcriptional activation (logRTA) was butyl paraben -1.63752 ($PC_{50}$, $1.25{\times}10^{-7}$ M) > isobutyl paraben -2.34008 ($PC_{50}$, $6.3{\times}10^{-7}$ M) > ethyl paraben -2.64016 ($PC_{50}$, $1.26{\times}10^{-6}$ M) > isopropyl paraben -2.73993 ($PC_{50}$, $1.58{\times}10^{-6}$ M) > propyl paraben -2.84164 ($PC_{50}$, $2.0{\times}10^{-6}$ M). Our data suggest that OECD test guideline TG455 may be useful as a screening tool for potential endocrine disruptors.

Research Trends in Agenda-setting for Climate Change Adaptation Policy in the Public Health Sector in Korea

  • Many studies have been conducted to assess the health effects of climate change in Korea. However, there has been a lack of consideration regarding how the results of these studies can be applied to relevant policies. The current study aims to examine research trends at the agenda-setting stage and to review future ways in which health-related adaptation to climate change can be addressed within national public health policy. A systematic review of previous studies of the health effects of climate change in Korea was conducted. Many studies have evaluated the effect of ambient temperature on health. A large number of studies have examined the effects on deaths and cardio-cerebrovascular diseases, but a limitation of these studies is that it is difficult to apply their findings to climate change adaptation policy in the health sector. Many infectious disease studies were also identified, but these mainly focused on malaria. Regarding climate change-related factors other than ambient temperature, studies of the health effects of these factors (with the exception of air pollution) are limited. In Korea, it can be concluded that studies conducted as part of the agenda-setting stage are insufficient, both because studies on the health effects of climate change have not ventured beyond defining the problem and because health adaptation to climate change has not been set as an important agenda item. In the future, the sharing and development of relevant databases is necessary. In addition, the priority of agenda items should be determined as part of a government initiative.

손자녀 양육에 참여하는 조모의 건강증진행위가 속박감에 미치는 영향 (Effects of Health-Promoting Behaviors of Grandmothers Participating in Their Grandchildren's Nurture on Entrapment)

  • This study examines the effects of health-promoting behaviors of grandmothers participating in their infant grandchildren's nurture and provides data for enhancing the quality of their lives. The sample included 218 consenting subjects who understood the purpose of the study. These subjects resided in three cities in Jeollanam-do, Korea, and were grandmothers entrusted by their adult children with a considerable amount of nurturing for their preschool-aged grandchildren under the age of six. First, the subjects scored an average of 3.05 for health-promoting behaviors and 2.42 for entrapment. Second, the entrapment measurement based on general characteristics showed a higher level of entrapment for those with better health, a lower education level, no religion, male grandchildren to nurture, and unsatisfactory compensation. Third, health-promoting behaviors and entrapment (r=-.304) were negatively correlated. Fourth, the analysis results for effects of health-promoting behaviors of grandmothers on entrapment based on general characteristics of grandmothers as a control variable reveal a significant effect of health-promoting behaviors on entrapment. In other words, the greater the employment of health-promoting behaviors, the less likely the entrapment was to be perceived. Fifth, the results for effects of subcriteria for health-promoting behaviors on entrapment reveal a decrease in entrapment when the subjects engaged in an appropriate level of physical activity with a relaxed state of mind. These results suggest that health-promoting behaviors of grandmothers participating in their infant grandchildren's nurture are likely to influence entrapment and thus that grandmothers should make efforts to actively engage in health-promoting behaviors to reduce any maladaptive effects on nurturing. Social support and related programs should be fostered to enable grandmothers to better practice health-promoting behaviors while nurturing their grandchildren.

곰팡이 노출에 따른 건강영향 및 민감 시설 내 곰팡이 분포 현황 (Health Effects of Exposure to Indoor Mold and the Levels of Mold in Facilities with Susceptible Populations in Korea)

  • Objectives: Exposure to mold is strongly associated with adverse health effects (development or exacerbation of allergic diseases). We reviewed the health effects of mold exposure and explored to determine the annual distribution of indoor mold in facilities with susceptible populations. Methods: The health effects of mold exposure were mainly summarized by reviewing related papers and WHO research reports. We selected 10 facilities, including daycare centers, postpartum care centers, medical institutions, and elderly care facilities within the Seoul Metropolitan. Mold sampling was performed once every week or once every quarter from February 2016 to 2017. In addition, fungal species analyses was performed, and distribution status by month and facility was analyzed in the same manner as concentration. Results: Adverse health effects attributed to fungal exposure are largely divided into allergic symptoms, toxic effects, and infectious effects. Monthly mean concentrations of mold indoors and outdoors was 368.8 CFU/㎥ (geometric mean 213.4 CFU/㎥) and 496.0 CFU/㎥ (327.9 CFU/㎥), respectively. The indoor concentration has begun to increase in February, peaked in July, declined in August, increased again until October, and then decreased in November. About 36 genera of indoor fungal species were found in each facility. Cladosporium sp., Penicillium sp., Fusarium sp., Aspergillus sp., Alternaria sp., and Arthrinium sp. were observed as the dominant species. Conclusions: Our findings showed that the overall level of indoor mold was below the 500 CFU/㎥ level recommended by the Ministry of Environment. The development of DNA-based assessment and expanding facilities to be monitored for mold would be necessary for preventive aspects.
