Degree of Parenting Stress and Fatigue Perceived by Primary Caregivers of Young Children (영유아의 주 양육자가 지각하는 양육 스트레스와 피로)

  • Purpose: The purpose of this study was to provide basic data for nursing interventions to assist primary caregivers of young children by determining the degree stress and fatigue that caregivers were under due to parenting young children. Methods: Participants were primary caregivers (181 mothers and 160 grandmothers) of young children. Results: 1) Mean score for degree of parenting stress was 2.61 (${\pm}.69$) and for fatigue, 1.97 (${\pm}.54$). 2) There was a significant difference in degree of parenting stress according to primary caregivers' age, employment status, income, whether the children were wanted babies, health status of the children and caregivers, personality of the children, and benefits from, and satisfaction with costs of childcare. Likewise, there was a significant difference in degree of fatigue according to relationship of primary caregivers to the children, primary caregivers' age, religion, income, whether the children were wanted babies, health status of the children and caregivers, personality of the children, and benefits from, and satisfaction with costs of childcare. 3) Stress and fatigue perceived by the primary caregivers were significantly correlated (r=.554 p<.001). Conclusion: To alleviate more effectively primary caregivers' parenting stress and fatigue, early nursing intervention and educational programs need to be developed.

Economic Evaluation of Hospital-based Home Care Services for the Breast Cancer Surgery Patients (유방암 수술 환자에 대한 가정간호서비스의 경제성 평가)

  • Purpose: This study conducted an economic evaluation of hospital-based home care services for the patients who had undergone breast cancer surgery. Methods: A total of 12,483 patients over 18 years of age who had received breast cancer surgery in 26 tertiary hospitals in 2018 were analyzed with the claim data from the Health Insurance Review & Assessment Service using cost-minimization analysis and societal perspectives. Results: There were 156 patients who utilized hospital-based home care services within 30 days after breast cancer surgery, and they received 2.17 (SD=1.17) hospital-based home care service on average. The average total cost was 5,250,028 KRW (SD=1,905,428) for the group receiving continuous hospital-based home care and 6,113,402 KRW (SD=2,033,739) for the group not receiving continuous hospital-based home care (p<.001). The results of the economic evaluation of continuous hospital-based home care services in patients who had undergone breast cancer surgery indicated a total benefit of 953,691,000 KRW, a total cost of 819,004,000 KRW, and a benefit-cost ratio of 1.16 in 2018. Conclusion: Continuous hospital-based home care was considered economically feasible as the total costs for the group receiving continuous hospital-based home care were lower than those of the group not receiving continuous hospital-based home care. Therefore, policy modification and financial incentives are recommended to increase the utilization of hospital-based home care services for patients who had undergone breast cancer surgery.

An Exploratory Health Outcome Analysis of Lumbar Surgery Patients Utilizing Korean Medical Services: Using Health Insurance Review and Assessment Service-National Patients Sample (HIRA-NPS 2019) Data (건강보험심사평가원 환자표본 데이터 분석을 통한 한의 의료 이용 요추 수술 환자의 탐색적 성과 분석)

  • Objectives This study aimed to analyze the medical utilization of low back pain (LBP) patients after back surgery and estimate the medical costs of Korean and Western medicine collaborative treatment, odds ratio, and hazard ratio between the two groups using the 2019 Health Insurance Review and Assessment Service-National Patient Sample (HIRA-NPS-2019). Methods Data management and descriptive analysis, logistic regression, and survival analysis were conducted for defining and estimating the LBP patients after back surgery in the NPS 2019 dataset. Results A total of 216,424 patients out of 991,189 were identified as having LBP. Among the patients with LBP, 1,734 were treated with surgery while 214,690 were not. Among those who had surgery, 937 were treated with conventional treatments only and 797 underwent Korean medicine treatments. The odds ratio of the logistic regression analysis was 0.7129, suggesting that Korean medical treatment experience group had a 28.7% lower risk of reoperation than the Western medical treatments only group. The hazard ratio of the survival analysis was 0.9145; thus, the risk probability of reoperation was estimated to be approximately 8.55% lower. The 50% risk of reoperation was 69 days (0.5044) for the conventional group, and 97 days (0.5008) for the Korean medical group in the survival analysis using the Kaplan-Meier graph. Conclusions These results could be utilized in future studies in conducting economic evaluation for estimating cost-effectiveness of Western medicine and Korean medicine treatment compared to Western medicine alone in LBP patients after back surgery in a South Korean perspective. mended and should be applied while taking the necessary precautions.

  • Background/Aims: The pancreatic pseudocyst (PP) is a type of fluid collection that typically develops as a delayed complication of acute pancreatitis. Drainage is indicated for symptomatic patients and/or associated complications, such as infection and bleeding. Drainage modalities include percutaneous, endoscopic, laparoscopic, and open drainage. This study aimed to assess trends in the utilization of different drainage modalities for treating PP from 2016 to 2020. The trends in mortality, mean length of hospital stay, and mean hospitalization costs were also assessed. Methods: The National Inpatient Sample database was used to obtain data. The variables were generated using International Classification of Diseases-10 diagnostic and procedural codes. Results: Endoscopic drainage was the most commonly used drainage modality in 2018-2020, with an increasing trend over time (385 procedures in 2018 to 515 in 2020; p=0.003). This is associated with a decrease in the use of other drainage modalities. A decrease in the hospitalization cost for PP requiring drainage was also noted (29,318 United States dollar [USD] in 2016 to 18,087 USD in 2020, p<0.001). Conclusions: Endoscopic drainage is becoming the most commonly used modality for the treatment of PP in hospitals located in the US. This new trend is associated with decreasing hospitalization costs.

Is Health Locus of Control a Modifying Factor in the Health Belief Model for Prediction of Breast Self-Examination?

  • Background: Breast cancer is one of the most common cancers among women in the world. Early detection is necessary to improve outcomes and decrease related costs. The aim of this study was to assess the predictive power of health locus of control as a modifying factor in the Health Belief Model (HBM) for prediction of breast self-examination. Materials and Methods: In this cross- sectional study, 400 women selected through the convenience sampling from health centers. Data were collected using part of the Champion's HBM scale (CHBMS), the Health Locus of Control Scale and a self administered questionnaire. For data analysis by SPSS the independent T test, Chi square test, logistic and linear regression modes were appliedl. Results: The results showed that 10.9% of the participants reported performing BSE regularly. Health locus of control did not act as a predictor of BSE as a modifying factor. In this study, perceived self-efficacy was the strongest predictor of BSE performance (Exp (B) =1.863) with direct effect, while awareness had direct and indirect influence. Conclusions: For increasing BSE, improvement of self-efficacy especially in young women and increasing knowledge about cancer is necessary.

Occupational Injuries Among Construction Workers by Age and Related Economic Loss: Findings From Ohio Workers' Compensation, USA: 2007-2017

  • Background: This study examined age-group differences in the rate, severity, and cost of injuries among construction workers to support evidence-based worker safety and health interventions in the construction industry. Methods: Ohio workers' compensation claims for construction workers were used to estimate claim rates and costs by age group. We analyzed claims data auto-coded into five event/exposure categories: transportation incidents; slips, trips, and falls (STFs); exposure to harmful substances and environments; contact with objects and equipment (COB); overexertion and bodily reaction. American Community Survey data were used to determine the percentage of workers in each age group. Results: From 2007-2017, among 72,416 accepted injury claims for ~166,000 construction full-time equivalent (FTE) per year, nearly half were caused by COB, followed by STFs (20%) and overexertion (20%). Claim rates related to COB and exposure to harmful substances and environments were highest among those 18-24 years old, with claim rates of 313.5 and 25.9 per 10,000 FTE, respectively. STFs increased with age, with the highest claim rates for those 55-64 years old (94.2 claims per 10,000 FTE). Overexertion claim rates increased and then declined with age, with the highest claim rate for those 35-44 years old (87.3 per 10,000 FTE). While younger workers had higher injury rates, older workers had higher proportions of lost-time claims and higher costs per claim. The total cost per FTE was highest for those 45-54 years old ($1,122 per FTE). Conclusion: The variation in rates of injury types by age suggests that age-specific prevention strategies may be useful.

Test on the Cost and Development on the Payment System of Home Health Care Nursing (가정간호수가 적정성 검증 및 수가체계 개선 방안)

  • Purpose: This study focused on analysing costs per home health care nursing visit based on home health care nursing activities in medical institutes. Method: The data was collected in three stages. First, the cost elements of home health care nursing services were collected and 31 home care nurses participated. Second, the workload and caseload of home care nursing activities were measured by the Easley-Storfjell Instrument(1997). Third, the opinions on improving the home health care nursing reimbursement system were collected by a nation-wide mailing survey from a total of 125 home care agencies. Result: The cost of home health care nursing per visit was calculated as 50,626\. This was composed of a basic visiting fee of $35,090{\\}({\fallingdotseq}355$)$ and travel fee of $15,536{\\}({\fallingdotseq}15$)$. The major problems of the home care nursing payment system were the low level of the cost per visit, no distinction between first visit and revisits, and the limitations in health insurance coverage for home health care nursing services. Conclusion: This study's results will contribute as a baseline for establishing policies for improvement of the home health care nursing cost and for applying a community-based visiting nursing service cost.

  • Moon Jae-in Government announced the Government's 5-Year Plan on July 19, 2017, President Moon directly announced the Government's Plan for Benefit Expansion in National Health Insurance on August 7, 2017. The main contents of the announced expansion include benefit coverage for all medically necessary services with control over non-covered service occurrence, a decrease in the cost-sharing upper limit, and monetary support for catastrophic medical costs. Although past governments have been continuously striving for benefit expansion in the last 15 years, this plan has its breakthrough aspect in that all medical services will be covered by the National Health Insurance. In alignment, there are important tasks to solve: attaining a proper fee schedule, reforming the healthcare delivery system, and improving healthcare quality. This plan is a symptom oriented action in that it is limited in reducing patients' out-of-pocket money, unlike the systematic approach of the National Health Insurance. The sustainability of the National Health Insurance is being threatened due to South Korea's low birth rate, rapidly aging society, and low economic growth, in addition to the unification issue of the Korean Peninsula, medical utilization of the elderly, management of non-communicable diseases, and so on. Therefore, the Government needs to plan the National Health Insurance system reformation including actions addressed toward medical consumers.

The Effects of Engel Coefficient, Angel Coefficient and Schwabe Index Influencing Household Head's Life Satisfaction : according to Income Quintile (가계의 엥겔계수, 엔젤계수 및 슈바베계수가 생활만족도에 미치는 영향 : 소득계층을 중심으로)

  • The purpose of this study was to investigate the effects of Engel coefficient, Angel coefficient and Schwabe index influencing Household head's life satisfaction. For this study, the data from the 8th analysis of the 2013 Korea Welfare Panel Survey conducted by Korea Institute for Health and Social Affairs were used. For the sample, 903 male Household heads with children under the age of 18, were selected. For statistical analysis, SPSS program (Ver. 21.0) was used. And for statistical methods, frequency and percentile, mean and standard deviation, Pearson's correlation, one way analysis of variance, Duncan's multiple range tests, multiple regression analysis were used. The findings are as follows. First, as a results of analyzing the food costs, education costs and housing costs depending on Income Quintile, the food costs and education costs in the 5th Income Quintile compared with other Income Quintile, were highest. Also, the highest housing cost was in the 2nd Income Quintile, while the least housing cost was in the 1st Income Quintile. Second, by analyzing the differences of Engel coefficient, Angel coefficient and Schwabe index according to Income Quintile, the results show that Engel coefficient and Schwabe index decreases as Income Quintile increases, and Angel coefficient increases as Income Quintile becomes higher. Third, the level of HH's life satisfaction according to Income Quintile, 1st Income Quintile, 2nd Income Quintile, 4th Income Quintile, 3rd Income Quintile, 5th Income Quintile in order, increased. Fourth, as the result of analyzing the influence of Variables related to household and demographics about Engel coefficient, Angel coefficient and Schwabe index, it was shown that the variables effecting Engel coefficient, Angel coefficient, and Schwabe index are age, occupations, Number of workers, House ownership, Income Quintile. Fifth, As a result of analyzing the Variables effecting life satisfaction, especially while Schwabe index is not that significant, Engel coefficient and Angel coefficient are shown to have a significant influence. Therefore, the influence of Food costs and education costs can be confirmed.

  • For smart structure technologies, the interests in wireless sensor networks for structural health monitoring are growing. The wireless sensor networks reduce the installation of the wire embedded in the whole structure and save the costs. But the wireless sensor networks have lots of limits and there are lots of researches and developments of wireless sensor and the network for data process. Most of the researches of wireless sensor network is applying to the civil engineering structure and the researches for the highrise building are required. And strain-based SHM gives the local damage information of the structures which acceleration-based SHM can not. In this paper, concept of wireless sensor network for structural health monitoring of highrise building is suggested. And verifying the feasibility of the strain-based SHM a strain sensor board has developed and tested by experiments.
