Effects of Household Type on Blood Pressure, Body Mass Index, Mini Nutritional Assessment Score, and Biochemical Indicators in Elderly Individuals Living Alone and with Families (독거노인과 가족동거노인의 가구형태가 혈압, 체질량지수, 간이영양평가점수 및 생화학적 지표에 미치는 영향)

  • Purpose: The purpose of this study was to identify the nutritional status of elderly individuals according to their household types and to investigate the predictors of their nutritional status. Methods: This study, which was a descriptive research study, involved physical measurements, surveys, and biochemical tests in 87 elderly individuals living in the community. Using SPSS/Win 24.0, logistic regression analysis was performed to identify the general characteristics, blood pressure, body mass index (BMI), mini nutritional assessment (MNA) score, and biochemical indicators in elderly individuals according to the household type. Results: Elderly individuals living with family members had higher MNA scores than those of elderly individuals living alone, while showing lower levels of systolic blood pressure and fasting blood sugar. The results of the logistic regression analysis showed that the risk factors were MNA scores (odds ratio (OR)=1.81, 95% confidence interval (CI)=1.36-2.42), systolic blood pressure (OR=0.96, 95% CI=0.92-1.00), and fasting blood glucose (OR=0.94, 95% CI=0.90-0.99). Conclusion: It was confirmed that elderly individuals living alone need differentiated nutrition intervention, since the results showed that they had lower nutritional levels and improper nutritional management than that in elderly individuals living with family members.

Comparison of Health Problems, Conditions, & Health Promoting Behavior and Risky Environment among Various Industrial Workers (일부 사업장 근로자의 업종별 건강문제, 건강상태, 건강증진행위 및 유해환경정도 비교)

  • Objective: This study was conducted to compare health conditions and problems, health promoting behavior, and risky environment of industrial workers. Methods: The subjects of this study were 1,075 workers in 40 factories, located in Daegu, Korea. Survey data were collected from April 17th, 2006 to April 28th, 2006. CMI (Cornell Medical Index) Health Checklist was used to assess the health problems, Health Rating Scale for health status, HPLP II for health promoting behavior, and environmental risk assessment tool for risky environment. The data were analyzed by One-way ANOVA and Pearson Correlation Coefficient using SPSS 12.0 for windows. Results: The results indicated a significant difference in health problems (F=4.70, p=.000), health conditions (F=2.47, p=.022), health promoting behavior (F=5.67, p=.000), and risky environment (F=14.75, p=.000) among these industrial workers. Conclusion: The results of this study suggest that there is a need to develop customized health care programs for each company to provide differentiated health care to different types of work places. Further studies are required to assess differences in work environment among different types of work places for measures to reduce harmful factors to the environment.

Human Risk Assessment for Exposure to Heavy Metals within Finishing Materials of Playground Facilities for Children in Gwangju (광주지역 어린이 놀이시설 마감재의 중금속 노출에 의한 인체 위해성평가)

  • Background: Children who use playground facilities are exposed to potential risks due to the high concentration of heavy metals contained in the finishing materials of facilities in children's playgrounds. Objectives: The purpose of this study was to investigate the concentration of heavy metals in the finishing materials of outdoor children's playgrounds where harmful heavy metals exist in Gwangju and to conduct human risk assessment for children and adults by age to find the risks and limitations. Methods: The bottom and top layers of double-painted paint were peeled off and collected together from the finishing materials of children's play facilities such as slides, swings, and seesaws in 147 children's parks in Gwangju. Heavy metals were analyzed using ICP-OES, etc., and human risk assessment was performed using the concentrations of heavy metals. Results: Based on 1.0E-04, which requires legal regulation, CTE was found to pose a carcinogenic risk for preschool children and no carcinogenic risk for the rest of the age groups. However, RME showed that both men and women of all ages had a carcinogenic risk. For reference, when the carcinogenic risk was based on 1.0E-06, CTE was found to pose a carcinogenic risk from infants to elementary school students, and RME was found to have a carcinogenic risk in all age groups. It was judged that there is a non-carcinogenic risk if the non-carcinogenic risk exceeds 1 based on the hazard index (HI) 1. In CTE, there was no non-carcinogenic risk, and RME for preschooler males (1.49E+00) and females (1.56E+00) were found to have non-carcinogenic risk. Conclusions: This study was meaningful in that it examines the differences in the current management of heavy metals concentration standards and potential carcinogenic and non-carcinogenic risks to the human body and discusses the relationship between heavy metals and human health effects.

Cable anomaly detection driven by spatiotemporal correlation dissimilarity measurements of bridge grouped cable forces

  • Stayed cables are the key components for transmitting loads in cable-stayed bridges. Therefore, it is very important to evaluate the cable force condition to ensure bridge safety. An online condition assessment and anomaly localization method is proposed for cables based on the spatiotemporal correlation of grouped cable forces. First, an anomaly sensitive feature index is obtained based on the distribution characteristics of grouped cable forces. Second, an adaptive anomaly detection method based on the k-nearest neighbor rule is used to perform dissimilarity measurements on the extracted feature index, and such a method can effectively remove the interference of environment factors and vehicle loads on online condition assessment of the grouped cable forces. Furthermore, an online anomaly isolation and localization method for stay cables is established, and the complete decomposition contributions method is used to decompose the feature matrix of the grouped cable forces and build an anomaly isolation index. Finally, case studies were carried out to validate the proposed method using an in-service cable-stayed bridge equipped with a structural health monitoring system. The results show that the proposed approach is sensitive to the abnormal distribution of grouped cable forces and is robust to the influence of interference factors. In addition, the proposed approach can also localize the cables with abnormal cable forces online, which can be successfully applied to the field monitoring of cables for cable-stayed bridges.

Educational Needs Assessment in Pediatric Nursing Handoff for Nursing Students (간호대학생을 위한 아동간호 인수인계 교육요구도 조사)

  • Purpose: This descriptive research was conducted to identify educational needs in pediatric nursing handoff training to improve students' handoff skills. Methods: Data were collected using a survey with 188 senior nursing students and 48 pediatric nursing professors and clinical nurses. The survey included items on general information as well as experiences in handoff training, necessity, training content, and items for a handoff training program in pediatric nursing. Results: Of the nursing students, 30.5% reported receiving handoff training during their clinical hours. After their handoff training, the students' confidence index was only 3.78 out of 10. Significantly, 98.3% of the respondents said that pediatric handoff training is necessary. In addition, participants reported that simulation practice (26.5%) is an appropriate educational method, and the time required for handoff training should be 8.16 hrs. Admission process was placed first as the most critical circumstance for handoff (56.8%). High demands were observed for the necessity of training content for patients with respiratory problems. Conclusion: The results of this study show the various educational needs for developing a patient safety pediatric handoff training program to promote nursing students' skills in handoff.

  • Structural health monitoring is important to maintain the safety and serviceability of the structures. The displacement in the structure should be precisely and frequently monitored because it is a direct assessment index indicating its stiffness. However, no practical method has been developed to monitor such displacement precisely, particularly for high-rise buildings and long span bridges because they cannot be easily accessible. To overcome such difficult accessibility, we propose to use a LIDAR system that remotely samples the surface of an object using laser pulses and generates the coordinates of numerous points on the surface. In this study, using terrestrial LiDAR, we develop a novel displacement measuring model for structural health monitoring and perform an indoor experiment to prove its performance.

Diagnostic Accuracy of Lactate Dehydrogenase/Adenosine Deaminase Ratio in Differentiating Tuberculous and Parapneumonic Effusions: A Systematic Review

  • Background: Tuberculous pleural effusion (TPE) and parapneumonic effusion (PPE) are often difficult to differentiate owing to the overlapping clinical features. Observational studies demonstrate that the ratio of lactate dehydrogenase to adenosine deaminase (LDH/ADA) is lower in TPE compared to PPE, but integrated analysis is warranted. Methods: We conducted a systematic review to evaluate the diagnostic accuracy of the LDH/ADA ratio in differentiating TPE and PPE. We explored the PubMed and Scopus databases for studies evaluating the LDH/ADA ratio in differentiating TPE and PPE. Results: From a yield of 110 studies, five were included for systematic review. The cutoff value for the LDH/ADA ratio in TPE ranged from <14.2 to <25. The studies demonstrated high heterogeneity, precluding meta-analysis. Quality Assessment of Diagnostic Accuracy Studies Tool 2 assessment revealed a high risk of bias in terms of patient selection and index test. Conclusion: LDH/ADA ratio is a potentially useful parameter to differentiate between TPE and PPE. Based on the limited data, we recommend an LDH/ADA ratio cutoff value of <15 in differentiating TPE and PPE. However, more rigorous studies are needed to further validate this recommendation.

Ecological Impact Analysis of a Stream on the Dam Construction Using Species Biotic Index (SBI) as a Tool of Ecosystem Health Assessment

  • Species biotic index (SBI), based on a fish assemblage, was applied to a stream assessment using long-term ecological fish data set (1996 to 2001) in Boryong dam area, which is located in the mainstream of Ungchun Stream of Chungnam province, Korea. According to the methods of Hilsenhoff (1988), the scores of tolerance guild assigned 10 classes to each species by its habitat and feeding guild, but modified current 7 criteria to 5 scoring standards due to unclear borderline among species. Relative abundance in the species number of upper stream guilds was only 7% of the total, whereas the abundance in the species number of middle to downstream upper stream guilds was 64%. Mean SBI, based on dataset in Site 1 during 1995-2001 averaged 5.10, which was judged as a "good" condition by the rank criteria of SBI. Before the dam construction, mean SBI in the Site 1 was 4.61, indicating a "good" condition, but after the dam construction, mean SBI was 5.60, indicating a "fair" condition. Trajectory analysis in the Site 1 showed significantly (One-way ANOVA, $F_{6,21}=3.26$, p=0.02) different among years, reflecting the changes of fish composition and population density by the dam construction, whereas Site 2 showed no significant changes ($F_{6,21}=1.00$, P =0.45) difference among years. Mean SBI prior to the dam construction in the Site 3 was 4.52 but after the construction, the value was 6.30, indicating a distinct difference between the pre- and post-dam construction. Trajectory analysis at the Site 3 supported this fact: Values of SBI showed significantly ($F_{6,21}$=14.37, p<0.01) different. Mean SBI was 4.67 in the Site 4, indicating a "good" condition in the health and the health rank was same as the sampling sites 1, 2, and 4. Trajectory in the Site 4 showed no significant ($F_{6,21}=2.35$, p=0.07) difference among the years. Overall, our trajectory analysis indicated that three of four sampling sites (sites 1, 3, 4) showed significant decreases (n=7, p<0.05) and that the proportions of sensitive species declined evidently in the sites 1 and 2 and the tolerant species increased in the dam sites. Our outcomes may be used as a key data for diagnosis of the long-term ecological impact in the future in the watershed.

Community Structure and Health Assessment of Macrobenthic Assemblages during Spring and Summer in the Shellfish Farming Ground of Wonmun Bay, on the Southern Coast of Korea (원문만 패류양식어장에 서식하는 대형저서동물의 춘계와 하계의 군집구조 및 건강도 평가)

  • This study investigated the community structure and health assessment of macrobenthic assemblages in Wonmun Bay, in the southern coast of Korea. The total number of species and mean density were 170 species and $1,269\;indivuduals/m^2$, respectively. The major dominant species were the bivalve Theora fragilis ($230{\pm}446\;indivuduals/m^2$) and the polychaetes Lumbrineris longifolia ($180{\pm}339\;indivuduals/m^2$), Sigambra tentaculata ($124{\pm}153\;indivuduals/m^2$), Paraprionospio patiens ($104{\pm}293\;indivuduals/m^2$) and Capitella capitata ($82{\pm}223\;indivuduals/m^2$). Community statistics [cluster analysis and nonmetric multidimentional scaling (NMDS) ordination] revealed that the macrobenthic community structure in the study area was classified into three station groups for both seasons. Several biotic indices based on macrobenthic communities have been developed to assess the ecological quality of coastal ecosystems. In this study, the ecological status was assessed by four biotic indices Shannon's H', the AZTI Marine Biotic Index (AMBI), multivariate-AMBI (M-AMBI) and the Ecological Conservation Index (ECI). The ecological statuses of the macrobenthic communities in Wonmun Bay were poorer in the summer than in the spring. The results of the present study indicate that three biotic indices ( i.e.; Shannon's H', M-AMBI and ECI ) were better correlated with bottom dissolved oxygen than the AMBI in Wonmun Bay.

Dementia Incidence Rate Before and After Implementing the National Responsibility Policy for Dementia Care in Patients With Vascular Risk Factors in Korea

  • Background and Purpose: The National Responsibility Policy for Dementia Care was implemented in September 2017 in Korea. This study aimed to compare dementia incidence in Seoul and Gangwon-do before and after the implementation of this policy. Methods: We extracted insurance claim data from the Korean Health Insurance Review and Assessment Service for people diagnosed with diabetes, hypertension, or dyslipidemia for the first time in Seoul and Gangwon-do, Korea. We defined two enrollment groups based on the policy implementation date: 1) January 1, 2015 to December 31, 2016 (Index 1, pre-implementation), and 2) January 1, 2017 to December 31, 2018 (Index 2, post-implementation). Each group was followed up for 1 year from the time of enrollment. Then, we calculated hazard ratios to compare the incidence of dementia between the two groups, and between Seoul and Gangwon-do. Results: In Seoul, the incidence of dementia was significantly lower in Index 2 than in Index 1 (hazard ratio [HR], 0.926; 95% confidence interval [CI], 0.875-0.979). However, the incidence rate did not differ between the 2 groups (HR, 1.113; 95% CI, 0.966-1.281) in Gangwon-do. In Index 1, the incidence of dementia did not differ between Seoul and Gangwon-do (HR, 1.043; 95% CI, 0.941-1.156), but in Index 2, was significantly higher in Gangwon-do than in Seoul (HR, 1.240; 95% CI, 1.109-1.386). Conclusions: After implementing the National Responsibility Policy for Dementia Care, the dementia incidence rate decreased significantly in Seoul, consistent with other studies, but not in Gangwon-do.
