Purpose: While all midshaft clavicle fractures have traditionally been treated with conservative measures, recent operative treatment of displaced, communited midshaft clavicle fractures has become more common. Though a recent increase in operative treatment for midshaft clavicle fractures, we have done the operative methods in limited cases. The aim of this study is to present indications, operative techniques and outcomes of the experienced cases that have applied to this limited group over the previous 10 years. Methods: This study consists of a retrospective review of radiological and clinical data from January of 2005 to July of 2015. Operative criteria for midshaft clavicle fractures having considerable risk of bone healing process were 4 groups - a floating shoulder, an open fracture, an associated neurovascular injury, and a nonunion case after previous treatment. Results: The study consisted of 18 patients who had operative treatment for midshaft clavicle fractures in adults. The most common surgical indication was a floating shoulder (10 cases, 55.6%), followed by nonunion (5 cases, 27.8%), an associated neurovascular injury (4 cases, 22.2%), and open fracture (3 cases, 16.7%). All cases were treated by open reduction and internal fixation in anterosuperior position with reconstruction plate or locking compression plate. Bone union was achieved in all cases except 1 case which was done bone resection due to infected nonunion. Mean bone union period was 19.5 weeks. There were no postoperative complications, but still sequelae in 4 cases of brachial plexus injury. Conclusion: We have conducted an open reduction and internal fixation by anterosuperior position for midshaft clavicle fractures in very limited surgical indications for last 10 years. Our treatment strategy for midshaft clavicle fractures showed favorable radiological results and low postoperative complications.
Chronic wounds, pressure sores, lesions, and infections of microbial origin in bedridden, paralyzed, or malnutrition patients remain the object of study of many researchers. A variety of factors behind the development of these disorders are related to the patient's immune system, making it unable to respond effectively to the treatment of the wound. These factors can be properly controlled, giving particular importance to the ethiology and stage of the wound, as well as the time periods corresponding to the replacement of the dressings. The present research reports a novel foam/soft material, ${{\small}L}$-Cys-g-PCL, with an application for decubitus/pressure ulcers, especially for wounds with a difficult healing process due to infections and constant oxidation of the soft tissues. During this work, the interactions between S. aureus and ${{\small}L}$-Cys-g-PCL foam were studied under conditions that simulate decubitus ulcers; namely, pH and exudate. The effects of duration of grafting (1 or 8 h) and pH (7.0 and 8.9) on wettability, surface energy, swelling, and porosity were also evaluated. Results showed an effective microbicidal activity exhibiting an inhibition ratio of 99.73% against S. aureus. This new ${{\small}L}$-Cys-g-PCL soft material showed saftey to contact skin, ability to be shaped to fill in sunken holes (craters) - pressure ulcers stage III - and to act as a smart material responsive to pH, which can be tailored to develop better swelling properties at alkaline pH where exudates are normally higher, so as to address exudate self-cleaning and prevention of desiccation.

Sometimes, literary therapy is to create an abscission layer at the mind and remove the signifier of love, by the abscission layer. The purpose of this study is to reveal the mechanism of literary therapy by mutation in human-created sentences and to activate literary therapy. In Gosijo, the abscission layer of Seo Gyeongdok's mind is present in the layer of the "falling leaf" of the his Sijo's last sentence, and the abscission layer of Hwang Jin-yi's mind exists in the layer of the "falling leaf" of the her Sijo's last sentence. The "falling leaf" created by the abscission layer of Seo Gyeongdok's sentence acts as an action potential to the nervous system of Hwang Jin-yi. In Hwang Jin-yi's Sijo, the abscission layer is also activated, forming a "leaf". Hwang Jin-yi's Sijo constructs a new sensory layer that is extended and mutated more than the meaning of space between lines of Seo Gyeongdok's Sijo. In her Sijo, Hwang Jin-yi twists and twists Seo Gyeongdok's sentences to squeeze the sound. She accepts the Seo Gyeongdok's sentences and twists in the first sentence and twists out of the second sentence once more to produce the mutation in the last sentence. As a result of her sentence twist, Seo Kyung-duk's visual sense of "falling leaf" is mutated into a "falling leaf's sound" of auditory sense. This mutation process works as a healing mechanism in the human body. If this study will continue in the future, it will be possible to activate literary therapy by Sijo.

Purpose: Resorption of the alveolar bone is an unavoidable consequence of tooth extraction when appropriate alveolar ridge preservation (ARP) measures are not taken. The objective of this trial was to test the hypothesis that dimensional changes in the alveolar bone after tooth extraction would be reduced by inserting an equine collagen membrane and a collagen cone to fill and seal the alveolus (as ARP), in comparison to extraction with untreated alveoli. Methods: In this randomized clinical trial, 31 patients were directly treated with the collagen material after extraction of a tooth from the maxilla (the ARP group). Twenty-nine patients served as the control group. After extraction, no further treatment (i.e., no socket preservation measures) was performed in the control group. Changes in the alveolar process immediately after extraction and after an 8 (${\pm}1$)-week healing period were evaluated 3-dimensionally. Blinded analyses were performed after superimposing the data from the digitalized impressions and surfaces generated by cone-beam computed tomography. Results: Both the ARP and control groups showed a reduction of bone in the alveolar area after tooth extraction. However, significantly less bone resorption was detected in the clinically relevant buccal region in the ARP group. The median bone reduction was 1.18 mm in the ARP group and 5.06 mm in the control group (P=0.03). Conclusions: The proposed hypothesis that inserting a combination material comprising a collagen cone and membrane would lead to a difference in alveolar bone preservation can be accepted for the clinically relevant buccal distance. The progress of periodontal disease and the wound healing process after treatment result in alveolar bone bone change. So, detection of it is very important in the diagnosis and the radiograph of periodontal disease. Various effects have been made to assess the subtle alveolar bone change and digital subtraction radiography (DSR) has been reported to be the best method in evaluating it qualitatively and quantitatively. The present study was performed to estimate the detectable alveolar bone change qualitatively with digital subtraction radiography. For the in vitro study, 10 intraoral standard radiographs were taken from porcine dry mandible which a rectangular cortical bone chip of 0.1mm to 1.0mm thickness with 0.1mm increment was attached on the buccal surface. The radiographs without and with bone plates were reviewed at the same time by 10 observers and requested to detect the presence of cortical bone plates. Digital Subtraction radiograph was reviewed subsequently by using the DSR system(digital converter-256 grey-levels,DT 2851,Data Translation Co., U.S.A;IBM 386 ; CCD camera, FOTOVIX, Tamrom Co., Japan). The detectable thickness of cortical bone plate was O.4mm on the intraoral radiograph and 0.2mm on the subtaction images. For the human study, radiographs were taken from patients by using intraoral film holding device and aluminum reference wedge before and 3 month after bone graft and 1 week after osteoplasty. The grey level change was estimated in the subtraction images and calculated to aluminum equivalent thickness. The grey level of the grafted site was higher that that of healthy controls. Average grey levels of change on healthy controls were O.48mm aluminum equivalent. However, the amount of changes in grafted sites were 1.87mm aluminum thickness equivalent and in the site of osteoplasty were -1.49mm aluminum thickness equivalent. In conclusion, digital subtraction radiography was more effective in detecting as subtle change of alveolar bone than intraoral standard radiography. With the aid of quantitative analysis of digital subtraction radiography, alveolar bone resorption of apposition can be estimated during diagnosis and treatment of periodontally diseased patients.
The purpose of this study was to observe the tissue response in applying staples and various suture materials to both scalp and buccal mucosa in rabbits. 18 rabbits were divided into 6 groups. The incised wounds of both scalp and buccal mucosa were sutured with staples, polyglactin 910, chromic catgut, mer silk and nylon. The experimental animals were sacrificed after 1, 3, 5, 7, 10, 14 days posto peratively 3 animals at one time. The tissue was stained with Hematoxylin and eosin, and Masson's Trichrom. In light microscopic examinations, the sutured sites were examined histologi cally according to 6 degrees about inflammation and collagen deposit. The results were obtained as follows, 1. The chromic catgut, an absorbable suture material, was absorbed by 7 days, whereas polyglactin 910 and mersilk began to get absorbed after 7 days. 2. Mersilk manifested a broad range of inflammation in the scalp, and both staple and nylon showed a severe inflammatory reaction in the buccal mucosa. 3. With polyglactin 910, both tissue samples showed only minor foreign body reaction, however in the scalp, the process of fibrosis took place compara tively slowly, whereas in the buccal mucosa, it occurred promptly and manifested active fibrosis by 7 days. 4. Mersilk showed widespread a matrix formation in both scalp and buccal mucosa, and showed the most severe inflammatory reaction by 3 days, which did not seem to decrease even after 7 days. 5. Both staple and nylon showed relatively a severe inflammatory reaction, however fibrosis took place rather promptly compared to the other groups. 6. Generally, in the buccal mucosa fibrosis occurred more promptly than in the scalp in both control and experimental groups. 7. Retention of the suture material and stability of the knot were the best with the staple, and better stability was manifested by the multi-stranded poly glactin 910 and mersilk than singlestranded chromic catgut and nylon. From above results, in the buccal mucosa absorbable suture materials especially polyglactin 910 showed better response in the aspect of inflammatory reaction, while in the scalp monofilament suture materials such as staple and nylon manifested a early fibrosis and collagen formation.
Background: Osteoradionecrosis is the most dreadful complication after head and neck irradiation. Orocutaneous fistula makes patients difficult to eat food. Fibular free flap is the choice of the flap for mandibular reconstruction. Osteocutaneous flap can reconstruct both hard and soft tissues simultaneously. This study was to investigate the success rate and results of the free fibular flap for osteoradionecrosis of the mandible and which side of the flap should be harvested for better reconstruction. Methods: A total of eight consecutive patients who underwent fibula reconstruction due to jaw necrosis from March 2008 to December 2015 were included in this study. Patients were classified according to stages, primary sites, radiation dose, survival, and quality of life. Results: Five male and three female patients underwent operation. The mean age of the patients was 60.1 years old. Two male patients died of recurred disease of oral squamous cell carcinoma. The mean dose of radiation was 70.5 Gy. All fibular free flaps were survived. Five patients could eat normal diet after operation; however, three patients could eat only soft diet due to loss of teeth. Five patients reported no change of speech after operation, two reported worse speech ability, and one patient reported improved speech after operation. The ipsilateral side of the fibular flap was used when intraoral soft tissue defect with proximal side of the vascular pedicle is required. The contralateral side of the fibular flap was used when extraoral skin defect with proximal side of the vascular pedicle is required. Conclusions: Osteonecrosis of the jaw is hard to treat because of poor healing process and lack of vascularity. Free fibular flap is the choice of the surgery for jaw bone reconstruction and soft tissue fistula repair. The design and selection of the right or left fibular is dependent on the available vascular pedicle and soft tissue defect sites.
Incipient changes of the periodontal tissue in the pressure zones of rat molar subjected to the experimental force were studied by the transmission electron microscope. Experimental animals were consisted in 3 control and 21 experimental rats, of which one maxillary first molar was moved buccally with a fixed appliance which were exerting the force of 15 gm. After experimental period of 1 hour, 3 hours, 6 hours, 24 hours, 2 days, 3 days and 7 days, the animal were sacrificed with cardiac perfusion of $2.5\%$ glutaraldehyde in the sodium cacodylate buffer and the experimental teeth with surrounding periodontal structures were processed for electron microscope. At the beginning of the tooth movement, periodontal ligaments of the pressure were compressed and collagenous fibers were arranged parallel to the root of the teeth and cell free zones in company with cell necrosis were followed. Cell free zones at the periodontal ligaments appeared in the 3 hour survival group, and getting severe with time lapse it became widespread in 2-3 day survival group and undermining bone resorption as a healing process was observed in 7 day survival group. Dilatation of mitochondria and swelling of the rER in the fibroblast and other connective tissue cells in the periodontal ligament were observed in the 3 hour survival group, which were characteristics of the incipient changes in the compressed periodontal ligament. Dilatation of nuclear membrane and pyknosis were followed by the destruction of the nucleus and cell membrane. There were no evidence in cell damage or necrosis of the alveolar bone adjacent to the hyalinized area of periodontal ligaments.

Taking these into account, this study focused on developing a thermal technology and design of the housing of an environment-friendly mobile home sauna, making the most use of the sauna's fundamental purpose and settling it as a tourist product, analyzing the marketing research on the existing sauna and considering the recent housing trends and lifestyles for a new concept sauna. Thus, regarding its characteristics and utilization, it was designed smaller than $10m^2$ (3 pyeong) so that it would be easy to install in any space and convenient to move. It can be installed in separate buildings and rest spaces such as country houses, resorts, pensions, camping grounds as well as outdoor houses, custom produced for a measure of pyeong that customers want so as to match up with the Enforcement Ordinance of the Agricultural Land Act in a concept of the farmer's hut and kitchen, bathroom and bathroom can be installed inside according to an option. In addition, regarding its efficacy, in order to give environment-friendly healing effects, materials such as Hinoki Cypress, red clay and hardwood charcoal were used, a fixed indoor temperature of $70{\sim}100^{\circ}C$ was maintained by heating methods such as electromagnetic wave free, energy saving and low-power boiler, and it was made to have excellent effects on fatigue recovery, relieving stress, skin care and diet through far-infrared emission.

This study investigated slow fashion consumers from a human desire viewpoint in order to examine the psychological aspects of slow fashion consumption. First, the concept of slow fashion was defined. Second, this study reviewed the concepts of desire found in Oriental philosophies and applied them to slow fashion consumption. This process identified the fundamental desire that motivates consumers of slow fashion. Lastly, the psychological aspects slow fashion consumers were examined through a comparison of fundamental desire and external reasons given by interviewees in regards to personal motivation to purchase slow fashion products. Data were collected through in-depth interviews with 10 unmarried women 25 to 37 years old with experience in buying slow fashion products. Exploratory data were classified into external reasons and fundamental reasons to derive the concept of slow fashion. First, slow fashion is defined as sustainable fashion that considers humans and nature. It also seeks to produce morally good products that transcend time and space. Second, we identified the following external reasons: social responsibility, pursuit of healthy products, and pursuit of beauty. Finally, we extracted four factors that reflected fundamental desire: Recognition Desire, Ostentation Desire, Sexual Appeal Desire, and Power Desire. Altruism was emphasized by respondents who expressed social responsibility among external reasons and the desire for recognition among fundamental reasons. However, this goodwill also originated from human desire due to the emotional healing they receive from helping others. Interviewees sought psychological benefits such as self-satisfaction and spiritual growth through slow fashion that implied an Association of slow fashion consumption with human desire. Slow fashion consumption was influenced by moral convictions as well as various desires; in addition, external reasons for slow fashion consumption could be explained by desire.
