Influences of Neck and/or Wrist Pain on Hand Grip Strength of Industrial Quality Proofing Workers

  • Background: The aim of this study was to analyze the interaction between neck and/or wrist pain and hand grip strength (HGS) and to investigate factors (age, sex, neck disorders, and carpal tunnel syndrome) influencing the HGS of industrial quality proofing workers (N = 145). Methods: Standardized questionnaires [Neck Disability Index (NDI), Boston Carpal Tunnel Questionnaire] were used to evaluate existing neck and/or wrist pain. HGS measurements were performed in different wrist positions. Results: Significant differences between participants with and without neck pain were found in different wrist positions, in neutral wrist position right [without neck pain (n = 48) 46.34 (43.39 - 49.30); with neck pain (n = 97) 38.46 (36.20 - 40.72), F(1,144) = 16.82, p < 0.001, ŋp2 = 0.11] and left [without neck pain 44.06 (41.19 - 46.94); with neck pain 37.36 (35.13 - 39.58), F(1,144) = 12.70, p < 0.001, ŋp2 = 0.08]. A significant difference between participants with and without wrist pain was found for neutral wrist position right [without wrist pain (n = 105) 42.53 (40.37 - 44.70); with wrist pain (n = 40) 37.24 (33.56 - 40.91), F(1,144) = 6.41, p = 0.01, ŋp2 = 0.04]. Regression analysis showed significant results especially for steps two (age and weight, NDI) and three (age and weight, NDI, Boston Carpal Tunnel Questionnaire) for neutral position right (R2 = 0.355, R2 = 0.357, respectively). Conclusion: Neck pain has an impact on HGS but should be evaluated in consideration of age and sex.

수근관증후군에서 수근관절굴곡이 신경전도속도에 미치는 영향 (Study on the Change of Nerve Conduction with Wrist Flexion in Carpal Tunnel Syndrome)

  • 정상대조군 20 hands와 수근관증후군환자 40 hands를 대상으로 각 1분, 2분, 5분간 수근관절굴곡 후의 신경전도속도의 변화를 측정하여 수근관증후군의 진단에 도움을 줄 수 있는지를 알아보고자 운동신경은 belly-tendon방법을, 감각신경은 역방향전도검사법을 각각 이용하여 wrist to finger segment에서 수근관절굴곡 전후의 정중신경의 SNCV와 MNDL을 측정하여 아래와 같은 결과를 얻었다. 대조군에서 수근관절굴곡 후의 신경전도속도의 변화가 있었던 경우는 감각신경에서 2 hands, 운동신경에 1 hands였으며 환자군에서는 감각신경에서 3 hands, 운동신경에서 2 hands였다. 그리고 1분, 2분 및 5분간 수근관절굴곡 후의 SNCV와 MNDL의 평균과 표준편차를 구해본 결과 대조군과 환자군에서 모두 유의한 변화가 없었다. Phlaen's wrist flexion검사에서 대조군은 5%에서 양성이었으며 환자군에서는 60%에서 양성이었다. Tinel징후는 대조군에서는 10%에서 양성이었으며 환자군에서는 33%에서 양성이었다.

Segmental Sensory Nerve Conduction Study in Vibration Exposed Subjects

  • The present study was performed to assess peripheral neural involvement by exposure to hand-arm vibration. Segmental sensory nerve conduction in the median and ulnar nerves were measured in shipyard workers exposed to vibration. The subjects were 47 male adults exposed to hand-arm vibration and 7 healthy male controls. The subjects underwent an extensive bilateral neurophysiological examination. Sensory compound nerve action potential (SNAP) of the median and ulnar nerves in palm-finger and wrist-palm segments were measured by antidromic method. And SNAP of the median and ulnar nerves in wrist-proximal finger and wrist-distal finger segments were measured by orthodromic method. Result of sensory nerve conduction study was abnormal in 31 patients $(66\%)$ and normal in 16 patients $(34\%)$ of subjects. The pathological pattern in the hand-arm vibration exposed group was 13 patients $(28\%)$ of carpal tunnel syndrome, 18 patients $(38\%)$ of distal sensory neuropathy, 7 patients $(15\%)$ of multifocal and 1 patient $(2\%)$ of Guyon syndrome. The present study indicates that vibration-induced nerve impairments exist both in the finger-palm and palm-wrist segment of median and ulnar sensory nerves. The results suggest that segmental sensory nerve conduction study would be useful as objective indication of peripheral nerve impairment induced by the hand-arm vibration.

2 자유도 자동손목관절을 가진 근전 전동의수 개발 (Development of the Myoelectric Hand with a 2 DOF Auto Wrist Module)

  • An essential consideration to differentiate prosthetic hand from robot hand is its convenience and usefulness rather than high resolution or multi-function of the robot hand. Therefore, this study proposes a myoelectric hand with a 2 DOF auto wrist module which has 6 essential functions of the human hand such as open, grasp, pronation, supination, extension, flexion, which improves the convenience of the daily life. It consists of the 3 main parts, the myoelectric sensor for input signal without additional attachment to operate the prosthetic hand, hand mechanism with high-torqued auto-transmission mechanism and self-locking module which guarantee the safety under the abrupt emergency and minimum power consumption, and dual threshold based controller to make easy for adopting the multi-DOF myoelectric hand. We prove the validity of the proposed system with experimental results.

Mean Value of Median Nerve Cross-sectional Area in Healthy 20s and 30s in Busan Area

  • This study was designed to investigate the median nerve cross-sectional area of the upper extremity which is the main cause of CTS in the 20s and 30s. The median nerve cross-sectional area (MNC-area) of each part of the upper limb was measured in healthy 20s and 30s females and males without neurological diseases or other diseases. This MNC-area was compared with the hand, wrist, finger, and other body indexes. The research group was divided into 20s female and male groups, and the 30s were also divided into female and male groups. In the comparison between the ages, the hand, and wrist configurations in the 30s were significantly higher than those of the 20s. The mean median nerve cross-sectional area was significantly larger in the male group than in the female group in both 20s and 30s, and it was larger in both men and women than in the 20s. Hand and wrist configurations were also positively correlated with the median nerve cross-sectional area in both 20s and 30s. The median values of hand ratio and wrist ratio were 2.26 and 0.65, respectively. This median value of hand ratio was inversely correlated with the median nerve cross-sectional area. The median nerve cross-area of the 20s was 6.88~7.38 ㎟ in the male group and 5.69~6.99 ㎟ in the female group, respectively. The median nerve cross-area of the 30s was 6.32~8.89 ㎟ in the male group and 6.15~7.17 ㎟ in the female group, respectively. The mean median nerve cross-sectional area was positively correlated with body mass index in both groups. Most of the variables were higher in their 30s than in their 20s.

신경망을 활용한 손목재활 수부 동작 인식 최적화 기법 (Optimization Technique to recognize Hand Motion of Wrist Rehabilitation using Neural Network)

  • 본 연구는 손목 재활을 위한 신경망을 이용하여 수부 동작 인식해주는 연구이다. 수부의 재활은 손상을 받은 수부의 기능을 최대로 회복시켜 일상생활과 직업, 취미생활을 가능하게 하는 것을 목표로 한다. 하지만 어느 장소를 찾아 치료를 하는 것은 경제적, 시간적으로 매우 비효율적이다. 이런 문제점을 해결하고자 본 연구에서는 환자가 직접 스마트 기기를 이용해 재활 치료를 하고자 한다. 이를 활용하면 비용이나 시간적인 면에서 매우 도움이 될 것이다. 본 연구에서는 총 10명의 사람으로부터 4 종류의 재활 운동에 대한 데이터를 수집하여 손목재활 데이터셋을 만들었다. 수부 제스쳐 인식은 신경망을 이용하여 모델을 구성하였다. 그 결과 93%의 정확도를 얻었으며, 본 시스템의 유용성을 검증하였다.

류마티스 관절염에서 손과 손목에 대한 수술적 치료 (Surgical Treatment for Hand and Wrist Problems in Rheumatoid Arthritis)

  • 류마티스 관절염은 만성 경과를 보이는 전신 질환이지만 최근 약물 치료의 비약적 발전으로 인해 많은 환자들이 질병 활성 조절을 얻고 있다. 이에 따라 손과 손목에 시행하던 수술이 예전에 비해 줄어드는 추세를 보이고 있지만 안타깝게도 약물 치료에 반응하지 않는 환자는 여전히 존재하고 있고, 그들에게 수술은 약물과 함께 필요한 필수적 치료 방법이다. 관절이 파괴 및 변형되고, 근력 저하가 발생한 환자의 손과 손목을 수술하기 위해서 집도의는 환자마다 각기 다른 양상으로 진행되는 류마티스 관절염의 병리를 이해해야 한다. 특히 류마티스 관절염은 동시에 여러 관절을 침범하여 문제가 발생하는 경우가 빈번하여 한 가지의 수술만 하기 보다는 여러 가지의 수술을 함께 시행하는 경우가 많으므로 류마티스 관절염 환자의 손과 손목에서 발생하는 문제들에 대해 종합적인 이해가 필요하겠다. 이번 종설에서 저자는 30여 년간 류마티스 관절염에 대한 수술을 하면서 중요하게 생각하였던 개념을 정리하였고, 각 수술의 임상 결과를 문헌 고찰과 함께 기술하였다.

완관절과 수부 질환의 초음파 소견과 치료 (Ultrasound Findings and Treatment of Wrist and Hand Diseases)

  • There are many wrist and hand diseases in which ultrasound can help in the diagnosis and treatment. Because many small structures are located, identifying anatomical locations and pathways is especially important. In De Quervain's syndrome, it is necessary to find tendon lesions located in the first compartment of the wrist. If injection therapy is required, administer the regimen accurately within the tendon sheath through ultrasound. In carpal tunnel syndrome, there are several methods to diagnose a disease by measuring cross-sectional area of the median nerve. Ultrasound has the advantage of administering injection therapy without damaging the nerve. Intersection syndrome can be diagnosed by observing swelling and hypoechoic appearances at the point where the tendons of the first compartment cross over the second compartment of the wrist. Ultrasound-guided injection is also safe and efficient. If there is a trigger finger lesion, the most representative findings is to observe a nodular hypoechoic thickening of the involved A1 pulley. When injection therapy is performed, it is effective to administer medication between pulley and flexor tendons as much as possible to reduce pressure on the attached structures.

정상 성인의 손목관절 위치가 파악력에 미치는 영향 (The Effect on Grip strength in Change of Wrist Position of Normal Adults)

  • Purpose: The purpose of this study was to investigate the effect of wrist position on grip strength. A Grip-Strength is measurement of muscle strength in hand. It is evaluated for the motor function and handicap of hand, projection of treatment plan. It is important because correct treatment. Methods: Total 80 college students were participated in this study, who consisted of 40 males and 40 females(age range: $20{\sim}26$). A Grip-Strength Dynamometer was used to measure the grip strength in three wrist position(flexion, extension, neutral position) and two elbow position(supination, pronation). The ANOVA was conducted to determine any significant difference in grip strength between the wrist position and the two elbow position. Results: The results were as follows: 1. The grip strength was affected by wrist position changes. 2. The difference of grip strength according to elbow position Was not significant difference (p>0.01). 3. The grip strength i1l neutral position was strongest among 3 position of wrist (p<0.01). Conclusion: A Grip-Strength was significant difference in wrist position. Pronation position was minimum grip-strength but not significant difference between supination and pronation.

A Study on the Robot Structure of Hand for the Rehabilitation Training of Stroke Patients

  • The rehabilitation training robots for treating the upper limbs of stroke patients were mainly focused on the upper proximal treatment of it, but recently studies of the distal parts of the upper limbs for rehabilitation of the hand is making some progress even though it is still a small number so far. In this paper, we present the hand robot for the rehabilitation training of stroke patients that is the fingertip contact-typed mechanism, and it has also equipped with the wrist rehabilitation unit to be worked like human hand that enables any movements through mutual cooperation by fingers while picking up or grasping objects. The robot that is presented for this purpose supports the movement of fingers with 5-DoF and the wrist with 3-DoF that moves independently, and operates with a structure that allows the joints of the wrist and fingers to be collaborated organically together to each other. Also, hereby the simulation and evaluation test on its robot mechanism are performed to ensure that fingers with 5-DoF and the wrist with 3-DoF of the serial kinematical mechanism are designed to comply with or exceed ROM for ADL.
