Effect of Tongue Scraping, ZnCl2 Mouth Rinse, and Periodontal Treatment on the Reduction of Oral Malodor (수종의 구취처치법의 구치감소효과에 관한 연구)

  • This study was performed to investigate the effect of three modalities commonly used for reduction of oral malodor and to review the contributing factors for halitosis. For this study, 45 subjects of good general health in their third decades were selected, and they were divided into three groups by each modality, that is, tongue scraping, $ZnCl_2$ mouth rinse, and periodontal treatment. Subjective sense of halitosis, diets, Oral hygiene index based on tooth brushing and tongue brushing habit, resting salivary flow rate, gingival index, degree of tongue coating were recored in the Halito-Chart designed by the author. Oral malodor converted from volatile sulfur compound was measured by the Halimeter$^{(R)}$(Interscan Co., USA), and the correlation between the oral factors and pre-treatment halimeter score were analysed. Data obtained were analysed by the SPSS windows program and the results of this study were as follows : 1. There were no significant correlation between oral factors such as oral hygiene index, salivary flow rate, gingival index, degree of tongue coating and pre-treatment halimeter score. 2. Difference between pre- and post-treatment halimeter score by the three modalities were significant, but correlation between pre- and post-treatment score within group was most significant in the periodontal treatment group followed by the tongue scraping group and lowest in the $ZnCl_2$ mouth rinse group. 3. Reduction of halimeter score was significant only in subjects with relatively more tongue coating in the tongue scraping group, but in the $ZnCl_2$ mouth rinse group and in the periodontal treatment group, the modality applied to each group had same significant effects without regard to the degree of oral hygiene index or gingival index. 4. Men had more tongue coating than women, and reduction of halimeter score was significant in men, but not in women.

Relationship between Halitosis and Gastric Fever Analysed by Questionnaire Cold-Heat Patternization (구취의 한열변증 분석: 구취는 위열증으로 발생하는가?)

  • Objectives: We performed this study to check the effect of Cold-Heat attribute analyzed quantitatively by questionnaire on halitosis patient. Methods: We made a questionnaire composed of 15 items(6 categories) about the contents of Cold-Heat and asked 105 subjects to answer in the form Likert-like seven-points score. And then, we extracted Cold-Heat attribute from 15 items, 6 categories, 6 weighted categories, heat score and cold score. On the other hand we measured the halimeter measurements. VAS, winkle tongue coating index. Afterward, the data were analyzed by statistical methods. Results:1) In the 15 items, question number 6, 7, 8 is negative correlated with halimeter measurements. Question number10 is positive correlated with halimeter measurements. And question number 11, 12, 14 is positive correlated with VAS. 2) In the 6 categories, constipation is negative correlated with halimeter measurements. Chill is positive correlated with VAS. And Cold-limbs is positive correlated with halimeter measurements. 3) In the 6 weighted score categories, constipation is negative correlated with halimeter measurements. Chill is positive correlated with VAS. And Cold-limbs is positive correlated with halimeter measurements. Conclusions: Further clinical research is necessary in the development of the questionnaire's items. And to get the better of this study, we should research more analytic method made casual relationship clear between Cold-Heat attributes and the index of halitosis.

Association Between Halitosis and Periodontal Disease Parameters, and Effect of Conscious Sedation on Measurement of Halitosis in Dogs (구취와 치주질환 지표와의 상관성 및 의식하 진정이 구취측정에 미치는 영향에 대한 연구)

  • The purpose of this study was to evaluate the relationship between periodontal disease and halitosis, and to develop a detail clinical protocol for assessing halitosis in animals. We measured the periodontal disease parameters, the degree of halitosis using organoleptic scale method and the concentration of volatile sulfur compounds(VSC) using portable sulfide monitor($Halimeter^{TM}$). In this study, VSC levels by $Halimeter^{TM}$ were found to be significantly associated with periodontal disease parameters(plaque index, calculus index)(P<0.05). We also found that conscious sedation did not affect the measurement of VSC levels.

Clinical Characteristics of Patients with Halitosis (구취 환자의 임상적 특징)

  • Objective : Halitosis, or bad breath, is a common concern for many people. The aim of this study was to investigate clinical characteristics of halitosis patients and correlation with their various associated elements. Methods: We surveyed 169 halitosis patients by reviewing questionnaires from the Halitosis Clinic in the Hospital of Oriental Medicine, Kyunghee University from January 2004 to March 2006. The questionnaires contained various items including sex, age, history of smoking and alcohol intake, duration of symptoms in a week, time and condition of severe halitosis, suggestive origin of halitosis, taste abnormality, oral hygiene, self-assessed halitosis severity, dry mouth, postnasal drip, tonsillolith, globus pharyngeus, reflux sensation, too much gas and Winkel tongue coating index. Volatile sulfur compounds were measured with a $Halimeter^{\circledR}$. Results : The halitosis patients actively carried out management methods such as non-smoking, moderation in drink, teeth brushing and tongue scraping in order to decrease their oral malodor. Tongue coating significantly affected the $Halimeter^{\circledR}$ reading score, and tongue coating and dry mouth significantly affected self-assessed halitosis severity. Conclusions: Our results suggest that tongue coating and dry mouth should be treated to improve satisfaction in halitosis patients.

Treatment Efficacy on Oral Malodor according to Pre-treatment Volatile Sulfur Compound Level (구취의 심도에 따른 치료 효과에 대한 비교 연구)

  • Considering various factors contributing oral malodor, the accurate prediction of prognosis is very important to both clinician and patients. The present study has been performed to invetigate the relationship between treatment effeicacy and pre-treatment volatile sulfur compounds (VSC) level. Ninety patients were divided into three groups, A(<150ppb), B(150< <200ppb), and C(>200ppb) groups, according to pre-treatment VSC level detected by Halimeter, and each group included 30 patients. Routine therapeutic measures for oral were provided to each patient which consisted of oral prophylaxis, tooth brushing and flossing instruction, tongue scraping by proper device, and gargling of 0.25% ZnCl2 Solution. The group with high pre-treatment VSC level (>150ppb) showed significant reduction of VSC level at 1 and 3 weeks after. However, the group with low pre-treatment VSC level (<150ppb) did not show any significant reduction during the experimental periods. Collectively, the results suggested that patients with high pre-treatment VSC level show better prognosis.

Relationship of Halitosis to Oral Hygiene : Pilot Study (구취와 구강위생과의 관계에 대한 예비실험)

  • The purpose of the pilot study was to know the relationship between halitosis and oral hygine, to examine the difference of the positon of the straw to be placed in the oral cavity by using Halimeter. Fourteen persons that has good health condition without halitosis were participated in this pilot study. They were treated twice prophylaxis and then they had not tooth brushing during two weeks. After two weeks, they were examied to gingival index, plaque index, VSC valuses. The obtained results were as follosws : 1. No significant difference of VSC values were observed during two weeks withoiut tooth brushing. But significant difference of gingival index, plaque index were observed. It was suggested that halitosis was not caused by only plaque, complex reasons(salivary function, stress etc) maybe caused halitosis. 2. Here were significant reduction of VSC values as the depth of plastic straw into the oral cavity decreased (p<0.05)

Clinical Evaluation of Halitosis Patients in Korea

  • Purpose: This study was designed to evaluate the sex and age differences in the Korean oral malodor patients. Methods: Subjects were the patients with oral malodor (male, 2,239; female, 3,496) visited the Department of Oral Medicine, Kyung Hee University Dental Hospital from January 2007 to December 2012 were recruited to this study and measured the volatile sulfur compounds (VSCs) in the oral cavity with Halimeter. Results: There were slightly more female than male (female : male=3 : 2), and the mean values of Halimeter result were 65.79 ppb (female) and 79.94 ppb (male), which was more higher in male. Total patients showed normal age distribution in both gender, and the peak was the age of 26-35 in female, the age of 36-45 in male. The percentages of pseudo-halitosis were 74.7% in female and 69.3% in male, which was more higher in female. Conclusions: Therefore, it was revealed that women are more likely to have oral malodor than men, and there was a difference in the largest age group between men and women. Also women had more pseudo-halitosis patient than men.

A study of oral malodor of denture wearers (의치장착자의 구취발생에 관한 연구)

  • In order to investigate the amount of oral malodor of denture wearers, concentration of volatile sulfur compounds of 20 denture wearers(10 male, 10 female) was measured by Halimeter(RH-17, Interscan, CA, USA) at following four conditions ; condition 1(with denture) : male - $415.0{\pm}90.1$, female - $430.9{\pm}101.3$ condition 2(immediately after denture removal) : male - $291.4{\pm}35.5$, female - $259.8{\pm}20.4$ condition 3(with denture after 30 minutes immersing in chlorhexidine) :male - $210.1{\pm}25.7$, 여자 - $197.4{\pm}22.2$ condition 4(with denture after 24 hours immersing in chlorhexidine) : male - $120.1{\pm}35.2$, 여자 - $109.2{\pm}20.0$ There was no sexual difference, but, value at the condition 1 showed statistically significant difference from the others(p<0.05). It is dentists' responsibility and privilege to understand and diagnose the strong association between mouth and oral malodor and treat it.

  • 본 연구에서는 환자들에 의해 구취와 관련이 있다고 일반적으로 믿어지고 있는 여러 요인들이 치료 후의 환자의 만족도에 미치는 영향에 대해 알아봄으로써 환자의 만족도의 예후를 판단할 수 있는 자료를 마련하고자 하였다. 서울대학교 치과병원 구강진단과에 구취를 주소로 내원한 환자 194명을 대상으로 설문지를 이용하여 구취와 관련성이 있는 것으로 알려져 있는 항목들을 조사하였다. 조사 항목으로는 1) 본인확인가능여부, 2) 타인확인가능 여부, 3) 구취의 기간, 4) 구취의 빈도, 5) 축농증, 6) 비염, 7) 코수술 병력, 8) 콧물이 목뒤로 넘어감, 9) 코막힘, 10) 비호흡 곤란, 11) 구강건조감, 12) 편도선부종, 13) 가래, 14) 신물의 역류, 15) 소화불량, 16) 흡연여부가 포함되었다. 구취의 평가는 portable sulfide monitor인 $Halimeter^{(R)}$(Interscan Co., Chatsworth, CA, USA)를 이용하여 치료 전, 후에 각각 측정하였고, 혀배면의 세정, 치면 세균막 조절, 0.25% $ZnCl_2$ 양치액의 사용을 내용으로 하는 구취의 치료를 시행한 뒤 자신의 구취개선정도에 대해 %로 질문하여 치료 후 만족도를 조사하였다. 각 조사 항목과 치료 후 만족도 사이의 상관관계 및 실제 $Halimeter^{(R)}$ 수치의 개선정도와 만족도 사이의 상관관계를 분석하였으며, Symptom Checklist - 90 Revised(SCL-90R)의 분석결과에 따라 환자를 두 그룹으로 분류하고 두 그룹 사이의 치료 후 만족도의 차이를 조사한 결과 다음과 같은 결론을 얻었다. 1. 본인 및 타인 확인가능여부, 구취가 항상 나는지 여부, 구취의 기간, 축농증, 비염, 코수술 병력, 코막힘, 비호흡 곤란, 콧물이 목뒤로 넘어감, 흡연여부에 따른 치료 후 만족도의 유의한 차이는 없었다. 2. 신물의 역류(p=0.003), 소화불량(p=0.007), 구강건조감(p=0.016), 편도선 부종(p=0.018), 가래(p=0.033)에 따른 치료 후 만족도는 통계적으로 유의한 수준의 차이를 보였다. 3. $Halimeter^{(R)}$ 수치의 개선정도와 만족도 사이의 유의한 상관관계는 관찰되지 않았다(r=0.092, p=0.201).

Comparison Characteristic on Psychological Status between Genuine and Pseudo Halitosis (진성구취 및 가성구취 환자군의 심리적 특성차이에 대한 연구)

  • Objectives : This study was designed to investigate the difference between genuine and pseudo halitosis about qi stagnation and stress. Methods : We recruited 63 halitosis patients who visited the halitosis clinic in the Kyung Hee University Korean Medicine Hospital from June 2012 to August 2013. We classified the halitosis patients into two groups, genuine and pseudo halitosis based on organoleptic assesment (OLT) score and evaluated the severity of self-reported halitosis using visual analog scale (VAS), amount of sulfide compounds using Halimeter, qi stagnation level using qi stagnation questionnaire and stress levels using heart rate variability (HRV) parameters. Results : Comparing the genuine halitosis group and the pseudo halitosis group, there was no significant difference in VAS of halitosis but significant difference in halimeter score. Distribution of qi stagnation and non-qi stagnation groups was significantly different between genuine and pseudo halitosis. Compared to the genuine halitosis group, the pseudo halitosis group showed a significant higher value of low frequency/high frequency ratio (LF/HF ratio). Conclusions : The result of this study suggest that there is significant difference about qi stagnation and stress level between genuine halitosis and pseudo halitosis patients, so when approaching pseudo halitosis patients, we should consider their psychological and emotional status.
