Evaluation of Metal Composite Filaments for 3D Printing (3D 프린팅용 금속 입자 필라멘트의 물성 및 차폐 능력 평가)

  • It is hard to get Filaments which are materials of the 3D printing Fused Deposition Modeling(FDM) method as radiation shielding in Korea. and also related research is insufficient. This study aims to provide basic data for the development of radiation shields using 3D printing by evaluating the physical properties and radiation shielding capabilities of filaments containing metal particles. after selecting five metal filaments containing metal particle reinforcement materials, the radiation shielding rate was calculated according to the Korean Industrial Standard's protective equipment test method to evaluate physical properties such as tensile strength, density, X-ray Diffraction(XRD), and weight measurement using ASTM's evaluation method. In the tensile strength evaluation, PLA + SS was the highest, ABS + W was the lowest, and ABS + W is 3.13 g/cm3 which value was the highest among the composite filaments in the density evaluation. As a result of the XRD, it may be confirmed that the XRD peak pattern of the particles on the surface of the specimen coincides with the pattern of each particle reinforcing material powder metal, and thus it was confirmed that the printed specimen contained powder metal. The shielding effect for each 3D printed composite filament was found to have a high shielding rate in proportion to the effective atomic number and density in the order of ABS + W, ABS + Bi, PLA+SS, PLA + Cu, and PLA + Al. In this study, it was confirmed that the metal particle composite filament containing metal powder as a reinforcing material has radiation shielding ability, and the possibility of using a radiation shielding filament in the future.

A Study of the Metal Artifact Reduction using Dual Energy CT : Clinical Applications of Dual Energy and MAR Algorithm (Dual Energy CT를 이용한 금속물질 인공물 감소방법 : Dual Energy와 MAR알고리즘의 임상적 응용)

  • Metal material inserted into the body have a large difference in density from human tissues or bones around the Metal during CT scans.. Therefore, the Metal material inserted into the body produces Artifact. Metal Artifact, which occurs around Metals, can degrade the quality of CT images, causing confusion when medical team diagnose lesions. Through this experiment, we confirm that the occurrence of Artifacts decrease by using Dual energy CT and MAR algorithm in Single source Dual energy CT. We also want to present basic data on clinical application methods by comparing and analyzing the characteristics of images obtained by each method. Using GE 750HD CT, artificial implants were scanned using general method and Dual energy. Then we apply the MAR algorithm to each image obtained. And all previously acquired images were compared and analyzed the characteristics of the examination, such as image quality evaluation and dose evaluation. Images with MAR algorithm and Dual Energy confirmed a decrease in Metal Artifact. Images with MAR algorithm have reduced Metal Artifact, but have the disadvantage of distorting the details of artificial joint implants. On the other hand images teseted with Dual Energy have the advantage of being able to implement details than those applied with MAR algorithms, it takes longer to reconstruct the image and the exposure dose was about four times higher than those applied with MAR algorithm. In order to locate Metals, such as the post-operative follow-up period, it is useful to apply MAR algorithm to obtain images. And it is more useful to examine with Dual Energy when micro lesion identification, such as cardiac examination, and surgical planning or when tests are performed in diagnostic way.

Comparison of the Effects of an Adductor Canal Block and Periarticular Multimodal Drug Local Injection on Pain after a Medial Opening High Tibial Osteotomy (내측 개방 근위 경골 절골술 후 통증 조절에서 관절 주위 다중 약물 국소 주사와 내전근관 차단술의 효과 비교)

  • Purpose: The efficacy of periarticular multimodal drug injection and adductor canal block after a medial opening-wedge high tibial osteotomy was compared in terms of the postoperative pain level. Materials and Methods: From November 2016 to March 2017, 60 patients underwent a medial opening-wedge high tibial osteotomy under spinal anesthesia. Preemptive analgesic medication, intravenous patient controlled anesthesia were used for pain control in all patients. Thirty patients received a periarticular multimodal drug injection (group I), and 30 patients received an adductor canal block (group II). These two groups were compared regarding the postoperative pain level, frequency of additional tramadol injections, total amount of patient-controlled analgesia, and number of times that the patients pushed the patient-controlled analgesia button at each time interval. Results: The visual analogue scale scores over the two-week postoperative period showed no statistical significance. The frequency of additional tramadol hydrochloride injections was similar in the two groups over time. The mean number of times that patients pushed the patient-controlled analgesia button was similar in two groups over time. The total amount of patient-controlled analgesia was similar in the two groups over time. Conclusion: This study shows that intraoperative periarticular multimodal drug injections and adductor canal block may have a similar effect on postoperative pain control in patients who have undergone a medial opening-wedge high tibial osteotomy for unicompartmental osteoarthritis of the knee.

3D Image Evaluation of Aneurysm in Cerebral Angiography (뇌혈관조영검사에서 동맥자루 3D 영상 평가)

  • In this study, four algorithms (Standard, Bone, Dual volume, and Stent Follow up) were applied to the image of the aneurysm in cerebral angiography to reconstruct the image in 3D, and quantitatively evaluate Noise, SNR, and CNR based on the reconstructed image to find out the optimal algorithm. As an analysis method, Image J program, which can analyze images and calculate area and pixel values, was used for images reconstructed with four algorithms. In order to obtain Noise, SNR, and CNR, the region of interest (ROI) is measured by designating the point where the abnormal artery (aneurysm) is located and the surrounding normal artery in the image are measured, and the mean value and SD value are obtained. Background noise was set to two surrounding normal artery to increase reliability. The values of SNR and CNR were calculated based on the given formula. As a result, the noise was the lowest in the stent follow-up algorithm, and the SNR and CNR were the highest. Therefore, the stent follow-up algorithm is judged to be the most appropriate algorithm. The data of this study are expected to be useful as basic data for 3D image evaluation of the vascular and aneurysm in cerebral angiography, and it is believed that appropriate algorithm changes will serve as an opportunity to further improve image quality.

Comparative Analysis on NIMBY Facility and Location - Suyeong·Nambu·Haeundae Sewage Disposal Plants Cases - (기피시설 입지의 지역별 비교 및 결정요인 분석 - 수영·남부·해운대하수처리장 사례중심 -)

  • The goal of this research is to explore the opinions of the resident's neighboring within sewage disposal plants, to investigate the differences in accordance with each location of sewage disposal plants, and to examine the determinants to impact on the sewage disposal plants. The multivariate analysis of variance model and regression model are employed as the empirical analysis for this research. The major findings are as follows; as a rule, most of residents represented the sewage disposal plants are essential public facilities. The sewage disposal plants could be positively considered under proper compensation and negotiation, It is found that the satisfaction level against accomplishing process of the sewage disposal plants facilities are very low. In addition, it was revealed that the determinants to impact on the sewage disposal plants showed differently according to each current location of sewage disposal plants. It means that there are no absolutely concrete reasons to oppose the sewage disposal plants and they can be somewhat different by the each local characteristics. Therefore, these findings provide for the policy makers related with the NIMBY facilities including the sewage disposal plants with valuable information.

Telemedicine Protocols for the Management of Patients with Acute Spontaneous Intracerebral Hemorrhage in Rural and Medically Underserved Areas in Gangwon State : Recommendations for Doctors with Less Expertise at Local Emergency Rooms

  • Previously, we reported the concept of a cloud-based telemedicine platform for patients with intracerebral hemorrhage (ICH) at local emergency rooms in rural and medically underserved areas in Gangwon state by combining artificial intelligence and remote consultation with a neurosurgeon. Developing a telemedicine ICH treatment protocol exclusively for doctors with less ICH expertise working in emergency rooms should be part of establishing this system. Difficulties arise in providing appropriate early treatment for ICH in rural and underserved areas before the patient is transferred to a nearby hub hospital with stroke specialists. This has been an unmet medical need for decades. The available reporting ICH guidelines are realistically applicable in university hospitals with a well-equipped infrastructure. However, it is very difficult for doctors inexperienced with ICH treatment to appropriately select and deliver ICH treatment based on the guidelines. To address these issues, we developed an ICH telemedicine protocol. Neurosurgeons from four university hospitals in Gangwon state first wrote the guidelines, and professors with extensive ICH expertise across the country revised them. Guidelines and recommendations for ICH management were described as simply as possible to allow more doctors to use them easily. We hope that our effort in developing the telemedicine protocols will ultimately improve the quality of ICH treatment in local emergency rooms in rural and underserved areas in Gangwon state.

Clinical Efficacy of Real-Time Sonoelastography for the Follow-Up of Congenital Sternocleidomastoid Muscle Torticollis (선천성 근육성 사경의 추적검사에서 실시간 탄성초음파 검사의 임상적 유용성)

  • Purpose To evaluate the clinical efficacy of real-time sonoelastography (RTS) for the follow-up of congenital muscular torticollis, based on measurements of muscle elasticity. Materials and Methods Thirty-four infants (23 male, 11 female) with congenital sternocleidomastoid (SCM) muscle torticollis underwent ultrasonography and elastography between November 2012 and December 2014. We evaluated the thickness, morphology (mass-like, fusiform, or overall thickened shape), and echogenicity of the SCM muscle on grayscale images and color patterns (homogeneous blue, mixed green < 50% and ≥ 50%, and green to red) on elastography. Strain ratios were measured using Q-lab software. A clinician classified the degree of neck rotation and side flexion deficits using a 5-point grade system based on angles of neck rotation and side flexion. Correlations between the ultrasonography and clinical findings were evaluated by statistical analysis. Results Twenty-two infants had right and 12 had left SCM torticollis, respectively. Linear regression analysis showed that involved/contralateral SCM thickness differences, morphology, elasticity color scores, and strain ratios of the affected SCM muscles were significantly correlated with neck rotation and side flexion deficit scores (p < 0.05). The elasticity color score of the affected SCM muscle was the most significant factor. Conclusion RTS might provide a reliable means for evaluating and monitoring congenital muscular torticollis.

  • To provide high-quality training to residents in a rapidly changing medical environment, it is very important to improve the annual training curriculum centered on competency and ensure that training hospitals maintain an environment suitable for training. The Korean Society of Radiology (KSR) has been steadily improving the training system and has suggested the improvement of the training system by strengthening the competency-based evaluation and faculty development. Currently, KSR was selected for the second annual training curriculum systematization construction project in July 2021, and developed entrustable professional activities, core competencies, and assessment guidelines required by the construction project. Therefore, the development process and assessment guidelines will be introduced to residents and the faculty.

Perception of Long-Term Oxygen Therapy for Chronic Lung Diseases May Affect Poor Adherence in Korea

  • Background: Long-term oxygen therapy (LTOT) improves the survival of patients with hypoxemia due to chronic respiratory diseases. The clinical outcomes of LTOT are strongly associated with patient adherence. To improve the adherence of patients, physicians have focused on the efficacy of LTOT. However, poor adherence may stem from patients' perceptions of LTOT. Herein we evaluated patients' perceptions of LTOT affecting adherence. Methods: We conducted a cross-sectional survey study using descriptive, open, and closed-ended questionnaire. Patients using oxygen therapy (OT) or requiring it but avoiding OT responded to the questionnaires at three university hospitals. Results: Seventy-nine patients responded to the questionnaires. The number of patients using home and portable OT was 69 (93%) and 37 (46.3%), respectively. Patients with good adherence were 22 (30.1%). Among patients with good adherence, 90.9% used oxygen according to physicians' prescriptions whereas only 37.3% of those with poor adherence followed physicians' prescriptions (p<0.01). The reasons for avoiding using home OT were fear of permanent use (50%), unwanted attention (40%), and lack of symptoms (40%). They avoided portable OT because of unwanted attention (39%), heaviness (31.7%), and lack of symptoms (21.6%). Conclusion: Patients on LTOT had the perception of the misunderstanding the effects of OT and of psychosocial barriers to initiate or use LTOT. Considering these findings, health professionals need to provide effective education on the purpose of LTOT to improve patient adherence to OT and provide sufficient support for the management of psychosocial barriers in patients using LTOT.

Effects of GV1001 on Language Dysfunction in Patients With Moderate-to-Severe Alzheimer's Disease: Post Hoc Analysis of Severe Impairment Battery Subscales

  • Background and Purpose: The efficacy and safety of GV1001 have been demonstrated in patients with moderate-to-severe Alzheimer's disease (AD). In this study, we aimed to further demonstrate the effectiveness of GV1001 using subscales of the Severe Impairment Battery (SIB), which is a validated measure to assess cognitive function in patients with moderate-to-severe AD. Methods: We performed a post hoc analysis of data from a 6 month, multicenter, phase 2, randomized, double-blind, placebo-controlled trial with GV1001 (ClinicalTrials.gov, NCT03184467). Patients were randomized to receive either GV1001 or a placebo for 24 weeks. In the current study, nine subscales of SIB-social interaction, memory, orientation, language, attention, praxis, visuospatial ability, construction, and orientation to name-were compared between the treatment (GV1001 1.12 mg) and placebo groups at weeks 12 and 24. The safety endpoints for these patients were also determined based on adverse events. Results: In addition to the considerable beneficial effect of GV1001 on the SIB total score, GV1001 1.12 mg showed the most significant effect on language function at 24 weeks compared to placebo in both the full analysis set (FAS) and per-protocol set (PPS) (p=0.017 and p=0.011, respectively). The rate of adverse events did not differ significantly between the 2 groups. Conclusions: Patients with moderate-to-severe AD receiving GV1001 had greater language benefits than those receiving placebo, as measured using the SIB language subscale.
