The Additional Filter and Ion Chamber Sensor Combination for Reducing Patient Dose in Digital Chest X-ray Projection (디지털 흉부엑스선 검사에서 환자선량 감소를 위한 부가필터와 Ion chamber 센서 조합)

  • In this paper, we studied additional filter and Ion chamber combinations to reduce patient dose without decreasing image quality in digital chest x-ray projection. The experiment set 125 kVp, 320 mA, AEC mode. Ion chamber sensors was divided by 4 cases of combinations, then, we measured patient dose and calculated organ dose using PCXMC. Also, physical image assessment using MTF was performed. As a results, The surface entrance dose and organ dose were the lowest when selecting both left and right Ion chamber sensors under the same conditions of additional filter. In image quality assessment, The spatial frequency scored 2.494 lp/mm which was highest when selecting both right and left Ion-chambers and 0.1 mmCu filter. And to conclude, to select both right and left Ion chamber sensors and 0.1 mmCu filter will help for acquiring good quality image as well as reducing patient dose.

The Application of TW3 method for Prediction about Bone Age in Hand AP Image of Children (소아 Hand AP영상에서 골연령 예측을 위한 TW3법의 응용)

  • The study is to recognize the interactions with bone ages by measuring the length between the end of the bone and the growth plate on selected highest weight of regions of seven for bone maturity in TW3 method. The experiment is subjected on seventy-two children (36 males, 36 females) who have examined the growth plate test from March, 2014 to March, 2015 and implemented a regression analysis by measuring the length between the end of the bone and the growth plate in Hand AP image of the children. In result, each bone age has produced a mean value and a standard deviation corresponding to the specific range and as bone age increases the length between the end of the bone and the growth plate decreased. In addition, female children showed lower mean value in comparison to male and also the measurement of the length between the end of the bone and the growth plate and its bone age are shown to be statistically valid(p<0.001) according to the results of regression analysis using its result value. Therefore, the probability of prediction on the bone age read off through the applied TW3 method and regression equation in the Hand AP image of the children.

Clinical Analysis of Inverse Planning for Radiosurgery ; Gamma Knife Treatment Plan Study (방사선 수술 역방향 치료계획 유용성 평가)

  • The purpose of this study is a comparison of forward planning(FP) and inverse planning(IP) of a radiosurgery procedure. 10 patients of acoustic schwannoma MR image were used for treatment plan. FP-1,2 and IP were established under the same condition. FP and IP were compared by number of shot, conformity index(CI), paddic conformity index(PCI), gradiant index(GI) and treatment time. On average the treatment plan produced by IP tool provided an improved or similar CI, PCI, GI and reduced treatment time as compared to the FP (CI;FP-1:0.85, FP-2:0.86, IP:0.94, PCI;FP-1:0.79, FP-2:0.81, IP:0.78, GI;FP-1:2.94, FP-2:2.94, IP:3.01). The inverse planning system provides a clinically useful plan while reducing the planning time and treatment time.

  • In this study, we developed a finite element model of the human middle ear has been developed to calculate itsfor sound transfer characteristics calculation. We usedThe geometric data forof ossicles, obtained byfrom micro-CT scanning, was used in order to develop the middle- ear FE model. A right- side temporal bone of a Korean cadaver was used for the micro-CT scanning. The developed FE model includes three ossicles, the tympanic membrane, ligaments, and muscles. We calculated theA sound transfer function from the tympanic membrane to the stapes footplate was calculated. The sound transfer function calculated vias of the FE model shows good agreement with measured responses over the 10- kHz frequency band. To measureidentify the sensitivityies of the middle- ear function due to material property variation, we studied several parameters studies have been fulfilled using the middle ear FE model. TAs a result the stiffness property of the incudostapedial joint had the greatest influence onwas the most influential to the middle- ear sound transfer function among the parameters.

  • For impact assessment of inundation in coastal area due to sea level rise (SLR), model for estimating future peak water level was constructed using observed mean sea level (MSL), storm surge level (SSL) data and calculated tide level (TL) data. Based on time series analysis and quadratic polynomial model for SLR and Monte-Carlo simulation for IC, SSL and TL, 100-year return peak water level is expected to be 2.3, 2.6, 2.8m, respectively (each corresponding to year 2050, 2080, 2100). Further analysis on future potential inundation area showed U-dong, Yongho-dong, Songjeong-dong, Jaesong-dong to be at high risk.

  • Diet exerts a major stress on the body and may affect gene expression and physiological functions. Understanding of cellular responses during starvation is necessary in developing strategies to reduce damage caused by diet. In this study, we isolated 10 genes (Comt, RGN, Scd1, Temt, Idi1, Fabp5, Car3, Cyp2c70, Pinx1, and Poldip3) that are down-regulated in starvation and are closely related to liver metabolism. Water supply during starvation had no effect on the induction of apoptosis, autophagy, and ERQC. The genes down-regulated by starvation were associated with many related pathways rather than limited to the liver homeostasis pathway. Water supply during starvation is important. However, maintaining NaCl homeostasis is more important. The results are thought to be closely related to gender-specific metabolism in starvation and NaCl.

Effects of Passive Scapular Alignment on Electromyographic Activity of Trapezius in People With Shortness of Pectoralis Minor Muscle (수동적 어깨뼈 정렬이 작은가슴근 단축된 사람의 등세모근 근활성도에 미치는 영향)

  • The purpose of this study was to assess the influence of scapular alignment on the electromyographic (EMG) activity of the trapezius in people with a short pectoralis minor muscle. For the study, we recruited 15 volunteers who had positive results for short on a pectoralis minor muscle length test. We measured the EMG activity of the upper, middle and lower trapezius muscles. The participants lifted their dominant arm to ear level with the thumb up toward the ceiling in the prone position on a table with the shoulder at a flexion angle of 180 degrees and a horizontal abduction angle of 120 degrees. Scapula was manually aligned by an experienced physical therapist prior to arm lift for the scapular alignment condition. A paired t-test was used to compare the effects of scapular alignment on the EMG activity of the trapezius muscles. The EMG activity of the lower trapezius muscle was significantly increased during the test with the scapular alignment compared to that without scapular alignment (p<.05), while the upper trapezius and middle trapezius exhibited no significant difference between the two conditions (p>.05). The findings of this study suggest that a scapular alignment may alter the recruitment of the lower trapezius muscle during arm lifting in the prone position in people with a short pectoralis minor muscle.

Current Research Trends in Randomized Controlled Trials Investigating the Combined Effect of Korean Medicine and Western Medicine Treatment

  • This review examined recently published (July 2014 to June 2017), randomized controlled trials (RCTs) which investigated the safety and effectiveness of combined Korean medicine/complementary alternative medicine (CAM) and Western medicine, to indicate the direction for integrative medical practice. The Korean Medicine Convergence Research Information Center evidence-based medicine database (KMCRIC EBM DB) was used to retrieve relevant RCTs indexed in the last 3 years. Study design, country, sample size, disease/condition with the Korean Standard Classification of Diseases code, interventions, direction of outcomes, and adverse events were extracted and summarized. A total of 93 RCTs were included in this review. Acupuncture/moxibustion was the most commonly used intervention (n = 47; 51%), and 19% (n = 18) of the studies treated musculoskeletal disorders, followed by circulatory disorders (n = 16; 17%), and mental and behavioral disorders (n = 9; 10%). Integrative treatment was reported as more effective than monotherapy in approximately 83% of these studies. Adverse events were poorly reported in most studies. This review suggests that integrative treatments are feasible, effective, and safe for various diseases/conditions, based on the evidence from recently published RCTs. Future studies on integrative healthcare are warranted.

Effect of neck design on peri-implant tissue responses in external connection type implant : a prospective pilot clinical study (외측연결형 임플란트 고정체의 경부 디자인이 임플란트 주위조직에 미치는 영향에 대한 전향적 예비 임상연구)

  • This clinical study was conducted to evaluate the clinical effects of a concave neck of external connection type implant fixture designed for platform switching on the peri-implant tissue responses. Two types of implants with different neck designs were implanted in 20 patients. For the experimental group, the bioseal(BS) implant fixtures with 's' shaped concave profile on the neck were used, and non-bioseal(NBS) implant fixtures with a straight profile on the neck were used as control(Total of 40 implants, NBS: n = 19, BS: n=21). During the one-year period after implant placement, implant survival rate, marginal bone resorption, bleeding, plaque, and complications were evaluated. The survival rate of NBS and BS group was 94.74% and 90.48%, respectively. There was no significant difference on marginal bone resorption, bleeding and plaque between the two groups (P>.05). Within the limits of the present study, implants with a concave neck design showed similar clinical results to implants with a straight neck design on the peri-implant tissue responses. Longitudinal clinical studies are necessary to confirm more effective clinical results.

Effect of Nutrition Support Team Management - Focusing on Medical Intensive Care Unit Patients - (영양집중지원에 따른 결과 비교 - 내과계 중환자실 대상으로 -)

  • Purpose : This study evaluated the nutritional status and effect of nutritional support team (NST) management in critically ill patients. Methods : From January 2015 to August 2017, the study retrospectively investigated 128 patients aged above 19 years admitted to a medical intensive care unit (MICU). The patients were divided into two groups: NST (n=65) and non-NST (n=63) groups. Nutritional status, classification of bedsore risks, incidence rate of bedsore and clinical outcomes were compared. Results : The study found a higher rate of the use of enteral nutrition in the NST group (${\chi}^2=45.60$, p < .001). The prescription rate of parenteral nutrition (PN) was found to be lower in the NST group (4.6%) compared to the non-NST group (60.3%). There was a higher PN of total delivered/required caloric ratio in the NST, compared to the non-NST, group (${\chi}^2=3.33$, p=.025). There were significant differences for higher albumin levels (t=2.50, p=.014), higher total protein levels (t=2.94, p=.004), and higher proportion of discharge with survival rates (${\chi}^2=18.26$, p < .001) in the NST group. Conclusions : Providing NST management to critically ill patients showed an increase in the nutrition support. Further, to achieve effective clinical outcomes, measures such as nutrition education and continuous monitoring and management for the provision of nutritional support by the systemic administration of a nutritional support team should be considered.
