Early Outcomes of Single-Port Video-Assisted Thoracic Surgery for Primary Spontaneous Pneumothorax

  • Background: Recently, single-port video-assisted thoracic surgery (VATS) has been proposed as an alternative to the conventional three-port VATS for primary spontaneous pneumothorax (PSP). The aim of this study is to evaluate the early outcomes of the single-port VATS for PSP. Methods: VATS was performed for PSP in 52 patients from March 2012 to March 2013. We reviewed the medical records of these 52 patients, retrospectively. Nineteen patients underwent the conventional three-port VATS (three-port group) and 33 patients underwent the single-port VATS (single-port group). Both groups were compared according to the operation time, number of wedge resections, amount of chest tube drainage during the first 24 hours after surgery, length of chest tube drainage, length of hospital stay, postoperative pain score, and postoperative paresthesia. Results: There was no difference in patient characteristics between the two groups. There was no difference in the number of wedge resections, operation time, or amount of drainage between the two groups. The mean lengths of chest tube drainage and hospital stay were shorter in the single-port group than in the three-port group. Further, there was less postoperative pain and paresthesia in the single-port group than in the three-port group. These differences were statistically significant. The mean size of the surgical wound was 2.10 cm (range, 1.6 to 3.0 cm) in the single-port group. Conclusion: Single-port VATS for PSP had many advantages in terms of the lengths of chest tube drainage and hospital stay, postoperative pain, and paresthesia. Single-port VATS is a feasible technique for PSP as an alternative to the conventional three-port VATS in well-selected patients.

  • At Haeundae Beach in Busan, people were swept away by the fast-moving rip current and rescued, on August 13th and 15th, 2009 & July 29th and 30th, 2010. In predicting the wave-induced current and consequent tube movement for public safety, the coastal flows and waves are calculated at each time step and grid point by means of systematic interfacing of hydrodynamic and wave models (Lee, 2001). To provide a user-friendly simulation tool for end-users, the forecasting system has been built in a software package called HAECUM. Outputs from the system are viewed as graphs of tube positions with combined current vectors for easy decision of emergency management officials. The tube-wave interaction is taken into account and the traces of swim tube are simulated by using a Lagrangian random walk (Chorin, 1978; Lee, 1994). In this study, we use the Lee's approach (Lee, 1993) in estimating the surface onshore currents due to wave breaking.

  • The rip current warning index function, which estimates the likelihood of dangerous rip current in the real-time rip current warning system operating to help mitigate against rip current accidents at Haeundae beach, was studied. The rip current warning index evaluated as a function of various real-time observations was developed based on Choi et al. (2013b). This study shows a version of rip current warning index improved by including the effect of wave direction and spreadings of frequency-directional spectrum on rip current likelihood. The wave and tidal observations in 2012 at Haeundae coast were applied to the rip current index function, and its performances at several real events found based on CCTV images were presented and analyzed.

  • The observation system has been developed to investigate the rip currents at Haeundae beach using X-band marine radar. X-band radar system can observe shape, size, and velocity of rip currents, which is difficult to obtain through field observation by conventional device. Algorithms which automatically detect locations, shapes, and magnitudes of rip currents were developed using time averaged X-band radar sea clutter images. X-band sea clutter images are transformed through 3D FFT into 2D wave number spectrum and frequency spectrum. Rip current velocities were estimated using differences in wave-number spectra and wave frequency spectra due to Doppler shift. The algorithm was verified by drift experiments. At Haeundae beach, the radar system exactly located the rip currents and found to be sustained for 1-2 days at fixed locations.

Characteristics of Seasonal Wave, Wave-Induced Current and Sediment Transport in Haeundae Beach (해운대 해수욕장의 계절별 파랑, 해빈류 및 퇴적물이동 특성)

  • To analyze the incident wave characteristics around Haeundae beach the long-term deep water wave data computed by wave hindcast method were used and a continuous wave observation was carried out for 1 year at the 20 m of water depth in front of Haeundae beach. Wave observation data showd that the prevalent wave direction was SSW-S in spring and summer seasons while E-SE in autumn and winter. A numerical modeling shows that the waves from E-SE are refracted strongly due to the shoal developed at the south-east side of Haeundae beach. The simulation also shows inflowing nearshore current along the east coast of the beach develops strongly in autumn and winter. Radioactive isotope tracer experiment for 155 days indicated that the tracers moves to the on-shore direction in the 1st and 2nd tracking then dispersed to the E-W direction along the shore.

Wind Effect on Tidal Currents in the Neighborhood of Haeundae Beach (해운대 해수욕장 전면 해상의 조류에 미치는 바람효과)

  • We observed tidal currents throughout all four seasons in 2007 at a single station, located 1.6km off Haeundae Beach and compared these current data with wind data. The direction of seasonal wind represented a similarity between the winds at sea and on land but the speed of wind at sea was almost three times stronger than the wind on land. In addition, the wind at sea turned out to considerably affect on tidal currents, particularly from late summer to autumn. On the other hand, the thickness of Ekman Layer, indicating a limitation of wind influence, was estimated to be 31.8 m on average, suggesting that the entire water column is under the influence of wind. Therefore, we are required to consider the wind stress into the analysis of tidal currents for the prevention of the loss of sand from Haeundae Beach.

Numerical Experiments on the Terrain Following Strong Wind Phenomenon Effecting to the Onset of Sea Breeze (해풍시작에 영향을 미치는 지형성 강풍현상에 대한 수치실험)

  • The onset time of sea breeze at Haeundae is faster than that at Suyoung in spite of the observation site at Suyoung being 5m and that of Haeundae being 1 km away from the coastline. We therefore simulate the effects of terrain on the onset time of sea breeze at Suyoung and Haeundae districts by using the LCM(Local Circulation Model). This phenomenon is due to the nighttime density flow, which is created by nighttime radiative cooling. It follows the slope of the highlands surrounding the urban area, gathers at a central area of Busan, and then flows out to a lower area like Suyoung river. This process continues after sunrise. In researching the AWS wind speed, we find an important thing. That is to say, the nighttime mean wind speed at Suyoung is three times greater than that at Haeundae. This property shows that Suyoung is an outflow channel of nighttime air mass. The above observed data shows that terrain effect has a important role on the onset of sea breeze.

The Clinical Reaction Study on 64 Cases with Essential Bee Venom Treatment (분리정제봉약침(Essential Bee Venom) 시술환자 64례에 대한 임상적 반응 보고)

  • Objectives The purpose of this study was to observe clinical reactions (itching, pain, etc.) after Essential Bee Venom (EBV) treatment. Methods This study was conducted 64 patients who had been injected with EBV in Korean Medicine Hospital of Woosuk University and Haeundae Ja-Seng Hospital of Korean Medicine from October 5, 2015 to February 20, 2016. We checked clinical reactions of treatment region. Results 8 patients (12.5%) complained of localized itching and 4 patients (6.3%) complained of localized pain after EBV injection. In itching reaction of patients, 2 patients (15.4%) occurred itching in head and neck, 4 patients (12.9%) occurred in lumbar and back. In pain reaction of patients, 2 patients (6.5%) occurred pain in lumbar and back, 1 patient (11.1%) occurred pain in upper limb. All clinical reactions were happened within 7 treatment times. Conclusions This study suggested EBV treatment can cause clinical reaction like itching and pain. Clinical reactions were more common in inpatients than outpatient, and all reactions were happened within 7 treatment times. Doctors should recognize physical reaction caused by EBV and explain to patient. Further studies are needed to more improved treatment.

  • Background: The purpose of this study was to identify the top-100 cited articles on genetic generalized epilepsy (GGE) published in journals that have made key contributions to the field of epilepsy. Methods: We searched the Web of Science website produced by Clarivate Analytics for articles on GGE, and sorted them according to the number of citations to identify the top-100 cited articles. We then manually reviewed the contents of the top-100 cited articles, which were designated as "citation classics". Results: The top-100 cited articles were published in 27 journals, with the largest proportion appearing in Epilepsia (19 articles). The articles originated from institutions in 17 countries, with 31 articles from the USA. The institution associated with the largest numbers of articles in the field of GGE was the University of Melbourne, Australia (9 articles). Panayiotopoulos C. P. was the first author of three articles, and was listed most frequently in the GGE citation classics. The publication years were concentrated in the 2000s, when 56 articles were published. The most-common study topics were genetics (35 articles) and neuroimaging (17 articles). Conclusions: This study has identified the top-100 cited articles on GGE. These citation classics represent the landmark articles on GGE, and they provide useful insights into international research leaders and the research trends in the field.

Boussinesq Modeling of a Rip Current at Haeundae Beach (Boussinesq 방정식 모형을 이용한 해운대 이안류 수치모의)

  • The rip current occurred at Haeundae beach was numerically investigated under directional random wave environment. The numerical simulation was performed using a fully nonlinear Boussinesq equation model, FUNWAVE which is capable of simulating nearshore circulation since it includes the effect of wave-induced momentum flux and horizontal turbulent mixing. The results of numerical simulation show the time-dependent evolution of the wave-induced nearshore circulation system (including rip current) that are caused by nonlinear transformation of directional irregular waves due to unique topography of Haeundae. From the results, it was found that rip current is well generated and developed where relatively lower wave height and relatively deeper water depth along the longshore direction, and sudden and strong events of rip current were observed.
