Experience with Pediatric Kidney Transplantation, 1985-2016: A Single Regional Center Study

  • Purpose: Kidney transplantation (KT) is an ideal treatment for pediatric patients with end-stage renal disease (ESRD). We report the clinical outcomes of pediatric ESRD patients who underwent KT in a single regional center. Methods: We retrospectively investigated the medical records of 60 pediatric patients who were diagnosed with ESRD and underwent KT in our hospital between January 1985 and June 2016. Results: A total of 60 children and adolescents (40 male, 20 female; mean age, $13.86{\pm}4.26$ years) were included in this study. Six patients (10.0%) underwent KT immediately after receiving the diagnosis of ESRD, while the others underwent KT after dialysis treatment (mean period of dialysis, $368.7{\pm}4,41.8$ days). The mean donor age (50 living-related [83.3%], 10 deceased [16.7%]) was $40.0{\pm}12.85$ years and the male:female ratio was 1.07:1. The most common cause of ESRD was chronic glomerulonephritis. The overall survival rates at 1, 3, and 5 years after KT were 98%, 98%, and 96%, respectively, while the graft survival rates at 1, 3, and 5 years were 93%, 86%, and 68%, respectively. Children who underwent KT before 10 years of age had better monthly growth rates than those who underwent KT later than 10 years of age. Conclusions: KT is performed less frequently in children than in adults, but causes of ESRD vary and clinical outcomes after KT greatly affect the growth and development of pediatric patients. Therefore, further analysis and monitoring of clinical progression after KT in pediatric ESRD patients are necessary.

  • Because traumatic tissue damage on hands and feet often lead to loss of function, permanent deformity, prompt and adequate reconstruction is essential. For children, future growth, as well as function and esthetics, must be taken into account. Several techniques have been employed to reconstruct hand and foot dorsum defects of children. However, skin graft and muscle free flap with skin graft cannot prevent contracture and will interfere with normal growth. Fasciocutaneous free flap reduces contracture and enables early physical therapy, decreasing the need for additional surgical intervention. Parascapular flap is particularly suitable because it has reliable pedicle and is relatively thin. There is little functional loss in the donor site, and also simultaneous extensor tendon reconstruction of hand and foot is possible. The disadvantage of this technique is that postoperative defatting is needed to adjust volume. Our department has achieved satisfactory results using this approach, and would like to report 13 cases of hand and foot reconstruction using parascapular flap in patients under the age of 15 (from March, 1998 to May, 2003).

A longitudinal study on the interrelationship between the growth change of the gonial ang1e and IMPA. (성장기 아동에 있어 gonial angle의 변화와 IMPA 변화에 관한 누년적 연구)

  • This study was undertaken to investigate the growth change of genial angle and interrelationship between the growth change of gonial angle and IMPA in the longitudinal data from 7 years to 15 years. By analyzing the serial lateral cephalograms or 15 males and 19 females of 7 years old to 15 yens old who had no abnomality in growth and development and no history of orthodontic treatment, the following conclusion were obtained. 1. Genial angle gradually decreased with aging in 25 children and in 9 children increased or maintained. 2. There was a tendency that children who decreased genial angle with aging showed gradual increase of IMPA and children who increased or maintained gonial angle with aging showed decrease of IMPA. 3. There was a tendency that regardless of the change of gonial angle, interincisal angle decreased with aging.

  • Among many factors that affect the development of the talent of gifted children, the family factor that first finds and cultivates the talent of gifted children is quite important. The researches on the family of gifted children are mostly concentrated on physical environment out of the home environment, neglecting psychological environment. Although the recreational activity of family with gifted children considerably contributes to the emotional, intellectual and social development of gifted children themselves and to the improvement of life quality of the family members, the recreation of family with gifted children has not yet been studied. Thus, the objective of this research is to present basic materials for seeking the direction to educate parents and to support family with gifted children in order to encourage synergy effect in emotional and social growth of family with gifted children by surveying the property of recreation of family with gifted children. The result of result is as follows. First, the type of family with gifted children tends to be remarkably biased to 'family-oriented activity' and 'elf-development activity.' Second, verifying the significant level of motive for recreational activity of family with gifted children at the variables such as number of family members, parents' age, parents' education level, parents' occupation, etc, it was concluded that they were not statistically significant. The result of this research suggests that education and support for efficient recreation of family with gifted children is necessary to achieve healthy talent of gifted children.

  • Purpose: Celiac disease, an autoimmune enteropathy triggered by exposure to gluten, is not uncommon in South Jordan. However, its prevalence is underestimated due to lack of physician awareness of the diversity of disease presentation. The clinical spectrum includes classic gastrointestinal manifestations, as well as rickets, iron-deficiency anemia, short stature, elevated liver enzymes, and edema. Our goal was to evaluate celiac disease presentation in clinically diagnosed children. Methods: Retrospective study included all children diagnosed with celiac disease between September 2009 and September 2015. Hospital charts were reviewed. Demographic data, clinical characteristics, and follow-up were recorded. Results: Thirty-five children were diagnosed with celiac disease during the study period. Mean age${\pm}$standard deviation was $6.7{\pm}3.8$ years (range, 2.0-14 years). There were 17 (48.6%) female patients. The average duration between onset of symptoms and diagnosis was $16.3{\pm}18.7$ months. Fifteen (42.9%) patients presented with classic malabsorption symptoms, whereas 7 (20.0%) patients presented with short stature. Positive tissue transglutaminase antibodies (tTg)-immunoglobulin A (IgA) was seen in 34 (97.1%) patients. The one patient with negative tTg-IgA had IgA deficiency. Although tTG-IgA values were not available for objective documentation of compliance, clinical data (resolution of presenting abnormalities and growth improvement) assured acceptable compliance in 22 (62.9%) patients. Conclusion: CD in children may present with diverse picture. Although of the small number, the non-classical presentations are not uncommon in our rural community. Gluten-free diet is the main strategy for treatment and associated with usually correction of laboratory abnormalities and improvement of growth.

Effect of Adenoidectomy on Dentofacial Skeleton in Naso-reapiratory Dysfunction Children (비호흡장애 아동에서 인두편도 절제가 치조안면 골격에 미치는 영향)

  • This study was made to investigate the effect of adenoidectomy on dentofacial skeleton in naso-respiratory dysfunction children. The clinical material compromised the 24 children in a previous study who had naso-respiratory dysfunction and 24 children who were the nasal breathing with normal occlusion. The cephalograms were taken at the initial examination and 1 year later for the control group and experimental group the paired sample statistical analysis was performed. The result were as follows. 1. In cranial base variable, difference between two groups were not statistically significant. 2. In craniofacial variable, experimental group showed brachyfacial pattern but control groups didn't show significant growth pattern. 3. In maxillary variables, experimental group showed flattening the palptal plane. 4. In mandibular variables, experimental group showed the decrease of mandibular plane angle and gonial angle. 5. In facial height variables, experimental group showed horizontal growth rotation.

Long-term Follow-up of Metaphyseal Sclerotic Lines in Children Treated with Pamidronate (파미드로네이트 치료받은 환자에서 발생한 골간단 경화성 선에 대한 장기간 추적 연구)

  • Purpose: Bisphosphonates are widely used for the management steroid-induced osteoporosis (SIO) in children. With the increasing use of bisphosphonates, there have been reports of abnormal radiological findings in the growing skeleton. Therefore, their use in pediatric patients remains controversial. The present study was conducted to evaluate the long-term follow-up radiographic features, particularly metaphyseal sclerotic lines, in children who receive pamidronate therapy for nephropathy. Methods: Twenty-four children with nephropathy treated with oral calcium and pamidronate (mean duration, 9 months; dose, 100 mg daily), were evaluated retrospectively. All patients had SIO secondary to chronic glucocorticoid therapy for treating nephropathy. Long bone radiographic imaging was performed before treatment with pamidronate, and at follow-up, several years later. Physeal growth rates were estimated by measuring the distance that the sclerotic lines moved on the radiographs during the corresponding time intervals. Results: The mean follow-up period was 138 months. Long bone radiographs showed well-defined sclerotic lines at the metaphyseal ends, progressively moving from the physeal plate to the diaphysis, in all patients. The mean rate of movement of the sclerotic line was 6.21 mm per year. In 12 patients, the lines disappeared. The mean rate of growth in height was 7.33 cm per year. Conclusions: Results of long-term follow-up suggest that the metaphyseal sclerotic lines associated with pamidronate treatment tend to disappear without affecting overall skeletal growth. Bisphosphonate treatment for SIO in children with nephropathy seems to be safe, although further studies in larger number of patients are needed.

Cognitive Function, Emotional and Behavioral Problems, and Temperament of Premature Children

  • Objectives: We aimed to compare preterm, neurodevelopmentally disordered and healthy full-term children. Methods: We enrolled 47 children who were born preterm, 40 neurodevelopmentally disordered children, and 80 healthy children as control participants, in order to assess the cognitive functioning and the risk of behavioral problems at the age of 5. Children were assessed using the Korean Wechsler Preschool and Primary Scale of Intelligence-4th edition (K-WPPSI-IV), the Child Behavior Checklist (CBCL), and the Temperament and Character Inventory (TCI). Results: The mean K-WPPSI-IV score of the preterm group was $87.19{\pm}17.36$, which was significantly higher than that of the neurodevelopmental disorder group ($69.98{\pm}28.63$; p<0.001) but lower than that of the control group ($107.74{\pm}14.21$; p<0.001). The cumulative CBCL scores of the preterm children were not significantly different from those of the control group. Additionally, the TCI scores for reward dependence of the preterm children were higher than those of the control group. Conclusion: The cognitive performance of preterm infants was lower than that of healthy full-term infants at the age of 5, and there was an association between slower growth and decreased cognitive ability.

A Qualitative Study on Experiences of Parenthood Among Mothers of Early School-Age Children (학령 초기 자녀를 둔 어머니들의 부모됨 경험에 대한 질적 연구)

  • Objective: The purpose of this study was to gain an understanding of parenthood of mothers of early school-age children, with emphasis on cognitive, behavioral, and emotional aspects of parenthood. Methods: Nineteen mothers whose children were 1st and 2nd grade elementary school children participated in this study. Data were collected through individual in-depth interviews. During the interview, mothers were asked to speak freely about their experiences as parents of early-school age children, including parenting beliefs, parenting behaviors, and emotional experiences related to parenting. These interviews were transcribed verbatim and analyzed in a thematic way using MAXQDA. Results: Using qualitative methods, five major themes were emerged: (1) Recognizing the role of parents as a supporter of growth; (2) recognizing parenthood of the present generation that imposes expanded roles to mothers; (3) co-parenting of early school-age children and marital relationship; (4) co-existence of different parenting styles based on specific mother-child interaction situations; (5) experiencing a wide spectrum of emotions including anxiety, guilt, and depression, as well as happiness. Conclusion: This study revealed the unique cognitive, behavioral, and emotional aspects of parenthood experienced by mothers with early school-age children. Results of this study can fill a gap in our understanding of parenthood of mothers when their children entered into the new developmental stage of middle childhood. The results could also be used as a basis for developing parent education and family relationship programs.

Perceived Uncertainty and Rearing Attitude of Mothers with Chronically ILL Children (만성질환아 어머니의 질병에 대한 불확실성 정도와 양육태도)

  • The purposes of this study were to measure the degree of perceived uncertainty and to identify the rearing attitudes of mothers with chronically ill children, and to examine the relationship between the perceived uncertainty and the rearing attitude of these mothers. The subjects of this study consisted of 133 mothers with chronically ill children, registered at 2 university hospitals in Seoul. Data was collected from April 1 to May 31, 1996. The Parents' Perception of Uncertainty Scale(28-item 4 point scale) and the Maternal Behavior Research Instrument (49-item 5 point scale) were used. Data was analyzed by Cluster analysis, ANOVA MANOVA and t-test. Results of this study are summarized as follows : 1. Mothers perceived their uncertainty to be slightly high (Mn 2.48). The degree of perceived uncertainty by the four components slightly differed unpredictability(2.72), lack in clarity(2.58), vagueness (2.52) and lack of information(2.04) . The degree of perceived uncertainty of mothers with ill children revealed to be influenced significantly by the age of the ill children, duration of illness after the diagnosis, and the experience of hospitalization. 2. Among the rearing attitudes : moderatlely high affective(Mn 3.98) and resrictive(Mn 3.58) attitudes of mothers toward their ill children were identified. Mothers tend to give positive evaluations of their childrens' behaviors (Mn 3.38) and less rejection(Mn 2.81). 3. Mothers' rearing attitude were correlated with the degree of perceived uncertainty in illness ; mothers in the Low Perceived Uncertainty Group (Mn 1.99) revealed the highest affective (Mn 4.08), the lowest resrictive(Mn. 2.72) attitudes and tendency to give positive evaluations of their childrens' behaviors (Mn 3.54) compared to the High Perceived Uncertainty Group(Mn 3.26) and Moderate Perceived Uncertaity Group(Mn 2.57). 4. The degree of perceived uncertainty, the duration of illness after the diagnosis and the experience of hospitalization revealed to be significantly influential to the rearing attitude of mothers with chronically ill children. From the above results, it can be concluded that predicting and controlling mothers' uncertainty are necessary for improved, efficient nursing interventions and normal growth & development of the chronically ill children.
