Association between Celiac Disease and Intussusceptions in Children: Two Case Reports and Literature Review

  • Association between celiac disease and intussusception has been reported in adult. Although intussusception is common in children, it rarely has been reported in association with celiac disease. Two children, 5 and 7 years old, with celiac disease are reported here, whose initial presentation was intussusception prior to investigation for concomitant failure to thrive. They presented with acute and severe abdominal distention with vomiting, and donuts and pseudo- kidney appearance in abdominal ultrasonography. One patient's intussusception had reducted spontaneously, however the other had required surgery. In investigation for concomitant failure to thrive, tissue transglutaminase levels were very high and duodenal biopsies revealed celiac disease. Thus celiac testing is recommended in children with intussusception and growth failure.

  • Congenital antral webs are a rare but relevant cause of gastric outlet obstruction in infants and children. The condition may lead to feeding refusal, vomiting, and poor growth. Due to the relative rarity of the disease, cases of congenital antral web are frequently misdiagnosed or diagnosed with significant delay as physicians favorably pursue diagnoses of pyloric stenosis and gastric ulcer disease, which are more prevalent. We report a case of an eight-month-old female who presented with persistent non-bilious emesis, feeding difficulties, and failure to thrive and was discovered to have an antral web. The web was successfully treated with endoscopic balloon dilation, which resolved her symptoms. Two years later, the patient remains asymptomatic and is thriving with weight at the 75th percentile for her age.

The Relationship between Children's Picky Eating Behavior, Physical Growth and Mother's Child Feeding Practice (까다롭게 먹는 유아의 식습관과 성장발육 및 어머니의 식사지도와의 관련성)

  • The purpose of this study was to determine the correlation between young children's picky eating behavior and mother's attitudes, beliefs and practices regarding the eating habits of children. Using a sample of 304 mothers of four or five year-old children, confirmatory factor analysis was conducted to test a 7 factor model, which included mother's beliefs and practices related to the feeding of their children. Following this, a t-test was conducted to examine the differences between the feeding behaviors of mothers of both of picky and non-oicky eaters, respectively. The results indicated that picky eater's eating behaviors were positively related to the pressures exerted by mothers and negatively related to the mother's concerns about their child's weight and child's BMI. These findings suggest that the pressures mothers exert upon their children to eat, their child's weight and picky eating behaviors are correlated to each other.

Research Trends and Challenges in Health Literacy among Children and Adolescents in Korea (아동·청소년 대상 헬스리터러시 연구의 국내 동향과 과제)

  • Purpose: This study aimed to investigate the trends in domestic research related to health literacy in children and adolescents. Methods: Six journal articles and four dissertations or theses related to health literacy in children and adolescents were analyzed according to the following criteria: study aim, research design, study population and sample size, sampling method, health literacy variables, and characteristics of health literacy measurement instruments. Result: The majority of studies focused on general children and adolescents. Correlational studies were most prevalent, while no experimental study was identified at all. The variable most frequently explored in relation to health literacy was health promoting behaviors. Functional health literacy instruments, assessing numeracy and reading comprehension, were most commonly used to evaluate child and adolescent health literacy. Conclusion: Considering the emphasis on health literacy improvement in order to achieve health equity across the entire life cycle, there is a need for both quantitative and qualitative growth in domestic health literacy studies for children and adolescents. School-based interventions are essential for enhancing health literacy. Furthermore, it is also necessary to develop an instrument to measure health literacy of children and adolescents that can be utilized and monitored at the national level.

A Study on the Longitudinal Trajectories of Use Time and the Related factors for the Children in Community Children Centers (아동의 지역아동센터 이용시간의 종단적 변화유형과 영향요인에 관한 연구)

  • The purpose of this study is to identify the trajectories in the use time of children from community children centers and to examine the predictive factors and developmental factors related to each trajectory. The data were derived from the second stage of the Community Children Center Panel Survey using from the first wave (2014) to the third wave (2016). A total of 606 samples were selected from the forth to sixth grades of elementary school. Latent class growth model was employed to identify the trajectories, and the multinominal logistic regression and the logistic regression analysis were used to examine predictive factors and developmental factors. Main results indicated that three types of trajectory were identified: high using group, low using group, and high initial using-rapid declining group. Sex, parental supervision, and use duration were found to be significant predictors. Regarding developmental factors, children who constantly use the community children centers were more likely to increase academic performance and school adaptation. However, no significant results were found for aggression and delinquent behaviors. Based on these findings, this study have suggested the future direction of the community children center.

Efficacy of Nutritional Therapy in Children with Crohn Disease (소아 크론병에서 영양 치료의 효과)

  • Purpose: Crohn disease (CD) in children is frequently complicated by malnutrition and growth retardation. Conventional treatment with corticosteroid may also affect the growth of children, which may be alleviated by nutritional therapy. Therefore, we performed this study to evaluate the role of nutritional therapy in the treatment of CD in children. Methods: The study cohort consisted of thirty children under the age of 17 years, admitted to the Asan Medical Center from 2001 to 2005. While 11 children received nutritional therapy (NT) with an elemental diet, 19 received conventional corticosteroid therapy (ST). Comparative analysis was performed for the rate of the induction of remission, recurrence, disease activity index of pre and post-treatment, and changes of the growth using mean standard deviation score for weight (zW) and height (zH). Results: Ten of 11 children with NT and 18 of 19 children with ST completed the therapy. Two of them were lost to follow-up during the treatment. All 10 children with NT and 15 of 18 children with ST achieved remission. While there was no recurrence in children with NT, 7 with ST showed recurrence within 1 year. Pre- and post treatment Pediatric Crohn disease Activity Index (PCDAI) did not show a difference between the two groups. There were no differences in zW and zH between the two groups after 1 year of follow-up. Conclusion: This study showed that nutritional therapy could be considered instead of steroid therapy for treatment of children with CD. However, a long-term study will be needed to determine the long-term efficacy.

THE EFFECTS OF HIGH PULL HEADGEAR IN MIXED DENTITION WITH CLASS II MALOCCLUSION (혼합치열기 II급 부정교합에 대한 high pull Headgear의 효과)

  • The purpose of this study is to observe the effect of high pull headgear on the craniofacial structures of mixed dentition with Class II malocclusion. The cephalometric headplates of 16 children treated by high pull headgeaar during 6 months and 18 children during 12 months were traced, digitized and statistically analyzed. The results were as follows. 1. Inhibition of foreward growth of maxilla was observed in both group. 2. Clockwise rotation of maxilla was observed in both group. 3. There were distal movement of maxillary 1st molar and inhibition of alveolar bone growth of maxilla. 4. There was compensatory extrusion on mandibular 1st molar. 5. The ratio of anterior facial height to posterior facial height was almostly not changed. In the treatment plan of C II malocclusion by high pull headgear, ire must prevent the mandibulasr 1st molar from extruding and for orthopedic effect, at least 6 months is needed.

  • The purpose of this study was observing the annual growth increment in Cephalogram. The Cephalograms of 28 boys & 22 girls were analized by method of Downs & Steiner, range from 7 to 9 years in Korean children. The results were as follows. 1. The change of gonial angle in both male and female is not presented in this age. 2. The most significant linear change of both male and female is the facial depth (Na-Go) and that of both male and female is the interincisal angle in this age. 3. The increment of the mandibular body length (Go-Me) and the anterior facial height (Na-Me) is presented in this age. 4. The change of SNA and SNB in both male and female is negligible is negligible in this age.

Trajectories of Marital Satisfaction of Married Couples From Childbirth to 5 Years Postpartum: The Panel Study of Korea Children 2008-2012 (한국 아동패널 2008-2012 자료를 활용한 자녀 출산 후 5년간의 부부의 결혼만족도 변화궤적)

  • Objective: This study aimed to explore marital satisfaction changes of married couples from childbirth to 5 years postpartum using the data from the Panel Study of Korean Children 2008-2012. Methods: A total of 2,072 coupes were followed longitudinally during the first 5 years after childbirth. Their repeatedly measured marital satisfaction were analyzed with growth modeling analyses. Results: Husbands and wives had different marital satisfaction trajectories regarding the rate of change and initial marital satisfaction scores. Further, the facets of postpartum marital satisfactions change varied depending on the couples' first, second, and third (or more) birth(s). Conclusion: In general, Korean married couples' marital satisfaction has a decreasing tendency after childbirth. Fathers' marital satisfaction is generally higher but more rapidly decreases compared to mothers' marital satisfaction. Also, couples after first child birth experience a faster decline of marital satisfaction compared to couples after second or third (or more) birth(s).

Understanding and Application of Hierarchical Linear Model (위계적 선형모형의 이해와 활용)

  • A hierarchical linear model(HLM) provides advantages over existing traditional statistical methods (e.g., ordinary least squares regression, repeated measures analysis of variance, etc.) for analyzing multilevel/longitudinal data or diary methods. HLM can gauge a more precise estimation of lower-level effects within higher-level units, as well as describe each individual's growth trajectory across time with improved estimation. This article 1) provides scholars who study children and families with an overview of HLM (i.e., statistical assumptions, advantages/disadvantages, etc.), 2) provides an empirical study to illustrate the application of HLM, and 3) discusses the application of HLM to the study of children and families. In addition, this article provided useful information on available articles and websites to enhance the reader's understanding of HLM.
