Use of Anti-TNF Alpha Blockers Can Reduce Operation Rate and Lead to Growth Gain in Pediatric Crohn's Disease

  • Purpose: Pediatric Crohn's disease (CD) is directly related to growth and has a high probability of requiring surgical intervention(s); therefore, more active treatment for CD is required for children. This study investigated the impact of biologics on growth and disease course associated with surgery. Methods: This was a retrospective cohort study involving patients diagnosed with CD at the Seoul National University Children's Hospital (Seoul, Korea) between January 2006 and October 2017. The aim was to determine the characteristics of pediatric patients with CD and whether biologics affected growth and the surgical disease course. Results: Among patients who underwent surgery for CD, the mean number of operations per patient was 1.89. The mean time from initial diagnosis to surgery was 19.3 months. The most common procedure was fistulectomy (34%), followed by incision and drainage (25%). In all patients, the use of biologics increased the height (p=0.002) and body mass index (BMI) (p=0.005). Among patients who underwent surgery, height (p=0.004) and BMI (p=0.048) were increased in the group using biologics. Patients who used biologics exhibited a low operation rate only within 2 years after diagnosis, with no differences thereafter (p=0.027). Conclusion: Although biologics could not mitigate the operation rate in pediatric patients who underwent surgery for CD, biological therapy delayed disease progression within 2 years of disease onset. Additionally, biologics conferred growth and BMI benefits in this window period. Therefore, it may be helpful to use biologics for optimal growth in pediatric patients with a high probability of undergoing future surgery.

소아 Hand AP영상에서 골연령 예측을 위한 TW3법의 응용 (The Application of TW3 method for Prediction about Bone Age in Hand AP Image of Children)

  • 본 연구는 TW3법에서 골성숙 가중치가 가장 높은 7부위를 선정하여 성장판과 골 말단부 사이의 길이를 측정하여 연령과의 상관관계를 알아보고자 하였다. 실험은 2014년 3월에서 2015년 3월까지 성장판검사를 시행한 소아 72명(남 36명, 여 36명)을 대상으로 하였으며, 소아 손 전후영상에서 성장판과 골 말단부 사이의 길이를 측정하여 회귀분석을 하였다. 그 결과 각 연령마다 특정범위에 상응하는 평균과 표준편차 값이 나타났으며, 연령이 증가할수록 성장판과 골 말단부 사이 길이가 감소하였다. 또한 여아가 남아에 비해 평균값이 작게 나타났으며, 회귀분석 결과에서 성장판과 골 말단부 부위 측정 길이와 연령이 통계적으로 유의(p<0.001)한 것으로 나타났다. 따라서 소아 손 전후영상에서 TW3법의 응용을 통한 회귀방정식으로 골연령의 예측이 가능함을 알 수 있었다.

  • Optimal nutrition in infancy and early childhood is the success behind good health, growth, and development of children. Failure to thrive may be the end point of any combination of a nutritional disorder, poor growth, and psychosocial deprivation. Hospital admission is rarely necessary and may be counterproductive. Day attendance, for investigation and observation of child-parent interaction, may be more valuable. Nutritional supplementation, together with nutritional counseling, can improve food intake and growth in children with failure to thrive.

3차원 스캔 데이터에 의한 초등학생의 발 성장에 관한 연구 (A Study of Elementary School Students' Feet Growth by 3D Scan Data)

  • This study was about to investigate the characteristics of elementary school students' feet growth, comparing and analyzed the measured values and the index values of 3D scan data. As the results from analyzing the measured values, the all measurement items excluding 'Toe 5 angle' showed some significant differences among the age classes, knowing that the elementary school students' feet were continually grown. Although, the items of length, thickness, width, height and circumference indicate some rapid growth of feet at 11 years old, but it appears slowing growth of 'Foot width' after the age of 11 years old. The angle item didn't show any sequential differences according to ages. As the results from analyzing the index values, the children in higher grades have longer 'Toe 5', thicker ankles, wider and thicker the superior part of feet than the children in lower grades. For the superior and the middle part of feet were lowly raised, their Arch height was low and thick. The inferior part of feet showed narrow width and higher height. Giving that the Medial ball width was wide and the Toe 1 angle is high for the children in higher grades, it is expected that the Metatarsophalangeal I might be more projected than that of children in lower grades. Likewise, knowing that the male students' feet shape was bigger than female students' feet from the result that the male students' index values were higher.

Evaluation of growth changes induced by functional appliances in children with Class II malocclusion: Superimposition of lateral cephalograms on stable structures

  • Objective: To compare short- and long-term dentoalveolar, skeletal, and rotational changes evaluated by Björk's structural method of superimposition between children with Class II malocclusion treated by functional appliances and untreated matched controls. Methods: Seventy-nine prepubertal or pubertal children (mean age, 11.57 ± 1.40 years) with Class II malocclusion were included. Thirty-four children were treated using an activator with a high-pull headgear (Z-activator), while 28 were treated using an activator without a headgear (E-activator). Seventeen untreated children were included as controls. Lateral cephalograms were obtained before treatment (T1), after functional appliance treatment (T2), and after retention in the postpubertal phase (T3). Changes from T1 to T2 and T1 to T3 were compared between the treated groups and control group using multiple linear regression analysis. Results: Relative to the findings in the control group at T2, the sagittal jaw relationship (subspinale-nasion-pogonion, p < 0.001), maxillary prognathism (sella-nasion-subspinale, p < 0.05), and condylar growth (p < 0.001) exhibited significant improvements in the Z- and E-activator groups, which also showed a significantly increased maxillary incisor retraction (p < 0.001) and decreased overjet (p < 0.001). Only the E-activator group exhibited significant backward rotation of the maxilla at T2 (p < 0.01). The improvements in the sagittal jaw relationship (p < 0.01) and dental relationship (p < 0.001) remained significant at T3. Condylar growth and jaw rotations were not significant at T3. Conclusions: Functional appliance treatment in children with Class II malocclusion can significantly improve the sagittal jaw relationship and dental relationships in the long term.

국민학교 3학년 아동의 아연의 구리 영양 상태 (Zinc and Copper Nutritional Status of Eight-Year-Old Children in Seoul)

  • The relationships among physical growth, dietary intakes, and Zn & Cu status were investigated in total of 128 eight-year-old children selected from the primary school in Seoul. The mean weight and height were 28.3Kg and 129.6cm and the average BMI and R hrer index were 16.75 and 129.25. These values were a little higher than the ones for the average Korean children at the same age. The average serum Zn & Cu levels were 109.98$\mu\textrm{g}$/100ml and 84.13$\mu\textrm{g}$/ml. About 2.4% of the subjects had blood Zn level below the normal range and 8.6% had above the normal level. For Cu, about 43% of the subjects fell into the subnormal level. Daily dietary intakes of calorie, protein and ascorbic acid were relatively adequate but Fe, thiamin, riboflavin, and niacin were insufficient. Ca and vitamin A intakes were very poor. More than 50% of the subjects onsumed Ca and vitamin A less then 2/3 of RDA. There was no consistant relationship among the serum Zn and Cu levels, growth, and the nutrients intakes. But it was observed that serum Zn contents were higher as consumption of milks, fats and oil groups were high in the serum Zn contents were higher as consumption of milks, fats and oil groups were high in the several BMI groups, and lower in taller and heavier children within the group of 110-119% BMI rate. Therefore, it could be summarized that the Zn status of the children was favorable but the Cu status was inadequate. Serum Zn and Cu levels were found to be related to the overall quality and/or the quantity of diet and physical growth pattern of children.

녹색성장교육에 기초한 프로그램이 유아의 환경보전지식, 태도 및 행동에 미치는 효과 (The Effects of a Green Based Growth Education Program on Young Children's Environmental Preservation Knowledge, Attitude and Behavior)

  • 본 연구는 유아가 함께 참여할 수 있도록 녹색성장교육에 기초한 프로그램을 구성하고 이 프로그램이 유아의 환경보전지식, 태도 및 행동에 미치는 효과를 분석하는데 목적이 있다. 녹색성장교육에 기초한 프로그램은 '생태계', '에너지', '자원', '녹색시민의식'으로 크게 4가지 내용 영역으로 구성하였고, 각 내용 영역에 가장 적합하다고 판단되는 구체적인 활동을 20회기로 계획하여 만 5세 유아를 대상으로 프로그램을 실시하였다. 프로그램 실시 후 환경보전지식, 환경보전태도, 환경보전 행동에서 실험집단 유아의 점수가 비교집단 유아보다 유의하게 높은 것으로 나타나 녹색성장교육에 기초한 프로그램이 유아의 환경보전지식 습득과 환경보전태도 및 행동의 향상에 효과적임을 알 수 있었다. 본 연구는 유아교육현장에서 녹색성장교육을 실행하는데 많은 어려움을 호소하고 있는 현실을 반영하여, 녹색성장교육을 실행하고자 하는 유아 교사에게 구체적이고 실질적인 정보를 제공할 수 있을 것으로 기대한다.

영유아 성장발달 크리닉 운영모형개발 및 시범사업 결과 분석 -서울시 일개 보건소를 중심으로- (The Study on the Model Development and Analysis of 'Child Growth and Developmental Clinic' - Focusing on the public health center -)

  • Purpose: This study was intended to develop the framework of public health center based 'Child growth and developmental clinic' and analyze the efficiency of services. This was the part of Mother-Child Health Guidance Project by the Public Health Center in Seoul. The purposes of this project were to improve children's health and development with early detection of developmental delay by screening test, and to assist and guide the parent for child care and rearing, so that children can reach their maximum potential. Method: Framework and standards for operation of 'Child growth and development clinic' were developed. We selected screening tool. developed parent education materials, and educated the personnel of public health center. Result: 243 children were enrolled for developmental screening, and we detect the children with developmental delay and referred them for further evaluation. The proportion of developmental delayed children was approximately 2% of the enrolled children. Also, we analyzed the characteristics of enrolled children, and their parental needs and attitude on child rearing. Some problems in operation of developmental clinic were discussed. Conclusion: Though we can found some limitations, we are confident that pubic health center can be charge of important role for improving child development for the majority. This study will be the basis of children's developmental clinic running by public health center.

발달장애 자녀를 둔 어머니의 양육과 성장 경험에 관한 포토보이스 연구 (The Child Rearing and Inner Growth Experiences of Mothers of Children with Developmental Disabilities: A Photovoice Study)

  • 이 연구는 발달장애 자녀를 둔 어머니의 양육부담과 성장경험을 이해하고자 한 것이다. 이를 위해 오케스트라 활동에 참여하는 발달장애 자녀를 둔 어머니를 대상으로 포토보이스를 활용한 초점집단인터뷰와 개별면접을 실시하였다. 분석 결과, 발달장애 자녀를 둔 어머니의 양육 경험은 "눈앞이 캄캄해도 걸어감"으로 나타났으며, 이 과정에서 어머니들은 "동행하며 함께 나아감"을 경험하는 것으로 나타났다. 어머니들은 자녀의 발달장애로 인해 여러 고통과 어려움을 경험하면서도 양육과정에서 또 다른 기쁨과 성장을 경험하고 있었다. 특히 오케스트라 활동이라는 매개체를 통해, 어머니는 자녀에 대한 인식 뿐 아니라 양육자로서의 자신에 대한 인식, 주변 지지체계와의 관계에도 의미있는 변화가 나타남이 확인되었다. 이러한 결과를 바탕으로 이 연구는 사회복지적 함의를 제시하고 있다.

한국 소아 청소년 정상 혈압 참고치 (Normative blood pressure references for Korean children and adolescents)

  • 목 적 : 소아 청소년 건강관리에서 혈압 측정의 중요성이 점차 강조되고 있는 시점에서 한국의 소아 청소년들을 위한 정상 혈압 참고 치를 만들고자 하였다. 방 법 : 2005년에 새로운 신체성장 표준치를 만들기 위해 시행한 전국 소아 청소년들의 신체계측사업에 혈압 계측사업을 포함시켜 7-20세에 해당하는 소아 청소년 57,433명(남: 29,443명, 여: 27,990명)에서 신장, 체중 및 혈압 계측치를 얻었다. 혈압은 자동 진동혈압계인 Dinamap Procare 200(GE Inc., Milwaukee, Wi, USA)를 이용하여 측정하였고 앉은 자세에서 안정한 후 우측 팔에서 5분 간격으로 2번 측정하였다. 두 측정치의 평균치를 수축기 혈압과 이완기 혈압으로 기록하여 원 자료로 하였다. 체중이 +3SD 이상인 과체중아들 2,373명의 혈압은 제외하고 분석 하였다. 혈압 백분위수 표는 성별, 연령별, 신장 백분위수별로 만들어 졌으며 통계기법은 2004 Task Force in NHLBI (USA)이 사용한 통계기법을 원용한 고정 변형 LMS 방법을 사용하였다. 결 과 : 성별, 연령별(7-18세), 신장 백분위수별(5th, 10th, 25th, 50th, 75th, 90th, 95th) 혈압 참고치가 완성되었으며 신장 백분위수에 대한 자료는 질병관리본부 홈페이지인 http://www.cdc.go.kr/webcdc/에서 확인 할 수 있다. 고혈압의 기준치는 성별, 연령별, 신장대비에서 95 백분위수 이상으로 하고, 전고혈압 기준치는 90-95 백분위수 또는 수축기 및 이완기 혈압 130/80 mmHg 이상으로 정하였다. 결 론 : 이번에 마련된 우리나라 소아 청소년 정상 혈압 참고치는 처음이기 때문에 사용기준 및 고혈압의 기준은 앞으로 지속적인 논의와 연구가 필요할 것으로 생각된다. 또한 본 혈압 표는 자동 진동혈압계로 마련된 것이기 때문에 수동 수은 혈압계를 이용한 혈압 치를 평가하는 기준표로 사용하는 것은 주의해야 할 것이다.
