Effects of Gyeonbi-Tang Treatment on the Monosodium Iodoacetate-induced Mild Osteoarthritis in Rats (견비탕(蠲痺湯)이 Monosodium Iodoacetate로 유발된 골관절염의 초기변화에 미치는 영향)

  • This study was to investigate the effects of Gyeonbi-Tang Treatment on the monosodium iodoacetate(MIA)-induced mild osteoarthritis in rats. Arthritis was induced by injection of MIA(0.25 mg) into knee joints of rats. Arthritic rats were divided into control(n=8) and treated(n=8) group. Control group was taken distilled water and treated group was taken extracts of Gyeonbi-Tang by orally for 20days. Body weights were measured at 0, 5, 10, 15, 20 days after MIA injection. At the end of experiment(20day after MIA injection), gross and histopathological examination on the articular structures of knee joints were performed. Proteoglycan(PG) contents in articular cartilages were analysed. And also, tumor necrosis factor-${\alpha}$(TNF-${\alpha}$) and interleukin-1${\beta}$(IL-1${\beta}$) contents in synovial fluids were measured by ELISA method. Body weights of the treated group were significantly increased compared with control group at 15, 20 days after injection. Grossly, the severity of osteoarthritis in the treated group were alleviated compared with control group. PG contents in articular cartilages of the treated group were increased compared with control group. Histopathologically, osteoarthritic scores of the treated group was decreased compared with the control group. TNF-${\alpha}$ contents in synovial fluids of the treated group were significantly decreased compared with control group. On the basis of these results, we concluded that Gyeonbi-Tang Treatment has anti-arthritic effects on the monosodium iodoacetate-induced mild osteoarthritis in rats. And it's effects were related with reduced secretion of TNF-${\alpha}$ from synovial membranes.

Age-related morphological studies on hemal node and hemolymph node in Korean native goat (한국재래산양 혈절과 혈림프절의 연령별 형태학적 연구)

  • Hemal nodes and hemolymph nodes are lymphoid organs which share morphologic and functional characteristics of lymph nodes and spleens. Hemal nodes and hemolymph nodes are normally present in Korean native goats. Hemal nodes bad extensive subcapsular and deep sinuses distended by a great number of erythrocytes, and no typical cortex and medulla were observed. Blood vessels commonly occurred, but lymph vessel was not observed in the hemal node. Hemolymph nodes had distinct cortex and medulla, and also had afferent and efferent lymph vessels. The aim of the present study was to obtain new information on the distinct morphological structures of hemal nodes and hemolymph nodes according to ages, and have the basic data for their functions. Goats are divided into 5 groups, consisting of 3 animals aged 1, 3, 6, 10, and 12 months. The morphological studies of the organs were carried out by gross anatomy, light microscopy and immunohistochemistry. During aging, there was an increase in the size of the organs, while there were no significant changes of their numbers, locations and colors. As the goat got older, the lymphatic nodules of hemal nodes were more developed, and the number of macrophage containing phagocytosed erythrocytes was more increased. As the goat was younger, the lymphatic tissues of hemolymph nodes were less developed. There was no difference in distribution of T- and B- lymphocytes according to ages.

Japanese Nursing Students' Learning Experience, Self-directed Learning Ability, and Self-efficacy in Nursing Practice Utilizing Portfolios (일본 간호학생의 학습포트폴리오를 활용한 임상실습교육의 학습경험과 자기주도학습능력 및 자기효능감)

  • Purpose: The purpose of this study is to investigate the learning experience, self-directed learning ability and self-efficacy of Japanese nursing students undergoing portfolio-based clinical practicums. Methods: The self-directed learning ability and self-efficacy of nursing students were examined using two scales. And using a text-mining approach, we constructed correspondence analysis followed by cluster analysis of open-ended responses forms. Results: The mean score of the self-directed learning ability was $60.89{\pm}5.28$ and the generalized self-efficacy was $68.37{\pm}11.56$. Moreover, the scores in the self-directed learning ability were positively correlated with scores in the generalized self-efficacy. In correspondence analysis, the distribution of extracted words showed that record was located on the negative side of the third quadrant, to the first principal component and that patient was located on the positive side of the first quadrant, contributing greatly to the second principal component. Conclusion: The results of this study contribute to approaching to "confidence, pride, stability," "growth and intention to development'' offers a key in developing self-directed learning ability. Students record what they see and learn the importance of visualizing it in learning portfolios. "Expression in detail of the learned contents" and "concerning to which objective evaluation is suggested" are important to the students.

Five Cases of Patients Treated with Injinchunggan-tang for Liver Cirrhosis and Chronic Hepatitis B (만성 B형 간염을 동반한 초기 간경변 환자에서 인진청간탕(茵蔯淸肝湯) 투여 관찰한 5례)

  • Liver cirrhosis is a liverdisease in which the normal microcirculation, the gross vascular anatomy, and the hepatic architecture have been variably destroyed and altered with fibrous septa surrounding regenerated or regenerating parenchymal nodules. Liver cirrhosis is regarded as a serious health problem with high prevalence in the world as well as in Korea. Liver cirrhosis is an irreversible disease. However, in cases of liver cirrhosis induced by chronic hepatitis B, we should treat the chronic hepatitis B, because it frequently progresses to cirrhosis and hepatoma. We observedimprovements of symptoms and laboratory tests with herb medicine Injinchunggan-tang (茵蔯淸肝湯) in five cases ofliver cirrhosis and chronic hepatitis B. The serum aminotransferase was preserved within normal value levels for a long time, 3 years to 7 years. However, further study is necessary to determine the effect of herb medicine on liver cirrhosis and chronic hepatitis B.

Whether Pinhole Scan or Single Photon Emission Computed Tomography (SPECT) in the Diagnosis of Bone and Joint Diseases (골격계진단에 있어서 핀홀스캔의 우월성)

  • Since the publication of the first bone scintiscans in 1962 three decades have elapsed. The bone scan has made great strides during this period, becoming one of the most commonly used nuclear imaging tests. In spite of the progress, however, the specificity of bone scan has remained relatively low. As the result it is a common practice to seek additional information from radiograph, CT scan and MR image, which is euphemistically termed as "image fusion or co-location." The basic reason is the inapplicability of the classical piecemeal analysis to interpreting planar and SPECT bone scans. Such analysis has its base on the observation of elemental features of morphology, which include the size, shape, contour, location, topography and internal architecture. The physiochemical profile may well also be included. Understandably, however, the miniatured images of the planar bone scan cannot provide these features in acceptable detail and the same holds true even with SPECT Images which are but sliced views of the reconstructed planar scans. Fortunately pinhole scanning has the capacity to portray both the morphological and chemical profiles of bone and joint diseases in greater detail through true magnification. The magnitude of pinhole scan resolution is practically comparable to that of radiography as far as gross anatomy is concerned. Thus, we feel strongly that pinhole scanning is a potential breakthrough of the long-lamented low specificity of bone scan. This presentation will discuss the fun-damentals, advantages and disadvantages and the most recent advances of pinhole scanning. It high-lights the actual clinical applications of pinhole scanning in relation to the diagnosis of infective and inflammatory diseases of bone and joint.

TISSUE REACTION FOLLOWING BY COMBINATION OF DISTRACTION AND COMPRESSION FORCE ON DISTRACTION OSTEOGENESIS OF THE MANDIBLE IN THE RAT (백서 하악골에서 신연골형성술시 신장력과 수축력 복합적용후의 조직반응)

  • Distraction osteogenesis is a biologic process of new bone formation between the surfaces of bone segments that are gradually separated by incremental traction. Distraction osteogenesis is clinically applied as a new treatment modality of mandibular hypoplasia or bony defect area in maxillofacial area by many studies of distraction devices and method. But, disadvantages of distraction osteogenesis shows unfavorably long consolidation period and relapse tendency. Therefore, this experiment was designed to investigate the effectiveness of combined application of distraction and compression force for improving of bone quality and shortening of treatment period during distraction osteogenesis. Twenty-five Sprague-Dawley rats with $300{\sim}350gm$ were used. These were divided into two group as distraction group and combination group. The distraction group was added with conventional method during distraction osteogenesis, but the combination group was applied with compression force in the consolidation period. The rats were sacrificed for gross finding, radiographic and histologic findings at 3, 6 weeks after distraction. The results were as follow: 1. On radiographic finding, all combination of distraction and compression force group appeared more radiopacity than distraction group both at 3 weeks and 6 weeks after distraction group. 2. On histologic finding, the formation of mature lamellar bone were showed increasingly in combined group at 6weeks after distraction group. From this study, we may suggest that compression force application in consolidation period during distraction osteogenesis can be useful method to improve bone quality and to shorten the treatment period. But more experimental and clinical studies is necessitated on ideal application timing and method of compression force application during distraction osteogenesis.

Effects of Buja-tang Treatment on the Early Change of the Monosodium Iodoacetate-induced Osteoarthritis in Rats (부자탕이 Monosodium Iodoacetate로 유발된 골관절염의 초기변화에 미치는 영향)

  • Objectives : This study was carried out to investigate the effects of Buja-tang treatment on the early change of the monosodium iodoacetate-induced osteoarthritis in rats. Methods : Arthritis was induced by injection of monosodium iodoacetate(MIA)(0.25 mg) into both knee joint cavities of rats. Arthritic rats were divided into control(n=8) and treated(n=8) group. The control group was taken distilled water and the treated group, extracts of Buja-tang by orally for 20 days. At the end of the experiment(20 days after MIA injection), gross and histopathological examinations on the articular structures of knee joints were performed. Proteoglycan(PG) content in articular cartilages was analyzed by safranine O staining method. And also, tumor necrosis factor-$\alpha$($TNF-{\alpha}$) and interleukin-$1{\beta}$($IL-1{\beta}$) contents in synovial fluid were measured by enzyme-linked immunosorbent assay(ELISA) method. Results : 1. Body weight(g) of the treated group was increased significantly compared with control group at 15 and 20 days after injection. 2. Grossly, the degree of osteoarthritis in the treated group was alleviated compared with the control group. 3. PG content in articular cartilage of the treated group was increased significantly compared with the control group. 4. Histopathologically, osteoarthritic score of the treated group was decreased significantly compared with the control group. 5. $TNF-{\alpha}$ content in synovial fluid of the treated group was decreased significantly compared with the control group. Conclusions : On the basis of these results, we suggest that Buja-tang have inhibiting effects on the progression of arthritis in MIA-induced osteoarthritis model. And it is related to inhibiting the activity of $TNF-{\alpha}$ in osteoarthritic chodrocytes and synovial membranes.

Synergistic Effect of Carboxymethyl Chitosan and Adipose-Derived Mesenchymal Stem Cells on Osteoarthritis Model in Rabbits

  • The purpose of this study is to investigate the clinical effects of carboxymethyl chitosan (CMC) and adipose-derived mesenchymal stem cells (MSCs) on osteoarthritis (OA). Thirty New Zealand white rabbits were used as cranial cruciate ligament transection and partial meniscectomy models. The rabbits were divided into five groups (n = 6) according to the intra-articular injection materials: the control group with PBS, the HA group with hyaluronic acid, the CMC group with CMC, the MSC group with MSCs emerged in PBS, and the MSC+CMC group with CMC and MSCs. Knee thickness, extension angle, gross morphology, histopathology and immunohistochemistry were performed to evaluate the effects of CMC and MSCs on rabbit OA. On the morphologic and histologic examination, the articular surfaces of the femur and tibia were markedly damaged in control group with higher Mankin score and lower cartilage surface thickness. However, OA related cartilage defects were alleviated by the treatment of MSC and/or CMC. The expressions of apoptotic and inflammatory cytokines were decreased and cartilage extracellular matrix (ECM) related collagens I and II were enhanced by the treatment of MSC and/or CMC. In conclusion, this study showed that CMC and MSC treatments have a beneficial effects on OA via the protection of cartilage damage, the stimulation of ECM, and the inhibition of inflammatory and apoptotic reaction.

A Case of Kallmann's Syndrome with Hypoplasia of Olfactory Bulb (후각구 형성부전을 동반한 칼만증후군 1례)

  • Kallmann's syndrome has both a general and specific connotation in describing general condition of gonadotropin-releasing hormone (GnRH) deficiency or a particular cluster of anomalies associated with primary eunuchoidism. The familial occurrence of hypogonadotropic hypogonadism associated with anosmia, color blindness, synkinesia, and mental defect is the classic Kallmann's syndrome. Interestingly, anosmia, or lack of smell, was not found in the absence of gonadal deficiency in the original study of this disorder. This disorder was found on both sexes, but the male to female ratio was 11:1, and Kallmann's syndrome is more often listed under disorders of male hypogonadism for this reason. Gross anatomy has shown disorders of the olfactory bulbs associated with Kallmann's syndrome and it was demonstrated a failure of GnRH-containing cells to migrate from the olfactory placode to the hypothalamus and preoptic area. We have experienced a case of Kallmann's syndrome which showed a hypoplasia of olfactory bulb in MRI during the workup of primary amenorrheic patient. So we report this case with a brief review of literatures.

Inhibitory Effects of Daeyeoungjeon on the Injury of Articular Cartilage Induced by Monosodium Iodoacetate in Rats (대영전의 Monosodium Iodoacetate 유발 관절연골손상 억제 효과)

  • Objectives This study was aimed to evaluate the effects of Daeyoungjeon (hereinafter referred to DYJ) treatment on the injury of articular cartilage induced by monosodium iodoacetate in rats. Methods Twenty-four male rats were divided into normal, osteoarthritic control and DYJ group. Rats of normal group were injected with 0.1 ml physiological saline, rats of control and DYJ groups were injected with 0.1 ml monosodium iodoacetate (3 mg/ml) into each left and right knee joint cavities. Rats of DYJ group were administrated extracts of DYJ during 60 days per orally. At 60 days after treatment, gross lesions, area and proteoglycan contents of articular cartilage, histopathological lesions, immunohistochemistry on matrix metalloproteinases (MMP-2, MMP-3, MMP-7) were evaluated. Results Grossly, degenerative changes of articular cartilages were observed weak in DYJ group. The areas of articular cartilages were broader significantly in DYJ group. The proteoglycan contents in articular cartilages were lesser significantly in DYJ group. Histopathologically, the chondrocyte score was lesser significantly in DYJ group. MMP-3 expression in articular cartilages was observed weak in DYJ group. Conclusions From above results, DYJ treatment has inhibitory effects on the injuries of articular cartilage induced by monosodium iodoacetate in rats, and it's effects may be related with down regulation of MMP-3.
