Clinical characteristics in Taiwanese women with polycystic ovary syndrome

  • Polycystic ovary syndrome (PCOS) is one of the most common hormonal endocrine disorders in women of reproductive age. It consists of a heterogeneous collection of signs and symptoms that together form a disorder spectrum. The diagnosis of PCOS is principally based on clinical and physical findings. The extent of metabolic abnormalities in women with PCOS varies with phenotype, body weight, age, and ethnicity. For general population, the prevalence of hyperandrogenism and oligomenorrhea decreases with age, while complications such as insulin resistance and other metabolic disturbances increase with age. Obese women with PCOS have a higher risk of developing oligomenorrhea, amenorrhea, hyperandrogenemia, insulin resistance, and lower luteinizing hormone (LH) to follicle stimulation hormone (FSH) ratios than non-obese women with PCOS. The LH to FSH ratio is a valuable diagnostic tool in evaluating Taiwanese women with PCOS, especially in the diagnosis of oligomenorrhea. Overweight/obesity is the major determinant of cardiovascular and metabolic disturbances in women of reproductive age.

Studies on the Bladder Worm, Cysticercus cellulosae -The Ulrastructure of C. cellulosae- (유구낭모충에 관한 연구 -유구낭모충의 미세구조-)

  • An electron microscopic study was performed to know the basic tegumental structure of Cysticercus cellulosae. The scolex and bladder portions of cysticerci (human and porcine strains) were prepared for transmission and scanning electron microscopy by conventional procedures. In general, the tegument of C. cellulosae showed the basic ultrastructure of cestode tegument on electron micrographs. The teguments of both scolex and bladder portions consisted of such components, i.e., an outer vesicular layer with numerous microtriches and inner filroug layer. Below the fibrous layer, there were layers of muscle bundles and tegumental cells. The microtriches which covered the surface of cysticercus revealed two distinctly different shapes. The characteristic bladder-like, elongated pyramid shaped "tetrahedrial form" was observed on the surface of the scolex portion, whereas the elongated cylindrical "filamentous form" was distributed on the stirface of bladder portion. In spite of the difference of isolated host and location, the cysticerci showed tole same result. But dimensional variations of the tegument according to topography of the worm were observed. The possibility of application in making differential diagnosis from other larval cestodes and possible functions of this larval tegument were discussed.

  • The purpose of study is to find out common factors and variables that affect common factors in level of right knowledge. attitude behavior with respect to AIDS. During the period form Apr.1. 1995 to Jun. 30. guestionnaires, written both in Korean and English. were distributed to Korean Malaysion employees woring at a certain Korean Video companies. and results of replies. given by 80 workers by country. were used as the research matrials. The gathered marials were analyzed through the SPSS package t-test. ANOVA. factor analysis. and the following results were obtained. The 4 extracted common factors could be named 'general publicity', 'transmissive knowledge', 'diagnosis. prevention'. and 'limitted publicity'. The means of trasimssive knowledge. diagnosos. prevention. limtted publicity factors showed higher means in groups who had beyond university level of education than in showed higher means in groups who beyond university level of education than in groups who had below high school level of education. There was a significant relationship between general publicity factor and econmic state. and between limitted publicity factor and preventive education

Useful Ultrasound Findings of Pediatric Patients with Equivocal Results of Appendicitis: Analysis Based on a Structured Report Form (애매한 초음파 결과를 보인 소아 환자에서 충수돌기염을 감별하는데 유용한 초음파 소견: 구조화 판독문에 기초한 분석)

  • Purpose To identify the features that can be used for differentiating appendicitis from non-appendicitis in pediatric patients with equivocal ultrasound (US) results. Materials and Methods A total of 53 pediatric patients (≤ 18 years old) with equivocal results on US examination for suspected appendicitis between November 2012 and October 2017 were included. US evaluation was conducted based on information retrieved from a predefined structured report form. Then, the likelihood of appendicitis was prospectively classified into five categories. The equivocal results were considered as grade 3 (indeterminate) and grade 4 (probably appendicitis). Results Of the 53 patients, 25 (47.2%) and 28 (52.8%) were classified into grade 3 and 4 groups, respectively. Among the individual US findings, increased vascularity of the appendiceal wall and peri-appendiceal fat infiltration were independent findings associated with the diagnosis of appendicitis (p = 0.005, p = 0.045, respectively) in the multivariate logistic regression analysis and showed the highest diagnostic accuracy (69.8% and 62.3%, respectively). Conclusion Increased vascularity within the appendiceal wall and peri-appendiceal fat infiltration were significant predictors of appendicitis in patients with equivocal US findings.

Automatic Sputum Color Image Segmentation for Lung Cancer Diagnosis

  • Lung cancer is considered to be the leading cause of cancer death worldwide. A technique commonly used consists of analyzing sputum images for detecting lung cancer cells. However, the analysis of sputum is time consuming and requires highly trained personnel to avoid errors. The manual screening of sputum samples has to be improved by using image processing techniques. In this paper we present a Computer Aided Diagnosis (CAD) system for early detection and diagnosis of lung cancer based on the analysis of the sputum color image with the aim to attain a high accuracy rate and to reduce the time consumed to analyze such sputum samples. In order to form general diagnostic rules, we present a framework for segmentation and extraction of sputum cells in sputum images using respectively, a Bayesian classification method followed by region detection and feature extraction techniques to determine the shape of the nuclei inside the sputum cells. The final results will be used for a (CAD) system for early detection of lung cancer. We analyzed the performance of a Bayesian classification with respect to the color space representation and quantification. Our methods were validated via a series of experimentation conducted with a data set of 100 images. Our evaluation criteria were based on sensitivity, specificity and accuracy.

An experience of Patients Who Follow Oriental Medicine After Cancer Diagnosis (암진단 이후 한방진료를 이용하는 암환자의 경험에 관한 연구)

  • Most of cancer therapy consists of surgery, chemotherapy and radiotherapy developed by modern western medicine. Often Korean patients use both modem western and oriental medicine through their cancer life. This study tried out to answer the the question : "What are the experience of a Korean cancer patients who follow oriental medicine after cancer diagnosis?" To answer to that, a micro-ethnographic research method was used. Total 6 patients were observed from March, 1996 to February, 1997. Data were obtained through interview, participant observation, audio-tape recording, field recoding, field note-taking, and ralated documents Using an analytical tool known as "pencil and scissors", the data were analyzed. First, I learned patietnts' accounts for cancer experience following oriental medicine, and I could found that they expereinced "feeling of uncertainty" through cancer life. Second, major argument was searched. : Feeling of uncertainty of cancer patients was extremely increased after cancer diagnosis. Oriental Medicine made cancer patients not only expect to improve general physical condition, but also gave them significnat emotional support to overcome their feeling of uncertanty. Third, I examined how did this argument form meanings in the context of individual life. Modem western mediacal service system could not satisfy cancer patients' informational and emotional need. But oriental medicine contribute to relieve the degree of their feeling of uncertainty. As a result of these understandings, I suggest that modern wetern medicine need to be concerned to feeling of uncertainty of cancer patietns and infomational service, and oriental medicine counsel with cancer patients much more systemically. Also nurses must improve cancer education with more accurate and practical information based on empirical data.

Causes of Chronic Hip Pain Undiagnosed or Misdiagnosed by Primary Physicians in Young Adult Patients: a Retrospective Descriptive Study

  • Background: Hip pain is a common musculoskeletal complaint in general practice. Although comprehensive diagnostic approach on hip pain is mandatory for adequate treatment, un- or mis-diagnosis is not rare in primary care. The aim of this study was to analyze descriptively un- or mis-diagnosed hip pain cases referred from primary care to a tertiary hospital, especially in young adults ${\leq}50years\;old$. Methods: We retrospectively analyzed a consecutive cohort of 150 patients (${\leq}50years\;old$) with chronic hip pain (${\geq}6weeks$), which was not diagnosed or misdiagnosed based on the information provided on the referral form. Results: Overall an average 32 cases/month were referred due to hip pain without a diagnosis or with an incorrect diagnosis. Among them, 150 patients were enrolled in this study and 146 (97.3%) could be allocated to a specific disease by using data from routine clinical practice. Four common final diagnoses were femoroacetabular impingement (FAI) syndrome (55.3%), hip dysplasia (HD, 13.3%), referred pain from the lumbar spine (9.3%), and spondyloarthritis (SpA, 7.3%). In patients with FAI syndrome, 37 (44.0%) had pincer-type FAI and 33 (39.8%) had combined-type. Although the pain site or gender was not tightly clustered, the distribution of final diagnosis was significantly different according to hip pain location or gender. Especially, SpA or HD was not observed in younger women subgroup or elder men subgroup, respectively, when stratified by the mean age of participants. Conclusion: Most (> 80%) young patients with hip pain, a difficult issue to diagnosis for many primary physicians, had FAI syndrome, HD, spine lesions, and SpA. This study could give a chance to feedback information about cases with un- or mis-diagnosed hip pain, and it suggests that primary physicians need to be familiar with the diagnostic approach for these 4 diseases.

  • Subject : Treatment of acupuncture & moxibustion in Huangdineijing Objectives : Discover the principles and substances of acupuncture & moxibustion treatment of Huangdineijing Methods : The author reviewed the Huangdineijing: Suwen, Lingshu to gain a realistic sense of the facts and the medical book's contents that deal with the practices of acupuncture & moxibustion treatment for clinical praxis. Conclusions : 1. The acupuncture & moxibustion of Huangdineijing can be defined as follows. A treatment for correcting of meridian's drift by proper selection of surgical site, using suitable implementation and appropriate manipulation skill from a correct judgement of meridian's flowing through sensing the pulse for diagnosis when the meridian's flowing which come and go constantly driven by the tidal order of defensive Qi tactually appeared hard or soft caused by diseases, thus bring out the soft & gradual 50 laps circulation of nutrient Qi moderately driven by defensive Qi. 2. Today's acupuncture & moxibustion treatments greatly differ from those of Huangdineijing in that we do not know the substance of that treatment which is correcting of meridian's drift and do not obligatorily carry out feeling of the pulse for diagnosis which is a measure & judgement of meridian's flowing against a preliminary treatment in parallel with treatment so reduce use and power of that treatment. 3. I could form a list of general principle from the substances of acupuncture & moxibustion of Huangdineijing like as the flowing or fate of meridian Qi(tidally going in order of defensive Qi and nutrient Qi), deficiency or excess of Meridian flowing, comparing diagnosis before and after treatment, bring out the soft & gradual Qi. That was imposing "the concept of time" on recognition and treatment to human body.

A Study on the Background Variables in the Patients with TMJ Dysfunction (악관절 기능장애의 원인에 관한 연구)

  • Background variables in 203 patients with TMJ pain dysfunction showing no bony change, ranging from 11 to 76 of age, were studied by use of the modified form of self-administered questionnaires deviced by Carlsson et al intended to give a review of the patient's history. All the patients were identified in the Dept. of Oral Diagnosis and Oral Diagnosis and Oral Medicine, Seoul National University Hospital, during the period from Feb. 1983 to Nov. 1983. The questions concerning general somatic and psychic health, educational and social condition were dealt with in this article. The obtained results were as follows : 1. The main symptoms reported by 203 patients were difficulties in opening the mouth wide(74.4%), clicking of TMJ(70.9%), chewing difficulties(69.0%), and pain on movement of mandible(57.6%) 2. The most fequently reported medical symptoms or histories were stomach disease (21.2%), insominia(15.3%), ENT disease(13.8%) and skin disease (12.8%) 3. Seventy-eight percent of the patients denied having had a nervous or psychic disorders while 4% were currently under treatment for such disorders. 4. Eleven percent of 203 patients with TMJ dysfunction thought their family situation was distressing(8.4%) or very troublesome(3.4%).

Connection of Myofascial Release to Cupping (근막이완술과 부항의 접목)

  • Although the physical therapy of both East and West has been based on an identical philosophy, they have had their own therapy with difference in its form and pattern. In general, cupping is used to diagnose and treat viscera by means of acupuncture point, and myofascial release is also used for both diagnosis and orthopedic treatment on the basis of trigger point and myofascial however, when they have a lot of identical facts such as using both mental and physical aspects of human beings for treatment, keeping nervous action balanced, and recovering depressed nervous functions and relieving the pain. In addition. their identical fact includes that they tend to treat patients by using symptoms and reaction shown in their skin, and that both East and West try to consider myofascia as an integrated totality and as a unified body of organic functions with correlations. Among the principles of myofascial release, recently, it has been very identical that stimulus given to the skin results in synapse to sympathetic nerve through dosal horn cell has an effect on viscera, and that cupping is sued for diagnosis and treatment of viscera. It is required, therefore, to continue to carry out studying on this field.
