A Prediction Model of Fear of Falling in Older Adults Living in a Continuing-Care Retirement Community(CCRC) in United States (미국 노인의 낙상에 대한 두려움 예측모형에 관한 연구)

  • Background: Falls are among the most common and serious health problems of older people. The psychological symptoms of falling have received relatively little attention compared to physical problems. Objective: The purpose of this study is to test a model to explain the factors that influence fear of falling among older adults living in a continuing care retirement community (CCRC) in Baltimore city, United States. Methods: A secondary analysis was conducted using data obtained from a Health Promotion Survey done on 149 older adults living in a CCRC. Data was originally obtained during face to face interviews with each participant. Descriptive statistics and bivariate correlations were used to describe the sample and evaluate simple correlations. A path analysis was done using the AMOS 4.0 statistical program. Results: Of the 49 hypothesized paths, 13 were statistically significant, and the model accounted for 22% of the variance in fear of falling among the elderly. There was support for the fit of the model to the data with a nonsignificant chi square at 0.478 (df=2, p=0.79), and the ratio of chi-square to degrees of freedom was 0.24, a CFI of 0.99 and RMSEA of 0.00. In particular, gender, a history of falling, and exercise were significant predictors of fear of falling. Conclusions/Implications: As anticipated, exercise is an important factor to prevent fear of falling. As a modifiable variable, self-efficacy and outcome expectation indirectly influence fear of falling through exercise.

Institutionalization of Care Labor and Differences among Women (돌봄노동의 제도화와 여성들의 차이)

  • This article explores the characteristics of care and care labor which is core keyword of the welfare state and the way of institutionalization of care labor, focusing specially on differences among women. Caring is defined by the expression of morality and labor accompanied by concrete action. But, care labor in the welfare state is defined by "activities involved in caring for the ill, elderly, handicapped and dependent", and I think, that definition is more useful than the narrow one for policy institutionalization. But the latter definition intentionally separates the domestic work from care work. Care labor is considered to be different from the market labor in terms of motivations, but there are some limits in standardization and commercialization of the traits of emotional and moral engagement. Thus, requiring of emotional motivation as one of the job descriptions is not realistic. Welfare state is institutionalizing women's unpaid care work in family through de-familization, and its policy tools are cash benefits and services for care-related, which influence to the female wage worker and fulltime housewife, care receiver and care giver, and polarization of women's class in a very different way. Cash benefits enhances the division of gender labor, polarizes the care laborer and weakens of expansion the care as decent job. The movement of feminist welfare state have a vision of universal service expansion and need the policy list for de-gendering of care labor.

Work-Life Balance Policies in Germany and the Participation of Private Companies (독일의 일·가정 양립정책과 민간 기업의 참여)

  • Traditionally, Germany, a conservative welfare state, has pursued a male breadwinner model based on gender division of labor. For a long time, Germany tried to address low fertility through economic support centered on cash benefits, but it was not successful. In 2007, the German government began to shift the paradigm of family policy for work-life balance under the slogan of "A mix of time policy, income transfer, and infrastructure." When the issue of low birth rates emerged as a national concern in Germany, there was a growing social sentiment that not only the government but also private companies should contribute to increasing fertility by providing family-friendly personnel policies and working conditions. Private companies have been voluntarily improving family-friendly working conditions beyond legal obligations, aiming to secure personnel and prevent turnover. Germany's fertility rate is currently rising toward the European average level in 2023, which can be attributed to the government's work-life balance policies and the participation of private companies. In terms of improving work-life balance policies in Korea, it has been proposed to change the perception of the need for fathers to participate in child care, to make parental leave compulsory for men, to guarantee employment for women after childbirth, to expand child care facilities, and to revitalize family-friendly policies in companies.

Impact of face masks on spectral and cepstral measures of speech: A case study of two Korean voice actors (한국어 스펙트럼과 캡스트럼 측정시 안면마스크의 영향: 남녀 성우 2인 사례 연구)

  • This study intended to verify the effects of face masks on the Korean language in terms of acoustic, aerodynamic, and formant parameters. We chose all types of face masks available in Korea based on filter performance and folding type. Two professional voice actors (a male and a female) with more than 20 years of experience who are native Koreans and speak standard Korean participated in this study as speakers of voice data. Face masks attenuated the high-frequency range, resulting in decreased Vowel Space Area (VSA) and Vowel Articulation Index (VAI)scores and an increased Low-to-High spectral ratio (L/H ratio) in all voice samples. This can result in lower speech intelligibility. However, the degree of increment and decrement was based on the voice characteristics. For female speakers, the Speech Level (SL) and Cepstral Peak Prominence (CPP) increased with increasing face mask thickness. In this study, the presence or filter performance of a face mask was found to affect speech acoustic parameters according to the speech characteristics. Face masks provoked vocal effort when the vocal intensity was not sufficiently strong, or the environment had less reverberance. Further research needs to be conducted on the vocal efforts induced by face masks to overcome acoustic modifications when wearing masks.

  • This study is a fundamental research in the field of home economics education to enforce vocational competencies. It was carried out in the purpose of examining the recent economical and social environmental changes and its management system related to the vocational training in the field of home economics education. It seeks change in direction in relation to the National Competency Standard(NCS) based on revisions in the educational system. The method of study was mostly through reference and data analysis, professional advisory and public hearing. The main research results are as follows. First, the main environmental change factors in relation to vocational training have been integrated to the changes in; population structure, gender related economic activities, generation composition, communications technology, and innovation of living technique. These change factors are forecasting innovations in related industries, lifestyle changes, demand for manpower and changes in capabilities required for each specific profession. Second, according to the analysis of current home economics education training, vocational home educations high school accounts for 9.4% of the total number of specialized high schools, where 8 standard departments are specialized in and characterized into 137 different department names. Despite differences among departments, overall employment rate of graduates were measured 44.7%, which rates above the entrance rate of 41.9%. These numbers show great change since 2010(overall employment rate 16.9%, entrance rate 75.2%), a meaningful outcome resulting from changes in policy from the previous employment-centered education system. Third, NCS based on high school vocational home economics education system revision and investigations in change of direction in vocational home economics, this study attempts to provide background for revision from the development of NCS. It also provides proposals for restructuring division of current classification and departments of home economics education, and propositions for further future research.

Recognition, purchase, and consumption of edible insects in Korean adults (한국 일부 성인의 식용곤충에 대한 인식, 구매 및 섭취 실태)

  • Purpose: This study aims to evaluate the recognition, purchase, and intake status of edible insects in adults, who are the main consumers of edible insects, and to provide the information necessary to expand the production and consumption of edible insects and related products in the future. Methods: A total of 453 adults (172 males and 281 females) aged 19 years and older were surveyed regarding their awareness of edible insects, purchase and consumption experience, and intention to purchase and consume, and the differences between them were analyzed according to gender and age groups. Data collection took place from December 2018 to January 2019. Results: Those who had knowledge of edible insects accounted for 87.0%, whereas those who had more than average knowledge were 75.9%, suggesting recognition was relatively high. Men had more experience than women in purchasing or consuming edible insects and related products (45.9% vs. 31.0%, p < 0.01). In terms of age, experience was 22.5% in 20-30s, 44.6% in 40-50s, and 63.1% in 60s and older, showing significant growth as age increased (p < 0.001). The satisfaction level of edible insects was highest in taste (3.4 points), and especially for shape and appearance, it decreased as age reduced (p < 0.001). The most common reason for not being willing to use edible insects was feeling repulsive (4.1 points), which was significantly more common in women than in men (4.3 vs. 3.9, p < 0.001), and lower with age (p < 0.001). The most needed information display for using edible insects was country of origin (63.8%), the main nutrient in edible insects was protein (93.6%), and the most preferred form of products was powder (39.5%). Conclusion: According to the above results, the recognition of edible insects was high, whereas experience of using edible insects and intention to use edible insects were low especially in women and younger groups. The reason for this is that consumers are dissatisfied with edible insects due to their appearance. This study is expected to be used as basic data for expanding the production and consumption of edible insects and related products in the future.

Comparison of Clinical Characteristics and Polysomnographic Features between Manifest and Latent REM Sleep Behavior Disorders (발현성 렘수면 행동장애와 잠재성 렘수면 행동장애의 임상적 특성 및 수면다원검사 소견 비교)

  • Objective: The purpose of this paper is to study the possible differences in clinical and polysomnographic findings, depending on the presence or absence of subjective complaints of abnormal sleep behavior, in patients with RWA on polysomnography. Method: We reviewed patient records and polysomnographic data of patients referred to the Sleep Laboratory at Seoul National University Hospital from June 1996 through October 2002. We defined the manifest RBD group (n=32) as patients having both complaints of abnormal sleep behavior and RWA on polysomnography. The latent RBD group (n=20) consisted of patients who exhibited RWA on polysomnography but did not complain of abnormal sleep behavior. The clinical characteristics and polysomnographic findings between the two groups were compared and analyzed. Results: Fifty-two subjects had RWA, as detected by polysomnography (42 males and 10 females, mean age of 55.1±19.1years). Subjects in the manifest RBD group were significantly older than those in the latent RBD group (61.59±13.5 vs. 44.70±2.76years, independent t-test, p<0.01). More subjects in the manifest RBD group exhibited abnormal REM behavior on polysomnography than did subjects in the latent RBD group (81.3 vs. 50.0%, Fisher's exact test, p<0.05). No significant differences between the groups were found in the prevalence of brain disorders and primary sleep disorders, gender proportion, and sleep architecture. Conclusion: No difference in sleep architecture was found between the manifest and the latent RBD groups. Only age and the presence of abnormal sleep behavior on polysomnography differentiated the two groups. We suggest that RWA on polysomnography without complaints of abnormal sleep behavior may be early manifestation of manifest RBD. Attention to RWA on polysomnography is necessary to help prevent full-blown RBD from developing.

Estimation of Annual Trends and Environmental Effects on the Racing Records of Jeju Horses (제주마 주파기록에 대한 연도별 추세 및 환경효과 분석)

  • This study was conducted to estimate annual trends and the environmental effects in the racing records of Jeju horses. The Korean Racing Authority (KRA) collected 48,645 observations for 2,167 Jeju horses from 2002 to 2019. Racing records were preprocessed to eliminate errors that occur during the data collection. Racing times were adjusted for comparison between race distances. A stepwise Akaike information criterion (AIC) variable selection method was applied to select appropriate environment variables affecting racing records. The annual improvement of the race time was -0.242 seconds. The model with the lowest AIC value was established when variables were selected in the following order: year, budam classification, jockey ranking, trainer ranking, track condition, weather, age, and gender. The most suitable model was constructed when the jockey ranking and age variables were considered as random effects. Our findings have potential for application as basic data when building models for evaluating genetic abilities of Jeju horses.

Significant Caries(SiC) Index Based on 2009 Korea National Health and Nutrition Examination Survey (2009년 국민건강영양조사에 근거한 Significant Caries(SiC) Index)

  • This study obtained DMFT index(9271 people) in population with more than 6 years old based on data of 2009 Korea National Health and Nutrition Examination Survey, and calculated SiC index(2,517 people) by classifying group with high risk, which is 1/3 in high rank out of it. The aim was to confirm existence of the group with high risk of dental caries, and was to emphasize necessity of preventive strategy for the group with high risk. Statistical analysis was carried out t-test by using STATA 11.0 program. 1. DMFT index accounted for 45.0%(4,174 people) for men and 55.0%(5,097 people) for women. SiC index stood at 2,517 people. Men accounted for 36.3%(914 people). Thus, women were larger with 64.7%(1,603 people). 2. Seeing the mean in DMFT index and SiC index, the more rise in age led to the bigger difference in the mean between two groups. Especially, the difference was shown more than 4 points on the average for 13 and 14 years old and more than double on the average from over 15 years. 3. As a result of comparing gender, DMFT index was higher in women than men from over 14 years old. SiC index wasn't significant. However, women were indicated to be 1 piece more than men at the age of 9 and 14. 4. As a result of comparing by region, both DMFT index and SiC index were indicated to be higher in the rural area for their over 60s. Seeing the above results, the group with high risk is showing the higher rate of experiencing dental caries than DMFT-index in the whole. Thus, the comprehensively & intensively preventive management seems to be likely needed on group with high risk.

Selection of Reference Equations for Lung Volumes and Diffusing Capacity in Korea (우리나라 성인 폐용적 및 폐확산능 정상예측식의 선정)

  • Background: The lung volume and diffusing capacity are influenced by ethnicity. However, there are no equations for predicting the normal lung volume in the adult Korean population, and there is only one equation for diffusing capacity. The aim of this study is to select the most suitable reference equation for the Korean population. Method: 30 men and 33 women at Hanyang University Guri Hospital, and 27 men and 34 women at Asan Medical Center in healthy nonsmoking adults were enrolled in this study. The subject's age, gender, height, weight, lung volume by plethysmography, and diffusing capacity by a single breathing method were obtained. The most suitable equation with the lowest sum of residuals between the observed and predicted values for lung volume and diffusing capacity was selected. Result: At Hanyang University Guri Hospital, the equations with the lowest sum of residuals in the total lung capacity were ECSC's equation in males (sum of residual: 0.04 L) and Crapo/Morris's equation (-1.04) in women. At the Asan Medical Center, the equations with the lowest sum of residuals in the total lung capacity were Goldman/Becklake's equation in males (sum of residual: -2.35) and the ECSC's equation -4.49) in women. The equations with the lowest sum of residuals in the Diffusing capacity were Roca's equation in males (sum of residual: -13.66 ml/min/mmHg) and Park's in women (25.08) in Hanyang University Guri hospital and Park's equation in all cases in the Asan Medical Center (male: -1.65, female: -6.46). Conclusions: Until a reference equstion can be made for healthy Koreans by sampling, ECSC's equation can be used for estimating the lung volume and Park's can be used for estimating the diffusing capacity.
