Associations of Communication Skills, Self-Efficacy on Clinical Performance and Empathy in Trainee Doctors (전공의 의료커뮤니케이션 능력과 진료수행 자기효능감, 공감능력과의 상관관계)

  • Objectives : This study evaluated the medical communication skills of trainee doctors and analyzed the relationship between medical communication skills, self-efficacy on clinical performance (SECP) and empathy. Methods : A total of 106 trainee doctors from a university hospital participated. The questionnaire comprised self-evaluated medical communication skills, modified SECP and the Korean version of the Jefferson Scale of Empathy-Health Professionals version. The mean difference in medical communication skills scores according to gender, age, division (intern, internal medicine group or surgery group) and position (intern, first-/second- and third-/fourth-year residents) were analyzed. Pearson correlation coefficients were determined between medical communication skills, modified SECP and empathy. The effects of each variable on medical communication skills were verified using the structural equation model. Results : There were no statistically significant mean differences in self-evaluated medical communication skills according to gender, age, division or position. Medical communication skills had a significant positive correlation with modified SECP (r=0.782, p<0.001) and empathy (r=0.210, p=0.038). Empathy had a direct effect on modified SECP (β=0.30, p<0.01) and modified SECP had a direct effect on medical communication skills (β=0.80, p<0.001). Empathy indirectly influenced medical communication skills, mediating modified SECP (β=0.26, p<0.05). Conclusions : Medical communication skills are an important core curriculum of residency programs, as they have a direct correlation with SECP, which is needed for successful treatment. Moreover, the medical communication needs a new understanding that is out of empathy.

Prevalence and trends in obesity among Korean children and adolescents in 1997 and 2005 (한국 소아청소년의 비만 유병률 추이: 1997년과 2005년 비교)

  • Purpose : The objective of this study was to provide current estimates of the prevalence and examine trends of overweight and obesity in children and adolescents. Methods : Height and weight measurements from 183,159 (112,974 in 1997, 70,185 in 2005) children and adolescents aged 2 to 18 years were obtained via the 1997 and 2005 National Growth Survey. Obesity among children and adolescents was defined as being at or above the 95th percentile of the gender-specific body mass index (BMI) for age in the 2007 Korean National Growth Charts or a BMI of 25 or higher; overweight was defined as being at or above the 85th percentile to less than the 95th percentile BMI. Results : In 2005, 9.7% (11.3% for boys, 8.0% for girls) of South Korean children and adolescents were obese; 19.0% (19.7% for boys, 18.2% for girls) were overweight or obese. The overall prevalence of obesity increased from 5.8% in 1997 to 9.7% in 2005 (from 6.1% in 1997 to 11.3% in 2005 for boys and from 5.5% in 1997 to 8.0% in 2005 for girls); the increasing trend was most evident in boys, especially those aged 13-18 years. Conclusion : The prevalence of obesity among children and adolescents increased significantly during the eight-year period from 1997 to 2005. This study suggests that we need to make a priority of developing strategies to control obesity in children and adolescents; the potential health effects of increases in obesity are of considerable public health importance.

  • Background: Chronic obstructive pulmonary disease (COPD) is a major cause of chronic morbidity and mortality throughout the world and is the only major disease that is continuing to increase in both prevalence and mortality. The second Korean National Health and Nutrition Survey revealed that the prevalence of COPD in Korean subjects aged 45 years was 17.2% in 2001. Further surveys on the prevalence of COPD were not available until 2007. Here, we report the prevalence of spirometrically detected COPD in Korea, using data from the fourth Korean National Health and Nutrition Survey (KNHANES IV) which was conducted in 2007~2009. Methods: Based on the Korean Statistical Office census that used nationwide stratified random sampling, 10,523 subjects aged 40 years underwent spirometry. Place of residence, levels of education, income, and smoking status, as well as other results from a COPD survey questionnaire were also assessed. Results: The prevalence of COPD (defined as forced expiratory volume in 1 sec/forced vital capacity <0.7 in subjects aged 40 years) was 12.9% (men, 18.7%; women, 7.5%). In total, 96.5% of patients with COPD had mild-to-moderate disease; only 2.5% had been diagnosed by physicians, and only 1.7% had been treated. The independent risk factors for COPD were smoking, advanced age, and male gender. Conclusion: The prevalence of COPD was 12.9% in the KNHANES IV data. Most patients with COPD were undiagnosed and untreated. Based on these results, a strategy for early COPD intervention is warranted in high risk subjects.

Improvements of the Relevant Act for Working of the Marriage Immigrants' Family in Korea (결혼이민자 가족의 국내 취업활동 허용을 위한 관련법 개선방안)

  • To explore the device to allow the marriage immigrants' family to take jobs in our country, the social and economic environments that they face are investigated and analyzed through literature survey and field inquiry. The principal motive of the marriage to Korean is economic problem. But their actual economic conditions are inferior, and other legal problems, such as getting jobs and remitting money to their home country, drive them to unstable status. The present hiring policy is applied only to foreign workers with no domestic relations(E-9), hence the marriage immigrants are excluded from the domestic employment. To make institutional devices for giving them the employment opportunity, the modification of the existing laws are proposed. For examples, the 'Act on employment of foreign workers' can be revised to permit immigrant's relatives to get jobs, and 'Immigration control law' can be amended to guarantee legal qualification for taking jobs. It is desirable that the overall control be made by the Prime Minister's Office, and operation as well as surveillance be performed by the Ministry of Gender Equality and Family (MGEF) and the Ministry of Employment and Labor (MEL).

The Relationship of Ego Resilience and involvement with the Experience of Anxiety and Perceived Stress in Nursing students (간호대학생의 불안 및 지각된 스트레스 경험과 자아탄력성과의 관계)

  • This study was conducted to identify the relationship between ego-resilience, anxiety and perceived stress. The participants were 500 students of nursing college located in K city, Korea. Data were collected from September, 9th to 14th 2013, and were analyzed by t-test, ANOVA, Pearson correlation coefficients. The result were as follows; 1) The mean scores were ego-resilience 3.36(0.50), anxiety 2.26(0.70) and stress 2.55(0.55). 2) Ego-resilience was significantly different according to gender, age, satisfaction for major, subjective career opportunity, subjective health. The score were higher at the male group than the female group, and the group who aged over 24 were higher than the group aged below 23. 3) Significant correlations were found between ego-resilience, anxiety and stress. Ego-resilience has negative relationship with level of anxiety(r=- .383, p<0.000) and perceived stress(r=- .454, p<0.000). While, the positive relationship were significant(r= .685, p<0.000) between level of anxiety and perceived stress. According to these result, ego-resilience promotion program is need to develop as a positive concept to adjust college life and nursing course.

A FAMILY-BASED AND CASE-CONTROL ASSOCIATION STUDY OF THE DOPAMINE BETA HYDROXYLASE GENE POLYMORPHISM IN ATTENTION DEFICIT HYPERACTIVITY DISORDER (한국인 주의력결핍 과잉행동장애와 Dopamine Beta Hydroxylase 유전자의 관련성 : 가족기반 연구 및 환자-대조군 연구)

  • Objective : Attention deficit hyperactivity disorder (ADHD) is the most common childhood psychiatric disorder, affecting 35% of school age children. Although the biological basis of ADHD is unknown, family studies provide strong evidence that ADHD has a genetic basis. Recent genetic studies have suggested associations between ADHD and Taq I polymorphism of dopamine beta hydroxylase gene(DBH) . The aim of this study is to test the association between ADHD and Taq I polymorphism of DBH in Korean population. Method : We processed DNA extraction and genotyping for 106 korean children with ADHD and their parents. Genotyping was additionally performed for 212 age and gender matched normal controls. Case-control association study was applied. And we tested the association using the transmission disequilibrium test (TDT) and haplotype-based haplotype relative risk test (HHRR). Results : There were no statistical differences of genotype distributions between cases and controls. However, we did observe preferential transmission of allele Al of DBH Taq T polymorphism in ADHD. Conclusion : On the whole, our results lend credence to the notion that the relationship between ADHD and DBH is complex. The number of cases and informative transmissions were small, therefore it would be premature to make any conclusions from our study concerning the role of DBH in ADHD. Further work is needed to support these findings.

The Influence of the Clinical Nurses' Emotional Labor and Resourcefulness on the Turnover Intention (병원간호사의 감정노동과 자원동원성이 이직 의도에 미치는 영향)

  • This is predictive correlational study to identify the influence of emotional labor and resourcefulness on turnover intention among clinical nurses. The participants were 138 clinical nurses and the data were collected by an online survey using a self-administered questionnaire from 10th to 17th April, 2016.The collected data were analyzed using SPSS program through t-test, One-way ANOVA, Scheffe's test and multiple linear regression. In the results, there were differences in emotional labor by age, marital status, job position, clinical career, shift work, in resourcefulness by gender, clinical career, and in turnover intention by age. As a result of multiple linear regression, emotional labor and resourcefulness were selected as significant related variables affecting nurse's turnover intentions. These factors accounted for 3.7% of turnover intention, which necessitates the consideration of a specific plan to reduce emotional labor and increase resourcefulness for decreasing clinical nurse's turnover intention.

Perception related to the Healthcare Accreditation Effects on Turnover Intention and Job Stress of employees' Quality Improvement Department (QI부서근무자들의 의료기관인증제관련 인식이 직무스트레스 및 이직의도에 미치는 영향)

  • The purpose of this study was to examine, among quality improvement (QI) department employees, the effects of perception of the healthcare accreditation on job stress and to identify factors affecting turnover intention. Research data was collected between 1 June and 15 June 2016. The sample included QI department employees who had participated in a healthcare accreditation survey. The collected data were analyzed to obtain frequency, percentage, mean and standard deviation results, which were assessed by using Student's t-test, ANOVA, and multiple regression analysis. The scores for QI employees' perceptions of the healthcare accreditation, job stress, and turnover intention were 3.35, 3.66, and 3.32 of the perception by general characteristics. There were significant differences between scores according to gender, age, QI department section, position, and job satisfaction. In addition, job stress and turnover intention scores showed significant differences according to position, age, QI department section, and job satisfaction. A positive correlation was observed between healthcare perception and job stress, while turnover intention was negatively correlated with healthcare perception. The results indicate that the perception of healthcare accreditation positively affects job stress; in contrast, it decreases the number of people intending to change departments. Among the sample of lower level workers, the younger age workers who were employed for more than 10 years expressed lower satisfaction with their job, which indicates that they are affected by their notion of healthcare accreditation more than by other factors. Thus, in order to lessen work stress and employees' thoughts of leaving the department, the QI department should be encouraged to lessen job stress and provide various supports to the employees.

How Do the Journalists Perceive the Civic Journalism? (시민저널리즘에 대한 언론인 인식유형 연구)

  • Nowadays, newspaper industry faces a crisis in terms of two aspects: First, in this multi-channel era, it has competed with the online journalism appearing in many digital media. Second, with the decrease of numbers of readers and political participation, the roles of newspaper has to be changed accordingly. The concept of civic journalism emerged as a solution in this context. The purpose of the study is to explore the factors in how journalists perceive the civic journalism. The Q-methodology was adapted to identify the factors composing of the belief, values, attitudes, ideas, and opinions toward the civic journalism. Total of 24 Q-sample (statements) were sorted by 27 P-sample (journalists) who were selected by their gender, years of service, and the like. The Quanl program finally produced 3 factors that were interpreted and named as follows: Factor I, the Complimentary Journalism, the civic journalism would fulfill the functions that traditional journalism has neglected. Factor II, the Alternative Journalism inducing the citizen's participation in the public areas. Factor III, the Functional Journalism, emphasizing dysfunction of the civic journalism.

The Application of Species Richness Estimators and Species Accumulation Curves to Traditional Ethnobotanical Knowledges in South Korea (남한지역 전통민속식물지식 자료를 활용한 종누적곡선 분석 및 종풍부도 추정 연구)

  • Under circumstances of rapid disappearing of traditional ethnobotanical knowledge, traditional ethnobotanical knowledge surveys are the major step in documenting useful species with a conservation priority. In the ethnobotanical research, the relevance to the survey intensity, ethnobotanical information and plant species richness is the most important research theme. We made up TEK database in south Korea using metadata which had been published by the Korea National Arboretum. We calculated species richness using species richness estimator like ACE, Chao1, Chao2, ICE, Jack 1, Jack 2, and Bootstrap. Species accumulation curves showed each province sampling efforts appeared to be wide range of variance so that Gangwon province need more sampling efforts, and Chungnam province approached a horizontal asymptote earlier. We found heterogeneous patterns in the rarefaction curves of TEK species between gender for each categories of use (medicinal, food and handicrafts). Comparing with regional floral diversities, it was predicted that more diverse species would be found in some provinces by carrying out additional survey.
