Review on the Changes of Liver Function Parameters after Administration of Herbal Medicine (간기능 정상자 또는 불특정 환자를 대상으로 한 한약 복용 후 간기능 지표 변화에 대한 문헌적 고찰)

  • The aim of this study is to analyze literatures researching changes of liver function parameters after administration of herbal medicine and the safety of herbal medicine on hepato-biliary system. Eight Korean databases were searched for researches on the safety of oriental herbal medicine. According to inclusion criteria, nineteen studies were selected and analyzed. As a result, seventy outcomes in five parameters(aspartate transaminase, alanine transaminase, gamma(${\gamma}$)-glutamyl transferase, alkaline Phosphatase, total bilirubin) were obtained. After administration, parameters decreased or sustained in fifty seven outcomes. In thirteen outcomes, parameters slightly increased. In two outcomes, parameters increased but not severely. The change of liver function parameters after administration of herbal medicine is expected to be harmless. But it requires additional large scale studies and modification in method to corroborate the safety of herbal medicine.

Comparing Biological Markers of Alcohol Dependence in the Patients of an Alcohol Counseling Center (알코올 상담 센터 환자를 대상으로 한 알코올 의존의 생물학적 표지자 비교 연구)

  • Objectives : Biochemical markers can provide an objective evidence of heavy alcohol drinking. The purpose of this study was to compare and evaluate the usefulness of biological markers detecting alcohol dependence, such as mean corpuscular volume(MCV), gamma-glutamyl transferase(GGT), and carbohydrate-deficient transferrin(CDT) in the patients of an alcohol counseling center. Methods : This study was done with 64 patients with alcohol dependence and 36 healthy subjects. Relative values(%) of CDT were determined in their sera with turbidimetric immunoassay(Bio-Rad %CDT assay, Axis-Shield ASA, Oslo, Norway), and were compared with conventional markers of alcohol consumption, GGT and MCV. Results : Among the patients with alcohol dependence, 78.1% showed abnormal %CDT levels compared with GGT(61.9%) and MCV(20.7%). The areas under the receiver operating characteristic(ROC) curves(95% confidence interval) for %CDT, GGT, and MCV were 0.934(0.866-0.973), 0.871(0.789-0.930), and 0.575 (0.472-0.673), respectively. Conclusion : %CDT seems to be the most reliable biological marker for the detection and monitoring of alcohol consumption in the patients with alcohol dependence of the alcohol counseling center.

A Cross-sectional Study on the Risk Factors Related to Fatty Liver (지방간의 위험요인에 관한 단면적 연구)

  • Generally fatty liver is attributed either to chronic alcoholism, diabetes mellitus, or obesity. Based upon this commonly held clinical brief, this study was conducted to investigate the contributing factors of fatty liver and odds ratio (OR) of known contributing factors. A sample of 310 male participants, who visited at Seoul Paik Automated Multiphasic Health Testing System from November 1991 to December 1991, was separated into 112 cases and 198 controls by ultrasonographic fin ding. There were statistically significant difference between fatty liver and normal in triglyceride (TG), body mass index (BMI), alanine aminotransferase (ALT), high density lipoprotein cholesterol (HDL-C), fasting blood sugar (FBS), alcohol consumption, low density lipoprotein cholesterol (LDL-C), total cholesterol, gamma-glutamyl transferase ($\gamma$-GT), duration of alcohol intake and alkaline phosphatase (Alk.P)(P<0.01, P<0.05). The statistically significant elevated odds ratio were noted for TG (4.48, confidence interval (CI) 2.66-7.55, P=0.000), alcohol consumption(3.24, CI 1.56-6.23, P=0.002), BMI(3.05, CI 1.87-4.97, P=0.000), and FBS (2.59, CI 1.53-4.40, P=0,000). In summary, it is suggested that the fatty liver could be preventive by avoiding such deleterious factors as high fat diet, alcohol and obesity.

  • To improve wellness and quality of life by recognizing the health efforts of stress, the author estimated the relationships between stress, subjective symptoms and clinical diagnosis through a questionnaire and a battery of specified laboratory tests; electrocardiography, blood pressure, cholesterol, aspartate aminotransferase(AST), alanine aminotransferase(ALT), gamma glutamyl transferase$(\gamma-GTP)$, fasting blood sugar, gastro-endoscopy or UGI, abdominal sonography, etc. The data was gathered from 337 clients who were undergoing multiphasic screening program at a University Hospital from January to March 1998. The mean age of subjects was $46.5{\pm}11.2$ years and the mean of body mass index was $24.0{\pm}3.7kg/m^2$. The mean vol of stress was $18.5{\pm}6.0$ expressed as the score out of 40. By general characteristics and lift style among male, mean level of stress was significantly higher in case of lower socioeconomic status, habitual drug use, longer daily working time(>10 hours), no regular exercise, drinkers, irregular meal, skip-ping breakfast(p<0.05). In case of female, that was significantly higher in case of lower education, lowe. socioeconomic status, longer daily working time(>10 hours), no regular exercise, drinkers, smokers, irregular meal, skipping breakfast(p<0.05). Significant correlations were observed between stress and subjective symptoms in all kinds of organ system (p<0.01). Correlation coefficients of stress among male were relatively high with neuro-psychiatric symptom$(\gamma=0.476)$ and cardio-vascular symptom$(\gamma=0.361)$ in order, and correlation coefficients of stress among female was highest with neuro-psychiatric symptom$(\gamma=0.371)$. The prevalence of the diagnosis through the battery of laboratory tests was high in upper gastrointestinal disorders and hypercholesterolemia in order in both sex group. Among male the mean score of stress was significantly high in ulcerative peptic disorder of upper gastrointestine and hepatopathy in order (p<0.05) . Among female that was significantly high in diabetes mellitus. In summary, it is likely that there are associations between stress, subjective symptoms and clinical diagnosis. To promote wellness and quality of life it would be of value that periodic stress evaluation program and stress management including apropriate control of smoking and drinking, regular exercise and meal.

The effect of duck meat treated with turmeric powder intake on the postprandial blood lipid profiles in female university students (울금 첨가 오리가공육 섭취가 여대생의 식후 혈중 지질 농도에 미치는 영향)

  • Purpose: This study examined the effects of duck meats with turmeric powder on blood lipids in 10 female university students. Methods: The subjects received duck meat with 0%, 0.1%, 0.2%, and 0.4% turmeric powder and glucose, total cholesterol, triglyceride (TG), high-density lipoprotein (HDL)-cholesterol, and low-density lipoprotein (LDL)-cholesterol in their serums after 30, 60, 90, 120, and 180 min were measured. Results: The average height, weight, and body mass index of subjects were $159.6{\pm}2.6cm$, $51.3{\pm}3.5kg$, and $20.1{\pm}1.0$, respectively. The fasting glucose, ${\gamma}-glutamyl$ transferase (GGT), glutamic pyruvic transferase (GPT), glutamic oxaloacetic transferase (GOT), c-reactive protein (CRP), and hemoglobin were within the normal range. The ${\Delta}-AUC$ (area under the curve) of postprandial glucose, TG did not change, but ${\Delta}-AUC$ of postprandial total cholesterol and LDL were significantly decreased, and HDL was increased by intake of the duck meat with turmeric powder. Conclusion: This study shows that duck meats with turmeric powder affected the postprandial blood lipid levels.

A Study on the Changes of Blood Mineral and Enzyme Activity During Growth Period of Velvet Antler in Korean Spotted Deer(Cervus Nippon) (꽃사슴의 녹용 성장기간 중 혈액 내 미네랄 및 효소활성 변화에 관한 연구)

  • This study was aimed at investigating the change of blood mineral and enzyme activity during growth period of velvet antler in Korean spotted deer. Samples of blood, obtained from the jugular vein of twenty-five deer(4 to 6 year-old males), were taken in 10 days interval from just after casting to 50 days. Deer were randomly selected from the farm, and samples were analyzed for blood parameters like mineral concentration and enzyme activities. No significant differences found in calcium and phosphorus concentration in blood whereas sodium, potassium and chloride concentration were significantly changed with antler growth. There were no significant differences in alkaline phosphatase, aspartate aminotransferase, alanine aminotransferase, $\gamma$-glutamyl transferase and lactate dehydrogenase during growth of antler, but alkaline phosphatase concentration was increased with growth of antler, and the highest concentration was obtained on the 50 days after casting. Creatine kinase and lactate dehydrogenase activities for the deer tested in this experiment were higher than those of other animals.

Correlation of Glasgow Prognostic Score or Procalcitonin to Clinical Variables in Patients with Pretreatment Lung Cancer

  • Unfortunately, the five-year survival rate of lung cancer is relatively low compared with other cancers. Therefore, better predictors are need for prognosis, therapeutic strategy, risk stratification and predicting long-term mortality of lung cancer. Recently, increasing data suggest that Glasgow Prognostic Score (GPS) and procalcitonin levels are useful predictor cancer prognosis. In this study, we retrospectively investigated the correlation of GPS or procalcitonin to clinical variables in patients with pretreatment lung cancer. In 135 patients with pretreatment lung cancer, GPS, procalcitonin, demographic characteristics, hematological, coagulation, biochemical, inflammatory and cardiac markers were measured. Monocyte, eosinophil, basophil, neutrophil to lymphocyte ratio, red cell distribution width (RDW), platelet to lymphocyte ratio, mean platelet volume to platecrit ratio, D-dimer and prothrombin time (PT) levels were higher, whereas mean platelet volume was lower than their normal ranges. Glucose and sodium levels were low, whereas gamma glutamyl transferase (GGT), total bilirubin, creatinine and inorganic phosphorus concentrations were increase compared their normal ranges. Procalcitonin, high sensitivity C-reactive protein and troponin-I concentrations were elevated compared with their normal ranges. GPS had significantly positive or negative relations to cancer stage, hematological, coagulation, biochemical, inflammatory and troponin-I. Based on the data, we suggest that GPS may be a potent and useful predictor for prognosis, therapeutic strategy, risk stratification and predicting long-term mortality of lung cancer.

A Case Report of the Efficacy of Saenggan-tang-gagam in the Treatment of a Patient with Chronic Hepatitis B Who Showed Conversion of the Immune-Tolerant Phase to Immune-Clearance Phase during Hospitalization for Intracerebral Hemorrhage (뇌내출혈로 입원치료 받던 중 만성B형간염 면역관용기에서 면역활동기로 전환된 환자에 대한 생간탕가감 치험 1례)

  • Objectives: The purpose of this case report is to describe the efficacy of Saenggan-tang-gagam treatment in a patient with immune-tolerant chronic hepatitis B who converted to the immune active phase during hospitalization for Intracerebral hemorrhage. Methods: A 61-year-old male patient with chronic hepatitis B was hospitalized from July 21, 2016 to October 24, 2016. On September 5, the patient showed fatigue and dyspepsia, and his urine was dark. These symptoms were accompanied by a dramatic elevation of aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatase (ALP), and gamma-glutamyl transferase (GGT) levels. Saenggan-tang-gagam was administered three times a day until the patient was discharged from the hospital. Results: After the Saenggan-tang-gagam treatment, AST and ALP levels decreased significantly, below the upper limit level. In addition, the levels of ALT and GG were significantly decreased. In addition, the fatigue, dyspepsia, and dark urine induced by hepatitis improved after the Saenggan-tang-gagam treatment. Conclusion: This case report suggests that traditional Korean medicine has a beneficial effect on the immune active phase of chronic hepatitis B.

Renal Tubular Carcinoma in a Cocker Spaniel Dog (코커 스파니엘에서 신장세포암종)

  • A 7-year-old neutered male Cocker spaniel was referred to Veterinary Medical Teaching Hospital, Kyungpook National University with a left abdominal mass, anorexia and weight loss. Hematologic test and biochemical panel showed severe leukocytosis, mild anemia, mild hypoalbuminemia and a marked elevation in gamma glutamyl transferase (GGT). Radiography demonstrated a large mass on the left kidney and there was no evidence of pulmonary metastases. Histopathologic features of the mass were consistent with renal tubular carcinoma. The dog was treated with unilateral nephrectomy. During the follow-up, the dog died 6 months after surgery.

Correlation between Arterial Stiffness and Physiological Parameters (동맥경화도와 생리학적 변수들 간의 상관관계)

  • Arterial stiffness(AS) is an important pathologic state of vascular injury. This study was carried out to elucidate the effect of physiological variables on brachial-ankle pulse wave velocity(BAPWV), index of AS. Four hundred adults(volunteers) participated in this study. Body indices, biochemical, cardiac and inflammatory markers, and right(Rt)- and left(Lt)-BAPWV were measured. Body mass index(BMI), Rt- and Lt-BAPWV, glucose, triglyceride, alkaline phosphatase(ALP), gamma-glutamyl transferase(GGT), creatinine, uric acid, troponin-I(TNI), NT-proBNP and high sensitivity C-reactive protein(hs-CRP) levels were higher than the reference value of each variable. Rt- and Lt-BAPWV were directly correlated with age, body weight, BMI, glucose, ketone, aspartate aminotransferase, alanine aminotransferase, ALP, GGT, total cholesterol, low density lipoprotein, lipoprotein(a), apolipoprotein-B, blood urea nitrogen, heart rate, TNI, creatine kinase, CK-MB, lactic dehydrogenase, myoglobin, hs-CRP, lipase, reumatoid factor, fibrinogen and D-dimer (P<0.05, P<0.01, P<0.001 or P<0.000, respectively), but inversely associated with total bilirubin, uric acid, apolipoprotein-A1 and GFR (P<0.05). These observations suggest that a variety of physiological variables may influence BAPWV, resulting in increased risk or prevention of cardiovascular and/or cerebrovascular attacks. Therefore, physiological variables affecting BAPWV should be regularly controlled.
