Many daily activities require people to complete a motor task while walking. Substantial gait decrements during simultaneous attention to a variety of cognitive tasks have been shown by a group of severely injured neurological patients of mixed etiology. And previous studies have shown that the attentional load of a walking-associated task increased with its level of difficulty. The purpose of this study was to analyze subjects' gait changes are affected by the effects of arithmetic task difficulty and performance level. Participants performed a walking task alone, three different Arithmetic tasks while seated, and among them, two kinds of the simillar Arithmetic tasks in combination with walking. Reaction time and accuracy were recorded for two of the Arithmetic tasks. The mean values of the gait were measured using a Timed Up and Go test among 11 with post-stroke patients while walking with and without forward counting (WFC) and backward counting(WBC).There was significant Arithmetic Task Difficulty level between the 10-forward counting task condition(FC) and the 10-backward counting task condition(BC)(p=0.008). The mean values of T.U.G time were significantly higher under backward counting dual-task condition than during a simple walking task(p=0.009) and WFC(p=0.009). The change in T.U.G time during WFC was higher when compared with the change during a simple walking, but there was no significant difference (p=0.246). This study suggesting that a high interference could be linked with a high level of difficulty, whereas adaptive task enabled participants to perfectly share their attention between two concurrent tasks. Future research should determine whether dual task training can reduce gait decrements in dual task situations in people after stroke. And the dual-task-based exercise program is feasible and beneficial for improving walking ability in subjects with stroke.
Background: The Functional Gait Assessment (FGA) was developed to measure of gait-related activities. The FGA was translated in Korean but only a few psychometric characteristics had been studied. Objects: The purpose of this study was to evaluate the validity and reliability of the Korean version of FGA scale using Rasch analysis. Methods: The study included 120 patients with stroke (age range=30~83 years; mean${\pm}$standard deviation=$58.3{\pm}11.1$). The FGA and Berg Balance Scale were performed, and were analysed for dimensionality of the scale, item difficulty, scale reliability and separation, and item-person map using Rasch analysis. Results: The 4 rating scale categories of FGA were satisfied with optimal rating scale criteria. The most items of the FGA showed sound item psychometric properties except 2 items ('gait with the horizontal head turns', and 'gait with narrow base of support'), and the 2 misfit items were excluded for all further analyses. The 8 items were arranged in order of difficulty. The most difficult item was 'gait with eyes closed', the middle difficult item was 'gait level surface', and the easiest item was 'gait with vertical head turns.' A person separation reliability was .93 and the person separation index was 3.57. Conclusion: This study suggests that the 8-item Korean FGA are valid measure of assess the gait-related balance performance, and to set the goal of rehabilitation plan in patient with stroke.

Purpose: This study aimed to investigate spatial and temporal features of motor control in an individual with hemiparesis during the curvilinear gait (CG) and proposed an exercise guideline. Research design, data and methodology: An individual aged 63 with hemiparesis by stroke disease was participated in the study. Autoencoder (AE) was used to extract four motor modules from eight muscle activities of the paretic leg during CG. After extraction, each module of four modules was operationally defined by numbering from M1 to M4 according to spatial and temporal features and compared with results reported in a previous study. Results: As a result, an individual with hemiparesis had motor module problems related to difficulty of weight acceptance (module 1), compensation for the weakness of ankle plantar flexor (module 2), a spastic synergistic pattern (module 3) and difficulty with transition from the swing to stance phase (module 4) in terms of spatial features. Also, a delayed activation timing of temporal motor module (module 2) related to the forward propulsion during CG was observed. Conclusions: Gait rehabilitation for the stroke will need to consider clinical significances in respect of the deterioration of motor module and provide the tailored approaches for each gait phase.
The Dynamic Gait Index (DGI) was developed and widely used as a clinical tool to assess balance performance during gait. The purpose of this study was to validate the Korean translated DGI using Rasch analysis. A total of 105 community-dwelling elderly was participated in this study (age range = 65~95 years; mean = 78.0 years). The translated DGI showed sound item psychometric properties, and the 8 items were arranged in order of difficulty for the total participants. The most difficult item was 'Steps' and the easiest item was 'Level surface'. Also, each of the original 4 rating scale categories satisfied the Linacre's essential criteria suggestions for optimal rating scale category effectiveness. Although, thirty eight person (36.2%) showed the maximal high score, but the most of them was no history of fall in the preceding year. For subjects who has falling history, all of the person's ability was arranged within the item's difficulty. The 8-item Korean translated DGI can be used to measure gain in elderly person with balance disorders without compromising important clinical measurement characteristics in Korea.
Purpose: This study suggests observational gait analysis (OGA) strategies for therapists with limited experience in clinics in evaluating patients with stroke during walking. Methods: The proposed model was the mnemonic STEP APP, whose initials refer to the process of OGA for clinical reasoning and decision-making by identifying problems during walking in patients with stroke. Results: STEP APP stands for step (S), tibia (T), events (E), phases (P), ankle (A), problems (P), and priority (P). It serves as a procedural guide for OGA in patients with stroke walking on the ground. Conclusion: This review suggests a simple evaluation of gait using OGA that can be used by therapists who have less experience and difficulty in evaluating patients with stroke during walking. However, it is important to consider the front and back views of motion as well as motion in the transverse plane in order to analyze problems accurately. Furthermore, small joint problems, such as those in the foot, should be considered when evaluating patients with stroke during walking.

A necessity of remote control robots or various searching robots etc. that accomplish works given instead of human under long distance and extreme environment such as volcano, universe, deep-sea exploration and nuclear power plant etc. is increasing, and so the development and the research regarding these mobile robots are actively progressing. The wheel mobile robot or the track mobile robot have a sufficient energy efficiency under this en, but also have a lot of limits to accomplish works given which are caused from the restriction of mobile ability. Therefore, recently many researches for the walking robot with superior mobility and energy efficiency on the terrain, which is uneven or where obstacles, inclination and stairways exist, have been doing. The research for these walking robots is separated into fields of mechanism and control system, gait research, circumference environment and system condition recognition etc. greatly. It is a research field that the gait research among these is the centralist in actual implementation of walking robot unlike different mobile robots. A research field for gait of walking robot is classified into two parts according to the nature of the stability and the walking speed, static gait or dynamic gait. While the speed of a static gait is lower than that of a dynamic gait, a static gait which moves the robot to maintain a static stability guarantees a superior stability relatively. A dynamic gait, which make the robot walk controlling the instability caused by the gravity during the two leg supporting period and so maintaining the stability of the robot body spontaneously, is suitable for high speed walking but has a relatively low stability and a difficulty in implementation compared with a static gait. The quadruped walking robot has a strong point that can embody these gaits together. In this research, we will develope an autonomous quadruped robot with an asaptibility to the environment by selectry appropriate gait, element such as duty factor, stride, trajectory, etc.

PURPOSE: This study aimed to provide supporting data for the clinical use of breathing exercise with a WBV stimulation as a sustained and safe intervention program, by examining the effect of breathing exercise with WBV stimulation on the pulmonary function, gait ability, and life quality in patients with severe chronic obstructive pulmonary disease (COPD), who have difficulty performing exercise. METHODS: For this study, after collecting the samples from 20 patients with severe COPD, they were placed randomly in an experimental group to perform breathing exercises with a WBV stimulation (n = 10) and a control group to perform breathing exercises only (n=10). Before the intervention, pulmonary function, six-minute gait distance, and health-related life quality were measured as pre-tests. After applying the intervention program to the patients for 30 minutes once a day, for four days a week, for six weeks, the post-test items were remeasured in the same way as the pre-tests, and the results were analyzed. RESULTS: In the within-group comparison, both the experimental and control groups showed significant differences in the forced expiratory volume in one second, six-minute gait distance, and health-related life quality (p < .01) (p < .05). In the intergroup comparison, there were significant differences in the forced expiratory volume in one second and the six-minute gait distance (p < .05). CONCLUSION: WBV stimulation was more effective for the patients by improving the muscular strength and muscular endurance through the reflexive contraction of muscles, and increasing the exercise tolerance. This result could serve as an alternative means to clinically improve the physical function of patients with severe COPD, who have difficulty performing breathing exercises in the future.
Objectives: The purpose of this study was to investigate the effects of Korean Medicine on postural instability and gait difficulty(PIGD) in patient with parkinsonism. Methods: We reviewed the charts of patients with parkinsonism who were admitted to the department of Korean internal medicine, stroke and neurological disorders center, Kyung Hee University Hospital at Gangdong from October 2009 to May 2017. We analyzed the data of UPDRS (Unified Parkinson's disease rating scale) and calculated PIGD related UPDRS before and after the admission. Results: The average of PIGD score for the 23 patients of Parkinsonism significantly decreased from $8.35{\pm}5.13$ to $4.52{\pm}3.68$ after treatment. The average of PIGD score for the 16 patients of idiopathic parkinson's disease and 7 patients of atypical Parkinsonism decreased from $9.13{\pm}4.46$ to $4.63{\pm}4.15$ and from $6.57{\pm}6.45$ to $4.29{\pm}2.56$ after treatment, respectively. Conclusions: These results provide that Korean Medicine has an effect on the motor function of patients who suffer from Parkinsonism with Postural Instability and Gait Difficulty. Furthermore it could be effective for improving activities of daily life.

Recently, technologies to help the elderly or disabled people who have difficulty in walking are being developed. In order to develop these technologies, it is necessary to construct a system that gathers the gait data of people and analysis of these data is also important. In this research, we constructed the development of sensor system which consists of pressure sensor, three-axis accelerometer and two-axis gyro sensor. We used k-means clustering algorithm to classify the data for characterization, and then calculated the symmetry index with histogram which was produced from each cluster. We collected gait data from sensors attached on two subjects. The experiment was conducted for two kinds of gait status. One is walking with normal gait; the other is walking with abnormal gait (abnormal gait means that the subject walks by dragging the right leg intentionally). With the result from the analysis of acceleration component, we were able to confirm that the analysis technique of this data could be used to determine gait symmetry. In addition, by adding gyro components in the analysis, we could find that the symmetry index was appropriate to express symmetry better.
Objectives : Postural instability and gait difficulty(PIGD) can develop in all Parkinson's disease patients, especially late in the disease course, but does not respond well to conventional dopamine treatment. This study aimed to report three cases of PIGD in Parkinson's disease patients treated with Korean and Western medicine treatment. Methods : We used acupuncture, bee venom acupuncture, herbal medicine and moxibustion to treat patients during hospitalization. They continued Western medication and received rehabilitation treatment. We observed the changes of PIGD using the Unified Parkinson's Disease Rating Scale(UPDRS). Results : After treatment, PIGD symptoms improved and UPDRS scores were decreased. Especially, scores of falling, walking, gait and postural stability subsections related with PIGD were decreased. In addition, general conditions of patients were improved. Conclusions : This study suggests that Korean medicine treatment could be effective in the treatment of PIGD in Parkinson's disease patients.
