Analysis of the Prevalence of Taurodont Deciduous Molars in Children (어린이의 유구치에서 우상치의 유병률 분석)

  • Taurodontism is an anomaly characterized by a long and broad pulpal cavity and consumed apical location of the furcation area. This study aimed to determine the prevalence of taurodontism in deciduous molars based on digital panoramic radiographs of children. The study was performed on a sample of panoramic radiographs taken from 2,473 Korean children who visited the department of pediatric dentistry, Yonsei University Dental Hospital between Nov. 2005 and Mar. 2018. Taurodontism was mainly categorized by Daito's method. Using panoramic radiographs, taurodontism was also categorized into mesotaurodontism, hypotaurodontism, and hypertaurodontism. Mesotaurodontism was the most prevalent type. A total of 2,473 panoramic radiographs were evaluated. The prevalence of taurodontism was 5.7% in general, 51.5% in the left quadrant, 48.5% in the right quadrant, 6.3% in the maxilla, and 93.7% in the mandible. The distribution of taurodontism stratified by gender showed a higher prevalence in males. This is a comprehensive study on the prevalence of taurodontism in children using the largest sample size to date.

Panoramic radiography can be an effective diagnostic tool adjunctive to oral examinations in the national health checkup program

  • Purpose: We investigated correlations between the findings of oral examinations and panoramic radiography in order to determine the efficacy of using panoramic radiographs in screening examinations. Methods: This study included patients who visited dental clinics at National Health Insurance Service (NHIS) Ilsan Hospital for checkups during 2009-2015 and underwent panoramic radiographic examinations within 1 year prior to the oral examinations. Among the 48,006 patients who received checkups, 1,091 were included in this study. The data were evaluated using the Cohen kappa and interrater agreement coefficients. Accuracy, sensitivity, and specificity were calculated using data from the panoramic radiographs as true positive diagnoses. Results: The interrater agreement coefficient for occlusal caries was 28.8%, and the Cohen kappa coefficient was 0.043 between the oral and panoramic radiographic examinations. Root caries and subgingival calculus were only found on the radiographs, while gingival inflammation was found only by the oral examinations. The oral examinations had a specificity for detecting occlusal dental caries of 100%, while their sensitivity for proximal dental caries and supragingival calculus was extremely low (14.0% and 18.3%, respectively) compared to the panoramic radiographic examinations. The oral examinations showed a relatively low sensitivity of 66.2% and a specificity of 43.7% in detecting tooth loss compared with panoramic radiography. Conclusions: Panoramic radiography can provide information that is difficult to obtain in oral examinations, such as root caries, furcation involvement, and subgingival calculus, which are factors that can directly affect the survival rate of teeth. It therefore seems reasonable and necessary to add panoramic radiography to large-scale health checkup programs such as that provided by the NHIS.

Three-dimensional analysis of the distal movement of maxillary 1st molars in patients fitted with mini-implant-aided trans-palatal arches

  • Objective: The aim of this study was to investigate three-dimensional molar displacement after distalization via miniscrews and a horizontal modification of the trans-palatal-arch (TPA). Methods: The subjects in this clinical trial were 26 Class II patients. After the preparation of a complete set of diagnostic records, miniscrews were inserted between the maxillary 2nd premolar and 1st molar on the palatal side. Elastic modules connected to the TPA exerting an average force of 150-200 g/side parallel to the occlusal plane were applied. Cone-beam computed tomography was utilized to evaluate the position of the miniscrews relative to the adjacent teeth and maxillary sinus, and the direction of force relative to molar furcation. The distances from the central point of the incisive papilla to the mesiopalatal cusps of the 1st maxillary molars and the distances between the mesiopalatal cusps of the left and right molars were measured to evaluate displacement of the maxillary molars on the horizontal plane. Interocclusal space was used to evaluate vertical changes. Results: Mean maxillary 1st molar distalization was $2.3{\pm}1.1mm$, at a rate of $0.4{\pm}0.2mm/month$, and rotation was not significant. Intermolar width increased by $2.9{\pm}1.8mm$. Molars were intruded relative to the neighboring teeth, from 0.1 to 0.8 mm. Conclusions: Distalization of molars was possible without extrusion, using the appliance investigated. The intrusive component of force reduced the rate of distal movement.

Positional relationship between the maxillary sinus floor and the apex of the maxillary first molar using cone beam computed tomograph (Cone beam형 전산화단층영상을 이용한 상악동저와 상악제1대구치 치근단괴의 위치관계)

  • Purpose: To assess the positional relationship between the maxillary sinus floor and the apex of the maxillary first molar using cone beam computed tomograph (CBCT). Materials and Methods: CBCTs from 127 subjects were analysed. A total of 134 maxillary first molars were classified according to their vertical and horizontal positional relationship to the maxillary sinus floor and measured according to the distance between the maxillary sinus floor and the maxillary first molar. Results: Type III (The root projected laterally on the sinus cavity but its apex is outside the sinus boundaries) was dominated between 10 and 19 years and type I (The root apex was not in contact with the cortical borders of the sinus) was dominated (P<0.05) between 20 and 72 years on the vertical relationship between the maxillary sinus floor and the apex of the maxillary first molar. The maxillary sinus floor was located more at the apex (78.2%) than at the furcation (21.3%) for the palatal root. The distance from the root apex to the maxillary sinus floor confined to type I was increased according to the ages (P<0.05). Type M (The maxillary sinus floor was located between the buccal and the palatal root) was most common (72.4%) on the horizontal relationship between the maxillary sinus floor and the apex of the maxillary first molar. Conclusion: CBCT can provide highly qualified images for the maxillary sinus floor and the root apex of the maxillary first molar.

A comparative study of clinical effects following treatment of class II furcations using allograft and PR with and without bioabsorbable membrane (2급 이개부 병변을 동종골과 혈소판 농축 혈장으로 치료시 차폐막 사용에 따른 임상적 효과의 비교 연구)

  • The present study evaluated of regeneration effect of platelet rich plasma on the treatment of classII furcation involvement, with allograft in humans. The control was treated without bioabsorbable membrane, and the test was treated with bioabsorbable membrane. Pocket depth, clinical attachment level, and gingival recession were measured at baseline, postoperative 3, 6months. Both groups were statistically analyzed by Wilcoxon signed Ranks Test & Mann-whitney Test using SPSS program (5% significance level). The results were as follows: 1. The change of pocket depth and clinical attachment level in both groups was decreased significantly at 3, 6months.(p<0.05) 2. The change of gingival recession in both groups was increased significantly at 3, 6months than at baseline.(p (0.05) 3. The change of pocket depth and clinical attachment level in both groups was increased significantly at 3, 6months, but there were no statistically or clinically significant differences with both groups. 4. The change of gingival recession in both groups was increased significantly at 3, 6months, but there were no statistically or clinically significant differences with both groups. 5. The significant reduction of the pocket depth and clinical attachment level exhibited marked changes at 3 months in both groups. In conclusion, the results of this study suggest that there are no statistically or clinically significant differences between with and without bioabsorbable membrane on treatment of classII furcations using allograft and PRP

Evaluation of canal preparation with Ni-Ti rotary files by micro computed tomography

  • The purpose of this study was to compare the effects of preparation with GT files and profiles .04 in shaping of root canals and reconstruct the three-dimensional root canal system using micro computed tomography 40 canals of the extracted human mandibular molars were used, and randomly distributed into two experimental groups. In group 1. canals were prepared by GT files. In group 2, Profiles .04. were used. Apical preparation size was #30. For each tooth pre and post operative cross-sectional images were obtained by the micro CT at 50 micron intervals. Pre and post operative cross-sectional images of 1, 2, 3, 5, and 8mm from the apex were compared. For each section. canal area and centering ratio were determined. For each tooth pre- and post-operative root canal volume from the furcation to the apex of the roots was calculated by three-dimensional image software. Following results were obtained: 1. At 8mm from the apex, area of dentin removed by GT rotary file was significantly larger than that by Profile .04. And at the other levels there was not a significant difference. 2. There was a trend for GT rotary file to remain more centered in the canals than Profile .04 at all levels. But at 3mm level. there was a statistically significant difference. 3. In root canal volume increments after instrumentation, there was no significant difference between two groups.

Comparative analysis of physicochemical properties of root perforation sealer materials

  • Objectives: This study evaluated the solubility, dimensional alteration, pH, electrical conductivity, and radiopacity of root perforation sealer materials. Materials and Methods: For the pH test, the samples were immersed in distilled water for different periods of time. Then, the samples were retained in plastic recipients, and the electrical conductivity of the solution was measured. The solubility, dimensional alteration, and radiopacity properties were evaluated according to Specification No. 57 of the American National Standards Institute/American Dental Association (ANSI/ADA). Statistical analyses were carried out using analysis of variance (ANOVA) and Tukey's test at a significance level of 5%. When the sample distribution was not normal, a nonparametric ANOVA was performed with a Kruskal-Wallis test (${\alpha}$ = 0.05). Results: The results showed that white structural Portland cement (PC) had the highest solubility, while mineral trioxide aggregate (MTA)-based cements, ProRoot MTA (Dentsply-Tulsa Dental) and MTA BIO ($\hat{A}$ngelus Ind. Prod.), had the lowest values. MTA BIO showed the lowest dimensional alteration values and white PC presented the highest values. No differences among the tested materials were observed in the the pH and electrical conductivity analyses. Only the MTA-based cements met the ANSI/ADA recommendations regarding radiopacity, overcoming the three steps of the aluminum step wedge. Conclusions: On the basis of these results, we concluded that the values of solubility and dimensional alteration of the materials were in accordance with the ANSI/ADA specifications. PCs did not fulfill the ANSI/ADA requirements regarding radiopacity. No differences were observed among the materials with respect to the pH and electrical conductivity analyses.

  • The ultimate goal of periodontal therapy is to fully reconstruct the periodontal attachment apparatus. Commonly used techniques for treatment of infrabony defects include a combination of root planing, curettage and root treatment. To prevent the apical migration of epithelial cells, the technique of guided tissue regeneration is used. The aim of this study is to compare the effects of root treatment with Citric acid & Tetracycline and Guided tissue regeneration in dogs. Experimental periodontitis was induced by the ligation of orthodontic elastic threads in the upper right and left premolars 3, 4 of five adult dogs for 6 weeks. 4 types of procedures were performed as follows; 1) Control graup : Mucoperiosteal flap 2) Experinental I : GTR used Gore-tex(R) membrane 3) Experinental II : Root treatment with citric acid (PHl) 4) Experinental III : Root treatment with tetracycline HCl (50mg/ml) There after, dogs were serially sacrificed at the 1, 2, 4, 5, 8 weeks, and the specimens were prepared, and stained with hematoxylin-eosin for the light microscopic evaluation. The results of this study were as follows; 1. Junctional epithelium reached to the notch through the furcation area in control group at 8 weeks. 2. In the aspects of the inflammatory cell infiltration, control group showed severe aggregation than experimental group I, II, III through the experimental period 3. New cementum was observed over the notch from 5 weeks in experimental group II 4. In the aspects of the amount of new bone formation, experimental group was better than control group, but there was not significant differences among the experimental group, I, II, III

THE PHYSICAL PROPERTIES AND HEALING EFFECT OF CALCIUM SULFATE-HYDROXYAPATITE COMPOUND ON ROOT PERFORATION (Calcium sulfate-Hydroxyapatite 혼합재의 물성 및 치근천공 치유효과에 관한 연구)

  • Treatment of root perforation elicits special considerations due to its blood-contaminated circumstances. It is known that conventional dental restorative materials are all leaking. Calcium sulfate is the material which react with water to become chemically set. This study, therefore, was performed to develop a new compound containing calcium sulfate and to evaluate its physical and biological characteristics. Three materials were used, IRM, calcium sulfate, calcium sulfate-hydroxyapatite compound. The composition of the calcium sulfate-hydroxyapatite compound was basically 50 % of calcium sulfate and 50 % of hydroxyapatite mixed with guajacol. The materials were mixed in conventional way and underwent four physical test procedures, setting time, solubility test, compressive strength, and marginal leakage test. All materials were evaluated under the scanning electron microscope to examine the marginal sealing ability. Animal experiment was also performed to test the materials' tissue response. Twenty-four dog's premolars were tested with either furcation perforations or apical retro-fillings. From the results, we found that calcium sulfate possess the good marginal sealing ability. However, calcium sulfate creates many voids which is caused by crystal thrusting action when it reacts with water. It seemed that the voids caused disintegration of the material which eventually lead to tissue reaction. By compounding calcium sulfate and hydroxyapatite, we were able to obtain the better physical properties but it showed larger marginal gap between the material and the root surface. Within the six weeks observation period, both IRM and calcium sulfate-hydroxyapatite compound showed good tissue responses in animal experiment. It is concluded that calcium sulfate would be the material of choice in root perforation repair, but the physical property needs to be further improved.

COMPARISON BETWEEN INTRAORAL AND PANORAMIC RADIOGRAPHS IN THE EVALUATION OF ALVEOLAR BONE LOSS (치조골소실의 평가에 있어서 구내 X선사진과 파노라마 X선사진의 비교)

  • The purpose of this study was to compare the interpretation results of the panoramic radiographs with those of intraoral radiographs in evaluation of alveolar bone loss. All radiographs were obtained from 100 patients who had visited the Dental Infirmary of Kyungpook National University Hospital for periodontal state evaluation. The results were as follows: The percentages of interpretable sites were 95.9%(94.6% in maxilla, 97.4% in mandible) on the intraoral radiographs, and 90.7%(84.0% in maxilla, 97.3% in mandible) on the panoramic radiographs. The concordance of interpretation scores of marginal bone loss between intraoral and panoramic radiographs was 66.3%(65.6% in maxilla, 66.8% in mandible). And according to the site, the highest concordance was in the distal surface of the mandibular 2nd premolar at 82.0%, and the distal surface of the mandibular 1st premolar(76.8%), the distal surface of the maxillary central incisor(75.8%), the mesial surface of the 2nd premolar (75.0%) in descending order of frequency. According to the interpretation scores of the marginal bone loss, the percentages of concordance between intraoral and panoramic radiographs were the highest on the score 10 at 76.4%, and the lowest on the score 8, 9. And the percentages of concordance were inverse proportional rate from the score 5 to the score 9. Number of the observed sites of the furcation involvement in bitewing and panoramic radiographs were 268 sites, and the percentage of interpretable sites was 92.9% in bitewing radiographs and 86.6% in panoramic radiographs. And the concordance rate of interpretation was 79.5%.
