Prosthetic rehabilitation of partially edentulous patient after hemimandiblectomy: Case report (하악골 부분절제술 시행한 부분 무치악 환자에서 보철 수복 증례)

  • Loss of continuity of the mandible destroys the balance and symmetry of mandibular function, leading to altered mandibular movements and deviation of the residual fragment towards the resected side. Apart from deviation, other dysfunctions include difficulty in swallowing, speech, mandibular movements, mastication, and respiration are accompanied. In general, surgical reconstruction is considered first then proceeds to the prosthetic restorations. However, patients with systemic disease such as BRONJ (Bisphosphonate related osteonecrosis of the Jaw), surgical reconstruction may be limited. Thus, the prosthetic restoration remains as the only resort. Numerous prosthetic methods are employed to minimize deviation and to improve masticatory efficiency, function and esthetics. If a removable partial denture is the selected treatment modality, maximum stability of the partial denture base may be accomplished with a functional impression procedure by means of eliminating lateral and horizontal forces caused by the functional movements of the lips, cheeks and tongue. Also, Twin occlusion is used to obtain a favorable occlusal relationship and check support for esthetics. The purpose of this case report is to demonstrate how neutral zone impression technique and twin occlusion scheme were applied to restore a hemi-mandiblectomy patient with BRONJ syndrome to achieve satisfactory results in functional and esthetic aspects.

  • As an aid towards improving the treatment of exchange and correlation effects in electronic structure calculations, it is desirable to have a clear picture of concepts of exchange-correlation functionals in computational calculations. For achieving this aim, it is necessary to perform different theoretical methods for many groups of materials. We have performed hybrid density functional theory (DFT) methods to investigate the density charges of atoms in rings and cages of carbon nanotube. DFT methods are engaged and compared their results. We have also been inclined to see the impression of exchange and correlation on nuclearnuclear energy and electron-nuclear energy and kinetic energy. With due attention to existence methods, B3P86, B3PW91, B1B96, BLYP and B3LYP have used in this work.

  • The azobenzene dendrimer is one of the dendrimeric macromolecules that include the azo-group exhibiting a photochromic character. Due to the presence of the charge transfer element of the azo-group and its rod-shaped structure, these compounds are expected to have potential interest in electronics and photoelectronics, especially in nonlinear optics. In the present paper, we give pressure stimulation to organic thin films and detect the induced displacement current. Functional photoisometrization organic molecular the photo-stimulus to organic monomolecular L films and LB films of dendrimer and SASH were performed. The SASH organic monolayer in case of pressure stimulus occurred that positive course but in case of the photo-stimulus compared positive and negative. It is assumed that generation forms of displacement current were measured when photo-stimulus for impression.

Verification of lavender(Lavandula angustifolia cv.Hidcote) and lawn(Zoysia japonica Steud.)'s Curative Effect, as a Green Space Composition Factor (녹지구성 인자로써 라벤더(Lavandula angustifolia cv.Hidcote)와 잔디(Zoysia japonica Steud.)의 치유적 효과)

  • This study has been carried out to verify curative effect of lavender and lawn, a green space composition factor. Thus, the physiological index value of blood pressure, heart rate and salivary amylase were measured with object of 16 women in their 20~30s for stress relaxation effect at lavender garden and lawn square, and impression assessment upon SD method as for psychological value was performed in this study. And a survey was conducted on necessity and reason of urban green space with object of 345 men and women urban workers. As a result of variance analysis between groups and multi test of Turkey upon object place, the blood pressure was lowered the same at lavender garden and lawn square, and especially, high blood pressure was greatly lowered to 30 mmHG at lavender garden. On the other hand, low blood pressure group showed the tendency of a little increasing at the same places. And for the pulse, there was difference between high blood pressure group and normal group at the same places. To conclude, at lavender garden and lawn square the blood pressure was made to be lowered when it's high, and when it's low, the effect of returning to sound status, the function of increasing, was verified. In case of impression assessment upon SD method, the lawn square was assessed as resting place and lavender garden diversional place. And over 90% of urban workers responded to the green space's necessity and curative effect, diversion and resting place were adopted for the reason in high-ranking items. As above the green space pattern that urban workers require and curative effect of lavender garden and lawn square were verified. Therefore, the functional green space plan is required, which curative effect, diversion and resting place are considered, for the future green space plan design.

  • The Applegate-Kennedy classification, the drawing of removable partial denture design using color coding, the selective tissue placement impression method in case of Class I and Class II removable partial dentures, the design of the swing lock attachment of an alternative approach to conventional removable partial denture, the design of the intracoronal or extracoronal attachment, and the removable partial denture design using a rotational path were presented. The following conclusions from the above things were presented : 1. The swing lock attachment removable partial denture can be effective to an alternative approach when the design of conventional removable partial denture is improper with markedly mobile remaining teeth or missing key abutments. 2. Intracoronal or extracoronal attachments must be selected care-fully considering the conditions of the abutment teeth and alveolar ridge whether more occlusal loads to the abutment teeth or to the alveolar ridge are distributed. 3. It must be almost prerequisite that a functional impression is taken in case of Class I and class II removable partial dentures and in case of tooth-borne removable partial dentures, a removable partial denture using rotational path is strong, hygienic, esthetic, and can be accomplished successfully in the clinical aspect when it is properly designed and fabricated through the complete understanding of an indication and a principle. 4. All necessary informations must be achieved with carefully investigated surveying procedure according to each clinical case by Applegate-Kennedy classification which can be helpful and useful in the clinical application and it is important that dentists themselves must be in the habit of drawing a reasonable partial denture design using a color coding in the paper sheet.

  • As digital technology has advanced in the field of dentistry, the use of computer-aided design/computer-aided manufacturing (CAD/CAM) has brought changes to the stages of dental treatment. The use of CAD/CAM technology in dental restoration offers clinical efficiency and convenience by reducing production time and appointment intervals, while also simplifying the fabrication process to reduce errors. In this case, digital replication and printing of temporary teeth were used to aid a patient with complex medical histories and physical disabilities. The final impression obtained with silicone impression material included information on the vertical dimension, centric relation, and the angle and length of the anterior teeth, which shortened the production time and appointment intervals and increased patient satisfaction. The final restoration was fabricated using milling and monolithic disc techniques, demonstrating appropriate stability, retention, and support, resulting in functional and aesthetic satisfaction.

Effects of Silicone Polymer Blends on Physical Properties of Dental Polyvinylsiloxane Impression Materials (실리콘폴리머의 혼합사용이 폴리비닐실록산 치과용 인상재의 물성에 미치는 영향)

  • The purpose of this study was to investigate the effects of combined use of several types of silicone polymers on the physical properties of the dental polyvinylsiloxane impression materials (PVS). Four types of silicone prepolymers having different molecular weight and vinyl group position, and two types of cross-linkers having differently located silyl hydride functional groups were used in various combinations for the formulation. The samples containing bimodal or trimodal prepolymers showed higher tensile strength, elongation at break, and elastic deformation than those containing only one type of prepolymer. The samples using CR210 cross-linker which has side- and terminal-silyl hydride groups showed higher elastic deformation and elongation at break than those using CR101 cross-liker which has side-only silyl hydride group. High vinyl content prepolymer having side vinyl group delayed setting even though it enhanced tensile strength. Further studies are needed to clarify the specific role of this component on setting time and to find appropriate controlling methods for making improved PVS with optimum workability.

Tissue conditioner in edentulous patients, depending on the time and the thickness of the changes affect the dimensional stability (무치악 환자에서 조직양화제(tissue conditioner)가 시간과 두께에 따라 체적안정성의 변화에 미치는 영향)

  • This study evaluated the linear dimensional changes with the factor of time and thickness using of three commercially available tissue conditioners (Coe-Comport, Visco-Gel, Soft-Liner). The thickness of materials were changed (1.5mm, 3.0mm) and the percentage changes in dimension were measured at 1h(baseline), 12h, 24h, 3 days and 7days after specimen preparation. The obtained data were analyzed by ANOVA with the SAS/PC statistical package. From the results, large differences appear between the various tissue conditioners. The results suggested that the period recommended for forming functional impression would be 36h to 3days after insertion in the mouth. Depending on the type of tissue conditioner over time, as there were significant differences in the elastic change(p<0.05). Tissue conditioner of the 1.5mm, 3.0mm thickness were significantly different by the dimensional stability(p<0.05). Elastic deformation of the ideal itself, and resilient when compared only the look, Visco-Gel 3.0mm group, stability was the most stable volume. In addition, it is important to select tissue conditioners suitable for functional impression because of the wide range of dimensional stability among the materials.

Palatal obturator restoration of a cleft palate patient with velopharyngeal insufficiency: a clinical report (구개인두 기능부전을 갖는 구개열 환자에서 폐쇄장치를 이용한 보철 치료 증례)

  • Cleft lip and palate is congenital deformity in oral and maxillofacial area. Normal soft palate has velopharyngeal closure action by connecting oral cavity and nasal cavity at rest and moving upward at swallowing and specific pronunciation. Cleft palate patients with velopharyngeal insufficiency have difficulty in mastication, swallowing and pronunciation because velopharyngeal closure is incomplete. At this time, a prosthetic device used to cover palate defects is called a palatal obturator. A palatal obturator separates oral cavity and nasal cavity and recovers pronunciation, mastication, swallowing and esthetic function. The purpose of this case study is to report the results because it reaches a satisfactory result in functional and esthetic aspects through functional impression procedures using modeling compound and tissue conditioner for restoration of a cleft palate patient with velopharyngeal insufficiency.

Applying Method of Arch Bar in Maxillofacial Surgery without Orthodontic Treatment (교정치료 비적용 악안면수술에서 치열활봉 거치법)

  • Temporomandibular joint(TMJ) and dental occlusion are important in functional aspect as well as in cosmetic aspect for craniofacial surgery such as orthognathic and traumatic surgery. During these operations, appropriate maxillomandibular fixation(MMF) is especially necessary to conserve or reconstruct, dynamic TMJ and functional occlusion. Arch bar is one of the most popular method to gain proper MMF. Seventeen patients including 5 patients with mandibular fracture, 12 patients with orthognathic surgery(6 patients had relatively normal occlusion, however 6 patients had facial anomalies such as hemifacial microsomia with irregular occlusal plane.) were joined in this study. Arch bar was contoured on the dental cast, which was prepared for model surgery, prior to apply it on the facial anomaly patient. On using pre-contoured arch bar, patients felt better during the procedure and surgeons saved time. Moreover, well-contoured arch bar promises precise transmission of force on the bone, which implies surgeons do not need to worry about the occlusion while fixing bony structures. Authors suggest how to apply arch bar to satisfy basic theories about dental row and occlusion based on the experience. In addition, it is proposed to contour arch bar on the dental impression prior to apply it on the patient who has irregular and complex occlusal plane.
