The Relationship Between Fear-Avoidance Beliefs and Functional Status in Patients With Low Back Pain: A Cross-Sectional Study (요통 환자에서 나타나는 공포-회피 반응과 기능 수행 수준의 관계: 단면 연구)

  • The objectives of this study were to investigate the relationship between fear-avoidance beliefs and functional status in individuals suffering from low back pain, and to identify factors that influence on fear-avoidance beliefs, functional status and pain. A cross-sectional study included 214 individuals with low back pain. Each of subjects completed a fear-avoidance beliefs questionnaire (FABQ) which divided into two subscales, FABQ for physical activity (FABQ-PA) and FABQ for work(FABQ-W), Oswestry disability index (ODI) and numeric pain rating scale (NPRS). Individuals with radiating pain were significantly higher in FABQ-PA, FABQ-total, and ODI scores than with no radiation pain (p<.05). Individuals who had received surgery were significantly higher in FABQ-PA, FABQ-total, ODI and NPRS scores when compared with individuals who had not received it (p<.05). And individuals with taking medication were significantly higher in all variables (FABQ-PA, FABQ-W, FABQ-total ODI and NPRS) than individuals not taking medication (p<.05). In correlation analysis. age appeared to be significantly correlated with FABQ-PA, FABQ-total and ODI scores (p<.05), and all variables was significantly correlated with each other (p<.01). These findings indicate that individuals suffering from low back pain have psychosocial problems such as fear-avoidance beliefs as well as functional status and pain. Fear-avoidance beliefs may contribute to aggravate physical symptoms, and therefore it should be carefully considered for the management of individuals with low back pain.

A Research on the Design Preferences among and the Development of Functional Clothing Designs for Disabled Women (지체 장애인 여성을 위한 디자인 선호도 조사 및 기능성 의복 디자인 개발 연구)

  • This research is aimed to develop the functional clothing designs for disabled women in the manner of investigating design preferences among them by means of questionnaire and interview with 150 disabled women as respondents and interviewees. The findings of this research are summarized as follows: 1. Regarding satisfaction with ready-made clothes, the respondents answered 'very satisfied' (1.6%), 'usually satisfied' (14.1%), 'moderate' (20.3%) and 'unsatisfied' (53.1%), suggesting that they had been generally unsatisfied with ready-made clothes. 2. There were more disabled women preferring to ready-made clothes with one-grade bigger (loose.) size than the actual one (53.1%) instead of completely fitted size (43.8%) when they purchased such clothes. This result indicates that they prefer to ready-made clothes with bigger size than the actual one because most of such clothes are made up of non-elastic materials which may be unfavorable for wearer's activities. 3. It was found that primarily worn upper garment among them was T-shirt (59.4%). The reason may be that T-shirt is favorable for using prosthesis and orthotics such as wheelchair, walking stick and crutches thanks to its remarkably high activity as well as simple to maintain, compared with other kinds of upper garments. 4. Regarding preferences to functional clothing designs, the primarily worn lower garment among them was trousers (85.9%); the reason was easiness to move. The main reasons of avoiding to wear a skirt included 'difficult to move' (40.6%) and 'exposed disabled region' (30.3%). Accordingly, functional clothing for disabled women should be developed in consideration for their individual characteristics of disability associated with the disabled region such as wheelchair user, crutch user or brace user, In addition, the designs should be made so that they are not different from those for non-disabled people.

Difference of Functional Outcome and Related Factors in Patients With Stroke (일부 뇌졸중 환자의 기능변화 및 관련요인)

  • The purposes of the present study were to determine the difference of functional outcome, and to identify factors associated with functional difference in patients with stroke. The sample consisted of 56 stroke patients who had received physical therapy at the physical therapy unit of the Dongeui Medical Center in Busan city between January 2000 and June 2002. Stroke patients were evaluated by physical therapists 3 times; The first day in physical therapy (PT) (T1), one month after the first day in PT (T2), and two months after the first day in PT (T3). Functional status was assessed with the Functional Independence Measure (FIM) instrument, a validated instrument for documenting the severity of disability and assessing the outcome of rehabilitation treatment. Functional gain was calculated over T2-T1, T3-T1, and T3-T2. SAS statistical software was used for the analysis. The Student's t-test, paired t-test, analysis of variance (ANOVA/Tukey and Scheffe), and analysis of covariance (ANCOVA) were used to examine the functional difference in variables. Repeated measures ANOVA was also used to analyze the functional difference by time (T1, T2, and T3). Multiple regression analysis was performed to determine the effects of independent variables on the difference of functional outcome as defined by the FIM score. A total of 56 stroke patients were evaluated, their average age${\pm}$standard deviation was $61.6{\pm}9.3$ years (range: 40~81 yr). The functional status of patients who received physical therapy for about 2~3 months was significantly improved (mean FIM scores, $20.5{\pm}1.8$, $28.9{\pm}1.9$, and $8.41{\pm}1.1$ points for each time period, respectively) (p<.0001). Diabetes was significantly associated with the FIM score for T2-T1 (p<.05). The type of diagnosis was significantly associated with the FIM score for T3-T1 (p<.05). Gender, smoking, and the FIM score on admission were significantly associated with the FIM score for T3-T2 (p<.05). In conclusion, gender, smoking, diabetes, the type of diagnosis, and the FIM score on admission were significantly associated with improved FIM scores. We recommend that further research should explore the functional outcome by using larger sample sizes, longer follow-up periods, and more sensitive assessment instruments.

The Effect of Korean Medical Treatments with Functional Cerebrospinal Therapy on Patients from Traffic Accidents with Temporomandibular Joint Disorder (기능적 뇌척주요법을 병행한 한방치료가 턱관절 장애가 있는 교통사고 환자에 미치는 영향)

  • Objectives : The purpose of this study is to determine the effect of Functional Cerebrospinal Therapy(FCST) on patients from traffic accidents with Temporomandibular Joint Disorder. Methods : We reviewed the medical records of 33 patients who were treated for injuries stemming from traffic accidents at the Dong-Eui University Korean Medicine Hospital from December 29th, 2015 to August 29th, 2016. The patients were divided into 2 groups: Group A(n = 15) and Group B(n = 18). In Group A, patients received Korean medical treatments with FCST using an Accurate Balancing Appliance(ABA) - a standard intra-oral appliance. In Group B, patients received Korean medical treatments without FCST. To estimate the efficacy of the treatments in decreasing pain, we analyzed the Visual Analogue Scale(VAS), Neck Disability Index(NDI) and Oswestry Disability Index(ODI). Results : In Group A, VAS of headache(VAS H), VAS of neck & nuchal pain(VAS N), VAS of shoulder pain(VAS S), and VAS of low back pain(VAS L) were significantly improved during each period. In Group B, VAS H was significantly improved during each period, except the period from the first visit to two weeks later. VAS N, S, L were significantly improved during each period, except the period from the first visit to one week later. The total VAS improvement during each period in Group A was significantly higher than Group B. In both groups, NDI and ODI were improved significantly during each period. The improvement of NDI and ODI during each period in Group A was significantly higher than Group B. Conclusion : According to the results, FCST using ABA may be an effective treatment for patients from traffic accidents.

The Effects of Lumbar Stabilizing Exercise on the Functional Recovery and the Range of Motion of Low Back Pain Patients (요부 안정화 운동이 요통환자의 기능회복과 가동범위에 미치는 영향)

  • The purpose of this study was to evaluate effects of lumbar stabilizing exercise on the functional recovery and the range of motion of low back pain patients. The subjects were consisted of sixty patients who had non specific chronic low back pain(32 females. 28 males; mean aged 37.3) from 19 to 65 years of age(mean age : 37.3). All subjects randomly assigned to the lumbar stabilizing exercise group, the modalities treatment group, the manual treatment group. Lumbar stabilizing exercise group received manual treatment with lumbar stabilizing exercise for 30minutes, modalities treatment group received hot pack used thermal therapy for 20minutes and ICT used electrical therapy for 20minutes and US or MWD used deep thermal therapy for 15minutes, manual treatment group received modalities treatment with therapeutic massage for 10minutes and joint mobilization or manipulation for 10minutes per day and three times a week during 4 weeks period. The Multilevel Roland-Morris Disability Questionnaire(MR-MDQ) was used to measure functional disability level. Visual Analogue Scale(VAS) was used to measure subjective pain level. Remodified Schober test(RST) was used to measure forward flexion range of motion of lumbar segment. Finger-to-Floor test(F-T-FT) was used to measure forward flexion range of motion of full spine of low back pain patients. All measurements of each patients were measured at pre-treatment and 4 week post-treatment. The results of this study were summarized as follows : 1. The MR-MDQ of lumbar stabilizing exercise group, modalities treatment group, and manual treatment group was significantly reduced between pre-treatment and post-treatment(p<.05). 2. The VAS of lumbar stabilizing exercise group, modalities treatment group, and manual treatment group was significantly reduced between pre-treatment and post-treatment(p<.05). 3. The RST of lumbar stabilizing exercise group, modalities treatment group, and manual treatment group was significantly reduced between pre-treatment and post-treatment(p<.05). 4. The F-T-FT of lumbar stabilizing exercise group, modalities treatment group, and manual treatment group was significantly reduced between pre-treatment and post-treatment(p<.05). 5. The results of analyzed effects of MR-MDQ, RST, F-T-FT were significantly reduced (p<.05), but VAS wasn't significantly reduced(p>.05) between treatment type of lumbar stabilizing exercise group and modalities treatment group and manual treatment group according to pre-treatment and post-treatment. 6. The results of LSD post-hoc to find difference between treatment type of lumbar stabilizing exercise group and modalities treatment group and manual treatment group according to pre-treatment and post-treatment that MR-MDQ was significantly reduced stabilizing exercise group than modalities treatment group(p<.05), and VAS wasn't significantly reduced all treatment group(p>.05), and RST was significantly reduced stabilizing exercise group than modalities treatment group(p<.05), and F-T-FT was significantly reduced stabilizing exercise group than modalities treatment group and manual treatment group (p<.05).

ICF Tool applied Intervention Strategy and Charting on Upper Extremity Functional Recovery of the Frozen Shoulder Patient - Case Study (동결견 환자의 상지 기능 회복에 대한 ICF Tool을 적용한 중재 전략과 문서 기록-사례연구)

  • Background: This study was to suggest the process of making strategy for effective intervention and evaluation on functional problems of the frozen shoulder patient applied International Classification of Functioning Disability and Health (ICF) Tools. Methods: The patient was 48years old woman with right shoulder adhesive capsulitis and calcific tendinitis. In first phase, therapists could list the information relating to functional problems used by the ICF Core set and then could confirm the interaction among the problems using the ICF assessment sheet. In second phase, therapist is needed to make the hypothesis and hypothesis testing and then set a primary functional goals and therapeutic goals in detail after prioritizing the problems to be managed based on the problem list. Lastly, after setting the confirmed problems as the purpose of intervention through the hypothesis testing, therapist could do some intervention after making a plan to solve these problems. Results: This report illustrates how to apply the process based on ICF concept into physical therapy practice. The decided hypothesis and goal that are to solve the problems the client faced were remarkably meaningful. Conclusions: Clinical decision making for the most effective intervention requires that therapists use the clinical reasoning process based on ICF concept.

  • Purpose: The purpose of this study was to investigate effects of mobilization combined active movement(SNAGS) on the pain and recovery of function of acute low back pain patients. Methods: The subjects were consisted of 135 patients with acute low back pain. All subjects randomly assigned to mobilization group, stretching exercise group and modality treatment group. The mobilization group received mobilization combined active movement(SNAGS) with modality treatment, exercise group received stretching exercise with modality treatment and modality treatment group received modality treatment. Visual Analogue Scale(VAS) was used to measure patient's pain level and Patient Specific Functional Scale(PSFS) was used to measure patient's functional disability level. Results: The results of this study were summarized as follows : 1. Visual Analogue Scale(VAS) was mobilization group showed significantly decreased more than comparison group(p<.01) and active treatment group showed significantly decreased more than passive treatment group(p<.01). 2. Patient Specific Functional Scale(PSFS) was mobilization group showed significantly increased more than comparison group(p<.01) and active treatment group showed significantly increased more than passive treatment group(p<.01). Conclusion: It maybe suggested that mobilization combined active movement(SNAGS) is beneficial treatment for acute low back pain patient.

Factors Associated with Physical Functioning among Community-Dwelling Older Adults (우리 나라 재가노인의 신체적 기능상태별 관련 요인)

  • Objectives: The aim of this study is to identify major factors associated with physical functioning among the Korean elderly. Methods: Data come from a survey of a nationally representative sample of 2,058 older people aged 60 years or elder living in the community. A hierarchical functioning scale was constructed, using upper and lower body mobility, IADLs, and ADLs. Socioeconomic characteristics, chronic conditions, measures of health status, health service use, and social support were analyzed to explore their influence on functioning. Polytomous logistic regression analysis was conducted to identify major contributing factors to different levels of functioning. Results: Persons of older age, female, with chronic diseases (heart disease, stroke, fracture/dislocation) were consistently more likely to show a higher degree of functional limitation. There were variations, however, among other factors; e.g., those with arthritis were more likely to be only mildly impaired, but not moderately or severely impaired. Conclusions: Older Korean adults living in the community with impaired functioning constitute persons with diverse characteristics. Policy and program activities need to address specific needs of older people in different functioning states.

  • Purpose : The main purpose of this study was to investigate the influence of lower extremity strengthening on balance ability of knee osteoarthritis. Methods : The walking exercise group with modality treatment and strengthening exercise group with modality treatment. The walking exercise and strengthening for 40 minutes per day and three times a week during 6 weeks period. Short Form McGill Pain Questionnaire(SFMPQ) was used to measure patient's pain level. Patient Specific Functional Scale(PSFS) was used to measure patient's functional disability level. BPM was used to measure sway area. Global Perceived Effect Scale(GPES) was used to measure recovery or worse of patient's condition. Results : 1. SFMPQ was strengthening exercise group showed significantly decreased more than walking exercise group(p<.05). 2. PSFS was strengthening exercise group showed significantly increased more than walking exercise group(p<.05). 3. Sway area was strengthening exercise group showed significantly limited area more than walking exercise group(p<.05). 4. GPES was strengthening exercise group showed significantly increased more than walking exercise group(p<.05). Conclusion : This study will be used as exercise method of patient with osteoarhtritis.

Effect on Muscle Questionnaire of Knee Osteoarthritis with Lower Extremity Patterns of the Proprioceptive Neuromuscular Facilitation (고유수용성 신경근 촉진법의 하지패턴이 퇴행성 슬관절염 환자의 근기능에 미치는 영향)

  • Purpose : this study was to investigate the influence on lower extremity pattern of PNF to muscle questionnaire ability of patients with knee OA. Methods : The subjects were consisted of 60 women patients with knee OA. And all subjects were randomly devide to two group which was modality group and PNF group. Each group had a treatment for 30 minutes per day and three times a week during 8 weeks period. VAS was used to measure pain, patient specific functional scale(PSFS) was used to measure patient's functional disability level, global perceived effect scale(GPES) was used to measure recovery or worse of patient's condition, muscle assessment questionnaire (MAQ) was used to measure patient's pain level. Results : 1. VAS was showed that PNF group had more significantly decreased than modality group(p<.05). 2. PSFS was showed that PNF group had more significantly increased than modality group(p<.05). 3. GPES was showed that PNF group had more significantly decreased than modality group(p<.05). 4. MAQ was showed that PNF group had more significantly decreased than modality group(p<.05). Conclusion : There are strong significant between PNF group and modality group.
