시력저하노인의 기능상태, 자기효능감, 삶의 만족에 관한 연구 (The Study on Functional State, Self Efficacy, and Life Satisfaction in the Elderly with Decreased Visual Acuity)

  • Purpose: This purpose of study was to investigate the relationships among functional state, self-efficacy, and life satisfaction in the elderly with decreased visual acuity. Methods: The subjects were 162 elderly people from the G university hospital. Functional state was measured by Late-Life Function and Disability Instrument (LLFDI) and Minimum Data Set-Home Care version 2.0 (MDS HC 2.0). Self-efficacy and Life satisfaction were measured by the tool of Rho & Lee (2011) and Yoon (2007). Data were analyzed using t-test, ANOVA, Pearson's Correlation Coefficient, and logistic regression. Results: The daily life function was significantly associated with self-efficacy and vision decrease. The regression model with these two variables explained 35.6% of the variance of daily life function. IADL was significantly associated with vision decrease, age, gender, and self-efficacy. The regression model with the three variables explained 52.9% of the variance of IADL. Life satisfaction is significantly associated with self-efficacy, daily life function, vision decrease and IADL. The last regression model with the four variables explained 51.8% of the variance of life satisfaction. Conclusion: The levels of functional state, self-efficacy and life satisfaction in the elderly with decreased visual acuity were low. Self-efficacy was an important factor that influences on the functional state and life satisfaction. Therefore, nursing interventions that can enhance the self-efficacy are required in order to increase the functional state and life satisfaction in the elderly with decreased visual acuity.

기능적 테이핑이 어깨둘레근 수술 후 환자의 어깨관절 통증과 근력, 관절가동범위에 미치는 즉각적 영향 (The Immediate Effects of Functional Taping on Pain, Muscle Strength, and Range of Motion of the Shoulder After Surgery in Patients With Rotator Cuff Tears)

  • Background: Patients after rotator cuff (RC) surgery experienced pain, weakness and limited of motion of the shoulder. Physical therapists have used heat therapy, electrotherapy, range of motion (ROM) exercise and other methods to treat patients after RC surgery. In addition, functional taping is also used to support joint movement and to increase shoulder joint stability. Objects: The purpose of this study was to determine the initial effects of functional taping using non-elastic tape on pain, strength and ROM of the shoulder following RC surgery. Methods: Forty-eight patients with who underwent RC surgery volunteered for this study. The subjects were randomly divided into an experimental group (EG, $n_1=25$) and a control group (CG, $n_2=23$). First, non-allergic tape was applied to the shoulder to prevent skin irritation. The EG applied functional taping using non-elastic tape and the CG applied sham taping using elastic tape. Assessment tools included the shoulder pain and disability index for functional activity score, visual analog scale for level of pain, shoulder muscle strength, hand grip strength and ROM testing. Results: Pain score in the both group significantly decreased (p<.05), and change in pain score of in the EG increased significantly than in the CG (p<.05). Shoulder strength and ROM in the both group significantly increased (p<.05). Especially external rotation and extension of the shoulder ROM in the EG increased significantly more than in the CG (p<.05), but the rate of change in the two groups showed no significant difference. Conclusion: These results suggest that functional taping using non-elastic tape was initially effective in decreasing pain score level in patients with RC surgery.

뇌졸중 환자군을 대상으로 한 작업대 높이가 수행도에 미치는 영향에 대한 연구 (Study on How the Height of the Table Influences of the Functional Ability Perfomence to Patients Group with Stroke)

  • Our study goal is to find out how the table height influences functional ability of upper extremity to patients with stroke when they are having therapy. In this study, we tested forty patients with stroke at the age of over sixty-five who do not have visual, auditory disability and have no difficulty in communicating and moving things by using upper extremity. And they were able to pinch grip in hospital. We used occupational table for occupational therapy as Lab Materials. We also used cube and pegs board to test upper extremity functional ability. The method is that we measured the recommended table height that takes the subjects' size into consideration and patients' preferred table height and therapists' preferred table height and we applied them and then we have done a comparative analysis and set significance level(p<0.05). As the result, we found the Statistically significant between the recommended height and the other two, which are patients' preferred height and therapists' preferred height. We also discovered that functional ability of upper extremity varies significantly depending on each table height. We came to a conclusion that the table height had effects on the upper extremity functional ability of patients with stroke who are over sixty-five years old. It is therefore considered that the proper table height is important to improve upper extremity functional ability of patients with stroke for their physical therapy.

자하거 약침 단독치료로 호전된 다발성 경화증 치험 1례 (A Clinical Case Report of Multiple Sclerosis Treated with Only Hominis Placental Pharmacopuncture)

  • Objectives: This case report describes the effect of Hominis placenta pharmacopuncture (HPP) on multiple sclerosis. Methods: A multiple sclerosis patient with gait disturbances, tremors, spasms of both lower limbs, and diplopia was treated with only HPP for two weeks. To evaluate the effects of HPP on the balance ability of the patient, the Functional System (FS) scale, Expanded Disability Statue Scale (EDSS), visual analog scale (VAS), manual muscle test (MMT), and Berg Balance Scale (BBS) were used. Results: The HPP treatment reduced the patient's gait disturbance, tremors, spasms of the lower limbs, and diplopia. According to the FS scale, the patient's cerebellar and optic functions were enhanced. In addition, the patient's performance on the EDSS improved from 4.0 to 3.0.3. As shown by the MMT, the lower extremities showed a mild improvement (from F+ to G.4). Furthermore, the VAS showed a considerable improvement, decreasing from 9 to 6.5 post-treatment. The patient's performance on the BBS, which denotes balance ability, showed a considerable improvement, increasing from 38 to 49. Conclusion: Treatment with only HPP could help to ameliorate the symptoms of multiple sclerosis.

각 관절 가동술(Angular Joint Mobilization)이 어깨 유착성 관절주머니염 환자의 통증, 관절 가동 범위, 기능 향상에 미치는 영향 -사례 보고- (Effects of Angular Joint Mobilization on the Pain, Range of Motion, and Functional Improvement in a Patient with Shoulder Adhesive Capsulitis -A Case Report-)

  • Purpose: This study aimed to investigate the effect of angular joint mobilization (AJM) on the shoulder pain, range of motion, and functional improvement in a patient with shoulder adhesive capsulitis. Methods: The patient diagnosed with right shoulder adhesive capsulitis by an orthopedic surgeon was a 60-year-old male, right hand/arm dominant, with a height of 175 cm and weight of 75 kg. The patient received 12 sessions of AJM once or twice per week for eight weeks. AJM was applied for 5 min each of flexion, abduction, external rotation, internal rotation, for a total of 20 min per session. The visual analog scale, the goniometer, and the Oxford shoulder score were used to measure pain, range of motion, and shoulder pain & disability index, respectively. Results: After all the treatments, the pain decreased from 6 to 2 points. The range of motion increased in flexion by $54.3^{\circ}$ from $125^{\circ}$ to $179.3^{\circ}$, abduction by $38^{\circ}$ from $140^{\circ}$ to $178^{\circ}$, external rotation by $54.4^{\circ}$ from $30.3^{\circ}$ to $84.7^{\circ}$, and internal rotation by $25^{\circ}$ from $45^{\circ}$ to $70^{\circ}$. The shoulder disability index decreased from 33 points to 17 points. Conclusion: This study found that AJM has a positive effect on the improvement of shoulder pain, range of motion, and function in a patient with shoulder adhesive capsulitis. Further studies on AJM are needed in the future.

ICF core-set를 이용한 뇌졸중 환자의 기능수행 분석 (Investigating Functional Level in Patients with Stroke using ICF Concept)

  • Purpose: The purpose of this study was to investigate level of functioning in patients with stroke using Modified Bathel Index (MBI), World Health Organization Disability Assessment Schedule 2.0 (WHODAS 2.0), and ICF core-set for stroke. Methods: Sixty-four patients with stroke were recruited for this study from nine medical institutes. The ICF core-set for stroke, WHODAS 2.0, and MBI were used to collect subjects' functional levels. ICF core-set was employed here as a standard frame to observe multi-dimension of functioning, that is physiological bodily function, activity and participation (AP) in daily life, and current environmental factors (EF) in patients with stroke. WHODAS 2.0 and MBI were also used in order to have a specific functioning level for subjects. The linkage of each item in WHODAS 2.0 and MBI into the ICF core-set for stroke was examined. Pearson correlation coefficient was used for analysis of their relationships. Results: Functioning level of participants showed moderate resulting from MBI and WHODAS 2.0 ($73.48{\pm}22.27$ and $35.55{\pm}12.53$, respectively). Strong relationship was observed between ICF core-set and WHODAS 2.0, and with MBI. Each item of disability scales was obtained its linkage into ICF in the domain of AP. However, lack of correlation between MBI and ICF in the domain of EF was found due to absence of related factors. Conclusion: MBI was found to be linked mainly into ICF in the domain of AP and to have limited linkage into EF. Therefore, it should be suggested that the ICF concept frame should be used as a multi-dimensional approach to patients with stroke.

Comparison of effectiveness for fluoroscopic cervical interlaminar epidural injections with or without steroid in cervical post-surgery syndrome

  • Background: Neck and back pain are leading sources of disability placing substantial burden on health care systems. Surgical interventions in managing chronic neck pain secondary to various disorders continue to increase. Even though surgical interventions are effective, a significant proportion of patients continue to have symptomatology and develop cervical post-surgery syndrome. This study was performed to know the effectiveness of cervical interlaminar epidural injections with or without steroids. Methods: The effectiveness of fluoroscopic cervical interlaminar epidural injections in post-surgery syndrome was evaluated in a randomized, active controlled trial. The study population included 116 patients assigned to 2 groups. Group 1 received cervical interlaminar epidural injections with local anesthetic alone and Group 2 received injection with local anesthetic and steroids. The main outcomes were defined as significant improvement (greater than 50%) of pain relief using the numeric rating scale and/or functional status improvement using the Neck Disability Index (NDI). Results: Both groups had similar results with significant improvement (${\geq}50%$ pain relief and functional status improvement) in 69% of the patients in Group I, whereas, in Group II, 71% of the patients showed significant improvement at the end of 2 years. During a 2-year period, the average number of procedures was 5 to 6, with an average of approximately 12 weeks of significant improvement per procedure. Conclusions: Fluoroscopic cervical interlaminar epidural injections administered in cervical post-surgery syndrome using local anesthetic, regardless of the use of steroids, may be effective in approximately 70% of the patients at 2-year follow-up.

NFlex Dynamic Stabilization System : Two-Year Clinical Outcomes of Multi-Center Study

  • Objective : Pedicle-based dynamic stabilization systems, in which semi-rigid rods or cords are used to restrict or control spinal segmental motion, aim to reduce or eliminate the drawbacks associated with rigid fusion. In this study, we analyzed the two-year clinical outcomes of patients treated with the NFlex (Synthes Spine, Inc.), a pedicle-based dynamic stabilization system. Methods : Five sites participated in a retrospective study of 72 consecutive patients who underwent NFlex stabilization. Of these 72 patients, 65 were available for 2-year follow-up. Patients were included based on the presence of degenerative disc disease (29 patients), degenerative spondylolisthesis (16 patients), lumbar stenosis (9 patients), adjacent segment degeneration (6 patients), and degenerative lumbar scoliosis (5 patients). The clinical outcome measures at each assessment were Visual Analogue Scale (VAS) to measure back pain, and Oswestry Disability Index (ODI) to measure functional status. Radiographic assessments included evidence of instrumentation failure or screw loosening. Results : Sixty-five patients (26 men and 39 women) with a mean age of 54.5 years were included. Mean follow-up was 25.6 months. The mean VAS score improved from 8.1 preoperatively to 3.8 postoperatively, representing a 53% improvement, and the ODI score from 44.5 to 21.8, representing a 51% improvement. Improvements in pain and disability scores were statistically significant. Three implant-related complications were observed. Conclusion : Posterior pedicle-based dynamic stabilization using the NFlex system seems effective in improving pain and functional scores, with sustained clinical improvement after two years. With appropriate patient selection, it may be considered an effective alternative to rigid fusion.

악교정 수술 환자의 주관적 만족도 조사 연구 (EVALUATION OF THE PATIENTS' SUBJECTIVE SATISFACTION ABOUT THE ORTHOGNATHIC SURGERY)

  • Purpose: As the interest and demand for esthetics grow up with medical development, the number of patients who want to correct dentofacial deformities by orthognathic surgery has increased. And the success of orthognathic surgery is dependent upon satisfaction of patients in great measure. Therefore the purpose of this study is to examine functional disability, sensory disturbance and esthetic satisfaction of the patients after orthognathic surgery. Material and methods: Among 89 patients who orthognathic surgery was performed at Seoul National University Bundang Hospital during the period from July 2003 to July 2008, 46 patients were considered in the study. Patients were surveyed with questionnaires and visual analogue scale(VAS) which author has designed. The analysis and comparison were performed according to gender, follow up span, and type of surgery based on answer of questionnaires. Results and conclusions: Total satisfaction rate was 76%. And there was statistical significant difference between subjective evaluation of facial esthetics before the surgery and that of after surgery(p<0.05). Differences in occurrence of functional disability, sensory disturbance between groups were not observed. In conclusion, the operator should understand the postoperative self-satisfaction of the patients and complications. If we can communicate with the patients before operation. patients' satisfaction rate will increase after operation.

분절별 등뼈 관절가동술이 만성허리통증환자의 통증과 관절가동범위에 미치는 즉시적 효과 (The Immediately Effects of Thoracic Mobilization by Segment on Pain and Range of Motion in Chronic Low Back Pain Patients)

  • PURPOSE: This study aims to identify the immediate effects of thoracic mobilization according to segment on disabling low-back pain and the range of motion in patients with chronic low-back pain. METHODS: The participants were divided randomly into two groups: a group (N=12) with pain and disability between the 5th and 9th thoracic vertebrae (T5-T9) and a group (N=12) with pain and disability between the 10th and 12th thoracic vertebrae (T10-T12). The same experiment was conducted in both groups: 10 minutes of thoracic mobilization and 10 minutes of functional massage. The visual analogue scale (VAS) and range of motion measurements were applied to the participants in both groups. All participants were measured again immediately after the program was completed. RESULTS: In both groups, the VAS and range of motion measurements showed statistically significant improvement after the experiment (p<.05). No statistically significant difference was revealed between the two groups (p>.05). CONCLUSION: The results of this study confirmed positive and immediate effects of thoracic mobilization of patients with chronic low-back pain. But, no significant difference in the thoracic mobilization by segment between the two groups. Based on these results, thoracic mobilization could help to improve pain control and functional activity in patients with chronic low-back pain.
