Study on the Relationship between Family Support and Functional Recovery in Cerebrovascular Accident Patients with Rehabilitation Treatment of Oriental Medicine (한방재활치료를 받은 뇌졸중 환자가 지각한 가족지지와 기능회복에 관한 연구)

  • Functional recovery of cerebrovascular accident (CVA) patients were studied by examining functional independence measure (FIM) to evaluate the functional state of the patients at admission to and at discharge from the hospital and its relationship with the family support. Study subjects consisted of 129 CVA patients, who were admitted and received rehabilitation treatment at K Medical Center of Oriental Medicine from August 3 to December 18, 1997. The results were as follows: 1) Total FIM score was $72.37{\pm}25.16$ at admission and $101.67{\pm}22.13$ at discharge. The difference of average score was 29.30, which was statistically significant by paired t-test. 2) The largest difference between FIM scores at admission and at clischarge was observed in items of walking and wheel-chair riding, and the smallest clifference in items of social interaction. 3) The recovery was faster with motor function than with cognitive function, because the difference of FIM scores at admission and at discharge was much larger with motor function. 4) Recovery was better in groups under age 49 than in groups above age 70. Functional recorvery was prominent especially in groups with normal sensory state and speech functions, and groups without urinary incontinence. Recovery was less significantly in patients with paraplegic patients hospitalized longer than 2 months, patients with family all the time, and patients with CVA over 11 days. 5) We could not find any relationship between functional recovery and family support. FIM scores were lower in groups of old age(r=-0.325), long stayed in hospital (r=-0.426), and long period of time after the onset of disease(r= -0.339) with a reciprocal correlation between FIM scores and these parameters. 6) Stepwise multiple regression analysis was done to evaluate factors to affect the recovery from CVA. FIM score at admission could explain 51.2 % of the functional recovery. Important factors were periods of hospitalization, state of sensory function, age, and education (listed in decreasing order of importance). In total, they could explain 64.89% of the functional recovery. These results indicate that functional recovery of CVA patients, who were admitted to oriental medicine hospital for rehabilitation treatment, could be estimated by measuring FIM scores. Recovery was significantly better at discharge from the hospital than at admission and motor function recovery rate was much faster than that of cognitive function. 2. Recommendation Based on these results, we recommend following further studies. 1) Comparative study of recovery of motor function and of sensory function would be necessary by measuring FIM scores once a week to evaluate the recovery of CVA patients. 2) It would be interesting to see whether there is any difference of functional recovery between patients treated with either western medicine or oriental medicine. 3) Psychological factors affecting the recovery of CVA patients need to be studied.

Effects on the Recovery of Motor Function, Change ECG and Troponin I According to Different Amounts Exercise in Ischemic Stroke Patients (운동적용 시간량이 허혈성 뇌졸중 환자의 운동기능회복과 심전도 및 Troponin I 에 미치는 영향)

  • Purpose : The purpose of this study is to investigate the effects of motor function recovery and change of the heart function factors(ECG & Troponin I) with ischemic stroke patients by different amounts(times) exercise. Methods : Forty-six consecutive chronic hemiparetic patients with cerebral infarct were randomly assigned to two groups: Group 1 (exercise time 60 minutes/day) and Group 2 (exercise time 120 minutes/day). Types of exercise included static bicycle, isokinetic exercise, and standing or gait exercise on a treadmill. Outcome measures included the level of motor recovery (Fugl-Meyer Scale, FMS) and heart function (ECG and Troponin I), and measurements were performed three times: pre-test, 8 weeks and 12 weeks. Results : There was a significantly different change of motor function recovery and ECG between two groups during treatment period. Especially there were significantly change period of pre-test to 8 weeks on ECG and pre-test to 12 weeks on motor function recovery. But Troponin I has no significantly different change between two groups during treatment period. Also there was no significantly different change of motor function recovery and ECG and Troponin I with between two groups during treatment period. Conclusion : The exercise program improved motor function and change ECG without Troponin I in two groups. The result of this study shows that no matter how different amounts of exercise to effect of motor function recovery and heart function test in chronic patients with cerebral infarct.

A Study on Recovery Time and Factors Related to Recovery after Sedative Gastroscopy (수면 위 내시경 검사 후 회복 시간과 회복 관련 요인에 관한 연구)

  • Purpose: The purpose of this study was to investigate the recovery time and its related factors after sedative gastroscopy. Method: The subjects of this study consisted of 103 clients. The data were collected from clients who visited a health care system at S national university hospital in Seoul. The sedative gastroscopy was performed by gastrointestinal endoscopists. Patients' demographics and medical characteristics were assessed by reviewing the patients' charts. Degree of sedation and recovery were assessed by the Ramsay's sedation scale and the Aldrete score, respectively. Result: The mean recovery time was $37.8{\pm}16.9$ minutes(range, 5 to 90 minutes). Hypoxia was resent in four clients during recovery. Alcohol intake(p=0.02) and pulmonary function(p=0.003) were significant factors affecting recovery time, whereas midazolam dose and sedation level were not. Conclusion: Current alcohol intake, pulmonary function were predicting factors on recovery time.

  • The number of the enterprises which utilize network technology has been increasing for solving problems such as productivity improvement, market trend analysis, and material collection for making decision. Especially, the management of equipment and the recovery time reduction when machines break down are very important factors in productivity improvement of the enterprise. Currently, most of the remote trouble diagnosis of equipment using the internet have just one function of transmitting the trouble information to the user. Therefore it does not directly reflect the user's recovery experience or the developer's new recovery methods. If the user's experienced recovery methods or the developer's recovery methods as well as the basic recovery methods are reflected online or on the internet, it makes it possible to recover faster than before. In this paper, we develop a Remote Monitoring Server (RMS) for chip mounters, and make it possible to reduce the recovery time by reflecting the user's experience and developer's new methods in addition to presenting the basic recovery methods. For this, trouble recovery concept will be defined. Based on this, many functions(trouble diagnosis, the presentation of the basic recovery methods, user's and developer's recovery method, counting function of the trouble number of each code, and presentation of usage number of each recovery methods) were developed. By utilizing the reports of the actual results of chip mounter and the notice function of the parts change time, the rate of operation of the chip mounter can be improved.

The Effects of a Recovery Education Program on Rehabilitation Motivation, Symptoms, and Function for Schizophrenic Patients (재기교육 프로그램이 정신분열병 환자의 재활동기, 증상 및 기능에 미치는 영향)

  • Purpose: This study investigated the effects of a recovery education program on rehabilitation motivation, symptoms, and function for schizophrenic patients. Method: The study employed a quasi-experimental design. Participants for the study were 27 patients with schizophrenia, 14 in the experimental group and the other 13 in the control group. Data were analyzed by using the SPSS/WIN 11.5 program with Fisher's exact test, t-test, and Repeated measures ANOVA. Results: After a 7 week intervention, participants in the recovery education program group reported increased rehabilitation motivation and function scores, which was significantly different from those in the control group. Conclusion: A recovery education program was effective improving rehabilitation motivation and function for schizophrenic patients. Therefore, this program is recommended as a rehabilitation strategy for schizophrenic patients.

Recovery Phase Spontaneous Nystagmus, Its Existence and Clinical Implication

  • Background and Objectives: Determination of the lesion side based on the direction of the nystagmus could result in confusions to the clinicians due to mismatch between the vestibular function tests and also between vestibular and audiologic features. To minimize these mistakes, we elucidated the clinical manifestation and vestibular function test results in cases with recovery spontaneous nystagmus (rSN). Subjects and Methods: Patients who visited ENT clinic of tertiary referral hospital for acute onset continuous vertigo from January 2008 to December 2011 were enrolled. In these patients, we assessed onset time of vertigo, time point of paralytic spontaneous nystagmus (SN) and time point of rSN. At each time point of SN, vestibular function tests and hearing function tests were performed. Results: We confirmed the rSN among patients with unilateral vestibulopathy and demonstrated that high gain of the rotatory chair test (slow harmonic acceleration) and/or mismatch of the SN direction and contralateral caloric weakness could indicate the recovery state of patients and nystagmus observed in this stage is recovery phase nystagmus. Conclusions: In acute vestibulopathy patients, recovery phase nystagmus was observed and on this stage of disease vestibular function tests shows several features that could predict recovery state.

The relationship of cognitive function and physical recovery stage on quality of life in stroke patients (뇌졸중 환자의 인지기능과 신체회복단계가 삶의 질에 미치는 영향)

  • Background: The patients common have side effects of cognitive and function dysfunction after a stroke. According to specific factors which influence quality of life(QoL), the QoL of stroke survivors are impacted resulting from diverse interactions. Therefore, This study aims to suggest that we determines the relationship between cognitive function and stage of physical recovery and the quality of life as well as the degree of recovery by cluster analysis of the relationship between the degree of physical recovery and the quality of life Design: Randomized Methods: The following tests were used in this study to evaluate cognitive function, recovery stages and quality of life respectively: Cognitive function was measured using Korea-Mini Mental State Examination(K-MMSE). For evaluation of recovery stages, Brunnstrom Rrecovery Stage(BRS), quality of life was measured using Stroke Specific -Quality Of Life(SS-QOL). The sample size of this study was calculated using G*Power Version 3.1.9.7 (Franz Faul, University kiel, Germany, 2020). Based on moderate effect size of 0.15, a significance level (α) of 0.05, and power of 0.90 in the two-sided test, the calculation revealed that 88 patients were required for questionnaires. Results: The results of this study showed significant positive correlation(p<0.05). As a result of cluster analysis, in the case of the physical recovery stage, the degree of physical recovery improves from cluster 1 to cluster 3 and in the case of the quality of life, the quality of life improves from cluster 1 to cluster 3. However, it was confirmed that the change in the quality of life of cluster 1 and cluster 2 was not significant. These results show that the degree of physical recovery has a greater impact on the quality of life in the late stages of physical recovery, while the degree of physical recovery does not have a significant effect on the quality of life in the early and mid-term of the physical recovery stage. Conclusion: This study confirms that cognitive function, recovery stages and quality of life have significant correlations, and the recovery status has impacted on quality of life.

  • The creep of concrete structures caused by variable stresses is mostly calculated by step-by-step method based on the superposition of creep function. Although most practical application is carried out by this linear assumption, significant deviations between predictions and experiments have been observed when unloading takes place, that is, stress is reduced. The recovery is overestimated. The main purpose of this study is to present the application method of the creep analysis model which is expressed with both creep function and creep recovery function to concrete structures where is expressed with both creep function and creep recovery function to concrete structures where increase or decrease of stress is repeated . To apply two function method to time analysis of concrete structures, this study presents the calculation method of creep strain increment for stress variation. Then, this paper executes the time analysis for an example using suggested method, and compares theses results with the previous analysis values and experimental results.

Impact of Enhanced Recovery Program on Colorectal Cancer Surgery

  • Surgical outcomes of colorectal cancer treatment depend not only on good surgery and tumor biology but also on an optimal perioperative care. The enhanced recovery program (ERP) - a multidisciplinary and multimodal approach, or so called 'fast-track surgery' - has been designed to minimize perioperative and intraoperative stress responses, and to support the recovery of organ function aiming to help patients getting better sooner after surgery. Compared with conventional postoperative care, the enhanced recovery program results in quicker patient recovery, shorter length of hospital stay, faster recovery of gastrointestinal function, and a lower incidence of postoperative complications. Although not firmly established as yet, the enhanced recovery program after surgery could be of oncological benefit in colorectal cancer patients because it can enhance recovery, maintain integrity of the postoperative immune system, increase feasibility of postoperative chemotherapy, and shorten the time interval from surgery to chemotherapy. This commentary summarizes short-term outcomes and potential long-term benefits of enhanced recovery programs in the treatment of colorectal cancer.
