Changes of the Cerebral Blood Flow During Event Related Petential Test to Auditory Stimuli : A Transcranial Doppler Study (인지유발전위중의 뇌혈류변화 : 초음파뇌혈류검사)

  • Background : The origin of P300 was still on debate nbut thought to be in the frontal, temporal or parietal lobe. As the transcranial doppler ultrasonography(TCD) gives us and opportunity to observe hemodynamic chaged dynamically and the middle cerebral artery feeds these ares of the hemisphere, we observed the change of mean flow velocity of MCA during the event related potential test(ERP) to determine the role of these structures in P300 generation. Method : Twenty normal subjects(male : 13, age : 24-29 years) performed ERP. An auditory oddball pardigm was used to elicit the ERPs. TCD examination was performed with 2-MHz probe monitoring the left MCA(Transscan, EME). After signal identification and adjustments to maximize the Doppler signal strength, the probe was mechanically locked during the monitoring. The changes of blood flow velocity of the left middle cerebral artery(MCA) induced by cognitive demands were monitored. The measurement of the meal flow velocities(MFV) of MCA were made while the subjects were prior to, during, and after ERP. We recorded the MFVs during ERP. Statistical analysis was performed using t-tests with SPSS-PC for windows release 6.0. Results : All subjects showed a relative increase in MFV of MCA during the task. The mean rise was about 3.2-4.2%(p <0.05). Although TCD does not measure absolute values of regional cerebral blood flow(rCBF) or absolute rCBF changes, changes of flow velocity can reflect relative rCBF changes. Conclusions : The generation site of P300 still remains unclear but the neocortical, thalamic and limbic region and temporal-parietal cortex have been proposed. The MCA supplies these anatomical structures. The Changes of flow veolocity of MCA during the ERP test suggest that the some part of the brain fed y the MCA activate of the temporal lobe or parietal lobes, we can deduce that some parts of brain fed by the MCA participate in the generation of P300.

The P300 Source Localization in the Patients with Obsessive-Compulsive Disorder using the LORETA Imaging and SPM (강박장애에서 LORETA 영상을 이용한 P300 국소원의 통계적 분석)

  • Objectives:We investigated the characteristics of P300 generators in obsessive-compulsive disorder(OCD) patients by using voxel-based statistical parametric mapping of current density images. Methods:P300 generators, produced by a rare target tone of 1500Hz under a frequent non-target tone of 1,000Hz, were measured in 15 right-handed OCD patients and 15 controls. Low Resolution Electromagnetic Tomography(LORETA), using a realistic head model of the boundary element method based on individual MRI, was applied to the 128-channel EEG. Statistical parametric mapping(SPM) was applied for the statistical analysis. Results:We found that both groups had the mean current density of P300 in the parietal, temporal and prefrontal lobe. There was a trend for decreased current density in the prefrontal area in OCD patients. The statistical comparison showed current density increase in the supraparietal area, a statistically significant longer P300 latency and a trend for reduced P300 amplitude in OCD patients. Conclusion:It suggests that P300 source of both groups exists in multiple brain regions at the same time. And both groups had no statistically significant differences in the current density of P300 except for increased current density in the supraparietal area in OCD patients. But, considering the statistically significant longer P300 latency, a trend for reduced P300 amplitude and relative mean current density reduction in the prefrontal area in OCD patients, this study suggests that the frontal lobe may have a reduced normal inhibitory process in OCD patients.

Global Cerebral Ischemia in a Beagle Dog (비글견에서 발생한 전반적 대뇌허혈)

  • Global cerebral ischemia occurs commonly in patients who have a variety of clinical conditions including cardiac arrest and shock. Cerebral ischemia results in a rapid depletion of energy stores that triggers resulting in excitotoxic death. Imaging studies of the brain with computed tomography(CT) or magnetic resonance imaging(MRI) are necessary to confirm the clinical neurolocalization, identify any associated mass effect, and rule out other causes of focal brain disorders. Cardiopulmonary arrest was occurred by propofol anesthesia in a 1 year old, intact female Beagle dog. After successful cardiopulmonary resuscitation was performed within 5 minutes, clinical signs such as vocalization, paddling, opisthotonus and seizure were represented. At the 12th day, CT and MRI examinations of the brain were performed to evaluate the brain. After euthanasia, histopathologic examination was performed. On transverse image of CT, lesions appeared as a hypodense in the right dorsal surface of the frontal lobe and level of optic canal, and dorsomedial surface of occipital lobe of cerebrum. No contrast enhancement was represented following intravenous contrast administration. On MR images of brain, the lesions were seen as a hyperintense on T2-weighted(T2W) images and a isointense or mild hypointense on T1-weighted(T1W) images. Hyperintense lesions both T2W and T1W images were observed at the surrounding cerebral sulcus. There was no significant signal changes on contrast T1WI. Histopathologic examination after euthanasia revealed that the lesion was necrosis of the cerebral cortex caused by cerebral ischemia.

Prognostic Factors Influencing Clinical Outcomes of Malignant Glioblastoma Multiforme: Clinical, Immunophenotypic, and Fluorescence in Situ Hybridization Findings for 1p19q in 816 Chinese Cases

  • Malignant glioblastoma multiforme (GBM) is the most malignant brain tumor and despite recent advances in diagnostics and treatment prognosis remains poor. In this retrospective study, we assessed the clinical and radiological parameters, as well as fluorescence in situ hybridization (FISH) of 1p19q deletion, in a series of cases. A total of 816 patients with GBM who received surgery and radiation between January 2010 and May 2014 were included in this study. Kaplan-Meier survival analysis and Cox regression analysis were used to find the factors independently influencing patient progression free survival (PFS) and overall survival (OS). Age at diagnosis, preoperative Karnofsky Performance Scale (KPS) score, KPS score change at 2 weeks after operation, neurological deficit symptoms, tumor resection extent, maximal tumor diameter, involvement of eloquent cortex or deep structure, involvement of brain lobe, Ki-67 and MMP9 expression level and adjuvant chemotherapy were statistically significant factors (p<0.05) for both PFS and OS in the univariate analysis. Cox proportional hazards modeling revealed that age ${\leq}50$ years, preoperative KPS score ${\geq}80$, KPS score change after operation ${\geq}0$, involvement of single frontal lobe, deep structure involvement, low Ki-67 and MMP9 expression and adjuvant chemotherapy were independent favorable factors (p<0.05) for patient clinical outcomes.

An Experimental Study on Physiological and Psychological Effects of Pine Scent (소나무 향기의 생리 심리적 효과에 관한 실험적 연구)

  • The scientific verification of the physiological and psychological effects that result from interaction with green plants would not only provide objective knowledge on the psychological effect of green but would also establish useful grounds for the creation of green spaces that consider human emotions. The present study measured the cerebral activity(cerebral blood flow) and the autonomic nervous system activity (blood pressure, pulse rate, amylase) of fifteen Korean male subjects as they inhaled the natural scent diffused by pine needles. Impression and mood state evaluations of the reactions to the pine scent were carried out using the SD method and POMS. Cerebral activity was observed to be significantly activated in the feeling, judgment, and motor areas of the frontal lobe, as well as the memory area in the temporal lobe. Verbal evaluations by the SD method and POMS indicated a pine scent left natural but stimulated and active impressions, provided vigor, and also reduced confusion. The autonomic nervous system activities, however, showed no significant differences. These findings verified scientifically that a pine scent vitalizes humans both physiologically and psychologically. These results could be useful as fundamental data for the design of green spaces that consider human emotional aspects.

  • The purpose of this study is to survey its characteristics through analyzing brain-wave activity in the scientifically-gifted and general children in the problem-solving process. The subjects of this study were 6 elementary school students, who are attending the institute of education for the gifted belonging to the regional office of education and 6 general children in the same region. The analysis was performed targeting total 12 people. As the task for measuring brain wave is Hanio tower, it is the effective task of researching into the problem-solving process. As the equipment of measuring brain wave is EEG System, it used equipment that was developed in Australia. The analysis of data was minimized noise. As a result of research, the gifted children are excellent in stable level compared to general people in a stable situation with opening the eyes, thereby being able to be known to be high in preparatory level for learning. This can be seen to be indicated as a result that the effect of learning is excellent due to being high in preparatory level for solving problem. Also, even in the process of performing task, the brain-activity level in the gifted children is high, thereby having been able to know that ${\alpha}-wave$ is formed that is significantly high in the regions of frontal lobe and occipital lobe. Accordingly, given developing task that is high in brain activity level of the gifted children, the higher educational effect will be able to be expected.

EEG based Vowel Feature Extraction for Speech Recognition System using International Phonetic Alphabet (EEG기반 언어 인식 시스템을 위한 국제음성기호를 이용한 모음 특징 추출 연구)

  • The researchs using brain-computer interface, the new interface system which connect human to macine, have been maded to implement the user-assistance devices for control of wheelchairs or input the characters. In recent researches, there are several trials to implement the speech recognitions system based on the brain wave and attempt to silent communication. In this paper, we studied how to extract features of vowel based on international phonetic alphabet (IPA), as a foundation step for implementing of speech recognition system based on electroencephalogram (EEG). We conducted the 2 step experiments with three healthy male subjects, and first step was speaking imagery with single vowel and second step was imagery with successive two vowels. We selected 32 channels, which include frontal lobe related to thinking and temporal lobe related to speech function, among acquired 64 channels. Eigen value of the signal was used for feature vector and support vector machine (SVM) was used for classification. As a result of first step, we should use over than 10th order of feature vector to analyze the EEG signal of speech and if we used 11th order feature vector, the highest average classification rate was 95.63 % in classification between /a/ and /o/, the lowest average classification rate was 86.85 % with /a/ and /u/. In the second step of the experiments, we studied the difference of speech imaginary signals between single and successive two vowels.

A Review of Domestic Research for the Brain-science Based Learning According to Age and Comparison and Consideration of Learning Methodology of Korean Medicine According to Age (뇌과학에 기반한 연령별 학습법과 연령별 한의학적 학습방법론 비교고찰)

  • Objectives: The purpose of this study was to research learning based on brain science and the learning methodology of Korean Medicine according to disparity of age. Through this, the study aimed to provide a guideline to related Korean Medicine treatments as well as the common nurturing/educational institutions. Methods: All journals and dissertations on brain science based learning methods studied in Korea to date that could be found in the National Assembly Library and the RISS were implemented in the analysis. The terminology used for search was as follows: 1st search, 'Brain'; 2nd search, 'Learning', 'Education'; 3rd search, 'Baby, 'Infant', 'Child'. For the learning methodology of Korean Medicine according to disparity of age, the related contents were extracted from Donguibogam and Liuyi, Sasang constitutional medicine. Results: A total of 30 studies, were collected as data. In the baby stage, the development and myelination of brain neurons are accelerated by experience and learning, highly influenced by social, cognitive and emotional movement. In infancy, the frontal lobe actively develops, so education for development of the prefrontal cortex is suggested. The brain of the infant at this stage can be developed by arts and physical education. In the child stage, the parietal and temporal lobe develop actively. Thus, programs to stimulate brain activity including brain respiration would be helpful in enhancing learning ability, concentration, etc. As evidence for learning and nurturing methodology according to disparity of age from Korean Medicine prospective, the following are listed: Location and time for sexual intercourse before pregnancy, stabilization during pregnancy, baby nurturing methods for nurturing from Donguibogam. Also Liuyi and Sasanag constitutional medicine can be the learning methodology according to disparity of age. And there are acupuncture points on each head section according to age in Donguibogam. Conclusions: Studies on 'brain-science based learning' are continuously being conducted. Based on these studies, diverse new brain-science based learning will be developed in the future. There is also a need to develop the learning methodology of Korean Medicine according to disparity of age in a more systematic and diverse way.

Comparison of Diagnostic Performance between Interictal F-18-FDG PET and Ictal Tc-99m-HMPAO SPECT in Occipital Lobe Epilepsy (후두엽간질 환자에서 F-18-FDG PET와 발작기 Tc-99m-HMPAO SPECT의 간질원인병소 진단 성능 비교)

  • Purpose: Interictal F-18-fluorodeoxyglucose (FDG) PET and ictal Tc-99m-HMPAO SPECT are found to be useful in localizing epileptogenic zones in neocortical lateral temporal or frontal lobe epilepsy. We investigated whether interictal F-18-FDG PET or ictal Tc-99m-HMPAO SPECT was useful to find epileptogenic Bones in occipital lobe epilepsy (OLE). Materials and Methods: We reviewed patterns of hypometabolism in interictal F-18-FDG PET and of hyperperfusion in ictal Tc-99m-HMPAO SPECT in 17 OLE patients (mean age=$27{\pm}6.8$ year, M:F= 10:7, injection time= $30{\pm}17$ sec). OLE was diagnosed based on invasive electroencephalography (EEG) study, surgery and post-surgical outcome (Engel class I in all for average 14 months). Results: Epileptogenic zones were correctly localized in 9 (60%) out of 15 patients by interictal F-18-FDG PET. Epiletogenic hemispheres were correctly lateralized in 14 patients (93%). By ictal Tc-99m-HMPAO SPECT, epileptogenic hemispheres were correctly lateralized in 13 patients (76%), but localization was possible only in 3 patients (18%). Among patients who showed no abnormality with MR imaging and no correct localization with ictal Tc-99m-HMPAO SPECT, interictal F-18-FDG PET was helpful in 2 patients. Conclusion: Ictal Tc-99m-HMPAO SPECT was helpful in lateralization but not in localization in OLE. Interictal F-18-FDG PET was helpful for localization of epileptogenic zones even in patients with ambiguous MR or ictal SPECT findings.

SVM-Based EEG Signal for Hand Gesture Classification (서포트 벡터 머신 기반 손동작 뇌전도 구분에 대한 연구)

  • An electroencephalogram (EEG) evaluates the electrical activity generated by brain cell interactions that occur during brain activity, and an EEG can evaluate the brain activity caused by hand movement. In this study, a 16-channel EEG was used to measure the EEG generated before and after hand movement. The measured data can be classified as a supervised learning model, a support vector machine (SVM). To shorten the learning time of the SVM, a feature extraction and vector dimension reduction by filtering is proposed that minimizes motion-related information loss and compresses EEG information. The classification results showed an average of 72.7% accuracy between the sitting position and the hand movement at the electrodes of the frontal lobe.
