현삼이 갑상선기능항진 유발 백서에 미치는 영향 (The Effects of Radix Scrophulariae (玄蔘) on the Hyperthyroidism of Rats)

  • Objective: The aim was to study experimentally the effects of Radix Scrophulariae (玄蔘) induced by Sodium levothyroxine. Methods: Sprague-Dawley rats Were separated into four groups. Except for the normal group, the other three were treated with sodium levothyroxine $160{\mu}g/kg$ for 5 days. Among the three groups, two (except the control) were treated with Radix Scrophulariae extract 0.5g/kg (HNS 1) or 1.0g/kg (HNS II). $T_3-uptake,\;T_3,\;T_4$, TSH, total cholesterol, glucose, ALP, AST, ALT, free fatty acid, $\beta-lipoprotein$, and change in the serum of rats were measured after medication of solid extract of Radix Scrophulariae. Results: As a result of this study, HNS II showed significant decrease in comparison with the control in the serum $T_3-uptake,\;T_3$ ALP and free fatty acid content. HNS I and HNS II showed significant decrease in comparison with the control in the serum of $T_4$ and glucose content. HNS I and HNS II showed significant increase in comparison with the control in the serum of TSH and total cholesterol content. In body weight, HNS I and HNS II showed significant increase in comparison with the control. Conclusion: These results suggest that Radix Scrophulariae is effective to cure hyperthyroidism.

준임상적 갑상선 기능저하증과 혈청 지질 및 비만도의 상관관계 (Correlations of Subclinical Hypothyroidism with Serum Lipid Profiles and Obesity Index)

  • Objectives: Though overt thyroid dysfunction is well recognized to affect serum lipid profiles and obesity, there are conflicting reports on the effect of subclinical hypothyroidism on serum lipid profiles and obesity. In most reports, the definition of the upper normal limit of serum thyroid stimulation hormone (TSH) of 4.0${\sim}$5.0mU/L has been used to diagnose. However, recent studies have suggested a much lower TSH cut off with an upper limit of 2.5mU/L, because >95% of rigorously screened normal euthyroid volunteers had serum TSH values between 0.4 and 2.5mU/L. Therefore we defined subclinical hypothyroidism as a TSH level greater than 2.5mU/L. We sought to evaluate the correlations of subclinical hypothyroidism with obesity index and serum lipid profiles Methods: TSH levels were measured in 6190 men and 4223 women that participated in health examination and free T4, lipid profiles (total-cholesterol, triglyceride, LDL-C), and obesity index (body mass index, body fat, waist circumference, C-reactive protein) were also measured. Results: There were significant differences of triglyceride between subclinical hypothyroidism men and normal control subjects. In women, there were also significant differences of triglyceride and LDL-C between subclinical hypothyroidism and normal control subjects. Subclinical hypothyroidism women showed significant correlations of TSH with total cholesterol, LDL-C, triglyceride, and C-reactive protein. Subclinical hypothyroidism men and women with a TSH level 2.5${\sim}$4.0mU/L had significant differences of triglyceride and body fat. In men, there were significant differences of waist circumference andC-reactive protein. In women, there were significant differences of LDL-C. Conclusions: We have demonstrated correlations of subclinical hypothyroidism with serum lipid profiles and obesity index. These findings suggested that subclinical hypothyroidism people had an increased risk of dyslipidemia and obesity. Subclinical hypothyroidism people with a TSH level 2.5${\sim}$4.0mU/L may be also considered suspect since it may signal a case of evolving thyroid underactivity eligible to be prevented.

Evaluation of Changes in Serum Thyroid Hormone Levels in Patients with Hepatitis B Infection

  • Purpose: We aimed to determine the differences in the levels of serum thyroid hormone (free T4 [FT4]) and thyroid stimulating hormone [TSH]) as biomarkers for hepatitis B virus (HBV) infection status, with respect to age and sex. Methods: We retrospectively analyzed serum samples from 200 patients who underwent HBV testing from August 2022 to September 2022. Serum samples were collected from patients suspected of having HBV infection who visited this hospital. Thyroid hormone levels were measured, and patients were grouped according to age and sex. Results: Differences in TSH and FT4 levels in the serum of patients in the HBV-positive and -negative groups were not significant. Among the HBV-positive patients in the younger age group (<60 years), TSH and FT4 levels were 1.78 ± 0.09 µIU/mL (normal: 0.4-5.0 µIU/mL) and 1.24 ± 0.02 ng/mL (normal: 0.8-1.9 ng/mL), respectively, whereas among the HBV-positive patients in the older age group (≥60 years), TSH and FT4 levels were 2.22 ± 0.17 µIU/mL and 1.24 ± 0.07 ng/mL, respectively. Conclusions: The presence of HBV did not markedly affect serum thyroid hormone levels. Our findings shed light on the conflicting evidence on the association between thyroid hormone levels and HBV infection. 자가면역성 갑상선질환에서 TSH 수용체 항체의 역활에 관한 연구 (The Roles of the TSH Receptor Antibodies in Autoimmune Thyroid Diseases)

  • To evaluate the clinical and pathogenetic roles of TSH receptor antibodies in autoimmune thyroid diseases, TBII were measured by TSH-radioreceptor assay methods in 352 patients with Graves' disease, 108 patients with other thyroid diseases and 69 normal persons. The normal range of TBII activity was less than 15%. The frequencies of detectable TBII in 169 patients with untreated Graves' disease, 31 patients with hyperthyroidism under treatment and 70 patients with euthyrodism under treatment were 92.4%, 87.1% and 54.3% respectively. However 12 (21.8%) out of 55 patients who have been in remission more than one year after discontinuation of antithyroid drugs treatment had detectable TBII activities in their sera. In 196 patients with untreated Graves' disease, the frequency of TBII increased by increasing size of goiter and the frequency of proptosis was significantly high in patients whose TBII activities were more than 60%. TBII activities were roughly correlated with total $T_3,\;T_4$ and free $T_4$ index but low $\gamma^2$ value(less than 0.1). In 67 patients with Graves' disease who were positive TBII before antithyroid drugs treatment, TBII activities began to decrease from the third months and it was converted to negative in 35.8% of patients at 12 months after treatment. There were no significant differences of the declining and disappearing rates of TBII activities between high dose and conventional dose groups. TBII activities were significantly increased initially (2-4 months) and then began to decrease from 5-9 months after $^{131}I$ treatment. There were two groups, one whose TBII activities decreased gradually and the other did not change untill 12 months after subtotal thyroidectomy. Although preoperative clinical and laboratory findings of both groups were not different, TBII activities of non-decreasing group were significantly higher than those of decreasing group$(74.6{\pm}18.6%\;vs\;39.2{\pm}15.2%;\;P<0.01)$. Thirty three(55.9%) out of 59 patients with Graves' disease relapsed within 1 year after discontinuation of antithyroid drugs. The positive rate of TBII at the end of antithyroid drug treatment in relapse group(n=33) was significantly higher than those in remission group (n=26) (63.6% vs 23.1%; P < 0.05). The mean value of TBII activities at the end of antithyroid drug treatment in relapse group was significantly elevated $(29.7{\pm}21.4%\;vs\;14.7{\pm}11.1%,\;P<0.05)$. Positive predictive value of TBII for relapse was 77.8%, which was not different from those of TRH nonresponsiveness(78.6%). The frequencies of detectable TBII in 68 patients with Hashimoto's thyroiditis, 10 patients with painless thyroiditis and 5 patients subacute thyroiditis were 14.7%, 20% and 0%, respectively. However in 25 patients with primary nongoitrous myxedema, 11 patients(44%) showed TBII activities in their sera. 9 out of 11 patients who had TBII activities in their sera showed high TBII activities(more than 70% binding inhibition) and their IgG concentrations showing 50% binding inhibition of $^{125}I-bTSH$ to the TSH receptor were ranges of 0.1-2.6 mg/dl. One patient who had high titer of TBII in her serum delivered a hypothyroid baby due to transplacental transfer of maternal TBII. These findings suggested that 1) TSH receptor antibodies are closely related to a pathogenetic factor of Graves' hyperthyroidism and of some patients with primary non-goitrous myxedema, 2) measurement of TSH receptor antibodies is helpful in evaluating the clinical outcome of patients with Graves' disease during antithyroid drug treatment and in predicting the neonatal transient hypothyroidism of baby delivered from primary myxedema patients. 3) there are 2 or more different types of TSH receptor antibodies in autoimmune thyroid diseases including one which stimulates thyroid by binding to the TSH receptor and another which blocks adenylate cyclase stimulation by TSH.

Thyroid-Stimulating Hormone Reference Ranges in the First Trimester of Pregnancy in an Iodine-Sufficient Country

  • Background: Thyroid dysfunction is associated with negative neonatal and obstetric outcomes. Large differences in thyroid function reference intervals exist across different populations. These differences can be explained by population-specific factors, such as iodine status. Many countries in Latin America report iodine sufficiency, but relatively few countries have published up-to-date data on iodine levels and thyroid function in the overall population, and especially in pregnant women. We evaluated the iodine status of pregnant women in Chile and determined thyroid hormone reference ranges in this population. Methods: This was a prospective observational study of healthy Chilean women at their first prenatal visit before week 14. Thyroid-stimulating hormone (TSH), total thyroxine ($T_4$), free $T_4$, antithyroid peroxidase antibody (TPOAb), and iodine levels from spot urine samples were measured. Iodine status and the reference ranges for TSH were calculated. Results: A total of 1,022 pregnant women in the first trimester were selected. Urinary iodine levels were measured in 302 randomly-selected women. The median urinary iodine concentration was $173.45{\mu}g/L$ (interquartile range, 108.11 to 249.35).The reference ranges of TSH were calculated in 670 patients selected according to the National Academy of Clinical Biochemistry guidelines. The median TSH level was $1.88{\mu}IU/mL$ (2.5th percentile: 0.13 to 97.5th percentile: 5.37). Using the reference range in the 1,022 women, the prevalence of clinical hypothyroidism was 1.76%, and that of subclinical hypothyroidism was 3.92%. TPOAb positivity was more common in women with TSH levels above $3.5{\mu}IU/mL$. Conclusion: We found adequate iodine intake and a right-shifted distribution of serum TSH levels in pregnant women in Chile. The prevalence of hypothyroidism in our sample of pregnant women was higher than has been described in the literature.

Final height of Korean patients with early treated congenital hypothyroidism

  • Purpose: Congenital hypothyroidism (CH) is the most common endocrine disorder in children. Thyroid hormone deprivation results not only in mental retardation but also growth retardation. This study investigates the final height (FH) in Korean patients with CH detected by newborn screening and examines factors that may affect the FH. Methods: The medical records of Korean CH patients (n=45) were reviewed. The FH was examined and target height (TH) was calculated based on mid-parental height. The FH z score (FHZ) and TH z score (THZ) were computed using the 2007 Korean National Growth Chart. The FHZ and THZ were compared with a Student t test. The impact of the etiology of CH (athyreosis, dyshormonogenesis, ectopic thyoid, hypoplastic thyroid), initial serum thyroid stimulating hormone (TSH) level, initial free thyroxine (T4) level, and time of therapy initiation based on FH was assessed. Results: The mean FHZ was $0.10{\pm}1.01$ for male patients and $-0.11{\pm}1.09$ for female patients. There were no significant differences between FHZ and THZ for both female (P=0.356) and male patients (P=0.237). No significant relationship was found between FH and the etiology of CH, initial TSH level, initial free T4 level, and the time of therapy initiation. Conclusion: Early intervention and satisfactory management do not appear to impede growth in Korean patients with CH. Thus, early detection and proper management of patients with CH detected by newborn screening program are necessary.

단일 뉴클레오타이드 결손으로 인한 Frameshift 돌연변이로 규명된 티록신결합글로불린 결핍증 1례 (A Single Nucleotide Deletion resulting in Frameshift in Two Korean Neonates with Thyroxine-Binding Globulin Deficiency)

  • TBG 결핍증은 X 염색체 장완의 TBG 유전자의 돌연변이에 의해서 발생하며, 낮은 총 $T_4$와총$T_3$, 정상 유리 $T_4$와 유리 $T_3$, 정상 TSH 농도를 특징으로 한다. 혈청 티록신글로불린 농도에 따라 완전 TBG 결핍증과 부분 TBG 결핍증으로 나눌 수 있으며, 적절하게 진단하지 못하면 불필요한 검사나 치료의 요인이 될 수 있다. 저자들의 완전 TBG 결핍증으로 진단된 2명의 남아에 대하여 TBG 유전자 분석을 시행하였다. 대상아들은 신생아 선별검사에서 측정된 낮은 총 $T_4$ 농도 때문에 내원하였다. 진찰 소견은 정상이었으며, 갑상선 기능 검사 상 유리 $T_4$, TSH 농도는 정상이었다. 방사면역측정법에 의한 혈청 TBG는 측정되지 않았다. 중합효소연쇄반응을 이용하여 4개의 TBG 유전자 엑손을 증폭한 후 자동염기서열분석을 시행하였다. 두 대상아에서 모두 엑손 4의 352번째 codon의 첫 번째 단일 뉴클레오티드 C의 결손에 의한 frameshift 돌연변이로 374번째 codon에 termination codon이 나타난 것을 확인하였다. 대상아의 어머니들에게서는 돌연변이 대립유전자와 정상 대립유전자의 이형접합체를 확인하였다. 한국인 TBG 결핍증의 역학과 유전적 특성을 규명하기 위한 더 광범위한 연구가 필요할 것으로 생각된다.

체외순환이 갑상선호르몬 농도에 미치는 영향에 관한 연구 (The effect of cardiopumonary bypass on the concentrations of thyroid hormone)

  • The hemodynamic effects of thyroid hormone are well established, and this hormone affects myocardial contractility, heart rate, and myocardial oxygen consumption. But the role of cardiopulmonary bypass on the thyroid function is not yet fully understood. We have studied twelve patients [male and female patients were equal in number] who were performed open heart surgery under cardiopulmonary bypass. The results are followed. 1] The serum level of T3 began to fall after cardiopulmonary bypass and sustained significantly till 24 hours after operation[p<0.05] 2] The concentrations of T4, Free T4, and TSH were slightly decreased after cardiopulmonary bypass but was maintained within normal range. 3] This above findings are similar to the "sick sinus syndrome" that is seen in severely ill patient. 4] We can propose that T3 would be effective in postoperative low cardiac output syndrome. syndrome.

  • Objective : This study was designed to evaluate oriental medicine therapy on a hyperthyroidism(T3 toxicosis) patient Methods : The clinical data was analyzed on a patient with hyperthiroidism(T3 toxicosis). This patient was treated with oriental medicine therapy only. After treatment, we analyzed a paient clinical symptom and thyroid function(T3, free T4, TSH) Results : After treatment, improvement was seen in clinical symptom, thyroid function test. conclusion : The study suggests that oriental medicine therapy is effective on the treatment of hyperthyroidism(T3 toxicosis).

한열변증 설문지와 일반적 건강 검진 결과와의 상호 연관성에 관한 비교 연구 (Health Test for Searching of Correlation and the Index of the Cold-heat Patternization Comparison of the Questionnaires for the Cold-heat Patternization and the Ordinary)

  • Background and purpose: The cold-heat patternization is one of the most frequently use for diagnostic method in oriental medicine. But it is still an unclear scientific mechanism and the objective index. the aim of this study is to search the objective index of the cold-heat patternization, comparing with ordinary health test. Method: The study group comprised 101 Korean, male:female ratio 51:50, with a mean age of 49.74 years. The results of the questionnaires for cold-heat patternization were compared with the western health test that it consist of height, weight, muscle mass, body fat, WBC, Hb, ALT, AST, TG, Total cholesterol, glucose, HDL-cholesterol, free-T4 and TSH, measured in the health promotion center of the hospital of Chungnam university Result: The cold patternization were significantly higher in the women than the man. The somothing of the questionmaires for the heat patternization were positive correlated with TG, TSH, glucose, Hb, free T4, weight, musle mass(p<0.05). But there were no correlation in the gross. The somothing of the questionnaires for the cold patternization were negative correlated with the weight, musle mass, body fat, glucose, Hb and ALT(p<0.05), Especially there were strong negative correlation with the weight and muscle mass(p<0,01). Conclusion: The present study shows there is no definitive index for the cold-heat patternization in the ordinary health test. But the weight and muscle mass can be useful index of the cold patternization.
