Diagnostic performance of dental students in identifying mandibular condyle fractures by panoramic radiography and the usefulness of reference images

  • Purpose : The purpose of this study was to evaluate the diagnostic performance of dental students in detection of mandibular condyle fractures and the effectiveness of reference panoramic images. Materials and Methods : Forty-six undergraduates evaluated 25 panoramic radiographs for condylar fractures and the data were analyzed through receiver operating characteristic (ROC) analysis. After a month, they were divided into two homogeneous groups based on the first results and re-evaluated the images with (group A) or without (group B) reference images. Eight reference images included indications showing either typical condylar fractures or anatomic structures which could be confused with fractures. Paired t-test was used for statistical analysis of the difference between the first and the second evaluations for each group, and student�fs t-test was used between the two groups in the second evaluation. The intra- and inter-observer agreements were evaluated with Kappa statistics. Results : Intra- and inter-observer agreements were substantial (k=0.66) and moderate (k=0.53), respectively. The area under the ROC curve (Az) in the first evaluation was 0.802. In the second evaluation, it was increased to 0.823 for group A and 0.814 for group B. The difference between the first and second evaluations for group A was statistically significant (p<0.05), however there was no statistically significant difference between the two groups in the second evaluation. Conclusion : Providing reference images to less experienced clinicians would be a good way to improve the diagnostic ability in detecting condylar fracture.

동측에 발생한 쇄골 골절과 견갑골 골절의 수술적 치료 (Operative Treatment of Ipsilateral Fractures of Clavicle and Scapula)

  • The superior shoulder suspensory complex is composed of glenoid fossa, coracoid process, coracoclavicular ligament, distal clavicle, acromioclavicular ligament, acromion. Traumatic double disruptions of this complex lose its suspensory action on the shoulder joint and result in functional loss and deformity. Careful radiologic evaluation and appropriate management are required for injuries to this complex. Ipsilateral fractures of clavicle and scapula create unstable anatomic situation on shoulder joint. Conservative treatment usually fails to achieve good functional recovery due to rotator cuff weakness, nonunion, delayed union, malunion and neurovascular injury. Authors studied the result of operative treatment of ipsilateral clavicle and scapular fractures to prevent such complications. Seven cases were treated with open reduction and internal fixations of clavicle alone or clavicle and scapula simultaneously and followed up for nineteen months(twelve months - thirty-eight months). All but one patient showed good or excellent functional result according to the scoring system of Rowe. Poor result was developed in the case which had brain injury. Rigid fixations of clavicle alone or clavicle and scapular fractures both can achieve stable reduction of the fractures and prevent sequelae. We concluded that operative treatment of ipsilateral fractures of clavicle and scapula is safe and yields predictable good results.

Fracture analysis of wild mammals in South Korea

  • This study was conducted to evaluate and analyze fractures types in wild mammals in Korea. Wild mammals treated at 3 wildlife centers in Korea were included. Of the 63 cases included in this study, water deer was the most common species encountered with fractures. We determined 85 different fracture sites, with maximum lumber vertebral fractures followed by femoral, tibial and pelvic fractures. External skeletal fixator (ESF)-intramedullary (IM) pin (IM+ESF tie-in fixation) was the most common method applied for the surgical treatment of mammals. Lumbar vertebrae and femur were the most common bones involved. The findings of this study are expected to provide information for veterinarians regarding the frequency and characteristics of fractures in wild mammals and serve as a basic database for the types and outcomes of fractures in wild mammals.

  • The purpose of this study was to assess the influence of the presence and impacted state of the mandibular third molars on the incidence of mandibular condyle fracture. A retrospective study was designed for patients presenting to the Department of Oral and Maxillofacial Surgery, Kyungpook National University Hospital and Tae-gu Fatima Hospital for treatment of mandibular fractures from January 2003 to January 2006. The independent variables in this study were the presence, degree of impaction of third molars, and the outcome variables were the incidence of mandibular condyle fractures. Hospital charts and panoramic radiographs were used to determine and classify these variables. The demographic data included age, sex, mechanisms of injuries and number of mandibular condyle fractures. The study sample comprised 136 mandibular condyle fractures in 105 patients. Result of this study demonstrated a statistically significant difference in ipsilateral condyle fractures and mandibular third molar absence(P=0.032) and bilateral condyle fractures without another fracture and mandibular third molar absence(P=0.028).

  • Today, endoscopic procedures are common in diagnostic and other surgical procedures, with endoscopically enhanced and magnified images permitting surgical access through minimal incisions. This has expanded the treatment options for many difficult anatomic sites, and the endoscope facilitated safe anatomic reduction and fixation. The use of the endoscope may reduce the disadvantages of open fracture repair and should be considered for broad application in the treatment of displaced facial bone fractures. Optical endoscopic magnification minimizes the disadvantages associated with open surgical repair, including the risk of facial nerve injury and external facial scarring, and no postoperative complications have been attributable to the endoscopic approach. This technique was used in 14 patients treated at Ulsan University Hospital, Korea, from September 2004 to August 2006, including six mandibular subcondyle fractures, five blowout fractures and three zygomaticomaxillary complex (ZMC) fractures. Careful preoperative evaluation and proper surgical technique were essential to achieve optimal results in the selected patients.

  • 양측 쇄골 골절은 양측 견관절 부위에 강한 외력이 동시에 작용하여 발생하는 드문 골절이다. 편측 쇄골 골절은 보존적 치료로 대부분 만족할 만한 임상 경과를 보이나 동시에 양측이 골절 되는 경우 극심한 통증과 재활 운동의 어려움을 겪게 되며, 호흡운동 장애를 초래할 수도 있다. 저자들은 다발성 늑골 골절 및 혈흉과 흉추 가시돌기 골절을 동반한 양측 쇄골 골절 환자에 대해 수술적 치료를 통해 견관절 기능 회복을 얻었으며, 호흡운동 장애 문제를 해결하였기에 문헌 고찰과 함께 보고하고자 한다.

하악골에 발생한 병적 골절 (Pathologic Fractures of the Mandible)

  • Pathologic fractures of the mandible can occur for many reasons including osteoradionecrosis, osteomyelitis, malignancy and cyst. Pathologic fractures are difficult to treat because management is needed not only for the fractures of the mandible but also the underlying disease the is weakening the bone. Additionally, the diseased mandible in the pathologic fracture frequently has abnormal bone healing capacity. We experienced three cases of pathologic fractures of the mandible resulting from cyst, malignancy and osteomyelitis. The treatment of these cases was complicated and time-consuming. Therefore, we present our three cases and discuss the management of pathologic fractures of the mandible.

지하 유류저장 공동의 지질구조와 공동누수량 상호관계에 관한 사례 (A Case Study of Correlation between Inflows and Geological Structures around Underground Caverns)

  • 일반적으로 공동 주변에 분포하고 있는 지질구조는 지하 암반 공동 굴착시 지수, 보강, 지하수 유동 등에 상당한 영향을 주게 되므로 지질구조의 분포 및 특성을 아는 것이 매우 중요하다. 본 연구에서는 거제 지역에 기 시행된 지하 비축시설의 조사, 설계 및 시공자료를 토대로 지질구조와 공동 누수량 상호 관계를 분석한 결과, 본역에 발달하고 있는 주 함수구조대의 방향은 N50∼60W, 경사는 거의 수직이며 인장 단열로서 추정된다. 이 구조대는 양수 시험시 타원형의 장축 방향과 일치하고 N10∼30E 방향의 단열들과 교차하고 있으며, 화강섬록암에서 나타나는 전형적인 장방형 단열계를 나타낸다.

Incidence of and Factors for Self-reported Fragility Fractures Among Middle-aged and Elderly Women in Rural Korea: An 11-Year Follow-up Study

  • Objectives: This community-based cohort study was performed to investigate the incidence of and factors related to self-reported fragility fractures among middle-aged and elderly women living in rural Korea. Methods: The osteoporosis cohort recruited 430 women 40 to 69 years old in 1999, and 396 of these women were followed over 11 years. In 1999, questionnaires from all participants assessed general characteristics, medical history, lifestyle, menstrual and reproductive characteristics, and bone mineral density. In 2010, self-reported fractures and the date, site, and cause of these fractures were recorded. Cox proportional hazards models were used to calculate hazard ratios (HRs). Results: Seventy-six participants among 3949.7 person-years experienced fragility fractures during the 11-year follow-up. The incidence of fragility fractures was 1924.2 per 100 000 person-years (95% confidence interval [CI], 1491.6 to 2356.8). In the multivariate model, low body mass index (HR, 2.66; 95% CI, 1.13 to 6.24), a parental history of osteoporosis (HR, 2.03; 95% CI, 1.18 to 3.49), and postmenopausal status (HR, 3.50; 95% CI, 1.05 to 11.67) were significantly related to fragility fracture. Conclusions: Fracture prevention programs are needed among postmenopausal, rural, Korean women with a low body mass index and parental history of osteoporosis Korea.

Prognosis of Pulmonary Function in Patients with Multiple Rib Fractures

  • Purpose: Rib fracture is the most common complication of blunt thoracic trauma. We investigated the effect of rib fracture on pulmonary function in the conservatively treated patients. Methods: From January 2000 to February 2017, we reviewed the records of 72 patients with rib fracture and pulmonary function tests were performed. According to the number of rib fractures, patients were classified into two groups: less than six fractured ribs (group A) and more than six fractured ribs (group B). The groups were compared concerning demographics, underlying diseases, associated thoracic injuries, surgery, mechanical ventilator times, days spent in the intensive care unit and pulmonary function test. Results: There were no statistically significant differences in the demographic data between the two groups. Mean hospitalization was 13.5 days in group A and 27.0 days in group B (p<0.001). There was no statistically significant difference between the two groups in the pulmonary function test. Conclusions: We conclude that pulmonary function is restored by conservative treatment in patients with rib fractures even if the number of rib fractures increases. In patients with multiple rib fractures, studies comparing open rib fixation and conservative treatment of long term pulmonary function are required.
