가상 기관지경으로 재구성한 기관지 내 이물 1예 (A Case of Bronchial Foreign Body Reconstructed with Virtual Bronchoscopy)

  • Computed tomography with virtual bronchoscopy shows inner shape of trachea and major bronchi with three-dimensional images. Virtual bronchoscopy can be used in cases of suspected foreign body aspiration. Virtual bronchoscopy shows the exact location of the obstructive lesion before conventional bronchoscopy. If the chest radiography is normal or suggests aspirated foreign body, virtual bronchoscopy can be considered before conventional bronchoscopy. We experienced a case of false-tooth foreign body extracted under bronchoscopy. In this case, virtual bronchoscopy was made retrospectively giving useful information about shape and orientation regarding foreign body.

  • Sometimes a pilomatrixoma on upper neck can be misdiagnosed as primary salivary gland tumor, skin adnexal tumor or metastatic carcinoma. On fine needle aspiration cytology(FNAC), characteristic features are ghost cells, basaloid cells, and calcium deposits, among which presence of ghost cells seems to be the key to recognize it. Here we present a cytologically misdiagnosed case of pilomatrixoma. A 32-year-old man presented a subcutaneous mass on the right posterior neck. It has grown slowly for 1 year, and was nontender, well circumscribed, hard, and movable mass. An Initial FNAC yielded only monomorphic population of highly mitotic basaloid cells, without anucleated ghost cells, chronic inflammatory cells or foreign-body giant cells, suggestive of a poorly differentiated carcinoma. However, that was inconsistent with patient's clinical findings. For further correct diagnosis, FNAC was repeated, which yielded the basaloid cells and foreign-body giant cells. The diagnosis of pilomatrixoma could be made and the mass was excised. This case demonstrates that the pilomatrixoma should be considered in the differential diagnosis of subcutaneous aspirates containing not ghost cells but a dominant population of basaloid cells.

기도이물: 10년간 101례의 경험 (Laryngo-tracheo-bronchial Foreign Bodies: 10-Year Experience of 101 Cases)

  • Aspiration of foreign bodies into the airway is a common problem in spite of efforts to educate the public, and it can be sometimes fatal accidents. We clinically analyzed 101 cases of airway foreign bodies treated in the Department of Otolaryngology-Head and Neck Surgery, Korea University College of Medicine for the past 10 years comparing data with the previous articles. There was male predominance in the occurrence rate, being 72% in the male patients. The peak age was 1 to 3 years of age, and 84.2% was below age of 10. The most common symptom after foreign body aspiration was intractable cough, and obstructive emphysema was most commonly found on the initial chest X-rays. Foreign bodies were mainly located at the right main bronchus and left main bronchus was the next. Peanuts were the most common airway foreign bodies. Ventilating bronchoscopy was performed in the 99 cases and foreign bodies were successfully removed in most cases. There was 2 deaths(2%) resulted from hypoxia after bronchoscopy. The fact that complication rates increase with the duration of the foreign bodies in situ is clear. Therefore, prompt bronchoscopy in patients with suspected airway foreign bodies is essential for lower complication rates.

  • Aspirated and ingested foreign bodies continue to present challenges to otolaryngologists. The major discussions were the accurate diagnosis and speedy, safe removal of foreign body. Many diagnostic tools have been tried and removal of foreign bodies has been facilitated by technical improvements with rod lens telescope, video endoscope, flexible fiberoendoscope and safer anesthesia. In spite of these advances, more than 3000 children's death occur per year in the world because of foreign bodies and untold number of parients survive with variable sequelae. In these study, 59 consecutive cases of children and adults with tracheobronchial foreign bodies were reviewed from 1992 to 2001. We studied the history, symptoms, ausculatory radiologic, bronchoscopic finding and post operative complications. 71% (42 cases in 59 cases) of patients had foreign body aspiration history or choking crisis. In 64% (38cases) cough was observed. 81% (48cases) had abnormal finding in chest auscultation and 78% in chest X-ray. Computed tomography was done in 12 cases, all were founded foreign body shadow. Main site of foreign body was right main bronchus (41%, 24cases) and most frequent foreign body was peanut (36% , 21cases) . 4 experienced ICU(intensive care unit) care. 2 cases were failed to remove foreign. In these cases 1 cases was improved by steroid therapy and physical therapy and the other was treated with thoracotomy. We concluded the morbidity and motality were much correlated with speedy decision making and experienced skill of operator.

A Case of Bilateral Tension Pneumothorax after the Successful CO2 Laser-assisted Removal of a Bronchial Foreign Body in a Child

  • Bronchial foreign body aspiration (BFA) is a common but emergent condition in infants and children. Furthermore, it can result in various complications such as atelectasis, pneumonia, bronchiectasis, and pneumothorax. Among these, pneumothorax is a very rare complication. However, it can be fatal without the swift implementation of appropriate treatment. We experienced a case of 16-month-old girl with an aspirated peanut. The foreign body was fixed in her left main bronchus. A CO2 laser was used to safely cut and break the foreign body. Removal was successful after breaking it. But after the process, inflammatory tissue of the tracheal mucosa was ruptured. Bilateral tension pneumothorax followed after the rupture. The patient was treated with bilateral chest tube insertion. Here we present this BFA case with a rare and unexpected complication. We also review the appropriate literature.

  • Foreign body (FB) aspiration remains a serious health problem, particularly in children. The complications due to occlusion by FB or related to procedures for removal can lead to morbidity and mortality. Most of the FBs are located in the unilateral bronchus, however, the organic FB can be multiple or bilaterally located when children chewed the FB. Here, we present a case of successful retrieval of bilateral bronchial FBs. Preoperatively, FB in the left main bronchus was diagnosed, however, FB of the right upper bronchus was not definitive on X-ray and CT scan. During the rigid bronchoscopy, occult FB in the right upper bronchus was detected and successfully removed.

  • 저자들은 최근 약 30년전 치과에서 발치도중 치아 흡인되었으나 환자가 모르고 지내오다 5개월 전부터 시작된 좌상부 흉통을 주소로 내원, 폐엽절제술후 치아 흡인으로 인한 기관지폐쇄로 확인된 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

  • 경막외기종은 자발기종격, 자발기흉, 천식 발작, 기침, 심한 구토, 외상 등에 의해 발생할 수 있지만 기관지 이물에 의한 경우는 드물고, 고환기종 역시 대장내시경, 진단적 복강경, 간생검 등에 의해 발생하는 경우가 보고되고 있지만, 기관지 이물에 의한 경우는 아직까지 국내 외에 보고된 적이 없다. 이에 본 저자들을 기관지 이물에 의해 심한 고환기종 및 경막외기종, 기종격, 기흉이 발생한 18개월된 남아에 대해 보고하는 바이다.
