Clinical Applications of Peroneal Perforator Flap (비골동맥 천공지 피판의 임상적 적용)

  • The perforator flaps are based on cutaneous vessels which are originated from a main pedicle and penetrate fascia or muscle to reach the skin. The lateral lower leg is one of the most suitable areas for harvesting perforator flaps because a number of perforator vessels exist. The authors applied peroneal perforator flaps in nine patients. Five flaps were reverse island flaps based on peroneal artery and septocutaneous perforator, and four flaps were free flap based on musculocutaneous perforator only. The recipient site was the posterior ankle in three patients, posterior heel in three patients, lateral malleolus, anterolateral ankle, and foot dorsum in one patient each. The flap size ranged from 5 to 12cm long, from 3 to 5cm wide, and the primary closure of the donor site was possible in most cases. All flaps, except for the flap in two patients in the reverse island flap series, survived completely. The peroneal perforator flap is a very thin, pliable flap with minimal donor site morbidity and is suitable for the reconstruction of small and medium sized superficial skin defects. Also, this flap may be considered as an alternative to radial forearm flap or other perforator flaps.

Reverse Dorsalis Pedis Flap Based on the Distal Communicating Artery of the Dorsalis Pedis Artery for the Reconstruction of the Forefoot Defect (원위교통동맥 기저 역행성 족배피판을 이용한 전족부 결손의 재건)

  • A 31-year-old female patient presented with a skin and soft tissue defect measuring $8{\times}6cm$ in size with exposure of the extensor hallucis longus tendon and the first metatarsal bone after metatarsal lengthening for brachymetatarsia. The defect was covered with a distally based dorsalis pedis flap based on the distal communicating branch of the dorsalis pedis artery. Secondary defect was covered by a split thickness skin graft. There was congestion of the flap tip after the operation; however, it was resolved using medical leeches and anti-coagulants. No necrosis or infection was encountered and the contour of the flap was satisfactory. There was no donor site morbidity. Reverse dorsalis pedis flap has not been commonly used due to the anatomical variation and uncertainty, which is different from the reverse radial forearm flap. However, when faced with the challenge of a moderate soft tissue defect of the distal forefoot, we believe that the reverse dorsalis pedis flap offers a good option with various advantages.

The usefulness of skin test in evaluation of immunity to varicella (수두에 대한 면역력 평가에 있어서 피부 시험의 유용성)

  • Purpose : The aim of this study is to assess the usefulness of skin test by an inactivated, 1/50 diluted solution of attenuated varicella vaccine in evaluating the immune status to varicella. Methods : Total 41 subjects (22 males, 19 females, aged 1-32 years) were enrolled from July to August, 2005. Past medical history including varicella infection, varicella vaccination were investigated through questionnaires. The skin test solution was prepared from solution of attenuated varicella vaccine(Oka strain) which was inactivated by exposure to room temperature for 10 days and diluted at 1/50 with normal saline. Skin test was done by injecting 0.1 mL of the solution intradermally into the volar surface of the right forearm and sterile normal saline was used as a control on the left forearm. Positive reaction was defined when the transverse diameter of the induration was 5 mm or more. Serum varicella zoster virus specific IgG antibody test by ELISA (enzyme-linked immunosorbent assay) was done. Results : In adults, the sensitivity of the varicella zoster virus skin test compared to ELISA was 94.7% and the positive predictive value was 100%. In children, both the positive predictive value and specificity were 100% but the sensitivity and the negative predictive value were 50% and 30.7% respectively. Children showed smaller skin test reactivity compared to adults. Conclusion : The varicella zoster virus skin test using inactivated, 1/50 diluted solution of attenuated varicella vaccine was proved as one of the useful tools for evaluating the immunity and susceptibility of the varicella zoster virus.

Changes in the Skin Temperature and Electrical Skin Resistance Following Ingestion of Red Pepper (고온환경에서의 한국인 작업능력 - 고추가 피부의 온도 및 전기저항에 미치는 영향 -)

  • In order to investigate the changes in the temperature regulation mechanism following ingestion of red pepper, twenty healthy man were given 2 gms of powdered red pepper each in 100 ml of water at room temperature, $20^{\circ}C$, Simultaneous recordings of temperature and electrical resistance of the skin of forehead were taken by means of Physiograph, using appropriate transducers. Temperature of various spots such as the cheek, upper tack and the forearm was also measured by the telethermometer at interval of two minutes. The results obtained were as follows: 1. Seventeen out of 20 subjects showed immediate rise in the skin temperature of the forehead, and 11 of them manifested the oscillating pattern of the skin temperature. The average of the largest peak amplitude was $0.58{\pm}0.355^{\circ}C$. 2. Even those who failed to show the immediate rise in the temperature did not keep the quiescence over 9 minutes, and delayed responses were revealed. 3. The rise in tile skin temperature is the favourable sign for the heat discipating mechanism only because the sweat glands are activated with the concomittant rise in the temperature of the skin. 4. There was a preceding or coincide fall in the electrical resistance of the skin, and it was also attributed to the glandular activity. 5. At rather cool room temperature, $20^{\circ}C$, no visible sweat was seen during the period of observation. Nevertheless it was obvious that latent activation of the sweat glands was triggered and the. threshold was lowered. This situation imitates the acclimatized condition in the hot environment, and it is likely to increase the tolerance in tropical climate from the view point of temperature regulation.

Standardization of Hydration in the Stratum Corneum Using by Polyols (폴리올을 이용한 각질층 수분량 측정의 표준화 연구)

  • The measurement of hydration level in the surface layer of the skin, stratum corneum (SC), gives important information on the biophysical properties and function of the skin barrier such as softness, flexibility, and healthiness of the skin. But it is difficult to measure a consistent hydration level from a sample to another sample due to individual variations and environmental changes. The aim of this study was to evaluate objective hydration after using various products in the SC. The SC Hydration was measured by capacitance (Corneometer$^{(R)}$, C+K, Germany) on the ventral site of forearm from 40 healthy volunteers. The skin surface was chronologically measured immediately after application of the test products and 3 and 6 hours later. We analyzed the averages of five measurements of each site and used the hydration increase rate for correction on untreated site variation. We found that most polyols including glycerol and butylenes glycol influenced directly the hydration increase rate in the SC previously. In this study, glycerol was used to prepare the standard products from 0 to 20 percents and applied to the same volunteers. The individual standard curve showed linear relation to glycerol concentrations. Based on the the standard curve, hydration of SC was converted into hydration increase rate to glycerol concentrations. The converted glycerol concentrations of products were repetitive and reproducible. In addition, the individual standard curve was used to relate the skin type of each individual. These results suggest that the hydration of the SC standardized regardless of external variation and individual skin condition can explain detailed skin state variation. Further studies will be conducted with other ingredients such as surfactants, lipids and aqueous materials, and with other methods for noninvasive measurement.

The Change of the Vital Sign by the Variables of Stimulated Areas in Interferential Current Treatment (간섭 전류 치료의 자극부위에 따른 활력징후의 변화)

  • Background: The purpose of this study is to have examined the influence on the blood circulation by comparing the differences between stimulating the sympathetic ganglion and the muscle group among the stimulation variables in interferential current stimulation. Method: The object of the study is the twenties(M=8, F=12), who are in great condition and have no pathological report for the blood circulation influence. The intensity of the inferential current stimulation is the medium degree, 100 bps constant current, which is the comfort and degree to confirm the muscle contraction. The areas stimulated are the stellate ganglion area in the seventh cervical vertebrae and the forearm muscle area. Results: We have made sure that there is no change in blood pressure and pulse and that the change in the skin temperature occurred highly. Conclusion: In considering the change of the blood circulation in case of stimulation area by the inferential current stimulation, we have seen that stimulating the sympathetic ganglion area is more effective than stimulating muscle area directly.

A Case Report on Superficial Second-Degree Burn of an Infant's Forearm (영아 전완부 표재성 2도 화상 치험례)

  • Objectives The purpose of this study is to report the effect of Korean medical treatment on an infant with burn injury. Methods We treated the infant suffering from burn injury by Korean medical treatment such as acupuncture, Hwangryunhaedoktang extract, Jaungo dressing from March 15th, 2016 to March 26th, 2016. This efficacy of treatment was evaluated with observation. Results After this treatment, burn wound size was reduced and skin tissue regeneration was accelerated. As time passes, pigmentation was faded. Conclusions This case report showed that the Korean medical treatment is effective in the treatment of pediatric burn. However further case studies are still required to confirm these findings.

Surgical Treatment of Cancer of Tongue and Floor of Mouth (설암 및 구강저암의 수술적 치료)

  • The records of 18 patients with squamous cell carcinoma of the tongue and floor of mouth treated surgically were reviewed. Surgical approaches, staging, treatment modalities, recurrence and vital status were evaluated. The distrubutions of involved sites were tongue(9 cases) and mouth floor(9 cases). Patients were treated by surgery primarily, combined chemotherapy and radiation, and by surgical salvage in the failure cases of radiation and chemotherapy. All ipsilateral necks of mouth floor cancer and advanced tongue cancer were treated with neck dissection. Cases of early tongue cancer could be excised with transoral route, and advanced cases needed transmandibular approach. Whereas, majorites of mouth floor cancer needed transmandibular approach, and other cases could be excised transoral and pull-through approaches. In the recontructions, we used primary closure, pectoralis major myocutaneous flap, forearm free flap, fibular osteocutaneous flap and skin graft. One year survival rate was 93% and 2 year survival rate was 60%.

A Clinical Experience of Direct Extension to Parotid Gland of Cutaneous Squamous Cell Carcinoma (귀밑샘을 침범한 피부 편평세포암종의 치험례)

  • Cutaneous squamous cell carcinoma has a high incidence. However, regional metastasis occurs infrequently because skin cancer is usually recognized and treated early. We report the case of squamous cell carcinoma around the earlobe in a 74-year-old male patient. The cutaneous squamous cell carcinoma invaded ipsilateral parotid gland directly without lymphatic spreading. Wide excision was made with 1.5 cm margin and immediate reconstruction was performed with radial forearm fasciocutaneous free flap. During operation facial nerve was preserved. No recurrence was noted for 5 years and the patient was satisfied with good aesthetic result. Cutaneous squamous cell carcinoma spreads to the parotid gland usually through lymph nodes and there are few reports of invasive organ damage by direct invasion. We experienced a case of direct invasion to parotid gland without lymph node involvement of cutaneous squamous cell carcinoma and treated the cancer adequately with wide excision and free flap coverage.

Defatting Vestibuloplasty for Functional and Esthetic Reconstruction of Tongue

  • The radial forearm free flap (RFFF) is a thin and pliable tissue with many advantages for tongue reconstruction. However, tongues reconstructed with RFFF occasionally need revision surgery because inadequate defect measurement at primary surgery can lead to bulkiness and limited movement of reconstructed tongue. In this case, the patient underwent partial glossectomy and RFFF reconstruction for treatment of tongue cancer five years prior. We could not make a lower denture for the patient, because the alveolo-lingual sulcus of tongue was almost lost. So we performed vestibuloplasty with a modified Kazanjian method on the lingual vestibule of the mandibular right posterior area, and defatting surgery to debulk the flap. After surgery, we observed that the color and texture of the revised tongue changed to become similar with adjacent tissue. The patient obtained a more functional and esthetic outcome. Accordingly, we present a case report with a review of relevant literature.
