MRI에 의한 한국인 신체분절의 생체역학적 모수치 산출 (Employing Magnetic Resonance Imaging(MRI) in the Estimation of the Biomechanical Body Segment Parameters of Korean Adults)

  • The purpose of this study was to employing MRI in the estimation of the biomechanical body segment parameters of Korean adults. for this purpose MRI study on 19 Korean living subjects were used to measurement. All the parameters that was concerned were inertial characteristics of human body mass of each segment, center of mass of them and the length of radius of gyration of them. The cross sectional images and saggital images of every 1cm interval were got using the 0.5 Tesla MRI from the top of head to the bottom of foot, whole body. And then, by tracing the images of the film and scanning them, got the area which the several tissues occupied in the image of slice. By summing the area of slice of each segment which were calculating and by multipling the density of the tissues, got the mass of segment and other inertial characteristics. Online Trajectory Planning for a PUMA Robot

  • Robotic applications, such as automatic fish cutting, require online trajectory planning because the material properties of the object, such as the bone or flesh conditions, are not known in advance. Different trajectories are required when the material properties vary. An effective online trajectory-planning algorithm is proposed using quaternions to determine the position and orientation of a robot manipulator with a spherical wrist. Quaternions are free of representation singularities and permit computationally efficient orientation interpolations. To prevent singular configurations, the exact locations of the kinematic singularities of the PUMA 560 manipulator are derived and geometrically illustrated when a forearm offset exists and the third link length is not zero.

Reducing the donor site morbidity in radial forearm free flaps by utilizing a narrow radial forearm free flap

  • Background The radial forearm free flap (RFFF) has remained a leading choice of many plastic surgeons as a fasciocutaneous flap due to its versatility, pedicle length, and simple elevation technique. However, donor site morbidity has led many reconstructive surgeons to limit their use of the RFFF and to use other flaps instead. We propose that using a narrow RFFF (nRFFF) decreases the aesthetic and functional morbidity of the donor site. Methods We report our experiences with the nRFFF from April 2012 through May 2015 at the Department of Plastic, Reconstructive, and Hand Surgery at Liaquat National Hospital, Karachi. The donor defects were closed primarily. The Stony Brook Scar Evaluation Scale and comparison with the contralateral hand were used to assess aesthetic and functional outcomes, respectively. Results A total of 24 patients underwent nRFFF procedures during the study period. The donor arm showed excellent motor function in 22 cases (91.7%), and very good function in the remaining two cases (8.3%). The aesthetic outcomes were excellent in four patients (16.6%), very good in eight patients (33.3%), good in 10 patients (41.6%), and fair in two patients (8.3%) who developed a hypertrophic scar. All flaps were successful and there were no cases of partial or complete loss. Conclusions For small to medium-sized soft tissue defects, the nRFFF had acceptable outcomes due to its thinness, pliability, and major reduction in donor site aesthetic and functional morbidity.

고압 전기감전시 발생한 수근관절 주위의 진구성 연부조직 결손에 대한 유리 피판술 (Free Flaps for Old High Tension Electrical Burns Around the Wrist)

  • With the advent of microvascular free-tissue transfer, this single stage resurfacing method for large scar and soft tissue defects around the wrist in the patients of electrical burn has distinctive advantage over the conventional multistage pedicle-flap transfer. Between 1992 and 1996, we treated 9 cases of 8 patients who had large scar around the wrist due to old electrical burn with free flaps as a preparation of staged tendon graft. Mean age was 30.3 years and average scar area was $6{\times}11cm$. The length of time the injury and free flaps was 9 months on an average. Prior to the free flap, we performed the angiography to all patients in order to evaluate the circulation of the forearm and hand and to choose the recipient vessel. In all cases, proximal ulnar arteries in the forearm remained intact and all radial arteries remained intact in 8 of 9 cases on angiogram. The interosseous arteries were well visualized in all cases. We used the ulnar arteries as a recipient artery. The types of flaps used were f scapular cutaneous flaps, 2 dorsalis pedis flaps and a radial forearm flap. Flap survial was 100 percents with satisfactory functional and cosmetic results. Free flaps using ulnar artery as a recipient artery is one of the useful reconstruction methods for the resurfacing of large scar around the wrist in the patients of old electrical burn.

소방공무원의 장갑제작을 위한 손 치수 연구 (Hand Sizing Study for Development of Firefighting Gloves)

  • This study measured and analyzed male firefighters hand sizes as well as suggested data for developing firefighting gloves with a good fit. Accordingly, body dimensions related to the hand were measured for 100 male firefighters. Twenty-three measuring items related to the hand and lower arm were selected. The firefighters' measurement data were compared to general adult male measurements from the 6th Korean Anthropometric data to analyze the hand sizes characteristics of firefighters. As a result, height, hand circumference, hand breadth, wrist circumference, elbow-wrist length, forearm-fingertip length and firefighter weight were significantly larger than adult males. A correlation analysis for 23 items was conducted to select items used as standards for setting a size when manufacturing firefighting gloves. Hand length and circumference were selected as representative hand parts for making firefighting gloves. Cross analysis between hand length and circumference were conducted to find the section showing the highest distribution ratio. As a result, the highest sections were between 19cm and 21cm for hand circumference and between 18.5cm and 19.5cm for hand length, which may be suggested as the most important ranges.

동맥화 정맥 유리 피판술을 이용한 수부와 수지 연부조직 결손의 재건 (Soft Tissue Reconstruction of Finger and Hand Using Arterialized Venous Free Flap)

  • Flaps are necessary, when important structures such as bone, tendon, nerve and vessel are exposed. Arterialized venous free flap is suited to the coverage of finger and hand because the thickness of venous flap is thin. Authors performed 65 cases arterialized venous free flap for the soft tissue reconstruction of the hand and finger. The size of donor defect were from $1{\times}1cm\;to\;7{\times}12cm$. The mean flap area was $9.1cm^2$. The recipient sites were finger tip in 34 cases, finger shaft in 29 cases and hand in 2 cases. The donor sites were volar aspect of distal forearm in 40 cases, thenar area in 17 cases and foot dorsum in 6 cases. The types of arterialized venous free flap were A-A type in 4 cases and A-V type in 61 cases. The length of afferent vein was from 0.5 cm to 3 cm (mean 1.7 cm) and efferent vein was from 1 cm to 10 cm (mean 2.2 cm). 58 flaps(89.2%) survived eventually. 42 flaps(64.6%) survived totally without any complication. 8 flaps(12.3%) showed the partial necrosis but they were healed without any additional operations. 8 flaps (12.3%) showed the partial necrosis requiring the additional skin graft. We had a satisfactory result by using arterialized venous free flap for the soft tissue reconstruction of finger and hand. We believe that volar aspect of distal forearm, thenar area, foot dorsum are suited as a donor site and the short length of the flap pedicle, the strong arterail inflow affect the survival rate of arterialized venous free flaps.

한국성인의 악력특성분석 (An analysis of grip strength for Korean adults)

  • Grip strength is a very important basic data for ergonomic design of hand tools, grips, handles and etc.. Excessive grip strength is contineously exerted to handle the machines or tools, it might cause a musculoskeletal disorder such as cumulative trauma disorders. Capal tunnel syndrome is a typical nerve disorder anomg CTDs. In this paper we have measured the grip strength under sitting and standing posture for Korean adults ; 113 male and 105 female aged from 20' to the above 50' . And this paper performed statistical analysis for driving out characteristics between grip strength and anthropometric data. The results are as follows ; (1) The maximum girp strength is exerted under standing posture with the elbow in full extension(180 .deg. ) for both of male and female (2) There is significant difference in posture, sex and right left hand (3) Grip strength decreases with age for male, but it traces an inverted U curve for female (4) Grip strength has a correlation with age, weight, height, BSA, forearm length, hand length, thickness of wrist, circumference of wrist and breadth of wrist for male

  • It is improtant to get accurate information about the grip strength for designing ergonomic grips and handles. It has been known that the excessive and repetitive use of grip force and unnatural posture of the wrist may cause cumulative trauma disorders (CTDs) such as carpal tunnel syndrome, tendrome, tendonitis, DeQuervan's disease, etc. In this paper we have measured the grip strength for sitting and standing posture for Korean adults ; 114 males and 105 females aged from the 20's to above the 50's. And this paper performed statistical analysis for deriving characteristics between grip strength and anthropometric data. The results are as follows ; (1) The maximum grip strength posture is exerted under standing posture with the elbow in full extension (180 .deg. ) for both males and females (2) There is significant difference among posture, sex and right and/or left handedness (3) Grip strength decreases with age for male, but it traces an inverted U curve for female (4) Grip strength has corre- lations with the age, weight, height, BSA, forearm length, hand length, thickness of wrist, circumference of wrist, and breadth of wrist for males.

Topographical measurement of the attachments of the central band of the interosseous membrane on interosseous crests of the radius and ulna

  • Background: To suggest a reasonable isometric point based on the anatomical consistency of interosseous membrane (IOM) attachment in association with topographic characteristics of the interosseous crests, the footprints of the central band (CB) of the IOM on the radial and ulnar interosseous crests (RIC and UIC) were measured. Methods: We measured the distance from the CB footprints from each apex of both interosseous crests in 14 cadavers and the angles between the forearm axis of rotation (AOR) and the distal slopes of the RIC and UIC in 33 volunteers. Results: The CB footprints lay on the downslope of both interosseous crests with its upper margin on average 3-mm proximal from the RIC's apex consistently in the radial length, showing normality (p>0.05), and on average 16-mm distal from the UIC's apex on the ulna without satisfying normality (p<0.05). The average angle between the UIC's distal slope and the AOR was 1.3°, and the RIC's distal slope to the AOR was 14.0°, satisfying the normality tests (p>0.05), and there was no side-to-side difference in both forearms (p<0.05). Conclusions: The CB attached to the downslope just distal to the RIC's apex constrains the radius to the UIC that coincides with the AOR of the forearm circumduction, maintaining itself both isometrically and isotonically.

생비골 이식술을 이용한 장골 골결손의 재건 (Reconstruction of Long Bone Defect with Vascularized Fibular Graft)

  • Purpose: The purpose of this study was to evaluate the effectiveness of limb reconstruction and functional recovery using vascularized fibular graft in the treatment of extensive bone defect of long bone caused by various diseases. Materials and Methods: From september 1995 to March 2005, 21 patients with segmental bone defects were managed with vascularized fibular graft: 13 males and 8 females, aged 39 years on average (range, $8{\sim}65\;years$). The reconstructed site was the humerus in 9 patients, the femur in 5, the tibia in 4 and the forearm bone in 3. The length of bone defect ranged from $8{\sim}17\;cm$. Results: Twenty grafts were successful. The mean period to obtain radiographic bone union was 5.7 months on average. Conclusion: Fibular grafts allow the use of a segment of diaphyseal bone and of sufficient length to reconstruct most skeletal defects of the long bone. The vascularized fibular graft is indicated in patients with intractable nonunions where conventional bone grafting has failed or large bone defects.
