Several types of soft tissue masses occur in the lower extremities. A mass associated with blood vessels is often difficult to diagnose. A 15-year-old male patient visited the author's hospital with discomfort and edema in his right calf that had persisted for six months. A physical examination showed no palpable mass other than mild edema. Three masses were found during the ultrasound scan along the small saphenous vein. The masses had a cyst-like appearance and were filled with thrombus. In duplex ultrasound, vascular reflux was represented inside the masses. During surgery, it was suspected that vascular deformation occurred in the small saphenous vein, and simple ligation and resection treatments were performed. The patient was finally diagnosed with venous aneurysms accompanied by thrombosis based on the histology tests. The symptoms disappeared after surgery, and there were no recurrences or unusual findings at the follow-up one year later. Venous aneurysms occurring in the superficial veins of the lower extremities are rarely reported, but treatment and diagnosis are important. This paper reports a case of an aneurysm on the small saphenous vein.
As a part of HMS(Human-Machine System), the car is very important thing in common life. It is also a significant part to study on the controllers of car that is intentively related with all sensibilities during driving. There are lots of controllers on seating buck of the car. However, there are few study on the sensibility analysis of them. Most of all, the foot controller could be easily overlooked because it could be invisible. This study was based on relationship that the controllers fitted to the driving posture in the drivers' sensibility difference of two transmission modes, automatic and manual transmission. The results show the driver's preference driving posture and sensibility in two kinds of transmission cars. Consequently, it should be designed the seating buck for two different types respectively to be taken comfort driving posture and improve the safety for drivers. Also, it could reduce the fatigue and discomfort in the task of driving. The design of the controllers strongly effects on the drivers' response time. hereby this study was accessed to the sensibility of Korean with analyzing the relationship, quantitative data, and sensibility difference between two kinds of transmission cars.
Purpose: Although the sural nerve is the most commonly used donor for autologous nerve graft, its morbidity after harvesting is sparsely investigated. The sural nerve being a sensory nerve, complications such as sensory changes in its area and neuroma can be expected. This study was designed to evaluate the donor site morbidity after sural nerve harvesting. Methods: Among the 13 cases, who underwent sural nerve harvesting between January 2004 and August 2009, 11 patients with proper follow up were included in the study. The collected data included harvested graft length, actual length of the grafted nerve, anesthetic and paresthetic area, presence of Tinel sign and symptomatic neuroma, and scar quality. Results: In 7 patients, no anesthetic area could be detected. Of the patients with a follow up period of more than 2 years, all the patients showed no anesthetic area except two cases who had a very small area of sensory deficit ($225mm^2$) on the lateral heel area, and large deficit ($4,500mm^2$) on the lateral foot aspect. The patients with a short follow up period (1~2 m) demonstrated a large anesthetic skin area ($6.760mm^2$, $12,500mm^2$). Only one patient had a Tinel sign. This patient also showed a subcutaneous neuroma, which was visible, but did not complain of discomfort during daily activities. One patient had a hypertrophic scar in the retromalleolar area, whereas the two other scars on the calf were invisible. Conclusion: After a period of 2 years the size of anesthetic skin in the lateral retromalleolar area is nearly zero. It is hypothesized that the size of sensory skin deficit may be large immediately after the operation. This area decreases over time so that after 2 years the patient does not feel any discomfort from nerve harvesting.
Purpose: The purpose of this study is to identify the current status and use of Gyungrodang located in Heungeop-myeon, Wonju-si and to present improvement and implication for community care in rural area. Methods: The survey of the current status was conducted with interview and observations. The purpose of these study was to analyze the contents and trend of researches done on oncology nursing in the past five years in Korea and to suggest its direction for the future research. Methods: 214 nursing studies published from 1998 to 2003 were selected for the analysis. They were examined for the source, design of study, type of subjects, the intervention outcomes of experimental research, the key words and theme of the oncology research. Results: 1. 139(64.9%) of 214 studies were masters thesis and doctoral dissertations. There were 196(91.6%) quantitative researches and 18 qualitative researches. Quantitative research consisted of 28(14.3%) experimental and 168(85.7%) non- experimental studies. Among non-experimental studies, there were 53 correlations, 78 descriptives, 14 comparatives and 23 methodological studies. 2.The main subjects of the study were cancer patients in general(43.7%) and breast cancer patients (18.3%). According to the treatment types, patients receiving chemotherapy were the highest number distribution as 23 of all researches. 3. Social support, fatigue, stress, hope, and quality of life were the most frequently used concepts in correlational studies.4. Education, providing information, oral care, exercise, relaxation and foot massage were the most frequent nursing interventions. Most frequently measured outcomes variable were self-care, pain control, nausea & vomiting, oral discomfort and anxiety. 5. Key words used most frequently were quality of life, educational need, pain controls and fatigue. 6. The themes of qualitative researches were illness experience, family experience and adaptation process. More than 50% of qualitative researches used phenomenologic method. Conclusion: Researches on cancer nursing in Korea have been increased. Descriptive design is being dominant and few experimental studies are being done For the future, it is necessary to conduct international studies to compare, replicate and to establish the effective nursing interventions.
Gait analysis trackers have been developed to monitor daily gait patterns to improve users' running performance and reduce the risk of injuries. A variety of gait analysis trackers are available on the market(e.g., foot pods, insoles). Depending on the type of gait analysis tracker, users' discomfort or satisfaction as well as required properties may differ. Hence, the purpose of this study was to compare and analyze user experience of three different types of commercial shoe-mounted gait analysis trackers and their mobile applications in a laboratory environment using questionnaires based on actual experiences of each product. Ten males and ten females who regularly enjoy walking and running exercises participated in the experiment. After the participants set up the tracker and application themselves without support from researchers, ten to thirty minutes' exercise was permitted on each product. Following this, the participants answered questionnaires containing evaluation variables on the device and mobile application, as well as satisfaction, intention to use, recommendation, and purchase. In addition, they were asked questions about the attractive features and shortcomings of each device and application. The results showed that the PRO-SPECS® smart insole was preferred over the others for ease of use, perceived durability, psychological burden of the design, and usefulness of the information provided by the application. Along with the results of questionnaire, this study also discussed strategies and recommendations for future product design and development.
Natural ventilation has proven to be an effective passive strategy in improving energy efficiency and providing healthy environments. However, such a strategy has not been commonly adopted to tall office buildings that traditionally rely on single-skin façades (SSFs), due to the high wind pressure that creates excessive air velocities and occupant discomfort at upper floors. Double-skin façades (DSFs) can provide an opportunity to facilitate natural ventilation in tall office buildings, as the fundamental components such as the additional skin and openings create a buffer to regulate the direct impact of wind pressure and the airflow around the buildings. This study investigates the impact of modified multi-story type DSFs on indoor airflow in a 60-story, 780-foot (238 m) naturally ventilated tall office building under isothermal conditions. Thus, the performance of wind effect related components was assessed based on the criteria (e.g., air velocity and airflow distribution), particularly with respect to opening size. Computational fluid dynamics (CFD) was utilized to simulate outdoor airflow around the tall office building, and indoor airflow at multiple heights in case of various DSF opening configurations. The simulation results indicate that the outer skin opening is the more influential parameter than the inner skin opening on the indoor airflow behavior. On the other hand, the variations of inner skin opening size help improve the indoor airflow with respect to the desired air velocity and airflow distribution. Despite some vortexes observed in the indoor spaces, cross ventilation can occur as positive pressure on the windward side and negative pressure on the other sides generate productive pressure differential. The results also demonstrate that DSFs with smaller openings suitably reduce not only the impact of wind pressure, but also the concentration of high air velocity near the windows on the windward side, compared to SSFs. Further insight on indoor airflow behaviors depending on DSF opening configurations leads to a better understanding of the DSF design strategies for effective natural ventilation in tall office buildings.

The purpose of this study was conducted in order to analyze the effects of the manual intervention and self-corrective exercise models of general coordinative manipulation(GCM) on the balance restoration of spine & extremities joints with distortions and mal-alignment areas. The subjects were the members who visited GCM Musculoskeletal Prevent Exercise Center from March 1 2012 to December 31 2013 because of spine & extremities joints distortion and mal-alignments, poor posture, and body type correction. All subjects were diagnosed with the four types of the GBT diagnosis. And according to the standards of the mobility vs stability types of the upper & lower body, they were classified into Group 1(40 persons) and Group 2(24 persons). For every other day for three times a week, GCM intervention models were applied to all subjects for four weeks, adding up to 12 times in total. Then the balance restoration effects were re-evaluated with the same methods. The results are as follows. 1) Balance restoration effects of VASdp(Visual analysis scale pain & discomfort) and ER(Equilibrium reaction: ER) came out higher in GCM body type(GBT) II III IV of Group 1. 2) In case of balance restoration effects in Moire and postural evaluation areas, Group 1 was higher and cervical and scapular girdle were higher in Group 2. The balance restoration of the four GBT types was significant in all regions(p<.05), and the scapular girdle came out as high in the order of GBTII IV I. 3) In case of thoracic-lumbar scoliosis and head rotation facial asymmetric cervical scoliosis ribcage forward, the balance restoration effects of the upper body postural evaluation areas came out the highest in Group 1 and Group 2, respectively. The balance restoration effects of the four GBT types were significant in all regions(p<.05), and came out the highest in lumbar scoliosis GBTIII I, ribcage forward and thoracic scoliosis GBTII IV. 4) The balance restoration effects of the lower body postural evaluation areas came out higher in Group 1 and Group 2 for pelvis girdle deviation patella high umbilicus tilt and hallux valgus foot longitudinal arch: FLA patella direction, respectively. The balance restoration effects of the four GBT types were significant in all regions(p<.05), and came out the highest in pelvis girdle deviation GBTIII I and patella high-direction GBTIV II I. 5) The balance restoration effects between the same GBT came out significant (p<.05) in all evaluation areas and items. The conclusions of this study was the manual intervention and self-corrective exercise models of the GCM about the mal-alignment of the spine & extremities joints across the whole body indicated high balance restoration effects(p<.05) in spine & extremities joints in all evaluation areas.
