Nutritional status of Korean elderly with dementia in a long-term care facility in Hongseong

  • BACKGOUND/OBJECTIVES: This study investigated nutritional status of the elderly with dementia in a care facility with the aim of improving the meal quality of the facility. SUBJECTS/METHODS: Data were collected from 30 dementia patients aged more than 65 years in a long-term care facility in Hongseong. The data were obtained from questionnaires and medical records. The food intake data was obtained using food photographs and the nutrient intakes were calculated using the CAN-Pro 5.0. The data were compared with the dietary reference intakes for Koreans (KDRIs). The nutrient density, diet quality such as nutrient adequacy ratio (NAR), mean adequacy ratio (MAR), and index of nutritional quality (INQ), as well as dietary diversity score (DDS) were evaluated. The data were analyzed using SPSS statistical programs. RESULTS: The average daily energy intakes for men and women were much lower than the estimated energy requirements of the KDRIs. The average intakes of energy and most nutrients in the general diet group were significantly higher than those of the other two groups. Significant differences in diet quality and diet diversity were observed according to the meal type groups. The NARs of some minerals (calcium, iron, and zinc) and vitamins (vitamin $B_6$ and folic acid) were less than 0.5 in all study groups. The NARs of protein, iron and MAR of the general diet group were significantly higher than those of the liquid diet group. The DDS scores of meats, fruits and diary food group were very low in all meal type groups, meaning that the diet qualities of the study subjects were not appropriate in all meal type groups. CONCLUSIONS: The food intakes of the study groups showed some limitations by a direct comparison with KDRIs because of the very low physical activities of the study subjects. The diet quality and diet diversity indices suggest the need for improvements in the nutritional quality in all types of diet. Overall, new intervention strategies targeting facility residents with dementia in Korea are needed as soon as possible.

성인의 연령에 따른 건강기능식품 섭취실태와 정보 요구도 분석 (Intake Pattern and Needs Assessment for the Development of Web-Contents on Health Functional Foods according to Age of Adults)

  • This study was done to analyze the consumption patterns of health functional food (HFF) as well as to perform needs assessment for the development of web-contents on HFF according to age of adults. The subjects were 238 male and female adults, divided into 4 groups by their age. This study collected all information by self-administrated questionnaires. The awareness on HFF was high in the older adults. The younger adults showed more negative responses to reliability and safety on HFF. The main reason for the consumption of HFF was to supplement nutrients and to prevent diseases. The main types of HFF consumed by adults were nutritional supplementary food, red ginseng products, and glucosamine products. There was higher consumption of nutritional supplementary food in the younger adults and glucosamine products in the older adults. Internet users had low level of satisfaction, with tendency to complain poor contents, reliability, difficulties in searching as problems of the pre-existing HFF websites. As useful methods for provision of information on HFF, most adults wanted general information, articles written by experts and videos. They also wanted to know the safety and side effects of HFF. Requirement of contents composition were various in-depth information, clear indication of citation, fresh updated data while that of display composition was easily-findable, uncomplicated, allowing mutual exchange of communication through bulletin board. These results can be used as basic data that reflect the consumer's needs for developing HFF web-contents according to age of adults.

Total Dietary Fiber and Mineral Absorption

  • The consumption of foods rich in TDF should not be associated with impaired mineral absorp-tion and long-term mineral status. In surveys of populations consuming high amounts of TDF e.g Third World populations and vegetarinas gross deficiencies in mineral nutrition have not been noted. If mineral status is low among these groups it is most likely caused by the inadequacy or imbalance of the diet and not by the TDF. The key word is interaction which should be inte-rpreted in dietary imbalances that produce nut-rient deficiencies. There are no strong data to support the concept that TDF inhibits mineral absorption through a binding chelation mechanism. Limited data sug-gest that positively charged groups on polymers such as chitosan and cholestyramine will decrease iron absorption in humans and animals. Because TDF does not contain positively charged groups future research should be directed at the possible role of protein consumed along with TDF and the combination of effects on mineral nutrition Phytic acid is acknowledged as a potent chela-tor of zinc. However its association with zinc and its propensity to lower Zn bioavaiability may enhance the absorption of other elements notably copper and iron. The importance of interactions among nutrients including TDF will gain addi-tional attention in the scientific community. Soluble and insoluble dietary fiber function di-fferently in the intestine. Insoluble fibers accele-rate movement through the intestine. Soluble die-tary fibers appear to regulated blood concentra-tions of glucose and cholesterol albeit by some unknown mechanism. In creased viscosity produ-ced by the SDF in the intestine may provide an explanation of how this class of polymers affects plasma glucose cholesterol and other nutrients. Employing a double-perfusion technique in the rat we demonstrated that viscosity produced by SDF will delay transfer of zinc into the circulatory system. This delayed absorption should not be interpreted as decreased utilization. A great deal of additional research is required to prove the importance of luminaly viscosity produced by SDF on slowing nutrient absorption or regulating bllod nutrient homeostasis. Increased intake of TDF in the total human diet appears desirable. A dietary intake of 35g/day should not be considered to have a negative effect on mineral absorption. It is important to educate people that an intake of more than 35g TDF/day may cause an imbalance in the diet that can adve-rsely affect mineral utilization. Acknowledgments. 미숙사과를 이용한 사과마스크팩의 정기적인 사용이 중년여성의 피부상태 개선에 대한 임상효과 평가 (Clinical Effectiveness of Regular Use of Unripe Apple Mask Pack on Skin Status of Middle-aged Women)

  • The effects of unripe apple mask pack made from unripe apple extracts on the skin status of middle-aged women for 4 weeks were investigated. The subjects were divided into three groups: placebo mask pack as the control (PM, n = 8), apple mask pack (AM, n = 16), and apple mask pack + apple intake (AAM, n = 12). The study included a survey questionnaire, and evaluations of nutrients intakes and of skin status. Skin status was measured by Aramo-TS. After 4 weeks, facial moisture, elasticity, evenness, and wrinkle level were improved in AM and AAM. The facial moisture level (AM: $+3.13\;{\pm}\;2.73$, AAM: $+2.25\;{\pm}\;2.93$) and elasticity level (AM: $+5.81\;{\pm}\;8.27$, AAM: $+5.50\;{\pm}\;6.13$) of AM (p < 0.001) and AAM (p < 0.05) were increased. The facial evenness level (AM: $-5.25\;{\pm}\;8.13$, AAM: $-9.17\;{\pm}\;4.26$) and wrinkle level (AM: $-10.88\;{\pm}\;11.74$, AAM: $-17.83\;{\pm}\;12.22$) were significantly decreased. AAM was more effective in improving skin elasticity, evenness, and wrinkle. The skin status of PM was not changed. While the macronutrient intakes of all the subjects were not different before and after the study, the daily intakes of some vitamins (vitamins $B_6$, C, and E and folic acid) during the study were significantly increased in AAM. These findings suggested that regular use of apple mask pack was effective in improving overall skin status. Furthermore, the consumption of apples with mask pack application was much better in improving skin status.

Associations of Dietary Calcium Intake and Serum Calcium Level with Blood Lead Levels in Korean Male Lead Workers

  • A cross-sectional study was performed to estimate the nutritional status of Korean male lead workers and to assess the relationship between calcium nutritional status and blood lead levels. A flood consumption survey was conducted by the 24-hr recall method with 118 lead workers and 63 non-lead exposed controls. Blood lead levels were analyzed from whole blood and serum calcium concentrations were also assessed. Results of dietary analysis showed Korean lead workers consumed relatively sufficient nutrients (more than 75% of RDA) except, calcium. Mean dietary calcium intake of lead workers was 502.2mg(72% of RDA) while that of the non-lead workers was estimated as 600.8mg(86% of RDA). Intakes of protein, iron, niacin and vitamin C of lead workers were significantly lower than those of non-lead workers. There was a wide range of blood lead levels(5.5 to 73.5$\mu\textrm{g}$/㎗) observed while mean blood lead level of lead workers was 30.9$\mu\textrm{g}$/㎗. However, 98% of lead workers showed normal serum ca1cium concentrations (range ; 8.9 to 10.7mg/㎗, mean ; 9.77mg/㎗) while 66% of lead workers were estimated to intake a dietary calcium lower than 75% of RDA. Mean blood lead levels of non-1ead workers were significantly lower(mean ; 5.1$\mu\textrm{g}$/㎗, p < 0.001) and the serum calcium concentration was significantly higher(mean ; 10.20mg/㎗, p < 0.001) than lead workers. Results of unadjusted correlation showed that serum calcium level and dietary calcium intake were negatively correlated with blood lead concentration. In a multiple regression of blood lead levels with variables known as affecting blood lead 1eve1 such as age, body mass index and occupational lead exposure, serum calcium was insignificant while dietary calcium intake showed statistically significant(p < 0.05) relation. Since calcium is a very important nutrient to reduce hazardous effects of lead, it should be strongly recommended that lead workers need to increase dietary calcium intake.

저나트륨 식생활 교육이 과체중 및 비만 중년여성의 식습관, 식사의 질 및 비만지표에 미치는 영향 (The Effects of Low-sodium Diet Education Program on Dietary Habits, Diet Quality and Obesity Index in Overweight and Obese Middle-aged Women)

  • Objectives: This study was conducted to investigate the effect of low sodium diet education program on dietary habits, diet quality, and measures of obesity in overweight or obese middle-aged women. Methods: Subjects were 81 individuals aged 45 years or over, who completed an 8-week nutrition education. The subjects were divided into a normal group (N = 30) and an overweight-obese group (N = 51) according to the BMI. The effects were evaluated by anthropometric measurement, biochemical analysis, questionnaire, and diet records before and after the program. Results: Overweight-obese group showed significant decreases in weight (p < 0.0001), BMI (p < 0.0001), percent of body fat (p = 0.0087), waist circumference (p < 0.0001), systolic (p = 0.0003) and diastolic blood pressure (p = 0.0261). Nutrients intakes were not different between the two groups and only sodium intake was decreased after education. Total score of general dietary habits, dietary behavior related to sodium intake, dietary diversity score (DDS), diet variety score (DVS), and diet quality index-international (DQI-I) were improved in both groups compared to the baseline. Overweight-obese group showed significant improvement in 'having fruits everyday', 'having fish everyday', 'trying to eat many kinds of food', 'eating less broth when eating soup, stew, and noodles', 'eating less kimchi and salt-fermented vegetable', and 'propensity to think that dishes should be pretty seasoned'. In addition, moderation of empty calories food (p = 0.0064) and macronutrient ratio (p = 0.0004) were improved in the overweigh-obese group, but in the normal group, the results did not reach statistical significance. Conclusions: These results suggested that low sodium diet education program may contribute to obesity management by improving diet quality and dietary habits in middle-aged women.

광주지역 일부 초등학교 아동의 식습관과 영양소 섭취량 및 혈액성상에 관한 연구 (The Eating Behaviors, Nutrient Intakes and Hematological Status of Primary School Children in Gwangju)

  • 광주시에 있는 한 초등학교에 재학중인 $10{\sim}12$세의 학생 210명을 조사 대상으로 식습관과 영양소 섭취량 및 혈액 성상에 관한 연구 결과는 다음과 같았다. 1. 조사 대상자의 연령 분포는 10세 남자가 65명, 여자가 71명, 11세 남자가 15명, 여자가 21명, 12세 남자가 21명, 여자가 17명으로 평균 신장은 남녀 각각 137cm, 136cm였고, 평균 체중은 32.2kg 31.0kg이었으며, 평균 가슴둘레는 67.1mm, 64.4mm, 평균 앉은 키는 74.0mm, 여자 74.7mm로 모두 유의적인 차이를 나타냈다. 2. 조사 대상자의 헤마토크리트치는 남학생이 37.1%, 여학생이 37.8%로 기준치와 거의 비슷하였고, 백혈구는 남 여학생 모두 $6.1{\times}10^3/mm^3$을 나타냈고, 적혈구는 남학생이 $4.7{\times}10^6/mm^3$, 여학생이 $4.6{\times}10^6/mm^3$이었다. 헤모글로빈은 남학생이 13.1g/dl, 여학생이 13.2g/dl로 기준치보다 약간 높았다. GOT는 남학생이 22.6unit, 여학생이 20.7unit였고, GPT는 남학생이 16.9unit, 여학생이 13.8unit였다. 혈액 중의 콜레스테롤 함량은 남학생이 170.3mg/dl, 여학생이 178.3mg/dl로 정상 범위였다. 3. 조사 대상자의 식습관 조사결과 고학년으로 올라 갈수록 식사시간이 불규칙해지고 식품의 배합이나 균형을 생각지 않고 섭취하고 있었으며 맵거나 짜게 먹고 있었고 아침 식사를 거르는 비율이 높게 나타났다. 4. 조사 대상자의 열량 섭취량은 남학생은 1,663kcal, 여학생은 1,427kcal를 섭취하였고, 단백질 섭취량은 남학생은 58.2g, 여학생은 52.1g, 지방 섭취량은 남학생이 43.2g, 여학생이 59.5g이었다. 칼슘의 섭취랴은 남학생이 469.6mg, 여학생이 513.9mg이었으며 인의 섭취량은 남학생이 896.4mg, 여학생이 823.6mg이었고, 철분의 섭취량은 남학생이 9.6mg, 여학생이 42.4mg, 나트륨의 섭취량은 남학생이 3,300mg, 여학생이 3,468mg이었고, 칼륨의 섭취량은 남학생이 2,169mg, 여학생이 2,192mg이었다. 비타민 A의 섭취량은 남학생이 388.4RE, 여학생이 466.0RE, 비타민 $B_1$은 남학생이 1.1mg, 여학생이 0.9mg, 비타민 $B_2$는 남학생이 1.1mg, 여학생이 2.2mg, 나이아신은 남학생이 12.2NE, 여학생이 26.3NE, 비타민 C는 남학생이 146.4mg, 여학생이 98.7mg 및 콜레스테롤 섭취량은 남학생이 251.4mg, 여학생이 159.3mg이었다. 이상의 결과로 보아 에너지, 단백질, 칼슘, 비타민 A 등 대부분의 영양소 섭취량이 권장량에 미치지 못하여 성장기에 있는 아동들의 영양소 섭취 상태에 문제가 있음을 시사해 주었다. 5. 조사 대상자의 식습관과 영양소 섭취량의 상관관관계는 식사 시간의 규칙성은 단백질, 식이성 섬유소, 비타민 A 및 티아민섭취량과 양의 상관을 나트륨섭취량과는 음의 상관관계가 있었다. 음식의 간(염도)은 인과 콜레스테롤 섭취와 음의 상관관계를 나타냈다. 들깨가루나 들기름 사용 여부는 열량, 탄수화물 및 인의 섭취량과 음의 상관을, 지방, 철분, 리보플라빈 및 나이아신섭취량과는 양의 상관관계를 나타냈었다. 결식여부는 지방, 철분 및 나이아신섭취량과는 양의 상관관계를, 열량, 탄수화물, 인 나트륨, 및 콜레스테롤섭취량과는 음의 상관을 보였다. 외식이 빈도수는 단백질, 지방, 식이성 섬유소, 철분, 레티놀, 티아민, 리보플라빈 및 나이아신섭취량과 음의 상관을 보였다. 이상의 연구 결과 $10{\sim}12$세 아동의 체위와 혈액 성상은 정상 범위에 속하며, 연령이 증가할수록 바람직하지 않은 식습관을 나타내었고, 에너지, 단백질, 칼슘, 비타민 A 등 대부분의 영양소 섭취량이 권장량에 미치지 못하였고, 특히 칼슘과 비타민 A의 섭취 증가가 요구되었다. 또한 식사시간이 규칙적일수록 단백질, 식이성 섬유소, 레티놀 및 티아민 섭취량이 높았고, 식이양이 많을수록 인과 콜레스테롤 섭취량이 높았으며, 들깨기름을 사용하지 않을수록 열량과 탄수화물 섭취량이 많았고, 외식의 빈도수가 높을수록 단백질, 지방 등의 섭취량이 높게 나타났다. 특히 초등학교 고학년 학생들의 불규칙적인 식사습관, 소금섭취량 및 외식의 빈도수가 여러 영양소 섭취상태에 영향을 주고 있음을 확인할 수 있었고 따라서 성장이 왕성한 학동기에 균형잡힌 좋은 식습관과 규칙적으로 가정에서 식사할 수 있도록 특별한 관심을 가져야 함을 시사해 주었다.

남녀별 직업군에 따른 대사증후군의 구성요소와 유병률 및 영양 상태의 비교 연구 : 2013년 국민건강영양조사 자료를 이용하여 (Comparative study on prevalence and components of metabolic syndrome and nutritional status by occupation and gender: Based on the 2013 Korea National Health and Nutrition Examination Survey)

  • 본 연구에서는 2013년 국민건강영양조사 자료를 이용하여 30~64세 무직을 제외한 근로자 1,750명 (남성 892명, 여성 858명)을 대상으로 화이트칼라와 블루칼라로 직업군을 분류하였으며 이를 통해 직업군에 따른 대사증후군의 구성요소와 유병률 및 영양 상태를 비교 연구하였다. 그 결과 대사증후군 영향 요인으로 작용하는 사회경제적 지표인 교육수준과 소득수준에서 남녀 모두 화이트칼라가 블루칼라보다 높았다. 대사증후군 유병률을 살펴보았을 때, 남성의 경우 블루칼라 35.1%, 화이트칼라 26.8%로 블루칼라가 화이트칼라보다 높게 나타났으며, 여성의 경우 블루칼라 24.8%, 화이트칼라 8.9%로 블루칼라가 화이트칼라보다 대사증후군 유병률이 높았다. 남녀를 비교하였을 때는 여성보다 남성에서 대사증후군 유병률이 높은 것으로 나타났다. 대사증후군 구성요소의 유병률에서는 남성의 경우 고혈당이 블루칼라가 화이트칼라보다 유의적으로 유병률이 높았으며 여성의 경우 복부 비만, 고혈압, 고혈당, 고중성지방혈증, 저HDL콜레스테롤혈증 모두 블루칼라가 화이트칼라보다 유의적으로 유병률이 높았다. 음식군별 1회 섭취량을 고려하여 주당 섭취빈도를 살펴본 결과 직업군별 남성의 경우 블루칼라가 화이트칼라보다 밥류의 주당 섭취빈도가 많았으며 빵 떡류의 주당 섭취빈도는 화이트칼라가 블루칼라보다 유의적으로 많이 섭취하였다. 여성의 경우는 블루칼라가 화이트칼라보다 국 찌개류의 주당 섭취빈도가 많았으며 과일류의 주당 섭취빈도는 화이트칼라가 블루칼라보다 유의적으로 많이 섭취하였다. 영양소 섭취량은 총 에너지 섭취량을 제외한 나머지 영양소 섭취량에서 남녀 모두 화이트칼라가 블루칼라보다 많이 섭취하였다. 따라서 본 연구 결과 남녀 모두 대사증후군 유병률은 블루칼라가 화이트칼라보다 높았으며 특히, 여성의 블루칼라에서 대사증후군과 그 구성요소의 유병률 비율이 높게 나타나 대사증후군 중재 및 예방을 위한 영양교육이 필요할 것이다. 또한, 남녀 모두 블루칼라가 화이트칼라보다 영양소 섭취 및 식사의 질이 전반적으로 저조한 것으로 나타나 블루칼라의 올바른 식습관 개선이 필요하다고 사료된다. 결론적으로 본 연구를 통해 성별에 따른 직업유형별 대사증후군 유병률과 영양소 섭취 상태가 상이하므로 이를 고려한 근로자들의 질병 예방을 위한 맞춤형 영양교육 서비스가 필요하다고 사료된다.

경남지역 농촌 의.식.주생활 양식에 관한 조사 연구 (A study on Rural Clothing, Dietray, Housing Style of Living in Kyeong-Sang-Nam-Do)

  • This study is proposed to research the rural clothing, dietary and housing style of living in order to adapt a basis which is supposed to improve the quality of rural life. The sample in this research comes from Daehang 1 Li, Bubuk Myun, Miryang Gun, Kyungnam, and the results are as following; Generally rural housewives are unconcerned with fashion or its information, instead, they care for the casual wear which represents economic and active life style. Because of their financial problems, inpurchasing, they spend only a small money without plan. When they launder Clothes, they use washing bat and pannel which is easy to break fabrics, and they ineffectively use to dry it by hands twisting. The total food intake per person in a day is 1170.6gram, and all nutrients intakes except fat and calcium exceed the recommended dietary allowances. But 90.3% of the food intakes are plant food, and the average food items taken per day per househod is 11.9items which are not so various, which represents the shortage of the nutrient quality. Both the nutrition knowledge score and the food habit score are very low. Therefore, in order to enhance the quality of clothing and diet in rural life, they need to learn clothing and nutrition management for their enlightment. The most comprehensive type of farmhouse site plan take the shape of ㄱ and ㄴ, and for the most part main building is the floor plan which includes 3-4 spans in front of the change from the type of traditional house, throught the revised one, to the type of newly built house. In main building of the farmhouse, kitchen, Kunbang and Chakunbang are extended to their real space, and both utility and the bathroom are added to build, and Malu is transformed into the living space.

Effect of feeding a by-product feed-based silage on nutrients intake, apparent digestibility, and nitrogen balance in sheep

  • Background: Literature is lacking on the effects of feeding by-product feed (BF)-based silage on rumen fermentation parameters, nutrient digestion and nitrogen (N) retention in sheep. Therefore, this study was conducted to determine the effect of replacing rye straw with BF-based silage as a roughage source on ruminal parameters, total-tract apparent nutrient digestibility, and N balance in sheep. Methods: The by-product feed silage was composed of spent mushroom substrate (SMS) (45 %), recycled poultry bedding (RPB) (21 %), rye straw (11 %), rice bran (10.8 %), corn taffy residue (10 %), protected fat (1.0 %), bentonite (0.6 %), and mixed microbial additive (0.6 %). Six sheep were assigned randomly to either the control (concentrate mix + rye straw) or a treatment diet (concentrate mix + BF-based silage). Results: Compared with the control diet, feeding a BF-based silage diet resulted in similar ruminal characteristics (pH, acetate, propionate, and butyrate concentrations, and acetate: propionate ratio), higher (p < 0.05) ruminal NH3-N, higher (p < 0.05) ether extract digestibility, similar crude protein digestibility, lower (p < 0.05) dry matter, fiber, and crude ash digestibilities, and higher (p < 0.05) N retention (g/d) Conclusion: The BF-based silage showed similar energy value, higher protein metabolism and utilization, and lower fiber digestion in sheep compared to the control diet containing rye straw.
