Effect of the Amount of Pigment on the Color of Silk Dyed with Fermented Indigo Powder Dye (발효쪽 분말염료로 염색한 견직물의 색소 함량이 직물의 색상에 미치는 영향)

  • This research investigated the relationship between the amount of fixed pigment and the color of silk dyed with three types of fermented indigo powder dye under different temperatures and pH, by reduction or nonreduction method. Amount of fixed pigment was analyzed using the Ultimate 3000 HPLC-DAD instrument and the color of dyed silk was measured using the X-rite spectrocolorimeter. All silk samples dyed by reduction method showed PB color. The amount of indigotin fixation was dependent on the dyeing temperature and pH regardless of the indigotin composition in the dye. Indirubin was less dependent upon the dyeing condition in the reduction dyeing and its fixation was minimum level. Dyeing conditions which can maximize the indigotin fixation were $50^{\circ}C/pH$ 11 and $70^{\circ}C/pH$ 7 conditions in reduction dyeing. Color of silk showed more redness ($a^*$) thus higher PB color when the indigotin fixation was low and indirubin fixation was relatively high. Indirubin fixation was very low with slightly better fixation by nonreduction method. More reddish color was obtained by nonreduction dyeing, and by more alkaline dyebath.

Results of Closed Reduction and Percutaneous Fixation of the Supracondylar Fractures of the Humerus in Adults (성인의 상완골 과상부 골절의 도수 정복 후 경피적 고정 방식의 치료 결과)

  • Purpose: To evaluate the results of the treatment of the supracondylar fractures of the humerus according to the fixation methods in adults Materials and Methods: Seven patients, aged 55 to 52 years (average,69 years), were reviewed after a mean follow-up of 37 months (range, 11-65 months). According to AO classification all fractures were classified as type A2 (simple transverse supracondylar fracture). Six patients underwent closed reduction and one patient, open reduction after failure of closed reduction. Percutaneous fixation with cannulated screws was performed to the 4 patients, per- cuta)leous fixation with Kirschner wires in 3 patients. All except one patients have associated medical problems. The results were assessed based on the Mayo Elbow Performance Score. Results: All the patients with cannulated screw fixation had stable bony union with excellent ranges of motion (mean: 5-125 degrees). All the three patients who received percuatnaous smooth K-wire fixation had nonunion with poor results, one of them had changed into cannulated screw. and then had good result. Conclusion: Although simple supracondylar fracture is similar to the pediatric fracture in nature, it should be firmly fixed with the method such as threaded cannualted screw rather than the simple fixation with K-wires.

Osteolysis in Kudo Type Total Elbow Arthroplasty (Kudo형 주관절 전치환술에서 골용해)

  • This study evaluated the development of osteolysis of Kudo total elbow arthroplasties according to the fixation with or without cement. Twenty-four elbows of nineteen patients of rheumatoid arthritis underwent total elbow arthroplasty from 1986 to 1993. The patients were followed for an average of 42 months ranging from 36 months to 59 months. In cemented humeri, 1 of 13 (7.7%) showed osteolysis. In the humeri of cementless fixation, 5 of II (45.5%) revealed osteolysis, representing higher rate compared to cement fixation (P<0.05). In the ulna, osteolysis was identified in none of 16 (0%) of cement fixation, which was significantly less than the incidence of osteolysis in cement less fixation (3 of 8,37.5%) (P<0.01). It was concluded that the cement fixation result in less osteolysis than the cementless fixation in Kudo type total elbow arthroplasty.

Development and Evaluation of Fixation Equipment for Transporting Unit Modules

  • The unit modular construction system is a building technique in which unit modules are manufactured at a factory and then assembled at the construction site. It has many benefits, including reduction of the construction period and improvement in quality. For this reason, it is utilized for various purposes in Japan and England. While it has been introduced in Korea, there have been few Korean studies conducted on the unit modular system. In particular, little research has been done on the method of safely fixating the unit modules to a truck. Therefore, this study reviewed the fixation methods of unit modules for transport, analyzed the problems, and designed a fixation device for unit modules. In addition, a device for the fixation of unit modules to a truck was developed, and a structural simulation was implemented for a safety test by considering the maximum stress generated during the transport of the unit modules fixed on a truck. When the device for the fixation of unit modules is manufactured based on the results of the structural simulation done in this study, it is expected to aid the development of a more practical fixation device for unit modules.

  • As measuring visual behavior by utilize eye-tracker, this study examines whether consumer concentrates on visual attention with certain object and measures visual behavior with fixation number, fixation duration, the first fixation, which are visual attention pattern of consumer about Mannequin, hanger, shelve as props of display advertising effect on clothing store. It has experimental purpose to recognize relationships in terms of regardless, appropriateness, purchase intention. During display of clothing store, first experimental consequence to measure visual behavior on consumer did appear to allocate more cautions than other props. Second, in terms of relationships with regardless, appropriateness, purchase intention from consumer, each fixation pattern about consumer's fixation number, fixation duration, the first fixation was confirmed that there is meaningful correlation with each other. Accordingly, as advertising effect of mannequin makes consumer to concentrate more visually cautious, it is translated that high favor about display product with mannequin has positive role on purchase intention. Also, there is a purpose that higher feasibility on study result provides practical point on clothing store's display marketing strategy of corporation.

A Study of Biomechanical Simulation Model for Spinal Fusion using Spinal Fixation System (척추경 고정 나사 시스템을 이용한 척추 유합 시술의 생체역학적 분석 모델 연구)

  • In general, spinal fusion surgery takes pressure off the pain induced nerves, by restoring the alignment of the spine. Therefore spinal fixation system is used to maintain the alignment of spine. In this study, a biomechanical study was performed comparing the SROM(Spinal Range Of Motion) of three types of system such as Rigid, Dynesys, and Fused system to analyze the behavior of spinal fixation system inserted in vertebra. Dynesys system, a flexible posterior stabilization system that provides an alternative to fusion, is designed to preserve inter-segmental kinematics and alleviate loading at the facet joints. In this study, SROM of inter-vertebra with spinal fixation system installed in the virtual vertebra from L4 to S1 is estimated. To compare with spinal fixation system, a simulation was performed by BRG. LifeMOD 2005.5.0 was used to create the human virtual model of spinal fixation system. Through this, each SROM of flexion, extension, lateral bending, and axial rotation of human virtual model was measured. The result demonstrates that the movement of Dynesys system was similar to normal condition through allowing the movement of lumbar.

Operative Treatment of the Clavicular Midshaft Fractures in Adult - A Comparison between Intramedullary Multiple Steinmann Pins Fixation and Reconstruction Plate Fixation - (성인 쇄골 간부 골절의 수술적 치료-골수강내 다발성 Steinmann핀 고정술과 재구성 금속판 고정술과의 비교-)

  • Purpose: Despite of the popular use of the reconstruction plate for the fixation of clavicular shaft fractures, some disadventages have been raised such as long period of immobilization, long skin incision, loosening of plate and screws, and increased chance of nonunion due to severe periosteal injury. Thus, the authors have performed intramedullary multiple Steinmann pins fixation that could reduce the disadvantages of plate fixation in order to compare the treatment results between the two groups. Materials & Methods: From 1994. Jan. to 1997. Dec. the department of orthopaedic surgery of the Kwak's hospital treated operatively for 56 cases of the clavicular shaft fractures in adult. 39 cases of them were treated with the plate fixation and 17 cases with the intramedullary multiple Steinmann pins fixation(SP group). Reconstruction plates(Plate group) were used for 26 out of 39 patients treated with plate fixation. Among the Plate group and SP group, each 15 cases were selected by age and sex and compared each other according to the bone union time, union rate, complication, and functional results. The follow-up period was 12 months at the shortest and 48 months at the longest and the average was 16 months. Results: The Plate group showed that the bone union time was 7 weeks and the bone union rate was 93%. The SP group showed 6.5 weeks and 100% respectively. In complication, the Plate group had 1 case of loosening of plate and screws and delayed union; SP group had 1 case of pin migration. The functional results according to Kang's criteria, 87% of the Plate group and 93% of the SP group showed good or excellent. Conclusion : The SP group showed very comparable results in terms of the bone union time, bone union rate, complication, and functional results comparing to the Plate group. The intramedullary multiple Steinmann pins fixation showed several advantages over the reconstruction plate fixation, which were simple operative technique, easy removal of pins, being able to perform immediate postoperative full range of motion exercise. Therefore, the intramedullary multiple Steinmann pins fixation is thought to be one of the useful operative techniques in treatment of the clavicular shaft fractures in adult.

Relationship of Fixation Disparity and Heterophoria According to Fixation Distance (원, 근거리에서 주시시차와 사위와의 관계)

  • Purpose: This study measured Y-intercept that means a fixation disparity, X-intercept that means a associated phoria and slope of a fixation disparity curve (FDC), which are variables of dissociated phoria and the FDC. We searched for the result value and examined the direction, distribution pattern and a variable that give an most affect on dissociated phoria and the FDC at distance and near. Also this study examined that there were statistically significant differences between distance and near, comparing the result value. Methods: We measured the dissociated phoria and the fixation disparity for 51 subjects at distance and near in June, 2007. All subjects ranged from 20 to 25 years of age (average 21.72${\pm}$1.88 years old) and had no eye disease. At distance the dissociated phoria measured with the distance MIM card (muscle imbalance measure card, Bernell co., USA), and the fixation disparity measured with the modified Mallett Far Unit (Bernell co., USA). At near the dissociated phoria measured with the near MIM card (muscle imbalance measure card, Bernell co., USA), and the fixation disparity measured with the Wesson fixation disparity card (American Optical co., USA). Results: The percentage distribution of types of fixation disparity curves was that at distance prevalence of Type I (74.6%) was the highest, followed by Type IV (17.6%) and Type II (3.9%), Type III (3.9%) and that at near prevalence of Type I (53.0%) was the highest, followed by Type III (29.4%), Type IV (13.7%) and Type II (3.9%). 2. There were significantly correlation in dissociated phoria, fixation disparity (Y-intercept) and associated phoria (X-intercept). 3. The fixation disparity at distance was most affected by associated phoria (X-intercept) (p=0.000). The distance dissociated phoria was most affected by fixation disparity (Y-intercept) (p=0.342), but the influence was weak. 4. The fixation disparity at near was most affected by associated phoria (X-intercept) (p=0.000). The near also dissociated phoria was most affected by associated phoria (X-intercept) (p=0.009). The result that compared the each variables with the same variables at distance and near had statistically significant on paired t-test for among dissociated phoria (t=7.529, p=0.000), X-intercept (t=5.860, p=0.000), the Y-intercept (t=4.640, p=0.000) but slope of the FDC did not differ significant (t=1.336 p=0.188). Conclusions: Relationship of fixation disparity and Heterophoria had close correlation at distance and near.

A Prospective Study with Cage-Only or Cage-with-Plate Fixation in Anterior Cervical Discectomy and Interbody Fusion of One and Two Levels

  • Objective : The authors prospectively analyzed the effect of one-level or two-level anterior cervical discectomy and fusion (ACDF), comparing stand-alone cages and cage-with-plate fixation constructs with respect to clinical outcomes and radiologic changes. Methods : A total of 84 patients who underwent one-level (n=52) or two-level ACDF (n=32) for cervical disc disease and who completed 2 years of follow-up were included in this study. The patients were divided by cervical level and grouped into ACDF-Cage-only and ACDF-Cage-with-plate groups. The following parameters were assessed using radiographs : subsidence, C2-C7 lordosis angle, fusion segment angle, adjacent disc space narrowing, and fusion status. Clinical outcomes were assessed using the neck disability index (NDI) and visual analog scale scores for arm pain. Results : In the comparison of one-level ACDF-cage-only and ACDF-cage-with-plate groups, the NDI score was better in the cage-only group at the 3-, 12-, and 24-month follow-ups : however, no significant difference in clinical outcomes was observed. In the comparison of two-level ACDF-cage-only and ACDF-cage-with-plate groups, no difference in any clinical outcome was observed between the two groups. At the 24-month follow-up, subsidence was observed in 45.8% of patients in the one-level cage-only group and 32.1% of patients in the one-level cage-with-plate fixation group. There was no statistically significant difference in the incidence rate between the two groups (p=0.312). Subsidence in the two-level cage-only group (66.6%) was significantly more frequent than in the two-level cage-with-plate fixation group (30%; p=0.049). The fusion rate for patients in the one-level cage-only group was not significantly different from that in the one-level cage-with-plate fixation group (cage-only, 87.5%; cage-with-plate fixation, 92.9%; p=0.425) ; fusion rate in the two-level patients were also similar between groups (cage-only, 83.3%; cage-with-plate fixation, 95%; p=0.31). Conclusion : Our clinical results showed that for single-level cases, plate fixation had no additional benefit versus cage-only; for two-level ACDF cases, the fusion rate and clinical outcomes were similar, although the cage-with-plate fixation group had a lower incidence of cage subsidence than did the cage-only group. We conclude that physicians should be aware of this possible disadvantage associated with using cervical plates in one-level ACDF. However, in two-level ACDF, subsidence is more likely to occur without plate fixation, and thus the addition of plate fixation should be considered.

CONSERVATIVE TREATMENT OF BILATERAL CONDYLAR FRACTURE BY A SIMPLIFIED TECHNIQUE OF MAXILLOMANDIBULAR FIXATION (단순악간고정법을 이용한 양측성 하악과두골절의 보존적 치료)

  • The conservative treatment of the condylar fracture has been used for a long time because of its simplicity, good prognosis and less complication. Traditionally the conservative treatment has been carried out by maxillomandibular fixation using arch bar and wire. But a simplified technique of maxillomandibular fixation introduced here is a procedure that 4 bone screws are placed above the apecies of the maxillary and mandibular canines and then ipsilaterally placed maxillary and mandibular bone screws are linked by a loop of wire each other. This procedure has several advantages compared with the traditional maxillomandibular fixation method. 1) it provides simplicity for the operators. 2) it sustains maxillomandibular fixation more rigidly compared with arch bar technique. 3) it keeps stable maxillomandibular fixation in the region of the anterior teeth so that anterior open bite tendency can be remarkably minimized. 4) it does not injure the periodontal tissue because the teeth are not engaged and causes less discomfort to the patient. 5) it decreases the possibility of operator's AIDS infection through inadvertent skin puncture. 6) it is highly recommended for the patients whose teeth are available for maxillomandibular fixation. The two male patients were diagnosed as bilateral condylar and symphyseal fracture of the mandible. They restored stable occlusion and functional mandibular movement only by a simplified technique of maxillomandibular fixation.
