Case Report for Severe Hepatitis A Treated in a Korean Hospital

  • A 25-year-old male presented with influenza-like symptoms and took Western anti-inflammatory analgesic drugs for 2 days. The symptoms aggravated, so the patient decided to rely on Korean medicine (KM). Based on the highly elevated hepatic enzymes (AST 4,621 IU/L and ALT 2,763 IU/L) with a positive result of anti-HAV IgM, he was diagnosed with hepatitis A. The patient was hospitalized and given herbal drugs (Chunggan-plus extract, Innae-Tang) and acupuncture, according to symptom differentiation, the accumulation of damp heat (濕熱蘊結)". The subjective symptoms (fatigue, nausea, gastric discomfort) including jaundice and dark urine as well as laboratory abnormalities gradually improved gradually in 10 hospital days, and the patient completely recovered in 25 days as an out-patient. This case presents a classic case of severe hepatitis A in 2019 Korean outbreak, and is informative to physicians for diagnosis and treatment in the traditional Korean medicine field.

A Case Report of Korean Medicine Treatment of a Decompensated Liver Cirrhosis Patient with Jaundice (황달을 동반한 비대상성 간경변 환자의 한방 치험 1례)

  • Objectives: The aim of this study was to report a clinical case of decompensated liver cirrhosis in a patient with jaundice who showed improvement with Korean medicine treatment. Methods: A patient with decompensated liver cirrhosis with jaundice was treated with herbal medicine (Injinoryung-tang, Galgeunjowi-tang), acupuncture, cupping, and moxibustion. Changes in jaundice, fatigue, and dysuria were measured with subjective parameters. Liver function was checked by performing a blood test 8 times. The levels of aspartate aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP), gamma glutamyl transpeptidase (GGT), total bilirubin (TB), albumin, and platelets (Plt), prothrombin time (PT), and international normalized ratio (INR) were recorded. Results: The Korean medicine treatment improved jaundice, fatigue, and dysuria and lowered the AST, ALT, ALP, GGT, and TB levels. Conclusions: Korean medicine treatment containing Galgeunjowi-tang positively improves clinical symptoms and liver function.

Factors Related to Psychosocial Stress and Fatigue Symptom Among Nurses Working at Ward and Operating Room in University Hospitals (대학병원 병동 및 수술실 근무 간호사의 사회심리적 스트레스와 피로수준에 관련된 요인)

  • The present study was intended to measure the level of psychosocial stress and fatigue symptom according to the various characteristics such as sociodemographic, health-related, job-related, job stress factors, and psychosocial factors among nurses working at ward and operating room in university hospitals, and to reveal the relation between these factors and psychosocial stress and fatigue symptom. The self-administered questionnaires were given to 220 nurses working at ward, and 147 nurses working at operating room in 4 participating hospitals located in Daejeon City during the period from July 1st to Aug 31st, 2012. As a results, the factors related to the psychosocial stress of nurses working at ward were age, subjective health status, job career, satisfaction of work, fit to the job, job demand, job control, coworker support, self-esteem, locus of control, type A behavior pattern. In operating room, there were age, sleep hours, subjective health status, job career, physical burden of work, satisfaction of work, fit to the job, consider quitting the job, job demand, job control, type A behavior pattern. The factors related to the fatigue symptoms of nurses working at ward were age, leisure time, subjective health status, satisfaction of work, consider quitting the job, job demand, locus of control, type A behavior pattern. In operating room, there were age, subjective health status, physical burden of work, supervisor support, coworker support, locus of control. Based on the study results, we suggest that the factors related to psychosocial stress and fatigue symptom of nurses were different from working station. We need development and application of programs to keep under management psychosocial stress and fatigue symptom.

A Study on a concept of Food Accumulation using Multidimensional Scaling - Comparison between Pyungweesan Questionnaire and Health Related Questionnaires - (다차원척도법을 이용한 식적(食積)의 개념에 관한 고찰 - 평위산(平胃散) 변증(辨證) 설문지와 건강 관련 설문지와의 비교 연구 -)

  • Objectives: In comparison with functional dyspepsia, food accumulation is known to have wider concept. On this study, we will make a proposal on the concept of food accumulation by a statistical comparison between pyungweesan questionnaire and health related questionnaires. Methods: We studied the similarities between six factors of pyungweesan questionnaire and four health related questionnaires(Rome II criteria of functional dyspepsia, subjective symptoms of fatigue test, beck depression inventory, state-trait anxiety inventory, etc.) using multidimensional scaling. Results and Conclusions: 1. Physical-emotional axis and acute-chronic axis were labelled in a two-dimensional scaling solution. 2. Seup-dam(濕痰) factor and sik-che(食滯) factor of food accumulation were closely related to fatigue. 3. Bi-seup(脾濕) factor of food accumulation was closely related to depression and anxiety. 4. The findings suggest that a concept of food accumulation reflects some aspects of physical fatigue and emotional depression as well as dyspepsia.

  • Most of the atrial flutter occurs in patients who have an organic disease of the heart. The symptoms of atrial flutter include anxiety with palpitation. sleep disorder and fatigue. For a long time, oriental medicine has clinically treated these symptoms, and the diagnosis has been usually based on the doctor's judgment on the subjective clinical symptoms. However, if oriental medicine includes new diagnostic aids like electrocardiogram or other various methods or combinations of techniques, the accuracy and objectivity of diagnosis and clinical data can be improved. In this case, a 65 year-old male patient who had anxiety with palpitation and abnormal P wave (atrial flutter) on the EKG was treated with Xiaofengbusindaotantang (疎風補心導痰湯) and Shengmaisan (生脈散). Significant improvement in the clinical symptoms and EKG results was observed. This result suggests that oriental medical treatment can be used for treatment of organic heart disease like an atrial flutter. In addition, subsequent studies should be followed to ensure the appropriateness of new diagnostic aids, such as EKG, and other various modalities.

Effects of Foot Bath Therapy on Menopausal Symptoms and Sleep in Women in Their 50s (족욕요법이 50대 여성의 갱년기 증상 및 수면에 미치는 영향)

  • Purpose: Purpose: This study was to study the effect of foot bathing on menopausal symptoms and sleep disorders in female subjects in their 50s with menopausal symptoms. Methods: After 12 weeks of foot bath therapy three times a week, pre- and post-menopausal signs were measured. Results: Subjects with hot flashes, heart discomfort, sleep problems, depression, irritability, anxiety, physical and mental fatigue, sexual problems, urination problems, vaginal dryness, and joint and muscle discomfort significantly improved after than before(p < .001). Subjective sleep quality, sleep incubation period, sleep duration, sleep disturbance score, sleep drug use, and sleep dysfunction significantly decreased after foot bath than before(p < .001). Habitual sleep efficiency increased considerably. Conclusions: The subjects showed overall improvement in menopausal symptoms and sleeping quality after a foot-bath. Therefore, foot bath therapy is evaluated as a natural healing therapy suitable for improving menopausal symptoms and sleep.

Clinical Study on Gadolinium-DPTA enhanced MRI of Bell's palsy (구안와사(口眼喎斜) 환자(患者)의 Gadolinium-DPTA enhanced MRI 소견(所見)에 대한 임상적(臨床的) 고찰(考察))

  • This study is designed to evaluate the clinical implications of Gd-DPTA (Gadolinium-diethyl enetriamine pentacetic acid) enhanced MRI(Magnetic resonance imaging) in Bell's palsy and find it's usefulness in Oriental Medicine In this study, 25 outparients with Bell's palsy were studied that MRI was performed. To evaluate degree of facial palsy, H-B(House-Brackmann) Grade was used. In Oriental Medical therapy, Acupuncture and Herbal medicine were treated. Subjective cause was divided into exposure to chill, fatigue, stress, mixed cases. Enhanced site was compared with symptoms which were disorder of eye, hearing, taste, and facial muscle palsy. Also, Relation between time which was performed MRI and enhancement was analyzed. The enhanced lesion in MRI was divided into five segments; Internal audiitory canal, Labyrinthine segment, Geniculate ganglion, Tympanic segment, Mastoid segment. In Bell's palsy, 20 of 25 patients(80%) had abnormal contrast enhancement of the facial nerve. The H-B grade and interval performed MRI from onset were directly proportionate to enhancement. That is to say, Severe facial palsy short interval show high possibility of enhancement. There was no relation between subjective causes and enhanced site of facial nerve in MRI. Also Clinical symptoms didn't coincide with MRI findings.

A Study on the Sex-identity and the Premenstrual Syndrome of Female High School Students (여자고등학교 학생의 여성 정체감과 월경전 증후군)

  • This study was conducted in order to contribute to female student health by providing fundamental data for health instruction and health counsel. The purpose of this study is to determine the frequency of PMS by a menstrual distress questionnaire, and determine sex-identity factors related to PMS. The data were collected by a questionaire conducted from December 1,1995 to December 20, 1995. The subjects of research were 1,102 high school students in Seoul. The data analyzed was done using the SPSS PC+, and the results were as follows; 1. The perceived subjective symptoms were clusted behavioral change, negative affects, pain, impaired concentration, water retention and autonomic reactions. The most common symtoms during the period of 2 to 10 days before menses were vaginal discharge(79%), back pain(70.4%), nervous tension(60.8%), fatigue(56.2%), abdominal bloating(54.5%), mood change(52.8), pelvic pain(46.8%), resistance to study(45.9) and talking(43.3). 2. There was a significant correlation between the clusters symptoms; negative affects and behavioral changes(r=.9326, p=.000), behavioral changes and impaired concentration (r=.8572, p=.000), negative affects and impaired concentration(r=.8411, p=.000) and autonomic reaction and pain(r=.7267, p=.000). 3. Sex-identity factors were related to PMS ; perception of the female(F=5.1811, p=.006) and pregnancy concerns (T=3.54, p=.000).

A Study on the Constitution Type-Specific Presentation of Physical Symptoms (사상체질에 따른 신체적 증상에 관한 연구)

Objectives: This clinical study was conducted to understand the differences between Sasang constitutional types and to identify the physical symptoms presentation specific to each Sasang constitutional type. 2. Methods: In this descriptive study, 2,629 subjects (1,061 Taeeum-type, 683 Soeum-type, 885 Soyang-type) were surveyed between Nov 1, 2007 and Jul 31, 2010. The subjective symptoms experienced by the subjects were collected using a Physical Symptoms questionnaire, and the subjects were interviewed by Sasang specialists who determined the subjects' constitutional type. The data (in crude number and percentage) was analyzed on the general characteristics, Sasang constitutional type, and physical symptoms using the SPSS 17.0 software. The symptomatic presentation in men and women were analyzed on Sasang constitutional distribution using the chi-square test. 3. Results: 1) The physical symptoms specific to each Sasang constitutional type were as follows: 'frequent aphthous lesions in the oral cavity', 'lingering fatigue after sleep', 'headache', 'common cold symptoms presenting as loss of appetite or indigestion', and 'physical deterioration presenting as problems with digestion' in the Soeum type; 'physical deterioration presenting as problems in perspiration' and 'swelling and puffiness' in the Taeeum type; and 'excessive forgetfulness' in the Soyang type. 2) The physical symptoms specific to each Sasang constitutional type in men were as follows: 'Unilateral or bilateral headaches, 'frequent aphthous lesions in the oral cavity', 'common cold symptoms presenting as rhinorrhea or nasal congestion', 'common cold symptoms presenting as loss of appetite or indigestion', and 'physical deterioration presenting as problems with digestion' in the Soeum type; and 'swelling and puffiness' in the Taeeum type. 3) The physical symptoms specific to each Sasang constitutional type in women were as follows: 'common cold symptoms presenting as headaches', 'common cold symptoms presenting as loss of appetite or indigestion', 'physical deterioration presenting as problems with digestion' in the Soeum type; and 'pain in knees', 'redness of eyes', 'dryness of mouth', 'common cold symptoms presenting as coughing', 'physical deterioration presenting as problems in perspiration', 'swelling and puffiness' in the Taeeum type. 4. Conclusions: This study demonstrates that physical symptoms present in constitutional type-specific patterns. Understanding of the personal Sasang constitutional type and systematic, personalized healthcare based on constitutional typology is anticipated to contribute to improved health management strategy.

The Effect of Preferred Music Listening on Police Officers' Occupational Stress and Fatigue (선호음악 감상이 경찰공무원의 직무스트레스와 피로 감소에 미치는 영향)

  • The purpose of this study was to examine the effect that preferred music listening had on police officers' occupational stress and fatigue. The musical intervention took place over a total of six sessions from October 11 to October 19, 2016. At each session, the experimental group (n = 19) received 20 minutes of music listening with individually customized playlists that were compiled based on their preferred music list. The control group (n = 21) simply took a break from work for 20 minutes. Every participant checked his or her occupational stress and fatigue level before and after the experiment using the Occupational Stress Inventory and Subjective Symptoms of Fatigue Test. The data were then analyzed by the SPSS 22.0 statistics program. To examine the homogeneity and pretest-posttest results between groups, an independent t test was conducted, and a paired t test was used to examine the pretest-posttest results within groups. The results confirmed all hypotheses. Findings support that the use of preferred music listening is an effective way to help police officers to control their occupational stress and fatigue. This study used a convenience sample and has limited generalizability. Future research should explore interactive approaches in music therapy as well as receptive music listening for alleviating police officers' occupational stress and fatigue.
