Antidiabetic Activities of Extract from Malva verticillata Seed via the Activation of AMP-Activated Protein Kinase

  • Stimulation of AMP-activated protein kinase (AMPK) signaling followed by increase of glucose uptake in L6 myotubes were studied with organic solvent extract of Malva verticillata (MV) seeds. Ethanol extract of M. verticillata seeds (MVE) significantly increased the phosphorylation level of AMPK, acetyl-CoA carboxylase (ACC), and glucose uptake in L6 myotube cells. The MVE was fractionated with n-hexane (MVE-H), chloroform (MVE-C), ethylacetate (MVE-E), n-butanol (MVE-B), and water (MVE-W). MVE-H (150 ${\mu}g$/ml) showed the highest phosphorylating activity and increased glucose uptake by 2.3-fold. Oral administration of MVE-H (40 mg/kg) for 4 weeks to type 2 diabetic (db/db) mice reduced non-fasting and fasting blood glucose levels by 17.1% and 23.3%, respectively. Phosphorylation levels of AMPK and ACC in the soleus muscle and liver tissue of db/db mice were significantly increased by the administration of MVE-H. MVE-H was further fractionated using preparative HPLC to identify the AMPK-activating compounds. The NMR and GC-MS analyses revealed that ${\beta}$-sitosterol was a major effective compound in MVE-H. Phosphorylation levels of AMPK and ACC, and glucose uptake were significantly increased by the treatment of MVE-S (${\beta}$-sitosterol) isolated from M. verticillata to L6 cells, and these effects were attenuated by an AMPK inhibitor (Compound C) pretreatment. These results, taken together, demonstrate that increased glucose uptake in L6 myotubes by MVE-H treatment is mainly accomplished through the activation of AMPK. Our finding suggests that the extract isolated from M. verticillata seed would be beneficial for the treatment of metabolic disease including type 2 diabetes and hyperlipidemia.

  • Objectives: The purpose of this study is to assess the efficacy and safety of Baekhogainsam-tang for type 2 diabetes without complications by systemic review and meta-analysis of randomized controlled trials (RCTs). Methods: For a systematic review and meta-analysis, we set a key question in accordance with PICOT-SD. We searched the following up to March 31. 2021: PubMed, EMBASE, The Cochrane Library, CNKI, CiNii, KISS, KMBASE, OASIS, and ScienceON. A meta-analysis was conducted by synthesizing the results, including fasting plasma glucose level, postprandial 2-hour plasma glucose level, and Glycated Hemoglobin (HbA1c). Results: A total of five trials are included in this systemic review. The treatment group (Baekhogainsam-tang plus conventional treatment) showed more statistically significant effect than did the control group (conventional treatment only) in fasting plasma glucose level, postprandial 2-hour plasma glucose level, and glycated hemoglobin (HbA1c). Conclusions: Baekhogainsam-tang showed statistically significant effects in hypoglycemic effect and in improving insulin resistance. However, the number of studies included in the meta-analysis was insufficient, and the BIT used in the included studies was not standardized. This topic requires further attention and more clinical research.

사상체질과 대사증후군 유발에 관한 코호트 연구 (Metabolic Syndrome and Sasang Constitution in Cohort Study)

  • Objectives This study is to investigate the related factors to contribute the metabolic syndrome according to Sasang Constitution. Methods Nine hundred twenty six persons out of 1774 persons, over 40 years old, participated in community-based cohort in Wonju City of South Korea from June 2006 to August 2009. The diagnosis of metabolic syndrome was carried out by NCEP-ATP III(National Cholesterol Education Program in Adult Treatment Panel III) and Asian Pacific Criteria for abdominal obesity. The related factors were checked using questionnaire and blood samples. Sasang Constitution was verified by a Sasang Constitution specialist using the results of PSSC(Phonetic System for Sasang Constitution), facial pictures and simplified Sasang Constitutional questionnaires. Metabolic syndrome incidence rate according to Sasang Constitution and binary logistic regression analysis were performed with SPSS 19.0. Results Metabolic syndrome incidence rate was 30.3% and the majority of newly categorized as metabolic syndrome was Taeeumin(40.7%). There were significant risk factors like systolic blood pressure, fasting blood sugar, triglyceride and female and a significant defense factor like HDL-cholesterol. In terms of constitutional view, there were significant risk factors like waist circumference, systolic blood pressure, triglyceride in Soyangin, female, waist circumference, systolic blood pressure, fasting blood sugar, triglyceride in Taeeumin, female, waist circumference, systolic blood pressure, triglyceride in Soeumin. And there was a significant defense factor like HDL-cholesterol in only Taeeumin. Conclusions Regimens on metabolic syndrome were considered to be changed according to Sasang Constitution. Taeeumin female and Soeumin female should be cautious of body weight and metabolic syndrome when elderly. There are more cautious risk factors in each constitution; systolic blood pressure and triglyceride in Soyangin and fasting blood sugar and serum lipids levels in Soeumin and Taeeumin.

CREB and FoxO1: two transcription factors for the regulation of hepatic gluconeogenesis

  • Liver plays a major role in maintaining glucose homeostasis in mammals. Under fasting conditions, hepatic glucose production is critical as a source of fuel to maintain the basic functions in other tissues, including skeletal muscle, red blood cells, and the brain. Fasting hormones glucagon and cortisol play major roles during the process, in part by activating the transcription of key enzyme genes in the gluconeogenesis such as phosphoenol pyruvate carboxykinase (PEPCK) and glucose 6 phosphatase catalytic subunit (G6Pase). Conversely, gluconeogenic transcription is repressed by pancreatic insulin under feeding conditions, which effectively inhibits transcriptional activator complexes by either promoting post-translational modifications or activating transcriptional inhibitors in the liver, resulting in the reduction of hepatic glucose output. The transcriptional regulatory machineries have been highlighted as targets for type 2 diabetes drugs to control glycemia, so understanding of the complex regulatory mechanisms for transcription circuits for hepatic gluconeogenesis is critical in the potential development of therapeutic tools for the treatment of this disease. In this review, the current understanding regarding the roles of two key transcriptional activators, CREB and FoxO1, in the regulation of hepatic gluconeogenic program is discussed.

여성수도자의 연령별 대사증후군 유무별 대사증후군 위험요인과 생활습관 차이 (Metabolic Syndrome Prevalence and Lifestyle by Age and Metabolic Syndrome Status in Women Religious)

  • Purpose: The purpose of this study was to analyze the prevalence, risk factors of metabolic syndrome, and lifestyle in religious women by age and metabolic syndrome status between the metabolic syndrome group and the normal group. Methods: As the subjects for this study, 125 religious women in the city of D, H, S, Y, participated in this study. The diagnostic criterion of metabolic syndrome used was the National Cholesterol Education Program Adult Treatment Panel III. Results: The prevalence of metabolic syndrome was higher as the religious women got older. The metabolic syndrome group in their forties showed higher waist circumference, triglycerides, and lower HDL-cholesterol than the normal group. Among those in their fifties, the metabolic syndrome group had higher waist circumference, fasting glucose, triglyceride and lower HDL-cholesterol than the normal group. In their sixties, the metabolic syndrome group had higher fasting glucose, triglyceride and systolic blood pressure than the normal group. Conclusion: The metabolic syndrome group in their forties showed that their practice rate of 'trying to avoid stresses at work', 'taking prescription medicines' was low. For those in their fifties, the practice rate of 'reducing overeating' and 'choice of low fat meats' was low. Finally, in the group of those in their sixties, 'reducing fried foods' was low.

  • Ileus is blockage of the intestine that prevents the contents of the intestine from passing to the lower bowel. In western medical treatment, surgery is mandatory for severe cases such as strangulation ileus. However more generally, conservative management is used for treatment. Examples of conservative management are decompression with tube, fluid treatment, enema and fasting cure. Until now, many oriental medical studies have been conducted on chronic ileus, but almost none on acute ileus. The results for the case in this report suggest that oriental medical treatment such as acupuncture, moxibustion, herbal medicine is effective in treating acute ileus. We think that further studies should be conducted on a combination therapy with other oriental medical treatment such as retention enema.

  • Objectives This study was to report the effect of oriental medical treatment and dietetic therapy on metabolic syndrome. Method We treated three cases of metabolic syndrome patients with herbal medicine, electroacupuncture, cupping a boil, dietetic therapy, aerobic exercise were done during the treatment period. Result After treatment the diagnostic index(waist circumference, triglycerides, HDL cholesterol, blood pressure, fasting glucose) was improved. Conclusion The patient with Metabolic syndrome was improved using oriental medical treatment and dietetic therapy.

후박(厚朴)이 ob/ob 마우스의 대사성 염증과 인슐린 저항성에 미치는 영향 및 관련기전에 대한 연구 (The Effect of Magnolia Bark on the Metabolic Inflammation and Insulin Resistance of ob/ob Mice)

  • Objective: This study was undertaken to investigate how magnolia bark extract affects ob/ob mouse in terms of metabolic inflammation and insulin resistance. Methods: Leptin-deficient ob/ob mice were divided into 2 groups (n=5): a normal saline treatment (=control) and magnolia bark treatment. Wild type mice were the lean group (n=5). After 5 weeks, we measured fasting blood sugar (FBS) and conducted oral glucose tolerance tests (OGTTs) in each group. After 6 weeks, we measured body weight, epididymal fat pad weight, liver weight, serum glucose, serum insulin, and gene expression of tumor necrosis factor-${\alpha}$, interferon-${\gamma}$, and interleukin-6. We characterized the phenotype of adipose tissue macrophages (ATMs) and analyzed fractions of the phenotype in each group by flow cytometry. Results: In the magnolia bark group, fasting blood sugar, oral glucose tolerance levels, and insulin resistance (HOMA-IR) were significantly decreased. The population and proportion of ATMs among leukocytes in adipose tissue were significantly decreased in the magnolia bark group. The population and proportion of M1 type ATMs among ATMs were significantly decreased in the magnolia bark group. Gene expression of tumor necrosis factor-${\alpha}$ was significantly decreased in the magnolia bark group. Conclusions: These results support a positive effect of magnolia bark on metabolic diseases such as insulin resistance and metabolic inflammation in leptin-deficient ob/ob mice.

복부 둔상에 의한 소장 천공 환자의 임상 양상 및 예후 인자 (Clinical Aspects and Prognostic Factors Of Small Bowel Perforation After Blunt Abdominal Trauma)

  • Background: The incidence of abdominal trauma with intra-abdominal organ injury or bowel rupture is increasing. Articles on the diagnosis, symptoms and treatment of small bowel perforation due to blunt trauma have been reported, but reports on the relationship of mortality and morbidity to clinical factors for prognosis are minimal. The purposes of this study are to evaluate the morbidity and mortality of patients with small bowel perforation after blunt abdominal trauma on the basis of clinical examination and to analyze factors associated with the prognosis for blunt abdominal trauma with small bowel perforation. Methods: The clinical data on patients with small bowel perforation due to blunt trauma who underwent emergency surgery from January 1994 to December 2009 were retrospectively analyzed. The correlation of each prognostic factor to morbidity and mortality, and the relationship among prognostic factors were analyzed. Results: A total of 83 patients met the inclusion criteria: The male was 81.9%. The mean age was 45.6 years. The mean APACHE II score was 5.75. The mean time interval between injury and surgery was 395.9 minutes. The mean surgery time was 111.1 minutes. Forty seven patients had surgery for ileal perforations, and primary closure was done for 51patients. The mean admission period was 15.3 days, and the mean fasting time was 4.5 days. There were 6 deaths (7.2%), and 25 patients suffered from complications. Conclusion: The patient's age and the APACHE II score on admission were important prognostic factors that effected a patient's progress. Especially, this study shows that the APACHE II score had effect on the operation time, admission period, the treatment period, the fasting time, the mortality rate, and the complication rate.

  • Objectives: The purpose of this study is to assess the antidiabetic effect and safety of Galgunhwangryunhwanggum-tang for type 2 diabetes without complications by analyzing related research. Methods: For a systematic review and meta-analysis, we searched for the antidiabetic effect and safety of Galgunhwangryunhwanggum-tang for type 2 diabetes without complications in 10 databases up to September 2021. Only randomized controlled trials were chosen. Results: In the treatment effectiveness analysis and meta-analysis, Galgunhwangryunhwanggum-tang had significant improvement effects on fasting plasma glucose level, 2-hour postprandial glucose level, glycated hemoglobin, fasting insulin, and homeostasis model assessment for insulin resistance compared to the control group when treated in parallel with oral glycemic drugs. Conclusion: Galgunhwangryunhwanggum-tang is effective in improving blood sugar and insulin resistance in type 2 diabetes patients without complications and can especially be considered in parallel treatment with oral hypoglycemic drugs. A large-scale randomized controlled clinical trial is required to complement the limitations presented in this study in the future.
