A Meta-Analysis of Influencing Soybean Food Interventions on the Metabolic Syndrome Risk Factors Utilizing Big Data (빅 데이터 분석을 활용한 콩 식품 중재가 대사증후군 위험요인에 미치는 영향 메타분석)

  • Big data analysis refers the ability to store, manage and analyze collected data from an existing database management tool. In addition, extract value from large amounts of structured or unstructured data set and means the technology to analyze the results. Meta-analysis is a statistical integration method that delivers an opportunity to overview the entire result of integrating and analyzing many quantitative research results. Meta-analysis is sometimes expressed as an analysis of another analysis. Commonly, factors of metabolic syndrome can be defined as abdominal obesity, high triglycerides, low high density lipoprotein cholesterol, elevated blood pressure, and elevated fasting glucose. This study will find meaningful mediator variables for criterion variables that affect before and after the metabolic syndrome studies, on the basis of the results of a meta-analysis. We reviewed a total of 5 studies related to metabolic syndrome published in Korea between 2000 and 2016, where a cause and effect relationship is established between variables that are specified in the conceptual model of this study.

A Relationship Study between Metabolic Syndrome and Glaucoma in a Middle-Aged Men over the Age of 40's -Based on the 5th Korean National Health and Nutrition Examination Survey- (40세 이상 중장년의 대사증후군과 녹내장과의 관련성 연구 -제5기 국민건강영양조사 자료중심으로-)

  • Pupose. This study aimed to analyze the relationship between metabolic syndrome and glaucoma in a middle-aged men over the age of 40's by using the data from the 5th Korean National Health and Nutrition Examination Survey(2010~2012). Methods. The subjects for the analysis were comprised of 410 patients who were at least 40 years or more with metabolic syndrome and glaucoma. The comparison between the metabolic syndrome and glaucoma according to the health demographic characteristics, health behavior and health conditions was analyzed with ${\chi}^2$-test. The complex samples logistic regression analysis was used to calculate the odds ratio of glaucoma according to the health demographic characteristics, health behavior and health conditions and the combination of the comprising factors of the metabolic syndrome. Results. The prevalence of glaucoma among the subjects with metabolic syndrome and glaucoma was significantly higher in the case of females, as their age increased, educational level decreased, income level decreased, who are non-smokers, non-drinkers, with exercising habits of less than 3 days a week (p<0.001). However, the glaucoma OR was significantly high in case of increasing age, with exercising habits of less than 3 days a week, and hypertension (p<0.01), while the OR was significantly low in case of the drinking group (p<0.05). The results of analyzing the odds ratio for the occurrence of glaucoma with the components of metabolic syndrome showed that the OR for glaucoma is significantly low (OR 0.63) in case of concurrently high fasting blood glucose, high triglycerides and low HDL cholesterol levels (p=0.05). Conclusions. Therefore the thorough management of metabolic syndrome and components is needed to prevent glaucoma.

Effects of Lifestyle and Dietary Behavior on Cardiovascular Risks in Middle-aged Korean Men

  • Lifestyle and dietary behavior intervention as the primary prevention of lipid disorder seems safe and compatible with other treatments of cardiovascular diseases. Cross-sectional associations between lifestyle factors and dietary behavioral factors with plasma lipid and lipoprotein levels were analyzed in 189 middle-aged men in Suwon, Korea. Overnight fasting plasma levels of total cholesterol, high-density lipoprotein(HDL)-cholesterol, triacylglycerol and glucose were analyzed. Blood pressure and anthropometric data were also measured. Lifestyle factors such as smoking status, alcohol consumption and frequency of physical exercise were evaluated by a self-administered questionnaire. Questions regarding dietary behavior were also asked. The subjects were 43.8%${\pm}$7.9 years old, and 23.8%${\pm}$2.6kg/m$^2$. From stepwise regression analyses, significant correlates with total cholesterol level were body mass index(BMI), alcohol intake(negative), age and coffee drinking(model R$^2$=14.3%). BMI, breakfast-skipping, age, and sleeping hours were significant for triacylglycerol level(model R$^2$=15.8%). BMI, alcohol drinking(negative), age, and coffee drinking were significant for low-density lipoprotein(LDL)(model R$^2$=11.7%). Age(negative), BMI(negative), alcohol drinking, stress level(negative), physical exercise, and cigarette smoking(negative) were significant for high-density lipoprotein(HDL)(model R$^2$=12.1%). From stepwise regression analyses, excluding BMI and age as factors in the model, alcohol intake(negative) and coffee drinking were significantly correlated with total cholesterol level(model R$^2$=4.4%) : breakfast-skipping with triacylglycerol(model R$^2$=3.2%) : alcohol intake (negative) with LDL level(model R$^2$=3.4%) : alcohol intake, physical exercise and stress level(negative) with HDL level(model R$^2$=6.3%). The findings suggest that a healthy daily lifestyle and dietary behavior may have an anti-atherogenic effect by altering plasma lipid and lipoprotein levels in middle-aged Korean men.   • Objectives: The objective of this study was to assess the gender differences in the relationship between alcohol use behaviors and the risk of metabolic syndrome among Korean adult population. Methods: Data from the Korean National Health and Nutrition Examination Survey (5th, 2010) was analyzed. Using the Alcohol Use Disorders Identification Test, subjects were classified into 4 groups including non-drinking, low-risk drinking, medium-level alcohol problem, and high-level alcohol problem group. Metabolic syndrome and its components were compared among the alcohol behavior groups by gender. Results: The odds ratio vs. non-drinking group for metabolic syndrome was significantly higher in male medium and high-level alcohol problem group. In female, high-level alcohol problem group showed significantly higher odds ratio for metabolic syndrome. Blood pressure and HDL-cholesterol were likely to be higher with the excessive drinking behaviors in both men and woman. Triglyceride level in men was significantly higher in the medium and high-level alcohol problem group than non-drinking group. Although fasting glucose level did not show differences among drinking groups, the odd ratios vs. non-drinking group for the hyperglycemia (${\geq}$ 100mg/dl) were significantly higher in female drinking groups. Conclusions: Problematic alcohol drinking is likely to increase the risk of metabolic syndrome in Korean adults.

Comparison of Nutritional Status among Obese, Overweight and Normal Subjects in Seoul (서울지역 중년에 있어서 정상체중군과 과체중군의 영양상태 비교)

  • The nutritional status of middle-aged overweight and control normal subjects were evaluated for the purpose of providing the background information of the degenerative disease control. A survey was conducted with 293 healthy subjects (121 males and 172 females) between the ages 40-64. The average BMI (body mass index) of male subjects was 24.5 $\pm2.75$, and that of females was 23.5 $\pm2.87$. The average values of WHR (waist-hip ratio) were 0.88 $\pm0.04$, for males and 0.82 $\pm0.07$, for females. The normal BMI group (BMI 18.5-22.9) comprised 28.9% of males and 47.1% of females. The percentage of overweight subjects (BMI 23-24.9) was 26.4% of males and 30.8% of females. The obese group (BMI > 25) was 44.6% of males ana 22.1% of females, showing the greater rate of obese state among male subjects. The average energy intakes were 76.6 $\pm14.9$% for males and 77.8 $\pm12.6$, % for females, protein intakes were 108.0 $\pm24.6$% for males and 111.2 $\pm22.7$, % for females of the RDA levels. The average intakes of other nutrients were above the 75% of RDA levels except calcium. The average nutrient intakes of the three subgroups according to their BMI values were not different for both males and females. There were weak correlations between obesity and blood biochemical indices. There were positive correlations between BMI or WHR and hemoglobin, hematocrit, fasting glucose, total cholesterol, triglyceride, AST or ALT. There were negative correlations between BMI or WHR and HDL-cholesterol. These results suggest that the obesity rate of middle-aged is an influential factor of chronic disease. The middle-aged subjects of this study with higher educational and socioeconomic background tend to have desirable nutrition knowledge and attitude, but the application of their knowledge or attitude was relatively poor.

The Presence or Absence of Osteoarthritis and Related Risk Factors in Korean Elderly using the 6th Korea National Health and Nutrition Examination Survey (2013~2014 KNHANES VI) (제6기 국민건강영양조사 자료를 이용한 한국 노인의 골관절염 유병 여부와 관련 위험 요인(2013~2014 KNHANES VI))

  • This study was performed to determine the prevalence and associated risk factors of osteoarthritis in the elderly using the Korea National Health and Nutrition Examination Survey (2013~2014 KNHANES VI). A total of 2,148 (936 males, 1212 females) elderly participants aged 65 years or older were selected. Prevalence of osteoarthritis was higher in 460 female (80.7%) and 167 rural (29.1%) groups, than in 110 male (19.3%) and 404 urban (70.9%) groups. While the mean values of LDL-cholesterol, triglyceride level, and waist circumference were significantly higher in the osteoarthritis group than the normal group, other parameters including height, HDL-cholesterol, fasting blood glucose, hemoglobin and hematocrit values were not. Quality of life (EQ-5D) was also significantly lower in the osteoarthritis group. The results of logistic regression analysis showed that smoking, alcohol intake, obesity, hypercholesterolemia and energy intakes were significantly different in regards to the prevalence of osteoarthritis. This study suggests that obesity and energy intakes were associated with osteoarthritis, whereas risk factors of chronic disease, such as hypertension, Hypo-high density lipoprotein, hypertrigly-ceridemia, anemia, diabetes, vigorous physical activity, moderate physical activity and walking were not. Long-term prospective study is necessary to investigate the effects of these factors on osteoarthritis.

Self-rated Health and its Associated Factors in the Elderly (노인의 주관적 건강상태와 관련요인에 관한 연구)

  • This study aimed to identify the self-rated health and its associated factors of the elderly. Study subjects were community-dwelling elderly of 60 or more. Survey was done by using questionnaire including general characteristics, lifestyle, and health related variables. Blood pressure, anthropometric variables, lipid profiles, and fasting glucose were checked. Descriptive statistics showed that good self-rated health was 78.2% in the elderly. Multiple logistic regressions revealed that good self-rated health was associated with disease(OR:10.83, 95% CI:2.47-47.43), depression(OR=2.50, 95% CI=1.20-5.18), and exercise(OR=3.01, 95% CI=1.40-6.47). Therefore, health promotion program considering associated factors of self-rated health should be developed.

Influence of Alcohol Consumption on the Serum hs-CRP Level and Prevalence of Metabolic Syndrome -Based on the 2015 Korean National Health and Nutrition Examination Survey - (알코올 섭취량이 혈청 hs-CRP 수준 및 대사증후군 유병률에 미치는 영향 - 2015 국민건강영양조사 자료 이용 -)

  • Elevated high-sensitivity C reactive protein (hs-CRP) levels and metabolic syndrome are considered important predictors of cardiovascular mortality. This study examined the influence of the alcohol consumption level on the hs-CRP level and the prevalence of metabolic syndrome in South Koreans. The study subjects were 3,884 participants (${\geq}19$ years) from the KNHANES in 2015. The subjects were divided into four groups according to their alcohol consumption; None (none-alcohol consumption), Low (less than 15 g/day), Medium (15~29.9 g/day), and High (over than 30 g/day). The odds ratio for high-risk hs-CRP of men was significantly lower in the Low or Medium groups compared to the None group. In women, the odds ratio for high-risk hs-CRP was not decreased in the Low or Medium groups but was significantly higher in the High group than in the None group. Compared to nondrinkers, the prevalence of metabolic syndrome was lower in the Low group and Medium group in both men and women, whereas it was significantly higher in the High group in men only. In particular, the prevalence of low HDL-cholesterol was lower in all drinking groups. When the confounding factors were adjusted, the odds ratio for low HDL-cholesterol appeared to decline in the Low groups in both men and women. On the other hand, in the Medium group, women showed an elevated odds ratios of high blood pressure (HBP) and high fasting plasma glucose but men showed an elevated odds ratios of HBP and high triglyceride levels. The overall results suggest that low alcohol consumption is more appropriate than medium alcohol consumption. Nevertheless, more study will be needed to evaluate the appropriate alcohol consumption level.

Association of serum ferritin level and depression with respect to the body mass index in Korean male adults

  • BACKGROUD/OBJECTIVES: Obesity is globally a major public health issue. Evidence suggests that elevated ferritin levels are associated with obesity, dyslipidemia, insulin resistance, and metabolic syndrome. This study was undertaken to examine the relationship between the serum ferritin level and depression in Korean male adults with respect to classification of the prevailing obesity. SUBJECTS/METHODS: This was a case-control study; subjects were classified into obese group (${\geq}25.0kg/m^2$, 28 subjects) and normal group ($18.5-22.9kg/m^2$, 27 subjects). A survey was conducted to assess the depression levels as per the guidelines suggested by the Center program for Epidemiological Studies-Depression (CES-D). Blood was collected from each group for assessing biomarkers, and isolated plasma was evaluated for fasting glucose, insulin, quantitative insulin sensitivity check index, and ferritin levels. Data were analyzed, and groups were compared with respect to Body Mass Index (BMI), depression scale and biomarkers. RESULTS: The average depression score of the obesity group was 16.86, which was higher than the normal group (12.56). Subjects scoring more than 16 points comprised 53.6% of the population in the obese group, which was more than double that in the normal group, as assessed by the CES-D program. Furthermore, the serum ferritin level of the obesity group was 207.12 ng/mL, which was higher than that of the normal group (132.66 ng/mL). Lastly, the BMI appeared to be significantly correlated with both depression (r = 0.320, P = 0.017) and elevated ferritin levels (r = 0.352, P = 0.008). CONCLUSION: This study provides evidence of existing correlation between ferritin and depression with obesity.

Increased prevalence of periodontitis with hypouricemic status: findings from the Korean National Health and Nutrition Examination Survey, 2016-2018

  • Purpose: The aim of this study was to investigate the relationship between serum uric acid (SUA) levels and the risk of periodontitis in Korean adults using data from the Korean National Health and Nutrition Examination Survey (KNHANES). Methods: This cross-sectional study used data from the KNHANES 2016-2018 and analysed 12,735 Korean adults aged ≥19 years who underwent oral examinations. Hypouricemia was defined as SUA <3 mg/dL in men and <2 mg/dL in women, and hyperuricemia was defined as SUA ≥7 mg/dL in men and ≥6 mg/dL in women. Results: The weighted prevalence of hypouricemia and hyperuricemia was 0.6% and 12.9%, respectively. The overall weighted periodontitis rate was 30.5%. The frequency of periodontitis in subjects with hypouricemia, normouricemia, and hyperuricemia were 51.1%, 30.3%, and 30.6%, respectively. Study participants with hypouricemia were significantly older, had significantly fasting blood glucose levels, and had better kidney function than non-hypouricemic participants. In univariate logistic regression analyses, hypouricemia was associated with periodontitis, but hyperuricemia was not. The fully adjusted model revealed that the adjusted odds ratio of hypouricemia for periodontitis was 1.62 (95% confidence interval, 1.13-2.33), while the relationship between hyperuricemia and periodontitis in the multivariable logistic regression model was not significant. Conclusions: The results of this study suggest that hypouricemia is associated with an increased risk of periodontitis.
