Anti-diabetic effects of the extract from Atractylodes lancea, Anemarrhena asphodeloides and Cinnamomum Cassia mixture in high fat diet-induced diabetic mice and regulation of the function in C2C12 mouse skeletal muscle cells (창출·지모·육계 복합추출물의 고지방식이 유도 당뇨병 마우스에서의 항당뇨 효능 및 C2C12 골격근세포에서의 조절기전 연구)

  • Objective : This study investigated the anti-diabetic effects of DM1, a herbal mixture with Atractylodis Rhizoma, Anemarrhenae Rhizoma, and Cinnamomi Cortex in high fat diet (HFD)-induced diabetic mice and the mechanism in C2C12 mouse skeletal muscle cells. Methods : The C57B/6 mice were fed high fat for 12 weeks, and then administrated DM1 extract (500 mg/kg, p.o.) for 4 weeks. The changes of body weight, calorie and water intakes, fasting blood glucose levels and the serum levels of glucose, insulin, triglyceride, HDL-cholesterol, AST and ALT were measured in mice. The histological changes of liver and pancreas tissues were also observed by H&E stain. C2C12 myoblasts were differentiated into myotubes and then treated with DM1 extract (0.5, 1, and 2 mg/㎖) for 24 hr. The expression of myosin heavy chain (MHC), PGC1α, Sirt1 and NRF1, and the AMPK phosphorylation were determined in the myotubes by western blot, respectively. Results : The DM1 extract administration significantly decreased the calorie and water intakes, glucose, triglyceride, AST and ALT levels and increased insulin and HDL-cholesterol in HFD-induced diabetic mice. DM1 extract inhibited lipid accumulation in liver tissue and improved glucose tolerance. In C2C12 myotubes, DM1 treatment increased the expression of MHC, PGC1α, Sirt-1, NRF-1 and the AMPK phosphorylation. Conclusion : In our results indicate that DM1 can improve diabetic symptoms by decreasing the obesity, glucose tolerance and fatty liver in HFD-induced diabetic mice, and responsible mechanism is might be related with energy enhancement.

  • Recent studies suggest a possible pathogenic linkage between the osteoporosis and atherosclerosis. We investigated the relationship between cardiovascular risk factors, including high sensitivity C-reactive (hs-CRP), hs-CRP and bone metabolism in females. Anthropometric measurements were performed on 300 women, and cardiovascular risk factors, including fasting blood glucose, lipid profiles and hs-CRP. An atherogenic index was calculated using the serum total cholesterol level divided by the high-density lipoprotein cholesterol level. The lumbar spine bone mineral density was measured using dual X-ray abosorptiometry. By bivariate analysis, the lumbar spine BMD showed negative correlations with age, systolic and diastolic blood pressures, serum total cholesterol, high-density lipoprotein cholesterol. The Age, BMI and high-density lipoprotein cholesterol showed significant correlations with the lumbar spine BMD. From linear regression analyses of all the study subjects, age, high-density lipoprotein cholesterol were found to be determinants of the lumbar spine BMD($R^2=0.272$).

The Association of Employment Status, Workplace Environment, and Hazard Exposure with Health Outcome in the Adult Korean Population according to KNHANES IV (제 4기 국민건강영양조사 자료를 이용한 근로자들의 근로형태, 작업환경 및 유해요인 노출과 건강검진결과의 관련성)

  • Objectives: This study was designed to investigate the current status of the association of job-related information such as employment status, workplace environment, and hazard material exposures with health examination outcomes. Methods: The study used data from KNHANES 2007-2009 representing the three years of 2007-2009, which was conducted annually using a rolling sampling design that involved a complex, stratified, multistage, probability-cluster survey of a representative sample of the non-institutionalized civilian population in Korea. The final analytical sample consisted of 17,240 participants. Information on age, education, smoking history and alcohol intake was collected during the health interview. Job related information consisted of employment status, workplace environment, and hazardous material exposure. The selected indices of health examination were blood pressure, fasting glucose, blood cholesterol, HDL, SGOT, SGPT, and BUN. Results: In multiple logistic regression analysis using hypertension and pre-hypertension as dependent variables and job related categories as independent variables after covariate adjustments, the odds of hypertension and pre-hypertension were significantly lower in those with responsibility and power in their job activities. Interestingly, low odds for hypertension were observed among those who reported that their jobs were fast-paced. Conclusions: This study confirmed that some job-related categories in employment status, workplace environment, and hazardous material exposure had an association with health outcome status. It is worthwhile to comment that high responsibility and power in job activities were revealed as one of the important favorable factors to improve health condition of workers.

Nutritional Status and Utilization of Public Health Center of Elderly (보건소 내원 노인들의 영양상태 및 보건소 이용에 관한 연구)

  • This study was performed to assess the nutritional status, frequency of visiting and necessity of nutrition programs for 157 elderly(male:49, female:108) visiting public health centers in Puchon. The purpose of the study was to provide the basic data for developing a nutrition service model. The subject were investigated by interviews with a questionnaire to obtain dietary data and other information related to public health center. Blood tests for analyzing biochemical status were also carried out. The elderly showed low income status. Ninety two percent of them showed their monthly income was less than 400,000 won and 72.6% was observed as having 30,000 won/month as their pocket money. The most frequent disease reported as having or being treated were hypertension(32.6%), rheumatic arthritis(28.5%), diabetes(10.2%), and stomach disease(8.2%) for males and hypertension(33.1%), diabetes(19.4%), rheumatic arthritis(16.7%), anemia(11.1%) for females. The nutrients whose daily intakes were less than 2/3 of RDA were calcium(37.5% RDA),vitamin A(49.9% RDA), iron(60.0% of RDA) and protein(62.0% RDA) for males and vitamin A(27.7% RDA), vitamin B$_2$(33.3% RDA), calcium(44.1% RDA), iron(53.3% RDA), and niacin(60.0% RDA) for females, respectively. Prevalence of anemia, assessed by hemogloben using WHO definition, were 4.1% for males and 18.5% for females. The percentage of males with hypercholesterolemia( 220 mg/dl) was 2.1% and 19.4% fir females, Two percent of males and 12.0% of females were observed as having a LDL-C higher than 165 mg/dl. The mean fasting blood glucose(FBG) level of males and female was 84.2 mg/dl and 101.7 mg/dl respectively. Two percent of males and 8.3% of females were found with a FBG higher than 140 mg/dl. Seventy one percent of elderly reported they were visiting public health centers at least once per week or more frequently. They were satisfied most with the low medical bills but showed the lowest satisfaction for the facilities of the public health centers. What the nutrition service programs wanted most was nutrition counseling and guidance.

The Effects of Supungsunki-hwan Partitioned Prescriptions on Obese Type 2 Diabetes Mouse Model Induced by High Fat, High Carbohydrate Diet (수풍순기환 분할처방 투여가 고지방, 고탄수화물 식이로 유발된 비만형 제2형 당뇨병 동물모델에 미치는 영향)

  • Objectives : Recently a lot of research is being done for find antidiabetic medicine which has no side effects. This study aimed to investigate the antidiabetic and antiobesity effects of Supungsunki-hwan partitioned prescriptions on obese type 2 diabetes mouse. Methods : Type 2 diabetes mellitus and obesity were induced by Surwit's high fat, high sucrose diet for 8 weeks. Mice were divided into 3 groups of ND (normal diet, n=10) HFD (high fat and high sucrose diet, n=10) and SPP (high fat and high sucrose diet with Supungsunki-hwan partitioned prescriptions, n=10) groups. Body weights were measured every week. After 7 weeks, fasting blood sugar and oral glucose tolerance tests were conducted. After 8 weeks, blood samples of all mice were taken from their heart and analyzed biochemically. At the same time, epididymal fat pad and liver weights were measured. Histological size of white adipocyte were measured as well. Results : Compared with a HFD group, body weight, fructosamine, epididymal fat pad weight and white adipocyte size decreased. High-density lipoprotein cholesterol levels increased in the SPP group. Conclusions : These results suggest that SPP has antidiabetic and antiobesity effects in high fat, high sucrose diet induced obese mice.

Relationships among Serum Adiponectin, Leptin and Vitamin D Concentrations and the Metabolic Syndrome in Farmers (농업인의 혈중 아디포넥틴, 렙틴 및 비타민 D 농도와 대사증후군 간의 관련성)

  • The purpose of this study was to investigate the relationships among serum adiponectin, leptin and vitamin D concentrations and the metabolic syndrome in Korean farmers. 105 (26 males, 79 females) farmers (39~78 years, mean age $59.4{\pm}9.6$ years) in Gangwon - area were included in this study. Anthropometric measurements and biochemical blood analysis of subjects were carried out. The prevalence of obesity, abdominal obesity, hypertension, diabetes, hypertriglyceridemia, hypercholesterolemia and hyper LDL-cholesterolemia, metabolic syndrome were 51.9%, 65.7%, 49.5%, 15.3%, 17.3%, 13.5%, 11.5% and 40.9%, respectively. Serum adiponectin and leptin levels ($8.90{\mu}g/ml$ and 12.6 ng/ml) of females were significantly higher than those ($6.49{\mu}g/ml$ and 4.88 ng/ml) of males. But there was no significant difference in 25(OH)vitamin D concentration between males (15.4 ng/ml) and females (16.9 ng/ml). In the subjects with metabolic syndrome, the adiponectin levels were significantly lower and leptin levels were significantly higher than those of the subjects without metabolic syndrome. Serum adiponectin level had positive correlations with HDL-cholesterol level (r = 0.325, p < 0.001), but showed negative correlations with triglyceride and fasting blood glucose concentrations, body weight and waist/hip circumference ratio (r = -0.202 ~ -0.317, p < 0.05). Serum leptin and 25(OH)vitamin D concentrations were positively correlated with body fat (kg, %) and BMI, waist and hip circumferences (r = 0.244 ~ 0.682, p < 0.001). The results of this study suggested that adiponectin and leptin levels could be credible indices to predict chronic diseases in farmers. However, further research on vitamin D should be carried out considering another factors.

Case Report of Liver Cirrhosis with Diabetes Mellitus and Hepatic Encephalopathy (당뇨병과 간성뇌증이 동반된 간경변증 1례)

  • The liver is one of the principal organs involved in glucose metabolism. Diabetes mellitus(DM) commonly develops in patients with liver cirrhosis as a result of hepatocyte dysfunction. Hepatic encephalopathy(HE) is a major neuropsychiatric complication of liver cirrhosis. HE develops frequently in advanced stage of liver cirrhosis due to portal hypertension. We experienced a case of liver cirrhosis with DM and HE in 67 years old male patient. The patient's symptoms were lethargy, general malaise, asterixis, dizziness, and heavy headedness. Hwangryunjihwang-tang, Saengkankunbi-tang, Injinchija-tang and Injinho-tang were prescribed to the patient. Finally, the symptoms had been improved, however significant change was not observed from serum ammonia, fasting blood sugar and postparandial blood sugar. So we hope that this case report will be helpful in treating patient of liver cirrhosis.

Serum Adiponectin Level Association with Breast Cancer Risk: Evidence from a Case-Control Study

  • Background: Adiponectin is a protein, synthesized by adipose tissue, which has a questionable role in breast cancer pathogenesis. This study focused on serum levels of adiponectin among cases and controls, and its possible association with breast cancer risk. Materials and Methods: This case-control study covered 175 diagnosed breast cancer patients with positive histopathology from Breast Clinic, surgical unit-1, Civil Hospital, Karachi and 175 healthy controls from various screening programs. Blood samples were obtained after an overnight fast and later analyzed. Results: Fasting blood glucose was significantly raised, while serum adiponectin was significantly decreased in breast cancer cases when compared to control subjects. To find the precise influence of serum adiponectin on the risk of breast cancer, subjects were divided in to quartiles based on serum levels of adiponectin. It is observed that risk of breast cancer decreased with rising quartile. Q2 (25th-50th) had the highest OR=1.76 (CI: 0.93- 3.34), Q3 (50th-75th) has OR= 0.89 (CI: 0.48-1.64), while subjects in the highest quartile Q4 (>75th) had significantly (p< 0.05) decreased risk of breast cancer having OR=0.06 (CI: 0.02-0.15), when compared to the lowest quartile. Conclusions: We demonstrated a statistically significant association of elevated serum adiponectin with decreased risk of breast cancer. This signifies a protective role of adiponectin via an intricate mechanism of masking mitogenic growth factors in breast carcinogenesis.

Relationship between metabolic syndrome and oral diseases in the middle aged and elderly people (중·노년의 대사증후군과 구강질환 관련성)

  • Objectives: The purpose of the study was to identify the relationship between metabolic syndrome and oral diseases in the middle aged and elderly in Korea. Methods: The study subjects were 6,390 people over 40 years old from 2010 and 2012 Korea National Health and Nutrition Examination Survey. The survey questionnaire consisted of health, nutrition, and oral examination surveys. The independent variables included general characteristics, health behavior, oral health behavior, and metabolic syndrome. The dependent variables included dental caries experience and periodontal disease. The oral examination was carried out by the dentist based on World Health Organization standard. Results: The average prevalence rate of metabolic syndrome MS was 23.79%, including 54.84% of risk group and 21.37% of normal group. The missing teeth rate was 82.38%, DMFT rate was 90.28% and the periodontal disease rate was 33.15%. Those having abnormal fasting blood glucose had 1.17 fold(95% CI: 1.00~1.37) higher periodontal disease than the normal group. The abnormal HDL cholesterol group had 1.25 times higher odds ratio(95% CI: 1.07~1.46) and the obese group had 1.27 times higher odds ratio(95% CI: 1.07~1.51). The risk group had 1.20 times higher odds ration(95% CI: 1.00~1.44) and that of the metabolic syndrome group was 1.60 times higher(95% CI: 1.29~1.97) in periodontal disease. The high blood pressure group had 1.25 times of missing teeth prevalence rate(95% CI: 1.00~1.37). The metabolic syndrome group had 1.47 times of missing teeth prevalence rate(95% CI: 1.11~1.94). Conclusions: The middle aged and elderly people in Korea had higher rate of metabolic syndrome and oral disease. It is necessary to implement the preventive oral health examination for the control of metabolic syndrome and oral diseases prevalence.

Effect of Atractylodis Rhizoma Alba water extract on streptozotocin-induced diabetes in rats (백출이 streptozotocin 유발 당뇨흰쥐에서 췌장 및 신장에 미치는 영향)

  • Objectives : This study aimed to evaluate the effect of Atractylodis Rhizoma Alba water extract on streptozotocin (STZ)-induced diabetes in rats. Methods : Male Sprague-Dawley rats were divided into four groups ; normal, STZ-control and Atractylodis Rhizoma Alba (A) water extract-administrated group. Rats in which diabetic was induced by intraperitonal injection with STZ(60 mg/kg body weight). STZ-induced diabetic rats were orally administrated A extract daily for 5 weeks at doses of 200 or 500 mg/kg. Fasting blood glucose, total cholesterol, triglyceride and blood urea nitrogen were measured in sera of rats. Total volume of urine and urinary creatinine were also measured. Histopathological examination and immunohistochemical staining for the expression of insulin and ${\alpha}$-SMA in pancreas and kidney were performed, respectively. Results : There were no differences in body and kidney weights between STZ-control and A extract-administrated groups. However, serum triglyceride level was significantly decreased in A extract-administrated groups compared with those of STZ-control group. Histopathological analysis of pancreas and kidney revealed increased the number of islets and insulin-positive beta-cells in pancreas, and decreased morphological changes of glomerulus and ${\alpha}$-SMA expression in kidney after the administration of A extract. Conclusions : These results suggest that Atractylodis Rhizoma Alba has a biological action on STZ-induced diabetes in rats via decreasing the serum levels of total triglyceride, and suppressing the morphological changes of pancreas and kidney.
