The purpose of this study was to investigate the effects of systemic chronic inflammatory state on dry eye syndrome as a local chronic inflammatory condition. From June 16, 2016 to December 31, 2016, 726 patients who visited the general health examination center were screened using the Schirmer's test and the metabolic syndrome screening test. A total of 214 patients who were studied for hs-CRP and HbA1c were simultaneously selected for the study as well. Dry eye syndrome significantly increased in higher age groups (p<0.001) and women (p=0.020); there was no significant relationship with fasting plasma glucose, hs-CRP. In addition, as HbA1c increased the risk for dry eye syndrome also increased (β=1.960, p=0.025). Therefore, dry eye syndrome may not caused by microvascular changes in the lacrimal gland due to chronic inflammatory conditions. In diabetic patients, long-term blood glucose control may be more important than short-term blood glucose control. The high prevalence of dry eye syndrome in diabetic patients is thought to be due to autonomic dysfunction rather than microvascular changes caused by chronic inflammation.

To evaluate the health status of the aged in rural area, 476 men and women of 65 years old and older among the residents of Gunwee County in Kyungpook were examined at the county health center between 4th and 22nd of June, 1984. Physical examination included measuring height, weight, chest circumference and blood pressure and chest x-ray. Laboratory tests were done for serum cholesterol, glucose and GOT, and urine glucose and protein. The mean heights of male and female were 160${\pm}$7.0cm and 147.7${\pm}$6.9cm, respectively. The height decreased as the age increased. The mean body weight of male was 53.5${\pm}$7.8kg and 46.7${\pm}$7.5kg for female and the older, the eighter. The mean chest circumference of male was 85.4${\pm}$7.5cm and 79.5${\pm}$79cm for female and it had no remarkable differences by age group. The average height, body weight, chest circumference, and serum cholesterol level of the age 65 years and older in rural area were lower than the standard this may be due to undernutrition of the aged. The mean diastolic and systolic blood pressure of male were 75.9${\pm}$13.5 mmHg/126.3${\pm}$24.1 mmHg and those for female were 73.1${\pm}$13.9 mmHg/125.8${\pm}$32.4 mmHg. Both male an d female in the age group of 65-69 years showed the lowest mean blood pressure. Among the study population 18.3% the borderlin hypertension and 14.3% had hypertension. Average hemoglobin level of the male(12.5${\pm}$0.8 g/dl) was higher than that of the female(11.6${\pm}$0.8 g/dl) (p < 0.01). Mean serum G.O.T. level was 25.2${\pm}$8.7 IU for male and 23.6${\pm}$7.5 IU for female and all of the study population were within the normal range. Mean serum glucose level was 110.4${\pm}$16.0 mg/dl for the male and 110.5${\pm}$15.7 mg /dl for the female and 41.4% of the study population were above the normal limit of serum glucose level. However, there was only one person she had glycosuria. Hyperglycemia could be due to non-fasting before the test. Two of the study population had proteinuria and five persons (1.05%) were diagnosed as pulmonary tuberculosis by the chest x-ray.

The prevalence of type 2 diabetes mellitus has been rapidly increased in parallel with the westernization of eating behavior in Korea. Increased consumption of animal fat and simple sugar can be potential contributors for insulin resistance. The purpose of the study was to determine whether Western-(WD) and Korean-style(KD) diets altered insulin secretion and insulin resistance in male Sprague Dawley rats. Rats weighing 98$\pm$5g were provided by KD(77 En% of starch, 5 En% of corn oil and 13 En% of gluten plus 5 En% of casein), WD(42 En% of starch, 40 En% of butter and 18% of casein) or control diet(62 En% of starch, 20 En% of corn oil and 18% of casein) for 12 weeks. Body weights were lower in KD compared to WD. Fasting blood glucose levels were not different among diets. Insulin secretion from the beta cells was higher by 2.2$\pm$0.4 folds in WD than KD at baseline. In hyperglycemic clamp insulin secretion was higher in WD than KD and CD. Whole body glucose disposal rates referred to the state of insulin sensitivity were lowest in WD among groups. Glycogen deposits in soleus and quadriceps muscles were lowest in WD among all groups, but their triglyceride contents were highest. GLUT4 contents and glycogen synthase were lowest in WD in both muscles. In conclusions, westernization of diets needed more insulin to normalization of blood glucose levels due to increased insulin resistance. Thus, WD would lead to increased prevalence of diabetes mellitus when increased insulin resistance could not be compensated by insulin secretion in the case of elevated blood glucose levels. This study were investigated the effects of physiologically active compounds isolated from Platycodon grandiflorum on streptozotocin-induced diabetic rats. The experimental groups were divided into six groups, normal, diabetic control (CM), and four experimental groups (inulin (IN), inulin + saponin (IS), oligosaccharide (OS), and total extract group (WE)). The rats have free access to water and diet. Experimental groups were administered orally with inulin (630 mg/mL), inulin (630 mg/mL) + saponin (25.2 mg/mL), oligosaccharide (367.5 mg/mL), and total extract (225 mg/mL) into the diabetic rats for 5 weeks after STZ injection; The levels of fasting blood glucose in diabetic control increased by 10.0% for 5 weeks, but the level of blood glucose in inulin and total extract groups decreased by 21.3% and 21.2%, respectively. The concentrations of total cholesterol and triglyceride in serum of diabetic rats fed physiologically activity compounds were lower than those in diabetic control rats. HDL-cholesterol was similar among all the groups. Weights of testicle and heart were lighter, while weight of kidney was significantly increased in diabetic groups than normal group. The concentrations of serum protein insulin and albumin in diabetic groups were significantly decreased compared with those of the normal group. Urinary glucose excretion was decreased in inulin group than other diabetic groups.
There have been many studies to develop methods for predicting diabetes and to prevent diabetes. The validity of glycated hemoglobin (HbA1c), one of the commonly known tools in predicting diabetes, has been verified by many previous studies. In this study, we examined the cutoff value of HbA1c for diabetes and impaired fasting glucose (IFG). Based on this study, we proposed a proper clinical guideline and evaluated the validation of the guideline. Excluding those without blood glucose and HbA1c data, we used the data of 5,161 subjects (2,281 men and 2,880 women) over the age of 20 years from the 2015 Korean National Health and Nutrition Examination Survey. The correlation efficient of fasting plasma glucose (FPG) and HbA1c was 0.79, indicating a strong relationship. Howeve, the correlation efficient of FPG and HbA1c was low, showing 0.27 in non-diabetes, 0.39 in IFG, and 0.66 in diabetes, showing a strong relationship. The cutoff value of HbA1c for predicting diabetes using ROC curve was 6.05% (sensitivity 84.6%, and specificity 92.0%), and AUC was 0.941 (0.937 in men, and 0.946 in women). The cutoff value of HbA1c for predicting IFG using ROC curve was 5.55% (sensitivity 64.5%, and specificity 70.0%), and AUC was 0.733 (0.708 in men, and 0.764 in women). Therefore, it may not be appropriate to apply the guidelines for diagnosing IFG since sensitivity and specificity were below 70%. For future studies retarding the cutoff value of HbA1c in predicting IFG, high sensitivity and specificity are expected if we segment the reference range of IFG.
Objective: This study was conducted to investigate the hypoglycemic effect and safety of Ojeok-san in patients with type 2 Diabetes Mellitus. Methods: We investigated type 2 diabetes mellitus patients at Kyung-Hee University Korean Medical Hospital who were administered Ojeok-san for at least one day between January 2012 and September 2020, basal characteristics and laboratory tests were reviewed retrospectively. The hypoglycemic effect of Ojeok-san was assessed by comparing fasting blood sugar (FBS) and two hours post-prandial plasma glucose (PP2) levels from before and after taking Ojeok-san. Subgroup analyses were conducted according to baseline hypoglycemic treatments and glycated hemoglobin levels (< or ≥6.5%). The safety of Ojeok-san was assessed by comparing levels of aspartate aminotransferase, alanine aminotransferase, γ-glutamyl transferase, blood urea nitrogen, and creatinine. Results: After Ojeok-san administration, FBS and PP2 were significantly reduced to an average of 14.33 mg/dL and 27.67 mg/dL respectively. In the subgroup analysis, PP2 in patients receiving metformin mono therapy was significantly reduced to 28.67 mg/dL, and those receiving a dual therapy of metformin and DPP-4 inhibitor, it was significantly reduced to 32.33 mg/dL. In patients with glycated hemoglobin of lower than 6.5%, FBS was significantly reduced to 12.20 mg/dL, and both FBS and PP2 were significantly reduced, to 15.50 mg/dL and 40.00 mg/dL, respectively, in those with glycated hemoglobin levels of more than 6.5%. The safety profile showed no significant difference after Ojeok-san administration. Conclusions: Ojeok-san has significant hypoglycemic effects in patients with type 2 diabetes mellitus who are also taking hypoglycemic agents.
The blood components of diabetic patients who visited S- hospital in seoul from January 1982 to June 1983 were compared with the reference levels. Hemoglobin and hematocrit levels of diabetic patients were significantly lower than the reference. The diabetic patients showed 2-3 times higher levels of fasting blood glucose and postprandial -2 hours blood sugar. Levels of blood urine nitrogen and creatine were also significantly higher in the diabetes mellitus and the levels of potassium sodium and chlorine showed no differences although these were decreased gradually in older patients. Lower serum calcium levels were seen in the diabetic patients and this change was more significant at the ages higher than 40. The effect of an active vitamin D on serum-Ca level in diabetic patients was studied in comparison to that of non- diabetic persons. The serum calcium levels were slightly increased in control and insulin- dependent diabetic patients after a week- intake of 1,000mg calcium a day, while the intake of 1,25-dihydroxycholecalciferol ( 1,000 IU per day ) did not increase the serum calcium levels of these groups. Insulin - independent diabetic patients showed the rather lower serum calcium levels after a week- intake of 1,000mg calcium per day. However, the levels were increased after 2-weeks intake of the calcium and a week-in-take of the active vitamin D(1,000 IU/day ). This effect of vitamin D was seen in the groups with lower intake of calcium(500mg/day ) but not in the groups with 1,000mg calcium intake a day.

Objective : To investigate gamma-Glutamyltransferase (${\gamma}-GTP$) and its related factors in male industrial workers. Method : Five hundred and tony male workers without heart disease, diabetes mellitus, renal disease, hepatitis, and other liver diseases were surveyed in October 1998. Blood samples were collected to test for ${\gamma}-GTP$, total-cholesterol and fasting blood glucose. A self-administered questionnaire survey on life style was also done. Results : The total geometric mean value of ${\gamma}-GTP$ was 30.6 U/L. According to a univariate analysis: age, BMI(body mass index, $kg/m^2$), alcohol consumption, current smoking, stress, diastolic blood pressure, and blood total cholesterol were significantly associated with ${\gamma}-GTP$(p<0.05). From a multiple regression analysis: BMI, alcohol consumption, current smoking, diastolic blood pressure and total-cholesterol were significantly related to ${\gamma}-GTP$(p<0.05). Coffee consumption was negatively related to ${\gamma}-GTP$, but not significantly. Conclusion : We recommend that a change in health behavior (i.e. reducing alcohol intake, controlling BMI and not smoking) is necessary to decrease ${\gamma}-GTP$ in male workers.
Background: It is doubtful that aging causes deteriorated glucose metabolism and insulin resistance of skeletal muscle. Some researchers had different results about it. So we have studied the mechanism responsible for the abnormal glucose tolerance associated with aging in rapidly growing and matured rats. Materials and Methods: Animals were used S.D. rats. Growing rats were 7 weeks old (BW: 160-190 gm) and matured rats were 28 weeks old (BW: 420-525 gm). Results: Fasting blood glucose and plasma insulin levels were significantly elevated in matured rat compared with growing rats. And during oral glucose tolerance test the glucose level was also significantly elevated in matured rats. These results confirmed an insulin resistant state of aging. Insulin levels at 30 minutes of oral glucose tolerance test was significantly elevated in growing rat. But at 120 minutes it was maintained at higher level in matured rats than in growing rats. It suggested the possibility of increased insulin secretion by initial stimulation of beta-cells in growing rats, and increased secretion and decreased catabolic rate of insulin in matured rats. Glucose uptake rate of soleus muscle in matured rats was lower than that of growing rats, but the difference was not statistically significant. The dose(insulin)-responsive(glucose uptake) curve of soleus muscle was only slightly deviated to the right side. Conclusion: Glucose metabolism of rat skeletal muscle was worsened by aging. The data of glucose uptake experiments suggested the possibility of insulin resistance of skeletal muscle in matured rats. but the mechanism of insulin resistance of skeletal muscle need further studies.

Among patients who visited each clinical department for oriental medical treatments, anthropometric measurement, blood pressure, fasting blood glucose and blood lipid profile level were measured at their first initial visit. 55 subject patients who were diagnosed as having metabolic syndrome and 150 mg/dL or more of triglyceride were selected as subjects whose fasting blood glucose, triglyceride, total cholesterol, HDL cholesterol and LDL cholesterol were measured after fasting. According to each patient's disease, the subject received treatments such as herb medicine, acupuncture, moxibustion, cupping therapy, physical therapy and rehabilitation therapy from each clinical department, and after an average of 4.10${\pm}$0.31 weeks, another test was performed yielding the following results. Serum triglyceride was 243.72${\pm}$13.05 mg/dL before the oriental medical treatment and 188.11${\pm}$12.17 mg/dL after the treatment where although it continued to show an abnormal value even after the treatment, there was statistically significant decrease compared to pre treatment(P<0.05). Serum total cholesterol was 207.50${\pm}$5.89 mg/dL before the oriental medical treatment and 192.37${\pm}$5.53 mg/dL after the treatment which was statistically insignificant compared to pre treatment(P>0.05). Serum HDL cholesterol was 51.19${\pm}$3.95 mg/dL before the oriental medical treatment and increased to 52.53${\pm}$1.49 mg/dL after the treatment although it was statistically insignificant compared to pre treatment(P>0.05). Serum LDL cholesterol was 110.66${\pm}$5.86 mg/dL before the oriental medical treatment and decreased to 106.12${\pm}$4.82 mg/dL after the treatment although it was statistically insignificant compared to pre treatment(P>0.05). In regards to the change of triglyceride for each sex, it was 221.84${\pm}$14.01 mg/dL before the treatment and 187.00${\pm}$15.47 mg/dL after the treatment for men, and it was 271.50${\pm}$22.78 mg/dL and 189.53${\pm}$19.76 mg/dL respectively for women where even though men and women showed the decrease of 34.84${\pm}$12.79 mg/dL and 81.96${\pm}$20.01 mg/dL respectively, both men and women continue to show abnormal values after the treatments. However, there was statistically significant decrease compared to pre treatment(P<0.05). In regards to the change of total cholesterol for each sex, with 198.24${\pm}$7.60 mg/dL for men before the treatment and 188.93${\pm}$7.45 mg/dL after the treatment, values for both before and after the treatment were within the normal range where the change value was 9.30${\pm}$5.86 mg/dL and statistically insignificant(P>0.05). For women, it was 219.26${\pm}$8.87 mg/dL and 196.73${\pm}$8.43 mg/dL respectively for women where with 22.53${\pm}$7.60 mg/dL, it decreased to the normal level after the treatment, and there was a statistically significant decrease compared to pre treatment(P<0.05). With such results, serum triglyceride and cholesterol levels of patients who have been diagnosed as having metabolic syndrome were observed to decrease after the oriental medical treatment. Especially, for both men and women, abnormally high triglyceride level decreased while the effect of lipid profile improvement for women was more significant compared to men.
