After first successful mitral valvotomy by Harken and Bailey in 1948, improvement of surgical technique and cardiac device may last rapidly for several years, but there after many patients deteriorate because of various causes, so incidence of reoperation for cardiac valvular disease has increased time by time. This paper is concerned with 21 patients in whom a second operation has been carried out from Jan. 1963 to Aug. 1984 at the department of Thoracic and Cardiovascular department, National Medical Center. Of 21 patients, 7 were male and 14 were female, and ages ranged from 14 to 37 years The second operation are classified into groups of secondary closed mitral commissurotomy [3 cases], open commissurotomy following closed mitral commissurotomy [1 case], Valve replacement following closed mitral commissurotomy [14 cases] or bioprosthetic valve replacement [3 cases]. Main cause of reoperation was restenosis or steno insufficiency, and that of bioprosthetic valve failure was bacterial endocarditis [1 case], fibrous tissue overgrowth on the Xenograft [1 case] and technical failure [1 case]. Early operative mortality was absent, but during follow-up, 4 patients died, so late mortality was 19.0%, and main cause of death was congestive heart failure.

In this study a neural network based expert system designed to diagnose operating status of a rotating spindle system supported by ball bearings was introduced. In order to facilitate practical failure situations, five exemplary abnormal status was fabricated. Out of several possible data source locations, seven most effective spots were chosen and proven to be the most successful in predicting single and multiple abnormalities. Increased signal strength was measured around where abnormality was embedded. Signal mea-surement locations producing high prediction rate were also classified. Even though multiple abnormalities were hard to be decoupled into their individual causes, proposed diagnostic system was somewhat effective in predicting such cases under certain combination of sensor locations. Among several abnormal operating conditions, highest prediction rate can be expected when signal is spoiled by the failure or damage in outer race. Proposed diagnostic system was again proven to be the most effective system in analyzing and ranking the importance of data sources.
A simple mechanical model is proposed to demonstrate qualitatively the pancake progressive collapse of multi-storey structures. The impact between two collapsed storeys is simulated using a simple algorithm that builds on virtual mass-spring-damper system. To analyze various collapse modes, columns and beams are considered separately. Parametric studies show that the process of progressive collapse involves a large number of complex mechanisms. However, the proposed model provides a simple numerical tool to assess the overall behavior of collapse arising from a few initiating causes. Unique features, such as beam-to-beam connection failure criterion, and beam-to-column connection failure criterion are incorporated into the program. Besides, the criterion of local failure of structural members can also be easily incorporated into the proposed model.
Slope stability is affected by various factors. For safety management of slopes, monitoring systems have been widely constructed along railway lines. The representative data from the systems are variations of ground profile such like ground water level and pore water pressure etc. and direct displacement measured by ground clinometer and tension wire sensor. Slopes are mainly effected by rainfall and rainfall causes the decrease of factor of safety(FOS). Because FOS varies linearly by the variation of ground water level and pore pressure, it has a weak point that could not define the time and proper warning sign to secure the safety of the train. In this study, alternative of FOS such as reliability index and probability of failure is applied to slope stability analysis introducing the reliability concept. FOS, reliability index, probability of failure and velocity of probability of failure of the slopes by variation of ground water level are investigated for setting up the specification of safety management of slopes. By executing case study of a slope(ILLO-IMSUNGLI), it is showed to be applied to specification of safety management.
Despite a huge amount of foreign assistance and close economic guidance by international donors throughout the past decades, Zambia today still suffer from a high level of aid dependency and the absence of sustainable economic development. In this study, I investigate the factors that resulted in aid (and development) failure in Zambia, focusing on institutional/historical contexts. I propose that in Zambia, government has largely failed to implement (or even produce) effective economic policies that could lead to successful use of foreign assistance for long-term, sustainable development. In particular, I focus on the nature of state and politics in Zambia, and argue that failed politics is one of the main causes of development and aid failure in Zambia and highlight colonial legacies and other contextual/institutional factors to understand the nature of politics and state in Zambia. In particular, this paper proposes that the Zambian case demonstrates that foreign aid and donor influence could worsen the situation directly by simply providing wrong guidance and also by further weakening the state (and institutional) capacity of the recipient country.
Oxidative stress has been implicated in a range of disease states, including end-stage renal failure treated with hemodialysis [Westhuyzen J. et al, 2003]. Free radicals react with biological molecules and destroy the structure of cells, which eventually causes free-radical induced disease such as cancer, renal failure, aging, etc. Exogenous or endogenously produced nitric oxide (NO) inhibits superoxide-stimulated urea permeability. In the inner medulla, superoxide generation by local oxidases may stimulate urea transport, and the role of endogenous No may be to dampen this effect by decreasing superoxide levels [Zimpelmann J. et al, 2003 (Epub ahead of print)]. In general, capacitors have a large influence on the life of the system due to frequent charging and discharging. In this paper, we analyze the cause of the core failure of high voltage, high current HVDC sub-module film capacitor and analyze the precautions of the capacitor design and manufacturing process. First, the cause of the fault, the failure mode, and the effect are analyzed through the FMEA of the capacitor. To quantitatively evaluate the causes and effects of faults that have the greatest effect on the failure of a capacitor, a fault tree for the capacitor is presented and the failure rate is analyzed according to the design parameters and the driving conditions. It is verified that the main cause of capacitor failure is the capacitance change, and it is necessary to minimize the temperature rise, corona occurrence, electrode expansion, and insulation distance decrease during capacitor design and manufacturing process in order to reduce the failure rate of the capacitor.
The main purpose of this study is to analyze research trend of 'business failure' from academic papers published in Asia Pacific Journal of Small Business. In this review, first, we reviewed research trend of failure, published in academic journals at abroad, explored the major topics, and set forth the framework of classification. Second, we selected and analyzed 16 Korean articles in a refined search from total 1,060 articles published in Asia Pacific Journal of Small Business from 1979 to 2019. Third, in order to understand overall research trend in Korea, additional publication search was done by online database system using keywords, and 24 other articles were selected. As a result, five research themes were identified and analyzed: (1) bankruptcy prediction, (2) emotion before and after failure, (3) costs of failure, (4) causes of failure, and (5) reentry determinants. We believe that this purposed review will offer future research issues regarding business failure.
We studied variation in clutch size, hatching, fledging and breeding success of the Black-tailed Gull (larus crassirostris) at Hongdo Island, which supported the largest breeding colony of the species, in South Korea in 1997 and 2000 to 2003. Cluck size was recorded in 1,388 nests and breeding success was estimated in 83 nests. Mean clutch size was 1.89 t 0.65. A total of 63.9% of the eggs hatched and 53.5% of the chicks survived until fledged. Breeding success was 34.2%. Causes of breeding failure were eggs missing (14.9%), rotten eggs (15.5%) and predation (2.4%) during the incubation period and chicks missing(35.8%), starvation (7.4%) and pecked chicks (3.2%) during the chick rearing period. In 2002, the main causes of breeding failure were eggs missing (17.2%) and rotten eggs (6.9%) during the incubation period and missing chicks (15.5%), starvation (3.5%) and pecked chicks (6.9%) during the chick rearing period. Most chick mortality occurred within three days after hatching.

An asset tracking using wireless sensor network is concerned with geographical locations of sensor nodes. The limited size of sensor nodes makes them attractable for tracking service, at the same time their size causes power restrictions, limited computation power, and storage restrictions. Due to such constrained capabilities, the wireless sensor network basically assumes the failure of sensor nodes. This causes a set of concerns in designing asset tracking system on wireless sensor network and one of the most critical factors is location uncertainty of sensor nodes. In this paper, we classify the location uncertainty problem in asset tracking system into following cases. First, sensor node isn't read at all because of sensor node failure, leading to misunderstanding that asset is not present. Second, incorrect location is read due to interference of RSSI, providing unreliable location of asset. We implemented and installed our asset tracking system in a real environment and continuously monitored the status of asset and measured error rate of location of sensor nodes. We present experimental results that demonstrate the location uncertainty problem in asset tracking system using wireless sensor network.
