사암침으로 호전된 동공산대를 동반한 동안신경마비 1례 (Case Report of Oculomotor Nerve Palsy with Mydriasis Improved by Sa-Am Acupuncture)

  • This study aims to report a case of oculomotor nerve palsy with mydriasis improved by Sa-Am acupuncture (大腸正格). The patient visited our clinic due to Right oculomotor nerve palsy with symptoms such as periocular pain, diplopia, vertigo, ptosis, eye adduction disorder, and mydriasis on November 11th, 2019. For the treatment, Sa-Am acupuncture (大腸正格) which used to give a significant effect on eye diseases and headaches and to remove toxins from the body was performed every time the patient visited the clinic. About a month after the treatment, ptosis disappeared, and eye adduction disorder also changed from -2 to 2 points, showing a significant improvement. Subjective symptoms such as vertigo, diplopia, and eye pain also significantly decreased from 13 to 4 points. At the second month of treatment, the eye adduction disorder improved to a normal level, and subjective ocular discomforts disappeared. The pupil size decreased from 5 mm to 2.5 mm. After 3-4 months from the start of treatment, most of the symptoms including the light reflex returned to normal. After a total of 32 acupunctures treatment, subjective symptoms such as periocular pain, diplopia and vertigo, as well as ptosis and eye adduction disorder disappeared, and the pupil size, including the light reflex, also improved to the normal level.

  • Inherited metabolic disorders (IMD) are a large group of rare disorders affecting normal biochemical pathways. The ophthalmic involvement can be very varied affecting any part of the eye, including abnormalities of cornea, lens dislocation and cataracts, retina and the optic nerve, and extraocular muscles. Eye disorders can be initial symptoms of some IMD and can be clue for diagnosis of IMD. However, eye disorders can evolve later in the natural history of an already diagnosed metabolic disorder. Awareness of IMDs is important to facilitate early diagnosis and in some cases instigate early treatment if a patient presents with eye involvement suggestive of a metabolic disorder. Ophthalmological interventions are also an important component of the multisystem holistic approach to treating patients with metabolic disorders.

수면안대와 귀마개 적용이 중환자실 관상동맥질환자의 수면양상에 미치는 효과 (Effects of Eye Protective Device and Ear Protective Device Application on Sleep Disorder with Coronary Disease Patients in CCU)

  • Purpose: The purpose of this research was to analyze the effects of an eye protective device and ear protective device application on sleep disorder with coronary disease patients in CCU. Methods: The research design was set up as a nonequivalent control group non-synchronized design. The subjects were coronary disease patients in the CCU of a tertiary hospital. The subjects were composed of 20 in the group with an eye protective device, 18 in the group with an ear protective device, 17 in the group with an eye protective device and ear protective device and 20 in the control group. The data was analyzed by $X^2-test$, ANOVA, Repeated measures ANOVA, Sheffe-test, Simple main effect and Time contrast using SPSS Win 12.0. Results: The hypothesis, 'There are significant differences in sleep quantity among the four groups' was supported (F=1,342.71, p=.000). The hypothesis, 'There are significant differences in the subjective evaluation of the general sleep patterns among the four groups' was supported (F=3,638.73, p=.000). In addition the hypothesis, 'There are significant differences in degree of sleep among the four groups' was supported (F=1,616.61, p=.000). Conclusion: It is cost effective and a simple eye protective device and ear protective device should be applied to patients according to their preference and characteristics in the clinical setting.

포비유증후군을 호소하는 남성 한방 치험 1례 (A case of a patient with Foville syndrome treated with Traditional Korean Medicine)

  • ■ Objectives The purpose of this case report is to show the effect of Traditional Korean Medicine (TKM) on a patient with horizontal eye movement disorder due to Foville syndrome. ■ Methods A patient with horizontal eye movement disorder as Foville syndrome was treated with herbal medication, acupuncture, electro-acupuncture and transcutaneous electrical nerve stimulation. Then we evaluated the improvement with mobility impairment, length of movement and House-Brackmann scale before and after treatment ■ Results Improvements of mobility impairment, distance at left gaze and House-Brackmann scale were observed after the TKM treatment. ■ Conclusion This case report shows the effect of TKM treatment on horizontal eye movement disorder due to Foville syndrome.

상안검성형술 후 발생한 후천성 유사 브라운증후군과 안검하수의 치험례 (Acquired Simulated Brown Syndrome Combined with Blepharoptosis after Upper Blepharoplasty)

  • Purpose: Brown syndrome is motility disorder of the eyeball which shows limited elevation in adduction and occurs very rarely after eye surgery. The authors have experienced a case of strabismus-like Brown syndrome combined with blepharoptosis and report this case with the review of literatures. Methods: A 28-year-old female suffered from hypotropia in the primary gaze and severe blepharoptosis with diplopia of the right eye after upper blepharoplasty. Rotation showed an inability to elevate the adducted right eye. She underwent extraocular muscle surgery about the 7 mm tucking of the right superior rectus muscle and 6 mm recession of right inferior rectus muscle. Intraoperatively, injury of the superior rectus muscle and foreign body were observed. Seven months after the extraocular surgery, the patient underwent frontalis muscle transfer on the right upper eyelid for the correction of blepharoptosis. Results: Postoperatively, the patient was orthophoric in the primary gaze, and she had improvements in the correction of blepharoptosis and eyeball movement. Conclusion: Repeated eyelid surgeries increase the risk of ocular motility disorder. Careful approach is essential for the proper treatment and successful outcome in secondary surgeries.

마비성 사시환자 1례 (One Case of Paralytic Strabismus)

  • Strabismus is a misalignment of eye, and which devided paralytic and non-paralytic strabismus. Paralytic strabismus is generally a neurological problem, especially 3rd, 4th or 6th nerve. The abducent nerve was most common affected. Paralytic strabismus are treated based on the theory of Oriental medicine with herbal medicine bal-san(發散) and gue-pung(祛風) effects and acupuncture around the eyes and etc. Authors experienced one case of the strabismus of abducent nerve palsy, 35-year-old male, showed disorder abduction of Lt. eye, diplopia and dizziness. Disorder abduction of Lt. eye, diplopia and dizziness and cervical pain improved under the treatment of acupuncture and herbal medicine Thus further clinical study and treatment principle should be made to raise the recovery rate for paralytic strabismus and it's effectiveness of Oriental medical treatment should be informed.

Diagnosing Reading Disorders based on Eye Movements during Natural Reading

  • Diagnosing reading disorders involves complex procedures to evaluate complex cognitive processes. For an accurate diagnosis, a series of tests and evaluations by human experts are required. In this study, we propose a quantitative tool to diagnose reading disorders based on natural reading behaviors using minimal human input. The eye movements of the third- and fourth-grade students were recorded while they read a text at their own pace. Seven machine learning models were used to evaluate the gaze patterns of the words in the presented text and classify the students as normal or having a reading disorder. The accuracy of the machine learning-based diagnosis was measured using the diagnosis by human experts as the ground truth. The highest accuracy of 0.8 was achieved by the support vector machine and random forest classifiers. This result demonstrated that machine learning-based automated diagnosis could substitute for the traditional diagnosis of reading disorders and enable large-scale screening for students at an early age.

A Novel Scheme for detection of Parkinson’s disorder from Hand-eye Co-ordination behavior and DaTscan Images

  • With millions of people across the globe suffering from Parkinson's disease (PD), an objective, confirmatory test for the same is yet to be developed. This research aims to develop a system which can assist the doctor in objectively saying whether the patient is normal or under risk of PD. The proposed work combines the eye-hand co-ordination behaviour with the DaTscan images in order to determine the risk of this disorder. Initially, eye-hand coordination level of the patient is assessed through a hardware module. Then, the DaTscan image is analysed and used to extract certain geometrical parameters which shall indicate the presence of PD. These parameters are then finally fed into a Multi-Layer Perceptron Neural Network using Levenberg-Marquardt (LM) Back propagation training algorithm. Experimental results indicate that the proposed system exhibits an accuracy of around 93%.

한의원에 내원한 틱장애 환자 292례 증례분석 (Clinical Analysis of 292 Cases of Tic Disorder in Oriental Medicine Clinic)

  • Objectives : In this study, patients with tic disorders who visited an Oriental medicine clinic were examined for their demographic characteristics, characteristics of symptoms, relation to Attention-deficit Hyperactivity Disorder(ADHD) and peculiarity according to various variables such as motor and vocal tics. Methods : After surveying 292 patients who visited an Oriental medicine clinic with tic symptoms as main complaints for 17 months, SAS 9.1, a statistical program was used for statistical analysis. Results : 1. The BMI of male tic patients was significantly higher than female ones and it was similar to or higher than the normal group. 2. Patients who are eldest children were 1.7 times higher than those who are not eldest ones. 3. The most usual case of motor tics was the eye blink and the most one of vocal tics was a dry cough. 4. There was no significant difference between male and female patients for all symptoms of motor and vocal tics, but male patients had significantly more obsessions related to tics than female ones. 5. There was no significant difference in the age of initial occurrence of Transient tic disorder(TTD), Chronic tic disoder(CTD) and Tourette's disorder(TD). 6. For the general disorder of a tic and Conners' ADHD rating scale, there was no significance in TTD, CTD and TD. 7. 66% out of the total subjects of 197 cases were found to score more than 65 points in more than 1 items among 8 items such as the time, hearing, wrong alarm, mean response time and standard deviation in the response time, etc. of the ADHD diagnosis system(ADS). 8. The eye blink among motor tics was shown mainly by patients under 10 years old and the frown, movement of the head, shrug and movement of the arms were shown mainly by 11-19 years old patients. Conclusions : For the number, frequency, seriousness and inconvenience in life of tics, TD showed a significantly higher result than TTD and CTD.

베네딕트 증후군 환자의 동안신경마비 치험 1례 (A Case of the Oculomotor Nerve Palsy in Benedikt's Syndrome Patient)

  • The purpose of this case study is to present a case of oculolmotor nerve palsy due to midbrain infarction improved by acupuncture and herb medicine. Oculomotor nerve palsy is a disorder associated with dysfunction in the third cranial nerve, which causes eye movement disorder, diplopia and ptosis. The patient, who was diagnosed with Benedikt's Syndrome(Red Nucleus Syndrome), was given oriental medical treatment. Benetikt's Syndrome has the various symptoms of weakness on one side(contralateral) and eye movement disorder, ptosis, diplopia(ipsilateral) etc., but research on Benetikt's Syndrome or midbrain infarction is scant. Results of this study suggest a role for conservative therapy with herb medicine and acupuncture to treat oculomotor nerve palsy(eye movement disorder, ptosis, diplopia, etc., ipsilateral) and right motor weakness(contralateral) due to midbrain infarction. Further research into oriental medical treatment for such disorders will be forthcoming.
