경피적 확장 기관 절개술의 중대 합병증 (Severe Complication of Percutaneous Dilatational Tracheostomy)

  • Percutaneous dilatational tracheostomy (PDT) has become an increasingly popular method of establishing an airway for patients in need of chronic ventilator assistance. We report a rare case of a 42-year-old female who developed extensive subcutaneous emphysema, bilateral pneumothoraces, pneumomediastinum, and pneumoperitoneum after percutaneous dilatational tracheostomy. The patient suffered from amyotrophic lateral sclerosis, and underwent PDT after a period of mechanical ventilation. During PDT, tracheostomy tube was inserted into the paratracheal space. Follow-up chest radiography and computed tomography of chest and abdomen revealed extensive subcutaneous emphysema, bilateral pneumothoraces, pneumomediastinum, and pneumoperitoneum. The patient was treated successfully with insertion of the thoracostomy tube and conservative care.

Structural behaviour under wind loading of a 90 m steel chimney

  • This paper presents results from an investigation of the structural behaviour of a very slender 90 m high steel chimney erected at V$\ddot{a}$xj$\ddot{o}$ in southern Sweden in 1995. The chimney is equipped with a mechanical friction-type damper at the top. Due to a mistake during erection and installation of the chimney the transport fixings of the damper were not released properly and the chimney developed extensive oscillations in the very first period of service. This caused a great number of fatigue cracks to occur within a few months of service. After the functioning of the damper had been restored and the fatigue cracks were repaired an extensive program was initiated in 1996 to monitor the structural behaviour of the chimney under wind loading. In the investigation data were collected for more than six years of continuous measurements and regular observations of the chimney. The data obtained have some general relevance with respect to wind data, behaviour of a slender structure under wind loading, and the effect of a mechanical damper. Also some theoretical studies were performed as part of the investigation of the chimney.

  • The present treatment of respiratory failure, using cuffed endotracheal and tracheostomy tube has produced, apparently with increasing frequency, three lesions which have serious ceminical manifestations such as tracheal stenosis, tracheomalasia, and localized tracheal erosion. Extensive resection and reconstruction of the trachea must be necessary because conservative treatment has generally failed in the fully developed stenotic lesion. of the mediastinal trachea following extensive resection is best accomplished by direct anastomosis of the patient`s own tracheobronchial tissue. Any replacement of the mediastinal trachea must be air tight and laterally rigid, and must heal dependably. A variety of materials has been used for substitution following circumferential excision of tracheal segments within the mediastinum. These attempts have often failed because of early leak or late stenosis. We have successfully performed circumferential resection and primary end-to-end anastomosis of the trachea for 4 cases of post-intubation tracheal stenosis located a few centimeter below the tracheostomy stoma in the period of 3 years between 1974 and 1976. The lesion in one patient was found in the upper trachea which was approached anteriorly through a cervicomediastinal incision with division of the upper sternum. Other three located in the lower half of the trachea were operated through a high transthoracic incision with appropriate hilar mobilization in addition to cervical flexion for the development of the cervical trachea into the mediastinum. There were no hospital death, but suture line granulations occurred in two patients were managed by bronchoscopic removal of granulations without difficulties.

PLGA Microspheres in Hyaluronic Acid Gel as a Potential Bulking Agent for Urologic and Dermatologic Injection Therapies

  • In this study, we investigated whether PLGA microspheres in combination with hyaluronic acid (HA) gel have appropriate properties as a bulking agent for urologic injection therapies and whether the implantation of PLGA microspheres and HA gel induces angiogenesis in the newly formed tissues. In order to investigate whether this bulking agent is injectable, this material was injected through 24-gauge needles into the subcutaneous dorsum of the mouse. The bulking agent was easily injected without needle obstruction. Histological analyses of the hybrid tissues at 2 weeks showed that host cells at the surrounding tissues migrated into the spaces between the implanted PLGA microspheres and formed tissue-like structures. An inflammatory response to the implants was mild at 2 weeks and diminished at 8 weeks. Importantly, extensive ingrowth of blood vessels was observed in the hybrid tissues formed by the injection of PLGA microspheres and HA, whereas blood vessels rarely formed in the hybrid tissues formed by the injection of PLGA microspheres only. The implant volume was conserved for almost the entire implantation period. Histological analyses of the distant organs of the bulking agent-implanted animals, such as the lungs, liver, heart, brain, kidney, and spleen, showed no evidence of the injected microsphere migration. These results show that PLGA microspheres in combination with HA possess the appropriate characteristics for a bulking agent for urologic injection therapies and induce extensive blood vessel formation in the hybrid tissues.

Penile Reconstruction after Extensive Excision of Sclerosing Lipogranuloma: How to Make the Shape of Scrotum, Penile Shaft and Suprapubic Region with a Rectangular Radial Forearm Free Flap

  • The authors had five cases of penoplasty from more than half of the scrotum to the suprapubic region using a fasciocutaneous radial forearm free flap (RFFF) after extensive excision of sclerosing lipogranuloma. Although the harvested RFFF was a rectangular shape, the authors made the shape of scrotum, penile shaft, and suprapubic region by using well designed geometry and several quilting sutures on junction of scrotum and penis. The contour of scrotum and penis was well maintained, and there were no complications, such as scrotal contracture, penile deformity, and erectile dysfunction during the one year follow up period in all five cases. There were no recurrent lesions and no need for further surgery.

Arch-First Technique을 이용한 대동맥궁 대동맥류의 수술 - 2례 보고 - (Arch-First Technique in Aortic Arch Aneurysm - 2case report -)

  • To minimize the period of brain ischemia and the potential for neurologic damage during aortic arch replacement, we used the arch-first technique. First case was a 28-year-old female with extensive aneurysm involving ascending, arch and descending thoracic aorta. Exposure was obtained via a bilateral via a bilateral thoracotomy (clamshell incision) in the anterior 4th right and 3rd left intercostal space with oblique sternotomy. To prepare for arch perfusion, the side-arm graft(10mm) was anastomosed to the aortic graft, opposite the site of the planned anastomosis to the arch vessels. After completing the arch anastomosis under total circulatory arrest(37min) and retrograde cerebral perfusion(12min), aortic graft was clamped on either side and the arch was perfused via side-arm graft for 36min. When distal aortic anastomosis was finished, distal clamp of aortic graft was released and arch vessels were perfused via common femoral artery, and the proximal aortic anastomosis was accomplished. The patient was discharged with no event. Second case was a 48-year-old male with extensive aneurysm involving ascending, arch, and aortic regurgitaiton(grade III/IV). This case was also done using the clamshell incision. Aortic valve replacement was done by valved-conduit(Vascutek 30mm), both coronary artery anastomosis using Cabrol's procedure. Last operation procedure was the same as the 1st case.

Characteristics of tidal turbulence near the bottom at a coastal trench in Tongyoung, Korea

  • Tidal turbulence was examined using three-dimensional tidal velocity data observed at a trench offshore of Tongyoung, Korea. The kinetic energy and intensity, including the variation period of the flow velocity and direction, were used to investigate the relationships between tidal turbulence and fishing gear dynamics, including the effects of swimming fish during fishing operations. As the resultant velocity increased from 0.2 to 0.9 m/s, the kinetic energy also significantly increased, while the turbulence intensity decreased from 50 to 10%. Tidal flow in strong flow fields displayed shorter periods of between 4 and 10 s, as determined by fast Fourier transform, the global wavelet method, and peak event analysis, and the periods were compared with the period of response to swimming fish and to oscillation of fishing gear. As mean velocity increased, velocity amplitude also increased from 0.1 to 0.6 m/s, and its directional amplitude changed markedly from 20 and $90^{\circ}$. Our study suggests that tidal turbulence can influence fish behavior or fishing gear geometry during fishing operations, although our analysis considered only a limited area. In future work, observations should be carried out over a more extensive depth and area.

  • Remote sensing techniques using satellites or the scanning weather radars depend mostly on the presence of clouds or precipitation, and leave the extensive regions of clear air unobserved. But wind profilers provide the most direct measurements of mesoscale vertical air motion in the troposphere, even in the context of heavy precipitation. In this paper, the precipitation events during the Changma period was classified into 4 precipitation types - stratiform, mixed stratiform/ convective, deep convective, and shallow convective. The parameters for the classification of analysis are the vertical structure of reflectivity, Doppler velocity, and spectral width measured with the wind profiler at Haenam for a three-year period (2003-2005). In addition, the synoptic fields and total amount of precipitation were analyzed using the Global Final Analyses (FNL) data and the Global Precipitation Climatology Project (GPCP) data. During the Changma period, the results show that the stratiform type was dominant under the moist-neutral atmosphere in 2003, whereas the deep convective type was under the moist unstable condition in 2004. The stratiform type was no less popular than the deep convective type among four seasons because the moist neutral layer was formed by the convergence between the upper-level jet and the low-level jet, and by the moisture transport along the western rim of the North Pacific subtropical anticyclone.

포항신항내 파랑에 의한 Downtime 분석 (Analysis of the Wave Induced Downtimes in Pohang New Harbor)

  • 포항 신항에서 파향·파고계 및 수압식 파고계를 사용하여 단주기파 및 장주기파에 대하여 약 1.5개월동안 현장관측을 실시하고 그 기간 동안의 바람 및 downtime기록도 분석하여 포항신항의 downtime의 원인에 대해 종합적으로 분석하였다. 자료 분석 결과 과거 수치실험에 의한 정온도 평가는 포항신항의 정온도를 보다 안전측으로 예상하였을 것으로 사료된다. 포항신항내 downtime의 원인은 입사파 주기에 관계없이 항외파고가 큰 경우와 파고가 전자에 비해 작더라도 주기가 10초 이상의 너울인 경우의 두 가지로 분석되었다. 한편, 포항신항내 장주기 파고는 주기 수 분 이상의 부진동이 뚜렷한 경우 항의 구석진 부분에서 20cm전후로 상당히 크나 downtime과는 큰 관계가 없는 것으로 나타났다

인터넷 기반 분산컴퓨팅환경에서 자원할당을 위한 피어 가용길이 예상 기법 (A Peer Availability Period Prediction Strategy for Resource Allocation in Internet-based Distributed Computing Environment)

  • 과학 기술이 발전함에 따라 대량의 정보를 처리하기 위해 대두된 인터넷을 기반으로 하는 분산 컴퓨팅 환경은 대규모의 독립된 자원을 공유하여 과학 연구와 같은 문제를 해결하기 위한 구축된 환경이므로, 사용자 작업을 효율적으로 할당하기 위한 스케줄링 알고리즘이 필요하다. 현재까지 여러 스케줄링 알고리즘이 연구되어 왔지만, 대부분 피어의 자율성을 고려하지 않는 문제점을 가지고 있다. 본 논문에서는 이러한 문제점을 해결하기 위하여 인터넷기반 분산 컴퓨팅 환경에서의 피어 가용길이 예상 기법을 제안하였다. 또한 인터넷기반 분산 컴퓨팅환경에서 사용되는 SRTFIT 알고리즘에 적용하여, 시뮬레이션을 통하여 제안된 기법이 단순한 예상기법보다 성능이 뛰어남을 보였다.
