The Change of Isokinetic Trunk Muscle Strength after Reduction of Body Weight (비만치료 전후 체간 근력 변화에 관한 연구)

  • Objectives : In order to investigate change of isokinetic trunk muscle strength according to decrease of body composition analysis parameter after obesity treatment. Methods : 2 obese patients have been treated with oriental medical obese treatment for 1 month. One patient got the exercise treatment, another didn't. Before and after treatment, the segmental bioelectrical impedance analysis, isokinetic trunk muscle strength test were performed. Then we analyzed the relationship of data. Results : After obesity treatment, BMI(Body Mass Index), PBF(Percentage of Body Fat), WHR(Waist Hip Ratio) were decreased in all patient and LBM(Lean Body Mass) was increased. In non-exercise patient, Ext.PT(extension Peak Torque) was decreased and Flex.PT(flexion Peak Torque) was increased. In exercise patient showed the opposite results. E/F ratio became more imbalance. Conclusions: Ext.PT was decreased in non-exercise patient but increased in exercise patient. And the trunk muscle strength became imbalance in both patients, right after the treatment. So trunk muscle exercise should be carried out and it is necessary to do long term study.

Effects of Motivational Interviewing Intervention for Patients with Total Knee Replacement (슬관절 치환술 환자 대상 동기상담 적용 운동 프로그램의 효과)

  • Purpose: The study was conducted to examine the effects of motivational interviewing intervention on exercise performance and maintenance, exercise and pain self-efficacy, functional status, and quality of life in the patients with total knee replacement. Methods: This study used a nonequivalent control group non-synchronized design. A total of 100 patients with total knee replacement (33 patients in motivational interviewing group (MG), 33 patients in exercise group (EG), 34 patients in comparison group (CG)) were participated in this study. MG and EG had 7 scheduled sessions for 4 weeks. Data were analyzed with repeated measures ANOVAs by using IBM SPSS Statistics 20. Results: MG showed better exercise maintenance, higher exercise and pain self-efficacy, higher muscle strength, and mental component of quality of life than the other groups. MG and EG showed better knee flexion, less knee extension, longer walking distance for 6 minutes, and better functioning than CG. Conclusion: The results showed that motivational interviewing intervention is an effective intervention for elderly patients with total knee replacement. Future study examining long term effects of motivation interviewing intervention is necessary.

Changes in Flexibility and Muscle Elasticity according to the Self-Stretching Method of the Hamstring Muscle (뒤넙다리근의 자가 신장운동 방법에 따른 유연성과 근탄성의 변화)

  • PURPOSE: The purpose of this study was to investigate the changes in the flexibility and elasticity of the muscles when the hamstrings were stretched in one direction and when they were stretched in three directions. METHODS: In this study the subjects were divided into two groups, namely the 'one-direction stretching exercise of the hamstring muscle in the neutral position' group (female: 14 people, male: 14 people) and the 'three-way stretching exercise' group (female: 12 people, male: 14 people) considering the positions of the three hamstring muscles. The elasticity and flexibility of the hamstring muscles were measured before and after the self-extension exercise, and the average value of two measurements was noted. To evaluate the flexibility of the hamstring muscle, a 'sit and reach' test was performed, and muscle elasticity was measured using the MyotonPRO® device. RESULTS: After the stretching exercise, flexibility increased in both groups. In terms of muscle elasticity, the body dynamic strength and resonance frequency of the stretching one-direction stretching group decreased after the stretching exercise. CONCLUSION: To alter the muscle characteristics and increase the flexibility when performing a stretching exercise, selecting and stretching only one muscle with the lowest range of motion yields effective results. The ideal technique to be employed appears to be to stretch the entire hamstring muscle in one direction from the neutral position.

Evolution of Exercise Performance and Medical Assessments in U-12 Youth Football Players (U-12 유소년 축구선수의 운동수행력 및 의학적 평가)

  • This study aimed to examine the evolution of exercise performance and medical assessments in U-12 youth football players. Participants were recruited from an youth football players (YFG, n=15; $11.2{\pm}0.9yrs$) and normal youth (CON, n=15; $11.5{\pm}0.6yrs$). Both groups were tested for exercise performance (health related physical fitness, skill related physical fitness, functional movement screen; FMS and Y-balance) and medical assessments (lower extremity alignment, range of motion; ROM and manual muscle test; MMT). YFG showed a positive effects on pacer in health related physical fitness, 50-m run and Single-leg standing with eyes closed in skill related physical fitness, active straight leg raise and rotary stability in FMS, non-dominant composite score in Y-balance, respectively compared to CON. However, YFG showed a negative effects on Q-angle of dominant and non-dominant in lower extremity alignment, dorsiflexion of non-dominant, plantarflexion of dominant and non-dominan in ROM, hip extension and abduction, knee flexion and extension of dominant, knee flexion and extension of non-dominant in MMT, respectively compared to CON. These results indicated that U-12 youth football players have shown excellent exercise performance, but medical assessments has proved negative effects.

Effects of Therapeutic Exercise on Posture, Pain and Asymmetric Muscle Activity in a Patient with Forward Head Posture: case report (치료적운동이 전방두부자세 환자의 자세, 통증 및 비대칭적 근육활성에 미치는 영향: 증례보고)

  • PURPOSE: The purpose of this case report was to determine the effect of therapeutic exercise on posture, pain, and muscle activity in two patients with forward head posture (FHP). METHODS: A-31-year-old male (patient A) and a 19-year-old women (patient B) presented with FHP, neck pain, and headache. The therapeutic exercise program consisted of cervical mobilization, deep cervical flexors strengthening, and cervical extensors stretching, for 40 min/d, 2 d/week, for 8 weeks. Neck pain (VAS), neck disability (NDI), cervical range of motion (CROM), lateral view of cervical spine X-ray (indicating the FHP), and asymmetrical neck and shoulder muscular activity ratio were measured before, after 4 weeks, and after 8 weeks of corrective exercise. RESULTS: VAS and NDI decreased in patients A and B after exercise compared to before the program. CROM increased in patients A and B at flexion, extension, side bending, and rotation after exercise compared to before the program. FHP decreased in patients A and B at distance after exercise compared to before the program. In addition, asymmetrical neck and shoulder muscles activity ratio improved in patients A and B after exercise compared to before the program. CONCLUSION: We demonstrated in a case report that therapeutic exercise increases ROM, decreases pain and disability of neck, FHP, and asymmetry muscle activity ratio in patients with FHP. These finding have clinical implications for therapeutic exercise in patients with FHP.

The Effects of 3 Dimensional Exercises and Klapp's Exercise on Scoliosis (3차원적인 운동프로그램과 Klapp's 운동프로그램이 척추측만에 미치는 영향)

  • PURPOSE: The purpose of this study was to investigate the effects of 3 dimensional exercises and Klapp's exercise on parameters of scoliosis in subjects with non-structure scoliosis. METHOD: This study was performed on 24 subjects. Twenty-four subjects were divided into two groups; 3 dimensional exercise(n=12), Klapp's exercise(n=12). Both of the group performed the exercise 3 times a week for 5 weeks. The data was analyzed by the Paired t-test for comparing before and after changes of factors in each group and the Independent t-test for comparing the between groups. RESULT: The result are as follows. There was statistically significant difference of before and after arthopometric parameters, trunk flexion, Cobb's angle in 3 dimensional exercise(p<0.05). There was statistically significant difference of before and after acromion high, trunk extension, Cobb's angle in Klapp's exercise(p<0.05). There was no statistically significant difference of between the two group in arthropometric parameters, ROM, Cobb's angle(p>0.05). CONCLUSION: As a result of this study, we found that the 3 dimensional exercise may be effective more than Klapp's exercise.

Comparison of the Immediate Effect of Ankle and Hip Joint Thera-band Exercise on the Balance Ability

  • PURPOSE: This study compared the effect of training ankle joint and hip joint thera-band exercise on balance. METHODS: The participants were divided into two groups of 11 each. Group A performed hip exercise after ankle exercise, and Group B performed ankle exercise after hip exercise. Using a green thera-band, the dorsiflexion and plantarflexion and hip flexion and hip extension were exercised repeatedly for 15 seconds three times with a five-second rest between each set. After the exercise and measurement of one area were complete, the exercise and measurement of the other area were performed at one-day intervals. The balance ability was assessed using a Tetrax and Y-balance test and repeated three times; the best values were taken. RESULTS: In the stability index (ST) of the static balance, the hip joint exercise group (HTG) during the follow-up of normal eye open (NO) revealed notable improvement over the ankle joint exercise group (ATG), and in the follow-up of the normal eye closed (NC), the ATG showed significant improvement over the HTG. In the pillow with eye closed (PC) follow-up, the ATG showed significant improvements over the HTG. At the left (Lt) and Y-balance test (YBT), the ATG showed significant improvements in the follow-up over the HTG (p <.05). CONCLUSION: In static balance, the ATG showed significant improvement in the follow-up of NC and PC over the HTG. In the dynamic balance, the Lt. dynamic balance on the non-dominant side in the ATG showed significant improvement in the follow-up over the HTG.

  • Objective : We found out improvement at low back pain and lumber strength by isotonic lumbar extension training on lumbar strength and self low back exercise stretching program. Methods : We evaluated the symptom's changes VAS(Visual Analogue Scale) and lumbar strength by Medx which could estimate and exercise lumbar muscle. Conclusions: We can decrease low back pain by strengthening lumbar muscle, in additionally when patients are In admission state we may improve lumbar muscle more than Opd. state.

Effect of Ribcage Stabilization Using a Belt on EMG Activity of the Abdominal Muscles During Double Leg Lowering in the Supine Position (벨트를 이용한 가슴우리 고정이 누운자세에서 다리내리기 동안 복부 근육들의 근활성도에 미치는 영향)

  • Purpose : The purpose of this study was to determine the effect of ribcage stabilization using a belt in the supine position during double leg lowering (DLL) by investigating the electromyographic (EMG) activities of the abdominal muscles. Methods : Twenty-two subjects with lumbar extension syndrome were recruited. EMG activity was recorded from rectus abdominalis (RA) and internal oblique abdominalis (IO), external oblique abdominalis (EO) muscles while subjects performed three double leg lowering exercises: double leg lowering (DLL), double leg lowering with abdominal draw-in maneuver (DLL-ADIM), and double leg lowering with ribcage stabilization using a belt (DLL-belt). RA, IO, and EO EMG activity were analyzed via one-way repeated-measures analysis of variance (ANOVA). Bonferroni correction was performed where significant differences were identified (p<.017, .05/3). Results : RA, IO, and EO EMG activity differed significantly among the three exercises (p<.05). The use of post hoc pair-wise comparison with Bonferroni correction showed that RA muscle activity significantly differed among the three exercises (p<.017), and IO muscle activity in the DLL exercise was significantly decreased compared to the DLL-ADIM and DLL-belt exercises (p<.017). There was no significant difference between IO muscle activity for DLL-ADIM and DLL-belt exercises (p>.017). EO muscle activity in the DLL-belt exercise was significantly increased compared to both DLL and DLL-ADIM exercises (p<.017), but there was no significant difference between EO muscle activity for DLL and DLL-ADIM exercises (p>.017). Conclusion : DLL-belt is a more effective exercise for activating the abdominal muscles than DLL-ADIM exercise. Therefore, we recommend DLL-belt exercises for strengthening the abdominal muscles.

Effect of Disease-Specific Exercise on Temporomandibular Joint Function and Neck Mobility in Temporomandibular Joint Dysfunction Associated With Ankylosing Spondylitis (강직성척추염과 관계된 측두하악관절장애에 대한 특수 운동치료의 효과)

  • The aim of the study was to evaluate the effect of a disease-specific exercise (DSE) on temporomandibular joint (TMJ) function and neck mobility in TMJ dysfunction associated with ankylosing spondylitis (AS). Ten AS patients (seven males and three females) with TMJ dysfunction were recruited for this study. The DSE included exercises to correct head and neck posture and to improve the flexibility of the neck and TMJs. The patients attended treatment three times a week for 4 weeks, averaging 1 hour each session. Assessments were performed pretreatment, posttreatment, and 6 weeks after the completion of treatment. General physical status was assessed by four clinical measures (tragus-to-wall distance, modified Schober test, lumbar side flexion, and intermalleolar distance), the Bath ankylosing spondylitis function index (BASFI), and the Bath ankylosing spondylitis disease activity index. The main outcome measures included TMJ function (craniomandibular index (CMI)), and neck mobility (flexion, extension, rotation, and lateral rotation). None of the measures of general physical status, with the exception of BASFI, were significant1y different between the pretreatment, posttreatment, and 6-week follow-up (p>.05). However, CMI and all neck movements, except for extension, significant1y improved after the treatment (p<.05). These improvements were maintained during the follow-up period. The DSE used in the present study seems to be a clinical1y useful method for managing patients with symptoms from the stomatognathic system in AS. Further studies with more subjects and longer treatment times, including the follow-up period, will be conducted to validate these findings.
