초음파영상에서 골관절염을 가진 노인의 하지 근육에 대한 사례연구 (A Case Study on Lower Extremity Muscle of Elderly with Osteoarthritis in Ultrasonic Image)

  • ■ Objectives The purpose of the this study was to investigate organization change(pennation angle) of lower extremity muscle in elderly with knee osteoarthritis through ultrasonic image. Also, we examined organizational change of muscle was by influenced after total knee replacement(TKR). ■ Methods Rectus femoris image was taken at 50% of the distance between the anterior superior iliac spine and the superior border of the patella. Vastus medialis image was taken at the most distal insertion level of vastus medialis on the medial border of the patella. measurements included the most distal insertion level of the vastus medialis on the medial border of the patella. ■ Results Change of pennation angle in rectus femoris showed decrease from 5.11° in preoperation to 4.29° in 1 week after operation. Change of pennation angle in vastus medialis showed decrease from 6.94° in preoperation to 5.76° in 1 week after operation. ■ Conclusion We consider decrease of pennation angle after total knee replacement in this study was casued by organizational change of muscle and this pennation angle was influenced force created by muscle. Therefore, we thinks that this pennation angle through ultrasonic image will be available to plane individual muscle exercise program and to confirm muscle mass by muscle strength training.

Development of an Evidence-based Nutritional Intervention Protocol for Adolescent Athletes

  • [Purpose] Effective nutritional intervention can help reduce the risk of nutritional problems and improve athletic performance in adolescents. The five A's (assess, advise, agree, assist, and arrange) model is widely used as the theoretical framework for advice on nutrition, smoking, drinking, and physical activity and it recommends that practitioners in primary health care promote behavior change to facilitate positive outcomes. This model has also been useful in understanding the underlying processes of behavior change. This study aimed to develop both a novel evidence-based nutritional intervention protocol, rooted in sound nutritional theory, and a customizable nutritional intervention program to support sustainable healthy eating, enhance nutrient intake, and improve athletic performance in adolescent athletes. [Methods] In this study, we adapted the 5 A's behavioral change model and motivational interview to develop a theoretical framework to help adolescent athletes change their behavior and achieve their goals. [Results] During each step of the 5 A's protocol, a customized nutritional intervention protocol was developed by nutrition experts for each of adolescent athletes. Each plan was developed to improve the eating habits of adolescent athletes through group education and counseling. All nutritional counseling sessions were designed to enable participants to apply nutritional knowledge and practical action plans to their training and competition conditions to enable each of them to achieve individual athletic goals and facilitate self-management. [Conclusion] A theoretical and evidence-based nutritional intervention protocol was developed to identify and address obstacles to healthy dietary habits in adolescent athletes. This could be used as the basis for further studies aimed at improving nutrient intake and athletic performance in adolescent athletes.

뇌졸중환자의 신체활동수준에 따른 운동기능과 인지기능 (Motor and cognitive function according to level of physical activity in stroke patients)

  • Background: In the rehabilitation of stroke patients, regular physical activity is very important not only as a treatment for maximal functional recovery but also as a strategy to prevent the recurrence of stroke. The purpose of this study was to objectively measure the amount of physical activity in people with stroke, and to examine the differences in motor and cognitive function according to a level of physical activity. Design: A cross-sectional study. Methods: Physical activity (GENEActiv), motor function (Fugl-Meyer Assessment), cognitive function (Montreal Cognitive Assessment-Korean version), and the Korean version of Modified Barthel Index were evaluated in adult stroke patients with hemiplegia. Results: There was no statistically significant difference in the level of physical activity according to the motor and cognitive function. There was no statistically significant difference in motor and cognitive function according to the level of physical activity, but there was a statistically significant difference in the MBI (p<.01). Conclusion: As a result of the difference in the MBI according to the level of physical activity, it was found that the more moderate to vigorous physical activities are performed, the higher the independence in daily living. These results can be interpreted as that the more often you participate in physical activities such as physical therapy (gait training), the better your independence in ADL. Since regular physical activity participation of adult stroke patients can improve daily living performance, it is considered necessary to participate in physical activities such as continuous physical therapy.

Effects of Y-Balance Test Difference of the Ankle Dorsi-flexion Range of Motion in K3 Soccer Players

  • 본 연구의 목적은 비 체중지지 자세 상태에서 발목관절 발등굽힘 관절가동범위에 따라 동적 균형 능력을 평가하는 Y-Balance Test와의 관계를 규명하고자 하였다. 본 연구에는 10년 이상의 축구경력을 가진 K3리그에 정기적으로 참여하는 남자축구선수 27명이 참여하였다. 발목관절의 발등굽힘과 동적균형 능력과의 관계를 검증하기 위해 The Pearson Rank Correlation Coefficient를 사용하였다. 본 연구의 결과 HADR군에서 PLRD(P<.05)와 CS(P<.01)에서 유의한 차이가 나타났으며, 발목관절 발등굽힘의 관절가동범위와 PMRD, PLRD 및 CS에서만 중증도의 유의한 상관관계가 나타났다. 따라서 발목관절 발등굽힘의 감소는 균형 능력 감소와 관련이 있으며, 축구선수의 손상을 예방하기 위해서는 정기적인 발목관절 관절가동범위의 평가뿐만 아니라 무릎과 엉덩관절의 근력 및 고유수용감각을 향상시키기 위한 트레이닝이 필요할 것으로 생각된다.

심혈관질환 위험인자를 가진 중소규모 사업장 남성 근로자의 모바일헬스 앱 활용 건강파트너십 프로그램의 효과: 무작위 실험연구 (Effects of a Health Partnership Program Using Mobile Health Application for Male Workers with Cardiovascular Risk Factors in Small and Medium Enterprises: A Randomized Controlled Trial)

  • Purpose: This study aimed to apply a health partnership program using commercially available mobile health apps to improve cardiovascular risk factors in male employees and verify its effectiveness. Methods: Using a randomized control group pretest-posttest design, male employees with cardiovascular risk factors from five small and medium-sized workplaces were randomly assigned to an experimental group (n = 32) and a control group (n = 31). The experimental group was encouraged to use three mobile health apps for 12 weeks to acquire the necessary cardiovascular disease-related information and practice strengthening training, walking, and diet management appropriate to their level. They also received feedback on their weekly activities and motivational text messages from health partners. Hypotheses were tested using the SPSS WIN 22.0. Results: The experimental group showed a significant difference compared to the control group in terms of their perception of mobile health app (p < .05), self-efficacy for exercise and diet, self-management partnership, and cardiovascular disease prevention health behavior (p < .001). In particular, there were significant decreases in the body mass index, ratio, serum fasting blood sugar, total cholesterol, and triglyceride in the experimental group (p < .001); however, there was no significant difference in high-density lipoprotein-cholesterol. Conclusion: Intervention using mobile apps based on partnership with health managers is effective in improving the objective cardiovascular risk index in male employees; therefore, such intervention should be continuously used as a useful lifestyle modification strategy in the workplace.

스쿼트 동작 시 정적 스트레칭과 근막이완기법이 하지 관절의 운동학적 요인에 미치는 영향 (Effect of Static Stretching and Myofascial Release Techniques on Kinematic Factors of Lower Extremity Joints during Squat )

  • Objective: The purpose of this study was to analyze the differences in kinematic factors according to stretching treatment, myofascial release treatment, and static stretching treatment conditions during squat. Method: Twelve males with resistance training experience participated in this study. Participants performed squats without treatment (Pre-Test), and performed squats after treatment with the myofascial release technique (MRT) and static stretching (SS) on different days (post-test). Squat movements were captured using eight motion capture cameras (sampling rate: 250 Hz), and the peak joint angles of the ankle, knee, hip, and pelvis were calculated for each direction. One-way repeated ANOVA and Bonferroni post hoc analyses using SPSS 27 (IBM Corp. Armonk NY, USA) were used to compare the peak joint angle of the lower extremity joints and pelvis among the normal condition (squat without treatment), MRT condition (squat after MRT treatment) and SS condition (squat after static stretching). The statistical significance level was set at .05. Results: It was observed that the maximum ankle joint flexion angle during squats was statistically reduced under conditions of myofascial release and static stretching (p<.05), in comparison to the scenario where no stretching was performed. Furthermore, static stretching was found to enhance the maximum hip flexion angle during squat (p<.05), whereas the myofascial release stretching technique resulted in the minimal posterior pelvic tilt angle (p<.05). Conclusion: Employing myofascial release stretching as a preparatory exercise proved to be more efficacious in maintaining body stability throughout the execution of high-intensity squat movements by effectively managing the posterior tilt of the pelvis, as opposed to foregoing stretching or engaging in static stretching.

변형된 엉치엉덩관절 테이핑의 요통 효과 사례보고 (The effect of Modified Sacroiliac Joint Taping on Back pain_A case report)

  • 본 연구는 엉치엉덩관절의 불안정성에 초점을 맞춘 테이핑 방법이 요통에 도움이 될 수 있는 잠재 중재 방법인지에 대해 고려해 보고자 한 사례연구이다. 해당 사례는 58세 과거 디스크탈출증 및 협착증으로 인해 허리 수술 병력이 있는 남성이 지속적인 통증 감소를 위해 참여한 테이핑 교육 세션에서 나타난 주목할 만한 결과를 사례로 정리하였다. 중재 방법으로 테잎을 엉치뼈 2~4사이에 가시결절로부터 양쪽으로 80%< 장력으로 늘려 엉치엉덩관절을 지나게 붙이고 이후 끝 쪽은 양쪽 약 45° 방향으로 올려 중간볼기근(gluteus medius m.) 쪽으로 향하게 붙였다. 그 다음 엉덩뼈능선(iliac crest)로부터 척추기립근을 따라 10번째 갈비뼈 수준까지 이르는 양쪽 테이핑 방법이 적용되었다. 해당 중재를 통해 VAS와 ODI 두 검사도구로부터 VAS는 5에서 0, 장애지수는 13에서 0으로 각각 기록되는 긍정적 사례 결과를 관찰할 수 있었다.

Effects of Backward Walking Training with a Weighted Bag Carried on the Front on Craniocervical Alignment and Gait Parameters in Young Adults with Forward Head Posture: A case series

  • Purpose : This case study aimed to investigate the effects of backward walking exercises with a front-loaded bag on craniovertebral angle (CVA), craniorotational angle (CRA), and gait variables in subjects with forward head posture (FHP). Methods : Two individuals in their twenties with FHP performed backward walking exercises on a treadmill while carrying a front-loaded bag with a load equivalent to 20 % of their body weight, for 30 minutes per day, three times a week, over two weeks. CVA and CRA were measured before and after the intervention using side view photographs taken from 1.5 meters away. CVA was calculated by marking C7, the tragus of the ear, and the outer canthus of the eye, and CRA was determined using the same landmarks. Image J software was used for angle analysis, with measurements taken three times and averaged. Gait variables such as step length and cadence were recorded using a step analysis treadmill and analyzed with the software included with the equipment, with measurements taken at baseline and after the two-week intervention. Results : Both participants demonstrated notable improvements in the CVA, indicating enhanced head alignment relative to the cervical spine. There was also a marked decrease in the CRA, suggesting a reduction in rotational misalignment. Although differences were observed in gait variables, such as step length and cadence, these changes were not consistent across measurements. The results suggest that backward walking exercises with a load carried in front can positively influence postural adjustments by aligning the cervical spine in individuals with FHP. Conclusion : The findings of this case study indicate that backward walking exercises with a front-loaded bag can effectively improve cervical spine alignment in individuals with FHP. Differences were observed in gait variables, such as step length and cadence, but these changes were not consistent across measurements. Future studies should explore these effects more comprehensively and consider optimizing the exercise protocol for better therapeutic outcomes.

  • 장기간(長期間)의 체력단련(體力鍛鍊)이 폐기능(肺機能)에 미치는 영향(影響)을 알아보고자 운동선수군(運動選手群) 24명(名)과 비운동선수군(非運動選手群) 12(名)에 대(對)하여 실시(實施)한 본(本) 연구(硏究)의 결과(結果)를 요약(要約)하면 다음과 같다. 호흡수(呼吸數), 일회호흡량(一回呼吸量) 폐활량(肺活量)은 실시군(實施群)과 비실시군(非實施群) 사이에 유의(有意)한 차이(差異)가 없었으나, 최대환기능(最大換氣能)은 선수군(選手群)이 $148.1{\pm}3.01\;L/min$, 비선수군(非選手群)이 $1118.3{\pm}9.1\;L/min$ 로서 선수군(選手群)에서 비선수군(非選手群)에 비(比)해 유의하게(p<0.01) 높았다. 초시폐활량(秒時肺活量)은 선수군(選手群)이 $3.310{\pm}0.070\;L$, 비선수군(非選手群)이 $2.279{\pm}0.104\;L$였고, $FEV_1%$는 선수군(選手群)이 $83.63{\pm}1.29%$, 비선수군(非選手群)이 $75.33{\pm}1.75%$로서 둘 다 선수군(選手群)에서 비선수군(非選手群)에 비(比)해 유의하게(p<0.01)높았다. $FEF_{\;0.2{\sim}1.2}L$는 선수군(選手群)이 $297.7{\pm}13.5\;L/min, 비선수군(非選手群)이 $222.7{\pm}15.0\;L/min$였고, $FEF_{\;25{\sim}75}%$는 선수군(選手群)이 $3.543{\pm}0.109\;L/sec$, 비선수군(非選手群)이 $2.719{\pm}0.142\;L/sec$로서 둘다 선수군(選手群)에서 비선수군(非選手群)에 비(比)해 유의하게(p<0.01)높았다. 이상(以上)의 결과(結果)를 종합(綜合)하면 선수군(選手群)과 비선수군(非選手群) 사이에 폐용적(肺容積)은 별차이(別差異)가 없으나, 최대환기능(最大換氣能), 초시폐활량(秒時肺活量), $FEV_1%$, $FEF_{\;0.2{\sim}1.2}L$, $FEF_{\;25{\sim}75}%$등(等)은 선수군(選手群)이 비선수군(非選手群)에 비(比)해 유의하게(p<0.01) 높은 측정치(測定値)를 나타내었으며, 이것은 선수군(選手群)에서 비선수군(非選手群)에 비(比)해 호흡근(呼吸筋)의 힘이 더 강(强)하거나, 폐(肺) 및 흉곽(胸廓)의 용압률(容壓率)이 더 크기 때문인 것으로 사료(思料)된다.

베트남의 해양기름유출 현황과 국가대응역량 증강 방안 - 통계자료와 유출유 방제시스템에 대한 베트남과 한국 간의 비교를 통하여 - (The Present State of Marine Oil Spills and the Enhancement Plans of National Oil Spill Response Capability in Vietnam - Through the Comparison of Statistics and OSR System between Vietnam and Republic of Korea -)

  • 베트남은 3,444 km가 넘는 해안선, 수천 개의 섬 그리고 길이 4260 km 이상인 2,360개의 강과 수로가 있는 해양국이다. 선박에 의한 유류수송의 빈도와 수송량이 증가하면서 기름유출사고의 가능성이 과거 어느 때보다 높아지고 있다. 해상에서의 연료유 및 화물유의 유출은 해양생태계, 연안자원 및 인간건강은 물론 사회 경제에 대하여 광범위하고 장기적인 영향을 미친다. 본 연구는 20년 동안의 기름유출사고 건수와 유출량과 같은 베트남의 해양기름유출 현황 그리고 기름유출대응(OSR)에 대한 국가체제 등과 같은 국가대응체계에 관한 전반을 보여줄 뿐만 아니라, 특히 베트남과 한국 사이의 국가기름유출대응체제를 비교함으로써 기름유출사고에 대응하여 베트남의 국가역량을 강화하기 위한 권고안을 제시하고자 한다. 그 결과, 베트남의 해양기름유출사고 건수와 유출량은 한국의 하락 추세와는 대조적으로 상승 추세를 보였다. 이는 베트남의 연안 해역에서 실제적 기름유출 가능성이 높다는 것을 의미한다. 따라서 베트남의 국가기름유출대응역량을 강화하기 위한 3가지 주요 권고안을 다음과 같이 제안한다. 즉 (1) OPRC협약의 수락 이행 그리고 유류오염으로 인한 피해 및 손실을 보상하는 국가기금의 조성과 같은 기름유출사고에 대비하고 대응하기 위한 법률제도를 포함한 국가기름유출대응 시스템을 강화하기 위한 방안의 개발; (2) 일관된 보고, 경보 및 모니터링 시스템의 강화; (3) 표준 교육과정 내용으로 구성된 교육 및 훈련 프로그램의 개발을 제안하고자 한다.
