요추 질환에 대한 신기공 오금희의 동작연구 -10식, 15식, 18식을 중심으로- (Study on the Movement of New Qi-gong "WuQinXi" Exercise for Lumbar Spinal Disease : Based on 10 Mode, 15 Mode, 18 Mode)

  • Hua-Tuo(145-208) created five "WuQinXi" exercise by imitating the movements of a tiger, a deer, a bear, a monkey and a bird. The "WuQinXi" exercise, one of the medical Qi-gongs, is an exercise maximizing human's self healing power and has been effective significantly at several modern researches. There are many exercise therapies in western medcine, such as Willamss flexion exercise, Mckenzie's extension exercise, vertebral stabilization exercise and so on. However, there isn't a special exercise therapy which can be applied for medical practice in oriental medicine. So We selected some motions from "WuQinXi" exercise, which are suitable for lumbar spinal disease, and analyzed them. After that, We assorted them by kinds of lumbar spinal disease. First, We selected 22 motions which are related with lumbar movements from 3 type "WuQinXi" exercises ; 10 mode, 15 mode, 18 mode. And then, We classified them according to lumbar movements as flexion, extention, lateral bending and rotation, and also functions as stabilization and rubbing. Next, with these classifications, We assorted those motions by kinds of lumbar spinal disease as HIVD(herniation of intervertebral disc), spinal stenosis, spondylolysis and spondylolisthesis, facet joint syndrome, compression fracture and spondylosis. We expect that trying "WuQinXi" exercise at clinic in this way, the particular exercise therapy of oriental medicine, "WuQinXi" exercise will become more popular. And Oriental medical doctors will be able to teach patients "WuQinXi" exercise's motions easily at clinic, depending on kinds of lumbar spinal disease each patient suffers from. We plan to study more about 20 mode, 30 mode, 40 mode and the effect of "WuQinXi" exercise by comparing patients who do the "WuQinXi" exercise with the patients who do the western medical exercise therapy.

  • The "WuQinXi" exercise, one of the medical Qi-gongs, is an exercise maximizing human's self healing power and has been confirmed to be effective significantly at several modern researches. Comparison of Number of Repetitions and Repetition Rate in 5 Sets of 65%1RM Bench Press and Biceps Curl Exercise

  • The purpose of this study is to investigate the changes in the number of repetitions and the repetition rate according to the exercise mode when performing 65%1RM resistance exercise at the 1-minute rest interval and the 3-minute rest interval. Sixteen healthy male subjects were treated with Bench press and Biceps curl of 65%1RM intensity at 1 and 3 minute rest intervals. The number of repetitions for each set of 1 minute rest interval showed a significant decrease from 1set to 5set in bench press. biceps curl showed a significant decrease from 1set to 4set. The repetition rate according to the exercise mode with a 1-minute rest interval showed a significant difference from 2sets to 4sets. In the repetition rate for each set, bench press showed a significant decrease from 1set to 5set. biceps curl showed a significant decrease from set 1 to set 4. The number of repetitions according to the exercise mode with a 3-minute rest interval showed a significant difference from 2sets to 5sets. In the number of repetitions for each set, bench press showed a significant decrease from 1set to 5set. biceps curl showed a significant decrease from 1set to 4set. The repetition rate according to the exercise mode with a 3-minute rest interval showed a significant difference from 2sets to 5sets. In summary, the decrease in the number of repetitions according to the set progression in the resistance exercise of the endurance depends on the exercise mode, and the increase of the rest interval or the decrease of the weight-intensity should be considered when aiming for more exercise.

Dual Mode Feedback-Controlled Cycling System for Upper Limb Rehabilitation of Children with Cerebral Palsy

  • Background/Objectives: This paper proposes a dual mode feedback-controlled cycling system for children with spastic cerebral palsy to rehabilitate upper extremities. Repetitive upper limb exercise in this therapy aims to both reduce and analyze the abnormal torque patterns of arm movements in three- dimensional space. Methods/Statistical analysis: We designed an exercycle robot which consists of a BLDC motor, a torque sensor, a bevel gear and bearings. Mechanical structures are customized for children of age between 7~13 years old and induces reaching and pulling task in a symmetric circulation. The shafts and external frames were designed and printed using 3D printer. While the child performs active/passive exercise, angular position, angular velocity, and relative torque of the pedal shaft are measured and displayed in real time. Findings: Experiment was designed to observe the features of a cerebral palsy child's exercise. Two children with bilateral spastic cerebral palsy participated in the experiment and conducted an active exercise at normal speed for 3 sets, 15 seconds for each. As the pedal reached 90 degrees and 270 degrees, the subject showed minimum torque, in which the child showed difficulty in the pulling task of the cycle. The passive exercise assisted the child to maintain a relatively constant torque while visually observing the movement patterns. Using two types of exercise enabled the child to overcome the abnormal torque measured in the active data by performing the passive exercise. Thus, this system has advantage not only in allowing the child to perform the difficult task, which may contribute in improving the muscle strength and endurance and reducing the spasticity but also provide customizable system according to the child's motion characteristic. Improvements/Applications: Further study is needed to observe how passive exercise influences the movement characteristics of an active motion and how customized experiment settings can optimize the effect of pediatric rehabilitation for spastic cerebral palsy.

심근경색후 물리치료 (Physical Therapy for Post-Myocardial Infarction)

  • The purpose of this paper is to provide an overview of the clinical physical therapy program used at the University of Yonsei Rehabilitation Hospital, for the practicing university trained physical therapists who may be unfamiliar with patients who have suffered a myocardial infarction. The four primary phases of the cardiac physical therapy graded exercise program are: 1) coronary care unit program (phase I), 2) general ward program (phase II), 3) convalescence program (phase III), 4) maintenance program (phase IV). The exercise prescription defines the exercise intensity, duration, frequency, and mode of exercise a after pre- discharge low level graded exercise test(LL-GXT) or symptom limited maximum graded exercise test. A typical exercise routine consists of preparation warm-up exercise, therapeutic exercise, cool-down exercise. Physical therapy is involved in the acute care and rehabilitation of the patient after a myocardial infarction. Therefore, the physical therapist must throughly comprehened the cardiac anatomy, cycle, performance, conduction system, pathogenesis, risk factors, and exercise benefits.

The Effects of Stair Climbing Using Wearable Robot Bot Fit's Resistance

  • Objective: The purpose of this study is to confirm the exercise effect when combining wearable exercise assist robot, Bot fit's resist mode (Samsung Electronics) and stair climbing. Design: Cross-section study Methods: Targeting 53 adults and seniors, foot pressure and muscle activity were measured when climbing 3-story stairs using foot pressure measurement equipment (W-insole Science System) and surface muscle activity measurement equipment (sEMG; FreeEMG, BTS Bioengineering, Italy) using Bot Fit's resist mode. All subjects were measured without wearing Bot Fit, and the data between the two conditions were compared and analyzed. Results: The front area(p<0.01) and middle area(p<0.05) foot pressures of adults significantly increased when wearing the Bot fit. Frontal area foot pressure significantly increased in elderly people with knee arthritis and obesity(p<0.05). The gastrocnemius activity in all subjects significantly decreased after wearing Bot Fit(p<0.01). In elderly people with knee arthritis, the muscle activity of the rectus femoris was significantly reduced(p<0.05)., and in obese elderly people, the muscle activity of the gastrocnemius muscle was significantly reduced(p<0.05). Conclusions: Based on the results of this study, it is possible to induce correct stair climbing posture when climbing stairs using Bot fit resistance mode. In particular, it is expected to be an effective exercise for strengthening muscle endurance by increasing the activity of the rectus femoris muscle.

Comprehensive Relevance of AMPK in Adaptive Responses of Physical Exercise, Skeletal Muscle and Neuromuscular Disorders

  • PURPOSE: This study was conducted to understand the adaptive responses of different modes of physical exercises utilizing skeletal muscle and the comprehensive relevance of AMPK signaling that can be activated by physical exercise as a potential molecular target in human health problems such as neuromuscular disorders (NMDs). METHODS: Most of the contents in this review article are based on recent publications concerning the main topics of interest. The reference literatures cited were obtained by basic searches of overseas academic databases such as PubMed and ScienceDirect using EndNote X7.8. RESULTS: The phenotypic adaptive responses of skeletal muscle during endurance- and resistance-based exercise training (ET and RT respectively) appear to be distinct. To explain the adaptive responses in each single mode of exercises (ET, RT) along with combined exercise training (CT), AMPK signaling is proposed as an important molecular link among those differential modes of exercise and a promising molecular target of NMDs. CONCLUSION: Based on the available evidence, intracellular AMPK signaling activated by diverse stimuli including physical exercise can be a potential and promising therapeutic target for the prevention, amelioration or cure of various human health problems including NMDs and may also be beneficial for physical rehabilitation and emergency situations that may elicit acute metabolic stresses.

누운 자세에서의 자전거 운동 시 하지 근활성도 분석 (Analysis of Lower-Limbs Muscle Activity during Cycle Exercise in Spine Position)

  • 본 연구에서는 입원 또는 재활 초기단계에 환자가 이동하지 않고 침대에 누운 상태에서 자전거 운동이 가능한 하지 재활 훈련 시스템을 개발하였으며, 정상 성인을 대상으로 침대에 누운 자세에서 다양한 조건에서의 자전거 운동 시의 하지 근 활성도를 비교 분석하고자 하였다. 피험자는 근 골격계나 신경근육계 병력이 없는 20대~60대 사이의 건강한 성인 남녀 42명을 대상으로 하였으며, 운동 모드, 부하, 속도에 따른 자전거 운동 시 오른쪽 하지의 대퇴직근, 대퇴이두근, 전경골근, 내측 비복근, 가자미근의 근 활성도를 측정하였다. 그 결과, 부하를 1과 5단계로 제공하였을 때보다 10단계로 제공하였을 때 내측 비복근의 근활성이 높아짐을 보였다. 또한, 속도가 증가할수록 내측 비복근을 제외한 모든 근육에서 근활성도가 감소하는 결과를 보였다. 즉, 운동 조건에 따라 크게 활성화되는 근육들이 각기 달랐으며, 이러한 결과는 침상에 누워 재활훈련이 가능한 자전거 시스템 개발을 위한 기초 자료로써 활용될 것이다.

심장재활에 참여한 급성 관상동맥증후군 환자에서 약물 반응과 운동 형태의 변화에 따른 심혈관과 운동자각도의 반응 (Cardiovascular and Perceived Exertion Response to Treadmill Running and Cycle Ergometer Exercise in Responder and Nonresponder Acute Coronary Syndrome Patients)

  • 본 연구는 심장재활에 참여한 고혈압을 동반한 급성관상동맥질환자중 내과적인 약물로 조절되지 않은 고혈압 환자의 경우와 약물로 조절되는 정상 혈압 환자군에서 트레드밀 운동과 고정식 자전거 운동 수행시 강도별 혈압과 심근산소소비량, 자각지수에 대한 반응을 구명(究明)하고자 목적하였다. 이를 위해 6주간의 심장재활 프로그램에 참여한 심혈관질환 환자를 연구대상으로 약물에 대한 혈압이 조절되는 혈압약물반응군과 약물에 대한 혈압이 조절되지 혈압약물비반응군 환자를 동원하여, 트레드밀 운동부하검사를 실시하여 예비심박수를 구하였으며, 이를 근거로 중강도 운동강도(60%HRR)와 고강도 운동강도(85%HRR)의 심박수에서 트레드밀 운동과 고정식자전거 운동을 실시하였다. 혈압의 조절유무에 따라 강도별 트레드밀과 고정식자전거 운동 시에 나타난 심혈관반응의 결과는 다음과 같다. 첫째, 중강도(60%HRR)와 고강도(85%HRR)에서 운동형태(exercise mode)에 따른 혈압약물반응군과 혈압약물비반응군과의 심혈관 반응에 대한 결과 모든 운동강도에서 고정식자전거 운동이 트레드밀 운동보다 수축기혈압, 이완기혈압, 평균동맥혈압, 심근산소소비량, 운동자각도 모두 유의하게 높았다(p<0.05). 또한 고정식 자전거의 60%강도에서 나타난 심혈관계에 미치는 부담이 85%강도의 트레드밀에서 받는 부담과 비슷하였다. 둘째, 혈압약물비반응군은 모든 운동방법(중강도 트레드밀, 고강도 트레드밀, 중강도 고정식자전거, 고강도 고정식자전거)에서 혈압약물반응군보다 유의하게 높은 이완기혈압(DBP)을 나타냈고(p<0.05), 이와 함께 평균동맥혈압(MAP) 역시 모든 운동방법에서 혈압약물비반응군이 혈압약물반응군 보다 유의하게 높았다(p<0.05). 이때, 수축기 혈압에서는 혈압약물비반응군이 중강도자전거 운동방법(60%HRR-cycle exercise)에서만 정상군보다 유의하게 높게 나타났다(p<0.05). 즉, 혈압약물비반응군이 중강도자전거 운동방법에서 수축기혈압과 이완기기혈압, 평균동맥혈압이 혈압약물반응군 보다 유의하게 높았다. 셋째, 심근산소소비량(RPP)에서 혈압약물비반응군과 혈압약물반응군과 중강도자전거 운동에서 유의한 차이는 없었으며 중강도 트레드밀 운동과 중강도자전거 운동 간의 평균 차이에서 중강도자전거 운동이 혈압약물반응군보다 혈압약물비반응군에서 유의하게 높게 증가하였음에도 불구하고 운동자각도(RPE)는 중강도자전거 운동에서 혈압약물반응군과 혈압약물비반응군 간에는 차이를 보이지 않았다. 이상의 결과, 심장재활운동전에 내과적으로 혈압 조절과 상관없이 고혈압 심장질환과 정상혈압 심장질환환자 모두 같은 강도에서 트레드밀보다 고정식 자전거 운동 시에는 모두 철저한 혈압감시가 필요하였다. 특히 고혈압심혈관질환환자는 혈압의 상승폭이 보다 큰 결과를 보여 트레드밀에 적용한 운동 강도를 동일하게 고정식자전거에 적용하는 것은 적합하지 못하였다.

BIOFIT - Smart, Portable, Wearable and Wireless Digital Exercise Trainer Device with Biofeedback Capability

  • Today Human Personal Trainers are becoming very famous in this health conscious world. They teach user to achieve fitness goals in managed way. Due to their high fee and tight schedule they are unavailable to mass number of people. Another solution to this problem is to develop digital personal trainer portable instrument that may replace human personal trainers. We developed a portable digital exercise trainer device - BIOFIT that manages, monitors and records the user's physical status and workout during exercise session. It guides the user to exercise efficiently for specific fitness goal. It keeps the full exercise program i.e. exercises start date and time, duration, mode, control parameter, intensity in its memory which helps the user in managing his exercise. Exercise program can be downloaded from the internet. During exercise it continuously monitors the user's physiological parameters: heart rate, number of steps walked, and energy consumed. If these parameters do not range within prescribed target zone, the BIOFIT will alarm the user as a feedback to control exercise. The BIOFIT displays these parameters on graphic LCD. During exercise it continuously records the heart rate and number of steps walked every 10 seconds along with exercise date and time. This stored information can be used as treatment for the user by an exercise expert. Real-time ECG monitoring can be viewed wirelessly (RF Communication) on a remote PC.
