비만 치료 한약 무작위 대조 임상시험에서의 음식 섭취량과 운동량 실태분석 (Analysis of Food Intake and Physical Activity in Randomized Controlled Trials on Herbal Medicine for Treatment of Human Obesity)

  • Objectives: The objective of this study was to analyse the methods being used to control food intake and physical activity in RCTs of human obesity. Methods: A total of 21 randomized controlled trials (RCTs) were investigated. Nine of which were domestic studies from "http://oasis.kiom.re.kr" and the other of which were foreign studies from systematic reviews of RCTs on herbal medicine for treatment of human obesity. Results: According to domestic studies, "low calorie diet" were recommended in five cases of the domestic studies, "maintain current dietary habit" were recommended in two and no information on diet was two. Considering the seven cases where the information on diet was available, patients' food intake were checked at every visit in six cases. Only two cases among the six had been dropped owing to the violation of dietary habit by patients. Exercises were prohibited in two cases, "maintain current level of phisical activity" were recommended in three cases and, from the rest, no information was available. The level of physical activity were not strictly controlled by any means hence no drop out. According to foreign studies, "low calorie diet" were recommended in two cases, "very low calorie diet (less than 700 kcal/day)" in one case, "maintain current dietary habit" in two cases, "do not eat fat" in two cases and no information was available in the rest five cases. Exercises which concerns spending about 300 kcal/day was recommended in one case, "moderate exercise" were recommended in three cases, "maintain current level of physical activity" were recommended in three cases and no information available in the rest five cases. Conclusions: In order to improve the accuracy of RCT, for the dietary side, researchers should record patient food intake at every visit by means of 24-hour dietary recall methods. This can be supplemented by multiple choice survey that are designed to help patients to diagnose themselves more accurately leading to less bias. For the exercise side, it is highly recommended to confine the exercises to walking only so as to quantify the amount of physical activity more easily by using pedometer.

Effects of Lifestyle and Dietary Behavior on Cardiovascular Risks in Middle-aged Korean Men

  • Lifestyle and dietary behavior intervention as the primary prevention of lipid disorder seems safe and compatible with other treatments of cardiovascular diseases. Cross-sectional associations between lifestyle factors and dietary behavioral factors with plasma lipid and lipoprotein levels were analyzed in 189 middle-aged men in Suwon, Korea. Overnight fasting plasma levels of total cholesterol, high-density lipoprotein(HDL)-cholesterol, triacylglycerol and glucose were analyzed. Blood pressure and anthropometric data were also measured. Lifestyle factors such as smoking status, alcohol consumption and frequency of physical exercise were evaluated by a self-administered questionnaire. Questions regarding dietary behavior were also asked. The subjects were 43.8%${\pm}$7.9 years old, and 23.8%${\pm}$2.6kg/m$^2$. From stepwise regression analyses, significant correlates with total cholesterol level were body mass index(BMI), alcohol intake(negative), age and coffee drinking(model R$^2$=14.3%). BMI, breakfast-skipping, age, and sleeping hours were significant for triacylglycerol level(model R$^2$=15.8%). BMI, alcohol drinking(negative), age, and coffee drinking were significant for low-density lipoprotein(LDL)(model R$^2$=11.7%). Age(negative), BMI(negative), alcohol drinking, stress level(negative), physical exercise, and cigarette smoking(negative) were significant for high-density lipoprotein(HDL)(model R$^2$=12.1%). From stepwise regression analyses, excluding BMI and age as factors in the model, alcohol intake(negative) and coffee drinking were significantly correlated with total cholesterol level(model R$^2$=4.4%) : breakfast-skipping with triacylglycerol(model R$^2$=3.2%) : alcohol intake (negative) with LDL level(model R$^2$=3.4%) : alcohol intake, physical exercise and stress level(negative) with HDL level(model R$^2$=6.3%). The findings suggest that a healthy daily lifestyle and dietary behavior may have an anti-atherogenic effect by altering plasma lipid and lipoprotein levels in middle-aged Korean men. Effects of Multidisciplinary Health Promotion Program Among Children in Community Childcare Center

  • In this study, the effects of a 12-month multidisciplinary education program on the health status, dietary quality, and eating habits of children and adolescents attending community childcare centers were investigated. A total of 88 participants aged 7 to 17 years from 7 community childcare centers in Gyeonggi-do were enrolled. The intervention consisted of 12 multidisciplinary education sessions covering topics such as nutrition, exercise, and psychological education. All participants received the same education, and the effectiveness of the program was evaluated by categorizing them into a high participation group (HPG) and a low participation group (LPG) based on their participation rates. After intervention, in physical activities, moderate-intensity exercise was significantly reduced in the LPG, and there was no significant difference in psychological parameters. However, notable differences were observed in nutritional data. After intervention, intakes of calorie, carbohydrate, protein, and fat were significantly increased in both groups, and in particular, the change was found to be greater in HPG. Additionally, dietary fiber intake compared to the 2015 Korean Dietary Reference Intakes was increased in both groups. Daily food intake also increased dietary fiber intake in HPG, and meat and fruit intake was increased in LPG. In the nutrition quotient, there was a significant difference in HPG's pre- and post-scores in the diversity category, and in nutrient adequacy ratio (NAR), the NAR of phosphorus was increased in both groups. The findings of this study suggest that multidisciplinary education implemented at community childcare centers primarily enhanced nutrition-related factors rather than physical activity or psychological aspects.

Acculturation and Dietary Intake of Korean American Women Living in California

  • The objective of this study was to examine the effect of acculturation stage on dietary intake of Korean American women (0=124) living in California and to compare the dietary intake with that of Korean women (0=191) in Seoul, Korea. The dietary intake of the subjects was collected by 24-hour recall method at cross-sectional surveys. Ouster analysis performed on immigration variables (e.g., length of residency, age at immigration, etc.) classified Korean American women into less (0=73) or more (n=51) acculturated group. Acculturation stage did not have a significant effect on macro nutrient intake. However, vitamin C intake was higher in the more acculturated group, while intakes of folate, calcium, iron, and zinc were higher in the less acculturated group. In comparison of three groups (the more and the less acculturated Korean American, and the Korean group), the more acculturated the women were, the less frequently they consumed rice and kimchi (p<0.05). Korean American women ate bread/noodle, meat/meat products, fruit juice, and soda more often and consumed vegetables less frequently, compared with Korean women (p<0.05). For breakfast, Western dishes were preferred in both more and less acculturated groups. Korean dishes were favored for dinner by both groups, even though the less acculturated group ate more Korean dishes than did the more acculturated group. The acculturation measured by immigration variables influenced nutrient intakes, food consumptions, and types of dishes eaten in Korean Americans. Cultural and health implications of dietary acculturation need to be studied in the future.

원주지역 초등학교 학생의 학교급식유형에 따른 영양섭취실태 조사 (A Study on Dietary Intakes of Elementary School Children According to School Foodservice Type in Won-Ju)

  • This study was carried out to survey dietary intakes of 739 elementary students, 4-6grades, in Won-Ju area, in 1998, to enhance the effect of school foodservice. The results were as follows : 1. Most investigated students had nuclear family with one brother or one sister. 2. The average height of boys and girls was $141.0{\pm}7.7cm\;and\;141.4{\pm}8.0cm$, and the average weight of them was $35.7{\pm}8.5kg\;and\;36.1{\pm}8.5kg$, respectively. There was no significant difference between boys and girls. 3. The rates of obesity were 12.2% in urban, 7.4% in rural and 0% in remote rural area by Rohrer index and 22.9% in urban 13.9% in rural and 0% in remote rural area by the rate based on standard weight. 4. All nutrients intake of investigated students were sufficient according to recommended dietary allowances. All nutrients intake of boys were higher than those of girls. Calcium intake of children with well educated mother was higher than that of children with poorly educated mother. Children in remote rural area had lower carbohydrate, calory and calcium intake than those in other areas. 5. Students without breakfast had lower protein and iron intake and higher calory from snack than those with breakfast. 6. Students who thought themselves to be obesity had low intake of carbohydrate, total calory and calory from snack and students with regular exercise had high intake of protein, iron, calcium and calory from snack. 7. Students with unbalanced diet, snack intake in large quantities and irregular meal had lower protein, fat, iron and calcium intake and higher calory from snack than other students. As a result, Students without breakfast often had lower all nutrients except calory from snack than other students, significantly. Problems of nutrients intake of elementary students in Won-Ju area were high rate of obesity, high intake of protein, unbalanced nutrients intake of students without meal or students with excessive eating and limited calcium intake of students with poorly educated mother. It is important to confirm desirable food habits for balanced nutrients intake in all areas and to increase calcium intake of students in remote rural area.

운동 유발성 알레르기 질환(FDEIA)에 미치는 영향과 기전분석 (Effects of Different Exercise Intensity on FDEIA and Related Mechanisms)

  • 본 연구에서는 우선 운동 강도(저강도, 중강도, 고강도)를 달리하여 훈련한 서로 다른 그룹에서 운동 유발성 알러지 아나필락시스의 차이를 규명하고 운동강도에 따른 운동 유발성 알러지 질환의 기전을 규명하고자 본 연구를 실시 하였다. 본 실험을 위하여 군당 25마리씩 통제군(S; control sensitized, n=25), 저강도 훈련군(S30, n=25), 중강도 훈련군(S50, n=25) 및 고강도 훈련군(S80, n=25)으로 구분하여 수영훈련 강도별 알러지를 유도하였을 때, 알러지 아나필락시스를 조사하고 아울러 비장지수, 림프구의 수, 림프구 증식반응, 싸이토카인(IL-4, INF-${\gamma}$), 항체 및 복강과 비장의 ROS를 함께 측정하였다. 본 연구 결과, 일반 감작군에 비하여 운동 감작군에서 알러지가 더 잘 일어남을 확인하였고, 운동강도의 증가와도 밀접한 것으로 확인되었다. 이는 운동강도가 증가할수록 OVA반응에 대한 림프구 증가가 현저하게 일어나는 원인과 복강활성산소의 증가 및 알러지 면역반응에 중요한 역할을 담당하는 싸이토카인인 IL-4의 증가 때문인 것으로 해석할 수 있다. 특히, 운동강도의 증가에 따라 INF-${\gamma}$의 감소도 의미 있는 결과로 해석된다. 따라서 추후 운동 강도뿐만이 아니라 운동빈도에 따라서도 FDEIA의 변화에 대한 심도 있는 연구가 필요하다고 여겨진다.

서울 지역 비구니 스님의 식생활과 건강상태에 대한 조사 연구 (A Study of Dietary Life and State of Health of Buddhist Nuns in the Seoul Area)

  • This survey was conducted to investigate the dietary behavior and health status of Buddhist nuns. In this study, 100 Buddhist nuns in Seoul were selected. The dietary survey was focused on the diet in winter. The age distribution was as follows; 24% of the subjects were in their 20s, 45% in their 30s, 23% in their 40s, and 8% in their 50's and over, respectively. The percentage of the subjects living in temples close to the downtown area was 77% and the rest resided in areas remote to the downtown area. Over fifty percent of the subjects were satisfied with their diet at temple. And 59% of the subjects were eating a snack more than once a day. The snacks included fruits (60%), raditional tea (20%), coffee (9%), bread and cookies (4%), md rice cakes (3%), etc. Among food groups, the intake of green-vegetables and fruits were the highest and intake of milk and bean products were low which may cause calcium and protein deficiency. Most of the subjects believed that their health conditions were average or above average. About 50% of subjects didn't exercise at all. The frequencies of gastro-intestinal diseases and anemia were extremely high. About 50% of subjects took some form of medicine. To improve their health and nutritional status, it is required that they practice a good dietary behavior, maintain a balanced diet, and exercise regularly.

과일식과 걷기 운동이 UCP2 유전자 다형성 특성에 따른 여대생의 신체 조성 및 혈액 성분에 미치는 영향 (Effects of a Fruit Diet and Exercise on Body Composition and Hematological Parameters according to UCP2 Genotype in Female Collegians)

  • The objective of this study was to examine the effects of a fruit diet and exercise on body composition and hematological parameters in female collegians according to their uncoupling protein 2 (UCP2) genotypes. Nine subjects were recruited from the SahmYook University. 4 subjects carried the ID genotype, 5 the DD genotype, and none of the subjects carried the II genotype. After 10 days of the program, BMI were significantly decreased (p<0.05) in the ID genotype and the DD genotype. Mean total energy intake from fruit diet was 888.1 kcal. Fat mass and % body fat were significantly decreased (p<0.05) in subjects with the ID genotype. Hemoglobin level was significantly increased (p<0.05) in subjects with the DD genotype. This study provided evidence that fruit diet and exercise could decrease the BMI in subjects with the ID and DD genotypes and increase the hemoglobin level in those with the DD genotype.

부산지역 일부 성인들의 신체활동, 식습관 및 영양소섭취상태에 관한 연구 (A Study on the Physical Activity, Food Habit and Nutrient Intakes of Adults in Pusan)

  • This study was performed to assess the physical activity, food habit and nutrient intakes by gender and age groups in 193 adults aged 20-59 years (84 men and 109 women) in Pusan. Data for physical activity and dietary survey was assessed by a questionnaire and 24hr recall method. The mean BMIs of men and women were 24.0 and 22.2 respectively and BMI of women in the 20-29 years group (20.9) was significantly lower than that of women in the 30-49 and 50-59 years group (22.7, 23.2) (p < 0.01). 56.0% for men and 44.0% for women exercised regularly. The mean exercise duration per once of men (69.7 minutes) was significantly higher than that of women (52.4minutes) (p < 0.01). The mean exercise duration per day was 36.0 minutes for men and 29.9 minutes for women. 67.9% for men and 78.0% for women often skipped meals and 68.4% for men and 69.4% for women skipped breakfast in the main. The mean energy intake of men was 2067.2 kcal and that of women was 1783.1 kcal comprised of 87.2% and 92.1% of the Estimated Energy Requirements (EER). The mineral intakes of men and women were over Recommended Intake (RI) and Adequate Intake (AI) except calcium and potassium. The mean calcium intake was 88.3% for men and 84.0% for women of RI. The mean potassium intake was 63.3% for men and 59.2% for women of AI. The mean vitamin intakes of men and women were over RI and AI except vitamin C and folic acid. The mean vitamin C intake was 92.5% for men and 85.6% for women of RI. The mean folic acid intake was 76.6% for men and 70.0% for women of RI. The mean energy, protein, sodium and zinc intakes of men were significantly higher than those of women (p < 0.01, p < 0.05, p < 0.05, p < 0.05). The mean vitamin $B_6$, vitamin C and folic acid intakes of men in the 20-29 years group were significantly lower than those of men in the 30-49 and 50-59 years group (p < 0.01, p < 0.05, p < 0.05). For energy, proportions of subjects with intake levels less than 90% EER were 64.3% for men and 56.0% for women. For calcium, proportions of subjects with intake levels less than Estimated Average Requirement (EAR) were 52.4% for men and 59.6% for women. For folic acid, proportions of subjects with intake levels less than EAR were 78.6% for men and 83.5% for women. For iron and phosphorus, proportions of women (36.7%, 14.7%)with intake levels less than EAR were significantly higher than those of men (6.0%, 1.2%) (p < 0.01, p < 0.001). For men, age was positively correlated with intakes of potassium, vitamin $B_6$, vitamin C and folic acid (p < 0.05, p < 0.05, p < 0.05, p < 0.01). For men, weight showed significantly negative correlations with intakes of carbohydrate, phosphorus, potassium, zinc, vitamin $B_6$ and folic acid (p < 0.01, p < 0.05, p < 0.05, p < 0.05, p < 0.01) and BMI showed significantly negative correlations with protein, lipid, phosphorus, potassium, zinc, vitamin E and folic acid (p < 0.05, p < 0.05, p < 0.05, p < 0.05, p < 0.05, p < 0.05, p < 0.05) For men, exercise duration per once showed significantly positive correlations with intakes of calcium, phosphorus, potassium, zinc, vitamin $B_1$, vitamin $B_2$, niacin, vitamin C and folic acid (p < 0.01, p < 0.05, p < 0.01, p < 0.05, p < 0.05, p < 0.01, p < 0.01, p < 0.01, p < 0.05). Therefore, nutritional education for adult health management is needed by gender and age groups.

갱년기 모델 유도 흰 쥐에서 갈근과 지황 복합물 및 유산소 운동이 대사이상 개선에 미치는 영향 (Effects of a Pueraria lobata-root based combination supplement containing Rehmannia glutinosa and aerobic exercise on improvement of metabolic dysfunctions in ovariectomized rats)

  • 본 연구는 갈근과 지황 복합물과 유산소 운동이 난소절제로 갱년기를 유도한 흰 쥐에서 나타나는 지질 및 당대사 이상에 미치는 영향을 확인하였다. 난소절제 후 8주간 AIN-76A 식이를 급여한 결과 체중증가량, 식이섭취량, 체 지방, 혈장 LDL-C, 공복혈당 증가를 통해 갱년기 동물 모델이 유도되었음을 확인할 수 있었다. 갈근과 지황 복합물과 유산소 운동은 서로 다른 기전으로 에스트로겐 분비 저하로 나타나는 비만 및 대사이상을 정상화하는 유익한 결과를 나타냈다. 지방 조직에서 확인된 관련 기전으로는 갈근과 지황 복합물로 인한 아디포넥틴 유전자 발현 증가 및 렙틴저항 억제, 유산소 운동으로 인한 아디포넥틴 및 인슐린 수용체 유전자 발현 증가, 갈근과 지황 복합물 및 유산소 운동의 병행으로 인한 아디포넥틴 유전자 발현 증가 상승효과가 관찰되었다. 이상의 결과로 갈근과 지황 복합물과 유산소 운동의 병행은 폐경기 에스트로겐 결핍으로 인한 대사이상을 개선하는 효과가 있을 것으로 기대한다. 향후에는 폐경기 유도 동물모델의 개선과 함께 단백질 및 신호전달분자 수준에서 추가적인 기전연구가 필요할 것으로 사료된다.
