관절염 환자의 운동행위 예측모형 (Pender의 재개정된 건강증진 모형에 의한) (Prediction Model of Exercise Behaviors in Patients with Arthritis (by Pender's revised Health Promotion Model))

  • The aims of this study were to understand and to predict the determinent factors affecting the exercise behaviors and physical fitness by testing the Pender's revised health promotion model, and to help the patients with rheumatoid arthritis and osteoarthritis perform the continous exercise program, and to help them maximize the physical effect such as muscle strength, endurance, and functional status and mental effects including self efficacy and quality of life, and improve the physical and mental well being, and to provide a basis for the nursing intervention strategies. Of the selected variables in this study, the endogenous variables included the physical fitness, exercise score, exercise participation, perceived benefits of action, perceived barriers of action to exercise, activity-related affect(depression) and perceived self-efficacy, interpersonal influences(family support), situational factors(duration of arthritis, fatigue) and the exogenous variables included personal sociocultural factor(education level), personal biologic factor(body mass index), personal psychologic factor(perceived health status) and prior related behavior factors(previous participation in exercise, life-style). We analyzed the clinical records of 208 patients with rheumatoid arthritis and degenerative arthritis who visited the outpatient clinics at H university hospital in Seoul. Data were composed of self reported qustionnaire and good of fitness score which were obtained by padalling the ergometer of bicycle for 9 minutes. SPSS Win 8.0 and Window LISREL 8.12a were used for statistical analysis. Of 75 hypothetical paths that influence on physical fitness, exercise participation, exercise score, perceived benefits of action, perceived barriers of action to exercise, activity-related affect(depression) and perceived self-efficacy, interpersonal influences(family support), situational factors(duration of arthritis, fatigue), 40 were supported. The physical fitness was directly influenced by life-style, perceived health status, education level, family support, fatigue, which explained 12% of physical fitness. The exercise participation were directly influenced by life-style, education level, past exercise behavior, perceived benefits of action, perceived barriers of action, depression and duration of arthritis, which explained 47% of exercise participation. Exercise score were directly affected by perceived self efficacy. BMI, life-style, past exercise behavior, perceived benefits of action, family support, perceived health status. perceived barriers of action, and fatigue, which explained 70%. Perceived benefits of action was directly influenced by BMI, life-style, which explained 39%. Perceived barriers of action were directly influeced by past exercise behavior, perceived health status, which explained 7%. Perceived self efficacy were directly influeced by level of education, perceived health status, life-style, which explained 57%. Depression were directly influeced by past exercise behavior, BMI, life-style, which explained 27%. Family support were directly influeced by life-style, perceived health status, which explained 29%. Fatigue were directly influeced by BMI, life-style, perceived health status. which explained 41%. Duration of arthritis were directly influeced by life-style, past exercise behavior, BMI, which explained 6%. In conclusion, important variables for physical fitness were life-style, and variable affecting exercise participation were life-style. Perceived self-efficacy of exercise was a significant predictor of exercise score. BMI, Life-style, perceived benefits of action, family support, past exercise behavior showed direct effects on perceived self-efficacy. Therefore, disease related factor should be minimized for physical performance and well being in nursing intervention for patients with rheumatoid arthritis, and plans to promote and continue exercise should be seeked to reduce disability. In addition, Exercise program should be planned and performed by the exact evaluation of exercise according to the ability of the patients and the contents to improve the importance of exercise and self efficacy in self control program, dedicated educational program should be involved. This study suggest that the methods to reduce the disease related factors, the importance of daily life-style, recognition of benefit of exercise, and educational program to promote self efficacy should be considered in the exercise behavior promotion and nursing intervention for continous performance. The significance of this study is also thought to provide patients with chronic arthritis the specific data for maximal physical and mental well being through exercise, chronic therapeutic procedure, daily adaptation and confrontation in nursing intervention.

관절염 환자의 운동행위 결정요인 (Determinant factors of Exercise behaviors in Patients with Arthritis)

Data were composed of self reported questionnaire and good of fitness score which were obtained by pedalling the ergometer of bicycle for 9 minutes. SPSS Win 8.0 and Window LISREL 8.12a were used for statistical analysis. 24 Of 54 hypothetical paths were supported in modified model, which was considered as a proper model with improved fit index. The physical fitness was directly influenced by exercise participation behavior and education level, and indirectly by physical fitness, while fatigue, physical disability, pastexercise behavior, life-style, self-efficacy, which explained 20% of physical fitness. The exercise participation were directly influenced by perceived benefits and self-efficacy, and indirectly influenced by life-style, fatigue and physical disability, and directly and indirectly by past exercise behavior, which explained 53% of exercise participation. Exercise score were directly affected by perceived health status, perceived benefits, self efficacy, and past exercise behavior, and were indirectly affected by fatigue, physical disability, and life-style, which explained 50%. Perceived health status were directly influeced by level of education, depression, sleep disorder, and physical disability, which explained 34% of perceived health status. Perceived benefit was directly influenced by fatigue, sleep disorder, physical disability, and life-style, which explained 45%. Perceived barriers was directly influenced by fatigue, sleep disorder, and lifestyle, which explained 9%. Self- efficacy was directly influenced by fatigue, physical disability, past exercise behavior, and level of education, which explained 61%. In conclusion, important variables for physical fitness were exercise participation and level of education, and variables affecting exercise participation were perceived self-efficacy, benefits, and past exercise behavior. Perceived self-efficacy of exercise was a significant predictor of exercise participation. Life-style, fatigue, and physical disability showed direct effects on perceived benefit, perceived barriers, and self-efficacy, and indirect effects on exercise behavior. Therefore, disease related factor should be minimized for physical performance and well being in nursing intervention for patients with rheumatoid arthritis, and plans to promote and continue exercise should be soaked to reduce disability. In addition, Exercise program should be planned and performed by the exact evaluation of exercise according to the ability of the patients and the contents to improve the importance of exercise and self efficacy in self control program, dedicated educational program should be involved.

류마티스 관절염 환자의 운동참여 예측요인 (A Predictors of Exercise Participation in Rheumatoid Arthritis Patients)

  • The purposes of this study were to examine and to predict the affecting factors on exercise participation of Rheumatoid arthritis Patients. The subjects were 161 adult out-patients who visited the hospital for rheumatic disease in H-university. Data were composed of self-reported questionnaire. The conceptual model of this study consisted of that personal characters(age, marriage, education, income), situational characters(pain intensity, fatigue, IADL, depression), behavioral characters(formerly exercise behavior, life-style), and cognitive-perceptional characters(perceived health status, perceived benefit, perceived barrier, perceived self-efficacy) affected directly to exercise participation. Logistic regression analysis was applied for testing model of this study. The results were as follows : 1. Personal characters(education), situational characters(pain intensity), behavioral characters(formerly exercise behavior, life-style), and cognitive characters(perceived barrier, perceived self-efficacy) were significant difference between current exercise participants(127subjects) and non-exercise participants(34). 2. Personal characters(income), situational characters(pain intensity), behavioral characters(life-style), and cognitive-perceptional characters(perceived barrier, perceived self-efficacy) were correlated to exercise participation. 3. Formerly exercise behavior, perceived barrier, and perceived self-efficacy were significant predictor of exercise participation. The logistic equation predicted overall 81.94% of this study subjects 161.

성인의 운동실천양상과 건강수준간 관련요인분석 - 초등학교 교직원을 중심으로 - (Relevance of Health Status and Exercises Patterns in Adults)

  • The purpose of this study is to analyze the relationship between life style and health status of the staffs of an elementary school to understand exercise practices and life styles and to analyze the self-efficacy and the factors that affect it concerning walking exercise. The subjects were m9 staff member of the 10 elementary school in Seoul and materials were collected through an organized examination table from October, 1999 to May 2000. The people health awareness table was used as a variable for life style, the 13 questionnaires made of a 5 points measure was for the self efficacy concerning walking exercise. The data was analyzed through a statistics package made of a Chisquare-test, t-test, ANOVA and a regression analysis. The major results of this study were as follows; 1) 50% of the staffs were practicing regular exercise, 3/4 of them practiced walking, jogging and mountain climbing that don't need equipment and exercise frequency was 1-2 times per week 64%. of the time and exercise span was under 30 minutes in 38%, the highest 2) The average of self-efficacy concerning walking exercise was 3.6 points but the answers "I can walk enough to be in a sweat", "I can walk according to my exercise plan" were relatively low. 3) The explanation degree of the Model was 14% in a regression analysis between self efficacy concerning walking exercise and as a dependent variable for a regular walking exercise plan, regular health examination, routine health status, and breakfast might serve as an explanation degree. According to the results of this study, the suggestions were as follows: 1) The chronic invalid was not enough in the change of life style after chronic disease, most of all, it need the awareness of change, of active health education and of public information for health promotion action practice through the prohibition of smoking, abstinence from drink, exercise and nutritional balance. 2) Concrete practice program enlargement is needed to enhance related walking exercise or of a type lecture meeting for subjects who have a positive attitude about walking exercise plans. 3) A program that can actively recommend a health promotion life style is needed when we make allowance for those who have an ordinarily healthy life style and show a high walking exercise self-efficacy.

고혈압 근로자를 대상으로 산업간호사가 제공한 맞춤형 생활습관개선 프로그램의 효과 (The Effects of Tailored Life Style Improvement Program for the Hypertensive Workers Provided by Occupational Health Nurse)

  • Purpose: To confirm the effect of tailored life style improvement program, provided by occupational health nurse for hypertensive workers on their blood pressure, self-efficacy, and life style changes in workplace. Method: The experimental and control group of thirty subjects each were randomly assigned for those who agreed to participate in the study among the pre-hypertensive workers. The tailored life style improvement program was provided to the experimental group and conducted once a week for 8 weeks. This program was composed of exercise and low salt diet to control high blood pressure and weight, and the measurement of carbon monoxide for non-smoking inducement. Result: In the experimental group, the systolic and diastolic blood pressure significantly decreased while self-efficacy increased after the program compared to the control group. Smoking and drinking habits of the experimental group were significantly decreased whereas exercise and nutritional habits were significantly improved compared to the control group. Conclusion: The tailored life style improvement program provided by occupational health nurses for the hypertensive workers was positively affected on their blood pressure, self-efficacy and life style changes in workplace.

일부 대학생들에 있어서 생활습관과 신체 및 정신건강도와의 관련성 (The Relationship between Life-style and Physical, Mental Health Status in College Students)

  • This study reports the relationship between life-style and physical, mental health status in college students. The questionnaire survey was carried out from September to October, 1991. The questionnaire included life-style, physical health condition centered on unhealthy habits and complaints about physical symptoms, and mental condition which was assessed by Zung's self-rating depression scale(SDS). Days absent from school during the last year in males(3.6) were significantly more than those in females (1.0). Days catching the common cold were more in females than in males. The amount of complaints and the SDS score were significantly higher in females than in males. In the relationship between physical and mental health status and individual health habits, abscence-days from school significantly correlated with regular life, snacking, physical exercise, smoking, drinking. The amount of complaints and the SDS score were significantly associated with regular life, regular meal-time, eating breakfast, nutritional balance and physical exercise. In the relationship between health practice index to health status, the group having good habits showed significantly less days catching the common cold, less complaints, less SDS score and less amount of stress. It was also shown that life-style correlated with physical and mental health status.

대전지역 중.장년층의 건강상태 및 건강생활 습관 (Health Status and Health-Related Life Style of Middle-aged People in Daejon)

  • Purpose of this study was to investigate the health status and life style of middle-aged people in Daejon. The subjects expressed more fear for disease as getting older(p<0.01). Sixty six percent of the subjects have suffered in the past or suffer at present certain kinds of disease. The older they are, the more disease they have. The alimentary disease, neuralgia, and arthritis were the most popular disease. The eye strain and exhaustion of body were indicated more frequently in fifties than in thirties or forties(p<0.01). They considered excercise as the most important factor to maintain health status, and then moderation in diet, relaxation, rest, decrease of alcohol intake, no smoking. Forty three percent of them were non-drinker. The number of drinker was four times more in male than female and increased as getting younger(p<0.01). Only male revealed smoker and 29% of them smoked 10-20 cigarettes per day. The number of smoker decreased as getting older(p<0.01). Male played exercise more regularly than female and subjects have better exercise habit as getting older(p<0.01). This study suggests that practical programs for self-health control should be developed for playing exercise regularly for female and for considering nutrition, no smoking, and no drinking in the daily life for male.

중학생 비만아들의 생활습관과 정신상태의 상관관계 (The Relationship between life Style of Obesity Teenager and Mental Condition)

  • Objectives The relationship between depression, obesity, life style of adolescents has been reported, but the results were not consistent. Thus this study was designed to find out the relationship. Methods The participants were the first and second grade middle school students. The participants were asked to measure their height and weight etc, and were asked to survey questionnaires. To diagnose the students' degree of depression, Children's Depression Inventory(CDI) by Kovas and Beck. Results The average depression score in the obese group was significantly higher than that of the normal body weight group. In addition, male obese group scored higher on CDI than the female obese group, and the obese group of participants who don't exercise, but like to play computer games and watch TV scored higher on CDI than the group who exercise and don't play computer games. Conclusions This study proved that depression in adolescent was closely related with obesity and obesity-related lifestyles. 청소년의 골밀도에 영향을 주는 요인에 관한 연구: 신체계측치, 생활습관 및 기타 환경요인 (A Study of Factors Affecting Bone Mineral Density in Korean Adolescents: Anthropometric Measurements, Life Style, and Other Environmental Factors)

  • This study was conducted to assess the bone mineral density (BMD) and the anthropometric measurements, life style, and other environmental factors affecting BMD in Korean adolescents. Subjects were 167 high school students (83 male students, 84 female students) in Seoul. BMD was measured in the lumbar spine (LS), femoral neck (FN), femoral trochanter (FT), and Ward's triangle (WT) by dual energy x-ray absorptiometry (DEXA). The questionnaire was used to assess the anthropometric measurements, life style, and other environmental factors. Bone mineral density of LS, FN, FT and WT were 0.967, 0.960, 0.795, $0.761 g/{\cal}cm^2$. The BMD of LS was not different by sex but the male students's BMD of FN, FT and WT were higher significantly than the female students (LS: 0.976 vs. 0.958, FN: 1.040 vs. 0.880, FT: 0.842 vs. 0.749, WT: 0.827 vs. $0.695 g/{\cal}cm^2$) Female students's BMD of LS, FN, WT, and FT was positively correlated with weight but male students's BMD of WT was not correlated with weight. The factors such as the life style, activity and exercise have significant influence on BMD. This study confirms that the major factor affecting BMD was body weight and the factors such as the life style, activity and exercise is related to accumulation of BMD. The classification of sexual characteristics is needed for further studies on BMD of adolescents.

생활양식과 식이성 요인이 성인의 혈장 총 유리기포집 항산화능(TRAP) 수준에 미치는 영향 (Effect of Life Style and Dietary Factors on Plasma Total Radical-Trapping Antioxidant Potential (TRAP) in Korea Adult)

  • Recently developed TRAP assay(Total Radical-trapping Antioxidant Potential) is a new technique which enable effectively the measurement of total antioxidant status in the plasma. Researches using TRAP method have rarely been reported in Korea. Therefore, we investigated effects of various life-style and dietary factors on plasma TRAP level in Korean adult population. The subjects consisted of 161 health adult male(n=105) and female (n=56) aged 19-57. A self-administered questionnaire ascertained life-style factors including smoking habits, alcohol use, physical activity. Dietary factors were analyzed by 24 hour recall method. TRAP was determined spectrophotometrically in plasma at 734nm. Mean plasma TRAP concentrations were slightly higher in males (1.35$\pm$0.02mM) and females(1.30$\pm$0.02mM) without significance. There were significant correlations between plasma TRAP levels and life-style factors including exercise and smoking Plasma TRAP levels were affected more by exercise than by smoking. Moderate amount exerciser(exercise time $\geq$30min/day) had significantly higher level of plasma TRAP levels in male smokers(1.33$\pm$0.03mM) were significantly lower than those in male nonsmokers(1.39$\pm$0.01mM, p<0.05), but the difference was not seen in female subjects. Intakes of vitamin C(r=0.288, p=0.007) or folate (r=0.258, p=0.016) were positively correlated with plasma TRAP levels in nonsmokers, but those relations were not seen in smokers. Dietary factors such as vegetables (r=0.288, p=0.007)or folate (r=0.258, p=0.016) were positively correlated with plasma TRAP levels in nonsmokers, but those relations were not seen in smokers. Dietary factors such as vegetables (r=0.277, p=0.009) and legumes(r=0.263, p=0.013) consumption were positively related to plasma TRAP concentrations also in nonsmokers only. Regular green tea drinkers showed significantly greater level of plasma TRAP than the non green tea drinkers(1.36$\pm$0.01mM vs 1.30$\pm$0.03mM, p<0.05). These results suggest that the regular exercise($\geq$30min daily), nonsmoking, and consumption of vegetables, legumes and green tea would contribute to improving plasma TRAP levels in Korean adults.
