Obesity rates are increasing worldwide, associated with excess acute and chronic disease risk. In most countries, obesity rates among women exceed rates in men, particularly during the post menopausal years. Many factors affect body weight and appetite, including age, metabolic rate, physical activity level, stress, cultural factors, socioeconomic status, health status and health literacy, diet composition, attitudes, and beliefs. Gender affects appetite and body weight indirectly by altering factors contributing to food choice. However, there is emerging evidence that gender affects appetite and body weight directly, altering the physiological control systems regulating appetite. The follicular menstrual cycle phase (estrogen-rich) is associated with relative suppression of appetite. Lower estrogen levels are associated with increased food intake, body weight gain, and altered body fat distribution in humans and animals. This paper reviews the linkages between estrogen and appetite regulation. While relationships among appetite, body weight, and gender-linked hormones are complex, research elucidating these interrelationships could lead to development of gender-specific treatment approaches for obesity and appetite dysregulation.
Two patients with chronic Budd-Chiari syndrome resulting from segmental obstruction of the inferior vena cava underwent operation. There were 1 man and 1 woman. The obstructed segment was directly visualized by a transthoracic, transdiaphragmatic, retroperitoneal approach. In these two cases, severe segmental obstruction of the inferior vena cava was observed just above the right hepatic vein. These patients underwent successful retrohepatic cavoatrial bypass with a polytetrafloroethylene [PTFE] graft [ 16mm plain and 16mm ringed graft ]. There were no operative mortality and postoperative complication. These patients have been followed up for 6months and 36months without evidence of re-obstruction. When there is a severe stricture of the IVC with hepatic veins draining freely into the obstructed segment of the IVC, a dorsal cavoatrial bypass with a PTFE graft, preferably ringed, is the method of choice.

In this article, free vibration attributes of double-walled carbon nanotubes based on nonlocal elastic shell model have been investigated. For this purpose, a nonlocal Flügge shell model is established to observe the small scale effect. The wave propagation is employed to frame the governing equations as eigenvalue system. The influence of nonlocal parameter subjected to different end supports has been overtly examined. A suitable choice of material properties and nonlocal parameter been focused to analyze the vibration characteristics. The new set of inner and outer tubes radii investigated in detail against aspect ratio and length. The dominance of boundary conditions via nonlocal parameter is shown graphically. The results generated furnish the evidence regarding applicability of nonlocal shell model and also verified by earlier published literature.

Historically, the decision to perform a hemiarthroplasty (HHR) versus a total shoulder arthroplasty (TSA) is based on the status of the glenoid and the status of the soft tissues (rotator cuff). In disease processes where the glenoid articular cartilage is relatively well preserved such as avascular necrosis and complex proximal humerus fractures, most orthopaedists recommend performing a HHR while preserving the native glenoid articular surface. At the other end of the spectrum, if the glenoid has excessive bone loss or is unreconstructible, a HHR is the preferred procedure. In patients who have deficient so(t-tissues (rotator cuff) such as rotator cuff tear arthropathy and, occasionally, rheumatoid arthritis, a HHR is the procedure of choice. The indications for HHR in osteoarthritis remain somewhat controversial. There is mounting evidence that performing a HHR for osteoarthritis is inferior to TSA. Recent developments, or 'third generation techniques and materials', in shoulder arthroplasty are expected to improve the longevity of TSA, particularly the glenoid component. In addition, newer designs of reverse-ball prostheses are entering the market with promising early results in patients with deficient rotator cuff mechanisms.
As a step preliminary to the factor-analysis of a child-rearing attitude scale under construction, its preliminary form containing a total of 368 self-answered questions was administerd to 88 married women with one or more children in the grade school age. all graduates of colleges and in possession of a home telephone and a house, twice from one week apart. The test-retest correlation coefficients, calculated for each test item based on the subject's choice among the three response alternatives on two test ocasions, ranged from .80 to .10. The cut-off point of r=.41 or lower led to the elimination of 48 items. As a point of incidental interest, the rate of response concordance. The namber of subjects giving the same categroy response on two occasions was calculated for each item and it was correlated with its test. retest correlation coefficient. The two different indices of item test-retest reliability were found not related to each other. Empirical evidence, as well as rational justifications, was presented to show that the correlation coefficient is the better of the tow indices of item test-retest reliability.
A modified $Brostr{\ddot{o}}m$ procedure has been widely accepted as a treatment of choice for patients with chronic lateral ankle instability. The procedure is highly reliable and provides successful outcomes in approximately 90% of cases. Severe degree of instability, absence or poor quality of the ligamentous remnant, obesity, and generalized joint hypermobility have been regarded as poor prognostic factors related to the modified $Brostr{\ddot{o}}m$ procedure. However, these perceptions are based on a low level of evidence studies or expert opinions. A traditional clerkship consists of a series of short rotations in specialty disciplines and is usually based in tertiary, urban teaching hospitals. Shortened inpatient stays and the shift toward ambulatory management have had a negative impact on student learning. There have been growing concerns that the traditional specialty-based clerkship in fragmented and highly specialized clinical environments may not be the optimal choice for basic clinical education. As a result, a new model of clinical clerkship called longitudinal integrated clerkships (LICs) has emerged. There is increased interest in LICs due to the growing evidence of positive outcomes for students, patients, and supervising clinicians. Emphasizing continuity as one of the main organizing principles of an LIC, this article reviews the introduction of LICs into medical education, the key concepts and educational theories which underpin LICs, and the typology of LICs. The author also offers some personal suggestions for contemplation before clerkship programs in Korea adopt LICs.
Main research question of this study is about whether gated community (GC) as private urban governance gets along with local public goods by locating near to them. We examine this question through testing the Tiebout hypothesis from case study of Seoul, capital city of South Korea, in which GCs are so common to test the assumption empirically. For this, we examine the meaning of GC in 3 Es viewpoints; conceptualize the framework of Tieboutian co-evolution of GC and local public goods by hedonic price modeling. As a result, possibilities are found that GCs are to be seen from different point of view, viz. co-evolutionary mechanism between private and public governance; GCs effectively capture and represent the demand of residents for local public goods through voting by their collective locational choice. It allows us different kind of approach to investigate APTs as a co-evolutionary form of private and public urban order rather than seeing them only as a tool of speculative investment, particularly in rapidly urbanizing countries like Korea.
Despite the increasing number of osteochondral lesions of the talus, there are a lack of definite evidence-based treatment protocols. Several types of treatments are available, each having their advantages and disadvantages. First-line therapy consists of well-conducted conservative treatment. Surgical treatment is the second choice. Treatments are chosen based on the size of the lesion, location, chronicity, and the condition of the neighboring cartilage. This article reviews the current updates in the treatment of osteochondral lesions of the talus to help clinicians use the available treatment strategies more efficiently.
More donors are formally assessing their multilateral aid disbursement policies as well as the multilateral institutions that they contribute to. Analyzing OECD Creditor Reporting System data from 2011 to 2019 of 23 donors and 34 multilateral organizations, we find evidence of institutional portfolio building of donors to align multilateral and bilateral aid channels. Such tendency is more pronounced for core-funding than multi-bi funding and much stronger at the recipient country level than at the sectoral level. Smaller donors that operate from a limited multilateral budget show greater preferences for geographical similarity. When donors give to institutions with sectoral specialization, they seek sectoral similarity with their bilateral aid.
