Study on the Retraction of Anterior Teeth for the Lingual Orthodontics with the Three-Dimensional Finite Element Method (유한요소법을 이용한 설측 치아교정시 전치부 후방견인에 관한 연구)

  • The orthodontic surgery including lingual orthodontics has recently attracted a person's attention due to its functional and esthetic appreciation. The skeletal anchorage with the miniscrew is newly adopted in the lingual orthodontics to assist the upholding ability. The appliciation needs to understand the mechanism of the orthodontic appliance and its related orthodontic correction for optimal orthodontic treatment. There is, however, few information about the qualitative and quantitative effect of the orthodontic appliance with the miniscrew has not been well identified. In this paper, three dimensional finite element analysis is introduced to the lingual orthodontics in order to investigate the effect of the anterior retraction force on the miniscrew and transpalatal arch wire. The analysis determines the adequate location of the miniscrew and the point of force application of the anchorage system in the lingual orthodontics. The analysis results demonstrate the effect of the position of the miniscrew and the transpalatal arch wire on the lingual orthodontics.

  • The odontogenic myxoma is a relatively rare, benign tumor that occurs on the jaw. A 41 year-old man visited Seoul National University Dental Hospital because of swelling of the mandible. Clinical and radiographic evaluation showed a huge mass invading most of the mandible. After biopsy, he was diagnosed with odontogenic myxoma. For resection of the lesion, partial mandibulectomy and reconstruction with a vascularized fibular free flap was done. The result showed successful removal of the lesion. Reconstruction resulted in satisfactory functional and esthetic outcomes. We conclude that huge benign neoplasms such as odontogenic myxomas can be successfully treated by using a wide margin of resection followed by vascularized fibular free flap reconstruction.

  • Gingival hyperpigmentation may cause esthetic problems, especially in patients with a gummy smile. This report presents the use of the $CO_2$ laser for gingival depigmentation. Two cases presented with the same chief complaint of unesthetic gingiva caused by melanin hyperpigmentation. The $CO_2$ laser was setted at 0.8 watt, 40Hz, 0.01sec. The procedure were performed with non-contact mode in all pigmented areas. Ablation of the gingival hyperpigmentation areas were accomplished without any bleeding complications or postoperative pain. After 2 weeks and 4 weeks later, healing is completed and hyperpigmented gingiva appeared pink and firm.

  • Cystic hygroma, a cystic subtype of lymphangioma, is relatively rare tumor of lymphatic origin. The lesion is a benign, painless, soft, compressible malformation of the lymphatic system. They can arise anywhere along the lymphatic system, however they are usually located in the head and neck regions and in most cases appear by the age of 2 years. The cases in the adult is rarely occurred and a few cases are described in the literature. Surgical excision remains the treatment of choice. But complete extirpation of these lesion is often impossible, because the tumor tends to spread along vital structures. Therefore recurrence rates are accordingly high. This is a case report about 19 year old male patient with cystic hygro a on right retromandibular area. We obtained the successful, functional and esthetic result by surgical excision of the mass. Therefore, we report the case with a review of literatures.

  • There has been a lot of research into the essence of the smile, but so far just a few studies have been done on the relationship between personality and smile. On the assumption that smile aesthetics are closely related to individual physical condition and psychological state, this study investigated the correlation between personality factors and smile scores. The Personality Factor Questionnaire test was administered to 60 university students (male : 30, female : 30) who have no teeth missing, no experience of orthodontic or prosthodontic treatment and good dentition. Then, portraits were taken in a full smile. Esthetic smile scores were estimated by 10 appraisers who were dentists. These scores were analyzed statistically using the SAS program. The following results were obtained. 1. Personality factors of Warmth, Stableness, Surgency, Boldness, Untroubled-adequacy, Group-dependence, Relaxation, Extraversion, Low anxiety were positively correlated to the aesthetic level of smile. 2. For females, personality factors had greater influence on smile aesthetics, whereas male smile aesthetics were less influenced by personality factors.

A literature review on the survival rate of single implant-supported restorations (단일 임플란트 지지에 의한 보철물의 생존율에 관한 문헌 연구)

  • Implant material, implant design, surface quality, status of the bone, surgical technique, and implant loading conditions were regarded as prerequisites for osseointegration which is a prime condition for implant success. The aim of this review paper was to investigate the survival rate of single implants in relation to the prerequisites for osseointegration. Fifty-eight papers reporting survival rates of single implants were selected by use of the 'PubMed' and hand searching. The survival rate of single implants were assessed with reference to factors influencing osseointegration. The results showed that single implants in general showed a high survival rate except a few failures in certain extreme conditions and early stages. Those failures and complications such as screw loosening and esthetic problem were almost solved with the development of implant components and surgical techniques and a better understanding of biology around a single implant. Single-tooth implant-replacement is now considered as a reliable and predictable treatment option for a single missing tooth and its application seems to expand to compromised situations which were previously thought to be impossible for single implant therapy.

  • Purpose: Surgical intervention into the maxillary posterior area can lead to inadvertent communication with the maxillary sinus. Spontaneous healing of 1 to 2 mm openings can occur. However, in patients with larger oro-antral communications and those with a history of sinus disease, surgical closure is often indicated. Materials and Methods: In this case, a modified free gingival graft technique was used. The oro-antral sinus track after extraction was about $2{\times}4\;mm$. But the patient had a history of Caldwell-Luc operation. Immediately after extraction, a modified free gingival graft was used for the closure of oro-antral sinus track. Result: A modified free gingival graft technique was successfully used for the closure of oro-antral sinus track. With this technique, esthetic results including similar colors, textures were achieved. Conclusion: It is thought that this technique is very useful in the closure of oro-antral sinus track.

  • In these days, the orthodontic surgery including lingual orthodontics has attracted a person' attention due to its functional and esthetic appreciation. The delivery of the optimal orthodontic treatment is greatly influenced by clinician' ability to predict and control the tooth movement by applying force system to dentition. The skeletal anchorage system with the miniscrew has been used recently in the lingual orthodontics to assist the anchorage control. Precise understanding of the force system produced from the various orthodontic appliances is necessary. However, the qualitative and quantitative effect of the miniscrew has not been identified well. In this paper, three dimensional finite element analysis is introduced on the lingual orthodontics to investigate the effect of anterior retraction force on the miniscrew and transpalatal arch wire. The purpose of this study is to determine the location of the miniscrew and the point of force application of the anchorage system in the lingual orthodontics. The analysis results indicate the efficient position of the miniscrew and the transpalatal arch wire in the lingual orthodontics.

A Clinical study of infraorbital wall fractures. (안와하부 골절의 임상적 연구)

  • This study was aimed at furnishing the data of infraorbital wall fractures and aiding treatment. This is the retrospective study on infraorbital wall fractures. The patients were treated in the Dept. of Oral & Maxillofacial Surgery of Chon-Buk National University Hospital from Jan. 1, 1996 to Sep. 30, 1999. The result were as follows : Male predominated over female by a ratio of 3.57 : 1. The most common reason was traffic accident(64.1%). The elapsed time from injury to operation was average 10.4 days. The most frequent site of fractures was Zygomatico-Maxillary complex fracture(61.7%). The highest department of associated injuries was neurologic Dept.(52.3%). The complication after fractures were the ophthalmologic(20.3%), esthetic(14.8%), facial numbness(4.7%), etc in this order. The highest ophthalmologic complications was the enophthalmos(7%).

  • Cystic hygroma is the benign proliferation of lymphatic tissue and has been categorized as a part of a larger spectrum that includes lymphangiomas recently. The majority of lymphangiomas occur in the head and neck as cystic hygromas with the posterior cervical region as the most common site. Cystic hygromas present in infancy or early childhood as compressible masses that may rapidly and intermittently enlarge. Cystic hygromas of the head and neck are especially difficult to manage since enlargement cause serious sequela such as airway obstruction, feeding difficulties, and speech pathology. Surgical exision remains the treatment of choice. But complete extirpation of these lesions is often impossible, and recurrence rates are accordingly different respectively ; these are high in suprahyoid lesions compared with infrahyoid involvement. This is a case report about 22 year old male patient with cystic hygrom. We obtained the successful, functional and esthetic results by surgical excision of the mass. Therefore, we report the case with literatural reviews.
